  1. A pharmacy led program to review anti-psychotic prescribing for people with dementia

    Full Text Available Abstract Background Anti-psychotics, prescribed to people with dementia, are associated with approximately 1,800 excess annual deaths in the UK. A key public health objective is to limit such prescribing of anti-psychotics. Methods This project was conducted within primary care in Medway Primary Care Trust (PCT in the UK. There were 2 stages for the intervention. First, primary care information systems including the dementia register were searched by a pharmacy technician to identify people with dementia prescribed anti-psychotics. Second, a trained specialist pharmacist conducted targeted clinical medication reviews in people with dementia initiated on anti-psychotics by primary care, identified by the data search. Results Data were collected from 59 practices. One hundred and sixty-one (15.3% of 1051 people on the dementia register were receiving low-dose anti-psychotics. People with dementia living in residential homes were nearly 3.5 times more likely to receive an anti-psychotic [25.5% of care home residents (118/462 vs. 7.3% of people living at home (43/589] than people living in their own homes (p  Of the 161 people with dementia prescribed low-dose anti-psychotics, 91 were receiving on-going treatment from local secondary care mental health services or Learning Disability Teams. Of the remaining 70 patients the anti-psychotic was either withdrawn, or the dosage was reduced, in 43 instances (61.4% following the pharmacy-led medication review. Conclusions In total 15.3% of people on the dementia register were receiving a low-dose anti-psychotic. However, such data, including the recent national audit may under-estimate the usage of anti-psychotics in people with dementia. Anti-psychotics were used more commonly within care home settings. The pharmacist-led medication review successfully limited the prescribing of anti-psychotics to people with dementia.

    Chlorpromazine is known to cause ocular pigmentary deposits. However, delayed presentation after cessation of chlorpromazine has not been reported. There are also no reports on whether newer generation of anti-psychotic agents contribute to ocular toxicity. We describe a case of ocular toxicity related to anti-psychotic agents. To the best of our knowledge, this is the first reported case of anterior segment pigmentary deposits associated with olanzapine use, 2 years after the cessation of chlorpromazine.We report a case of ocular toxicity in a patient with history of chlorpromazine usage of 100 mg per day for 13 years and subsequently switched to olanzapine 5 mg for 2 years. There were no signs of ocular toxicity while the patient was on chlorpromazine. However, when the patient switched to olanzapine, she developed the ocular side effect as described for chlorpromazine-induced ocular toxicity, with pigmentary depositions on both corneas and the anterior lens surface and decrease in vision.Olanzapine, a newer anti-psychotic agent, may play a role in the ocular pigmentary deposition, either directly causing pigmentary deposition itself or accentuating the effect of chlorpromazine as the 2 drugs act on the same receptors, although further studies are required to support this hypothesis. As patients with psychiatric conditions may not voluntarily complain of visual symptoms, ocular screening could be considered in these patients receiving chronic anti-psychotic treatment, so that any ocular toxicity could be diagnosed in a timely manner.   4. Anti-atherogenic properties of high-density lipoproteins in psychiatric patients before and after two months of atypical anti-psychotic therapy.

    Some of the medications used for the management of schizophrenia are associated with clinically significant increases in weight and adverse alterations in serum lipid levels. The aim of the study was to investigate the effect of short-term (two months) treatment with atypical anti-psychotics on coronary heart disease risk factors, including the functional properties of high-density lipoprotein (HDL), in psychiatric patients. Nineteen patients diagnosed with schizophrenia, schizoaffective, and bipolar disorder and ten healthy volunteers were enrolled in the study. In the present study blood was drawn at baseline and after two months of atypical anti-psychotic treatment. Wilcoxon non-parametric-test was used to examine differences in the psychotic group before and two months after treatment.Waist circumference and oxidative stress in psychiatric patients were higher compared with the control group. Serum-mediated cholesterol efflux capacity was lower in psychotic patients compared to controls. Two months of anti-psychotic therapy was associated with increased abdominal obesity, decreased paraoxonase lactonase activity, but with no further change in serum-mediated cholesterol efflux from macrophages. Psychotic patients have low serum-mediated cholesterol efflux from macrophages as a parameter of HDL functionality. Atypical anti-psychotic treatment for two months increased metabolic derangements in these patients but without further decrement in serum-mediated cholesterol efflux. PMID:26253619

  11. Effect of in utero exposure to the atypical anti-psychotic risperidone on histopathological features of the rat placenta.

    For clinical management of different forms of psychosis, both classical and atypical anti-psychotic drugs (APDs) are available. These drugs are widely prescribed, even during pregnancy considering their minimal extra-pyramidal side effects and teratogenic potential compared to classical APDs. Among AAPDs, risperidone (RIS) is a first-line drug of choice by physicians. The molecular weight of RIS is 410.49 g/mol; hence, it can easily cross the placental barrier and enter the foetal bloodstream. It is not known whether or not AAPDs like RIS may affect the developing placenta and foetus adversely. Reports on this issue are limited and sketchy. Therefore, this study has evaluated the effects of maternal exposure to equivalent therapeutic doses of RIS on placental growth, histopathological and cytoarchitectural changes, and to establish a relationship between placental dysfunction and foetal outcomes. Pregnant rats (n = 24) were exposed to selected doses (0.8, 1.0 and 2.0 mg/kg) of RIS from gestation days 6-21. These dams were sacrificed; their placentas and foetuses were collected, morphometrically examined and further processed for histopathological examination. This study revealed that in utero exposure to equivalent therapeutic doses of RIS during organogenesis-induced placental dystrophy (size and weight), disturbed cytoarchitectural organization (thickness of different placental layers), histopathological lesions (necrosis in trophoblast with disruption of trophoblastic septa and rupturing of maternal-foetal interface) and intrauterine growth restriction of the foetuses. It may be concluded that multifactorial mechanisms might be involved in the dysregulation of structure and function of the placenta and of poor foetal growth and development. PMID:27256515

Anorexia can have a direct impact on the carbohydrate and fat intake, which may, in turn, regulate antipsychotic induced dyslipidemia and Hyperglycaemia. Aim: To compare the effects of Modafinil- ADDON with Placebo add on with olanzapine, Clozapine and Risperidone in drug naive subjects and people who were started on the drugs within 15days of assessment. Materials and Methods: Randomized, Double blind, Placebo controlled study, which was conducted at two centres, one at department of Psychiatry, S.V Medical College, Tirupati and the other at Asha hospitals, Hyderabad. Seventy two patient were randomised, sixty three patients have completed the total study period of three months.The dose of Modafinil was 200 mgs constantly as Flexible doses of Olanzapine, Clozapine and Risperidone as per clinical need was given. A baseline, three week and twelve week assessments of Fasting blood Glucose and fasting Serum cholesterol were made and the groups were compared on these parameters. Results: From baseline to week 3 there was a significant raise in Fasting serum cholesterol followed by a fall from week 3 to week 12 in the Modafinil addon group, though it could not be considered a drug for hypercholesteremia like Statins in controlling hyperlipidaemia.   18. A 12-month controlled trial of methadone medical maintenance integrated into an adaptive treatment model.

    Methadone medical maintenance (MMM) reduces the reporting schedule for stable and well-functioning methadone maintenance patients to once a month, with counseling provided by medical staff. We report on the 12-month outcomes of 92 highly stable methadone maintenance patients randomly assigned to one of three study conditions: routine care, MMM at the methadone maintenance program, and MMM at a physician's office. Methadone medical maintenance patients received a 28-day supply of methadone, whereas routine care patients received five or six take-home methadone doses each week. All patients performed a medication recall once a month and submitted two urine samples each month. An adaptive stepped-care system of treatment intensification was used for patients who failed recall or who had drug-positive urine specimens. Seventy-seven patients completed the 12-month study period. Dropout was caused primarily by problems with handling methadone and disliking the recall frequency. There were low rates of drug use or failed medication recall. Treatment satisfaction was high in all groups, but the MMM patients initiated more new employment or family/social activities than did routine care patients over the study period. The stepped-care approach was well tolerated and matched patients to an appropriate step of service within a continuum of treatment intensity. PMID:17084792

  19. A model for preemptive maintenance of medical linear accelerators—predictive maintenance

    Unscheduled accelerator downtime can negatively impact the quality of life of patients during their struggle against cancer. Currently digital data accumulated in the accelerator system is not being exploited in a systematic manner to assist in more efficient deployment of service engineering resources. The purpose of this study is to develop an effective process for detecting unexpected deviations in accelerator system operating parameters and/or performance that predicts component failure or system dysfunction and allows maintenance to be performed prior to the actuation of interlocks. The proposed predictive maintenance (PdM) model is as follows: 1) deliver a daily quality assurance (QA) treatment; 2) automatically transfer and interrogate the resulting log files; 3) once baselines are established, subject daily operating and performance values to statistical process control (SPC) analysis; 4) determine if any alarms have been triggered; and 5) alert facility and system service engineers. A robust volumetric modulated arc QA treatment is delivered to establish mean operating values and perform continuous sampling and monitoring using SPC methodology. Chart limits are calculated using a hybrid technique that includes the use of the standard SPC 3σ limits and an empirical factor based on the parameter/system specification. There are 7 accelerators currently under active surveillance. Currently 45 parameters plus each MLC leaf (120) are analyzed using Individual and Moving Range (I/MR) charts. The initial warning and alarm rule is as follows: warning (2 out of 3 consecutive values ≥ 2σ hybrid) and alarm (2 out of 3 consecutive values or 3 out of 5 consecutive values ≥ 3σ hybrid). A customized graphical user interface provides a means to review the SPC charts for each parameter and a visual color code to alert the reviewer of parameter status.     The effective maintenance management of medical technology influences the quality of care delivered and the profitability of healthcare facilities. Medical equipment maintenance in Jordan lacks an objective prioritization system; consequently, the system is not sensitive to the impact of equipment downtime on patient morbidity and mortality. The current work presents a novel software system (EQUIMEDCOMP) that is designed to achieve valuable improvements in the maintenance management of medical technology. This work-order prioritization model sorts medical maintenance requests by calculating a priority index for each request. Model performance was assessed by utilizing maintenance requests from several Jordanian hospitals. The system proved highly efficient in minimizing equipment downtime based on healthcare delivery capacity, and, consequently, patient outcome. Additionally, a preventive maintenance optimization module and an equipment quality control system are incorporated.   4. In Silico Identification and In Vitro and In Vivo Validation of Anti-Psychotic Drug Fluspirilene as a Potential CDK2 Inhibitor and a Candidate Anti-Cancer Drug.

    Full Text Available Hepatocellular carcinoma (HCC is one of the leading causes of cancer-related deaths worldwide. Surgical resection and conventional chemotherapy and radiotherapy ultimately fail due to tumor recurrence and HCC's resistance. The development of novel therapies against HCC is thus urgently required. The cyclin-dependent kinase (CDK pathways are important and well-established targets for cancer treatment. In particular, CDK2 is a key factor regulating the cell cycle G1 to S transition and a hallmark for cancers. In this study, we utilized our free and open-source protein-ligand docking software, idock, prospectively to identify potential CDK2 inhibitors from 4,311 FDA-approved small molecule drugs using a repurposing strategy and an ensemble docking methodology. Sorted by average idock score, nine compounds were purchased and tested in vitro. Among them, the anti-psychotic drug fluspirilene exhibited the highest anti-proliferative effect in human hepatocellular carcinoma HepG2 and Huh7 cells. We demonstrated for the first time that fluspirilene treatment significantly increased the percentage of cells in G1 phase, and decreased the expressions of CDK2, cyclin E and Rb, as well as the phosphorylations of CDK2 on Thr160 and Rb on Ser795. We also examined the anti-cancer effect of fluspirilene in vivo in BALB/C nude mice subcutaneously xenografted with human hepatocellular carcinoma Huh7 cells. Our results showed that oral fluspirilene treatment significantly inhibited tumor growth. Fluspirilene (15 mg/kg exhibited strong anti-tumor activity, comparable to that of the leading cancer drug 5-fluorouracil (10 mg/kg. Moreover, the cocktail treatment with fluspirilene and 5-fluorouracil exhibited the highest therapeutic effect.   5. Evaluation of a risk communication approach for maintenance staff working with induced radioactivity in medical linear accelerators

    In order to promote consensus building on decommissioning operation rules for medical linear accelerators in Japan, we carried out a risk communication (RC) approach mainly providing knowledge for maintenance staff regarding induced radioactivity. In February 2012, we created a booklet (26 pages) to present an overview of the amended law, the mechanism and the distribution of induced radioactivity showing the actual radiation dose rate around a linear accelerator and actual exposure doses to staff. In addition, we co-sponsored a seminar for workers in this field organized by the Japan Medical Imaging and Radiological Systems Industries Association to explain the contents of this booklet, and answer questions regarding induced radioactivity of linear accelerators as an RC program. As a result, the understanding of staff regarding the regulations on maximum X-ray energy on linear accelerators (P<0.05), and the outline of clearance systems (P<0.01), were facilitated by RC. In addition, we found that about 70% of maintenance staff considered that the cooling time for decommissioning operation depended on the situation. We sought to determine whether our increased health education and screening intervention was associated with improved: 1) Improved drug addiction outcome (as measured by comparing percentage of opiate and cocaine positive drug screens from admission to most recent). 2) Basic health screening, (as measured by the patient's compliance with primary care physicians (PCP) appointments and current smoking status). 3) Management of co-occurring medical conditions (as measured by levels of LDL cholesterol, hemoglobin A1c, and systolic blood pressure (SBP). 4) Presence of QTc prolongation (difference in QTc between baseline and most recent EKG). RESULTS: Illicit drug use (opiate and cocaine) markedly decreased in patients overall. The effect was more robust for those successfully "retained" (n=55, pCompliance with PCP appointments was high (82% and 88% before and after the onsite intervention, respectively) for "retained" patients. LDL cholesterol level was within normal range for all patients. A1c improved by 40% after the onsite intervention as reflected by the decreased percentage of patients with A1c > 7 % from before to after the intervention (90% vs. 50%, p=0.05). However, the prevalence of uncontrolled hypertension did not significantly improve after the onsite intervention (38% vs. 28%, p=0.34).   8. Baseline Substance Use Interferes with Maintenance of HIV Medication Adherence Skills.

    Cognitive behavioral therapy for adherence and depression (CBT-AD) in HIV improves both adherence and depression outcomes relative to enhanced treatment as usual (ETAU). However, in persons with injection drug use (PWIDU) histories, adherence gains seen during treatment have not been maintained postintervention. Therefore, we examined whether heroin or cocaine use at study entry moderated acquisition or maintenance of adherence gains after CBT-AD. HIV-positive adults in treatment for opioid dependence (n = 89) were randomly assigned to CBT-AD or ETAU and completed 3-, 6-, and 12-month assessments. Participants were majority male (61%), white (48%), and heterosexual (79%). Hierarchical linear modeling was used to evaluate whether heroin or cocaine use at baseline interacted with intervention assignment to predict change in adherence during active treatment and follow-up. CBT-AD-related improvement in adherence during the active intervention period did not vary by baseline substance use. However, cocaine use (but not heroin use) at baseline interacted with intervention assignment to predict a significant decline in follow-up adherence (cocaine use × group condition coefficient = -0.77, t = -2.44, p = 0.02) such that by 12 months, adherence among CBT participants was significantly lower among those who used cocaine (45.0%) compared to those who did not (72.3%; t = 2.50, p = 0.018). HIV-positive PWIDU who use heroin or cocaine at baseline can benefit from the CBT-AD intervention to improve adherence to antiretroviral therapy; therefore, providers should not withhold an active psychosocial treatment for HIV-positive PWIDU who are using. Cocaine use at baseline may limit the degree to which gains are maintained postintervention, and therefore, booster sessions may be needed. PMID:27158849

  11. 精神分裂症患者不典型抗精神病药治疗前后甲状腺激素的变化%Changes in thyroid functions of schizophrenic patients before and after treatment with non-typical anti-psychotic drugs

    Objective To study the effects of non-typical anti-psychotic drugs on thyroid functions of schizophrenic patients. Methods We recruited 30 healthy controls and 63 schizophrenic patients under the treatment of risperidone or olanzapine for 4 weeks. Their plasma levels of thyroid hormones (T3, T4 and TSH) were measured before and after treatment. Results The levels of plasma T3,T4 and TSH were lower in the schizophrenia group than in the control group, with T3 and T4 levels being significantly different (P<0. 05). Plasma T3 level increased significantly after risperidone treatment compared with that after olanzapine treatment (P<0.05). Before anti-psychotic drug treatment, patients with initial onset of schizophrenia had lower plasma T3 and T4 levels than the healthy controls (P<0.01). Conclusion Schizophrenic patients may have abnormal thyroid functions, which could be ameliorated by the treatment with risperidone.%目的 探讨不典型抗精神病药对精神分裂症患者甲状腺功能的影响.方法 对30例健康对照及63例用利培酮或奥氮平治疗4周的精神分裂症患者,检测治疗前后患者及健康对照的血浆T3、T4、促甲状腺素(TSH)水平.结果 精神分裂症患者血浆T3、T4、TSH水平较健康对照组低,T3、T4均有统计学差异(P<0.05).利培酮治疗后血浆T3水平明显升高,较奥氮平组差异有统计学意义(P<0.05).治疗前首发精神分裂症患者血浆T3、T4水平均显著低于健康对照组(P<0.05).结论 精神分裂症患者本身可能存在甲状腺功能异常.利培酮治疗有利于改善甲状腺功能.

Methods Through January 2012 to July 2014 in our hospital for treatment of 72 cases of patients with schizophrenia group, A group were treated with didgeridoo, Capitoline patients in group B, group C patients Implementation olanzapine treatment, choose the same period of 50 health examination staff. Comparison of serum thyroid hormone levels were observed difference between groups. Results Patients with thyroid hormone treatment and control group was not statistically significant (P> 0.05), in patients with T4, T3 level was significantly decreased after treatment with treatment, TSH levels were significantly increased, the difference was statistically significant (P <0.05). Conclusion The atypical anti psychotics may lead to changes in serum thyroid hormone levels in patients with schizophrenia, the clinical course of treatment for thyroid hormone levels should be regularly tested.%目的:探讨非典型抗精神病药物对女性精神分裂症患者甲状腺激素水平的影响。方法通过对2012年1月—2014年7月在该院进行治疗的72例精神分裂症患者进行分组,A组患者给予利培酮治疗,B组患者应用氯氮平治疗,C组患者实施奥氮平治疗,选取同期进行健康体检的50名人员。观察比较各组血清甲状腺激素水平差异。结果治疗前患者甲状腺激素与对照组比较差异无统计学意义(P>0.05),治疗后患者T4、T3水平同治疗前比较明显下降,TSH水平明显增加,差异具有统计学意义(P<0.05)。结论非典型抗精神病药物可导致精神分裂症患者血清甲状腺激素水平变化,在临床治疗过程中应定期对甲状腺激素水平进行检测。

Methods: Each trajectory file is decoded and a total of 131 axes positions are recorded (collimator jaw position, gantry angle, each MLC, etc.). This raw data is processed and either axis positions are extracted at critical points during the delivery or positional change over time is used to determine axis velocity. The focus of our analysis is the accuracy, reproducibility and fidelity of each axis. A reference positional trace of the gantry and each MLC is used as a motion baseline for cross correlation (CC) analysis. A total of 494 parameters (482 MLC related) were analyzed using Individual and Moving Range (I/MR) charts. The chart limits were calculated using a hybrid technique that included the use of the standard 3σ limits and parameter/system specifications. Synthetic errors/changes were introduced to determine the initial effectiveness of I/MR charts in detecting relevant changes in operating parameters. The magnitude of the synthetic errors/changes was based on: TG-142 and published analysis of VMAT delivery accuracy. Results: All errors introduced were detected. Synthetic positional errors of 2mm for collimator jaw and MLC carriage exceeded the chart limits. Gantry speed and each MLC speed are analyzed at two different points in the delivery. Simulated Gantry speed error (0.2 deg/sec) and MLC speed error (0.1 cm/sec) exceeded the speed chart limits. Gantry position error of 0.2 deg was detected by the CC maximum value charts. The MLC position error of 0.1 cm was detected by the CC maximum value location charts for every MLC. Conclusion: SPC I/MR evaluation of trajectory log file parameters may be effective in providing an early warning of performance degradation or component failure for medical accelerator systems

Results The MEMMS is convenient and flexible to operate. Compared with traditional management model, it only takes 0.5~1min to input the maintenance information.   20. Non-medical use of opioids among HIV-infected opioid dependent individuals on opioid maintenance treatment: the need for a more comprehensive approach

    Full Text Available Abstract Background Opioid maintenance treatment (OMT has a positive impact on substance use and health outcomes among HIV-infected opioid dependent patients. The present study investigates non-medical use of opioids by HIV-infected opioid-dependent individuals treated with buprenorphine or methadone. Methods The MANIF 2000 study is a longitudinal study that enrolled a cohort of 476 HIV-infected opioid-dependent individuals. Data were collected in outpatient hospital services delivering HIV care in France. The sample comprised all patients receiving OMT (either methadone or buprenorphine who attended at least one follow-up visit with data on adherence to OMT (N = 235 patients, 1056 visits. Non-medical use of opioids during OMT was defined as having reported use of opioids in a non-medical context, and/or the misuse of the prescribed oral OMT by an inappropriate route of administration (injection or sniffing. After adjusting for the non-random assignment of OMT type, a model based on GEE was then used to identify predictors of non-medical use of opioids. Results Among the 235 patients, 144 (61.3% and 91 (38.9% patients were receiving buprenorphine and methadone, respectively, at baseline. Non-medical use of opioids was found in 41.6% of visits for 83% of individual patients. In the multivariate analysis, predictors of non-medical use of opioids were: cocaine, daily cannabis, and benzodiazepine use, experience of opioid withdrawal symptoms, and less time since OMT initiation. Conclusions Non-medical use of opioids was found to be comparable in OMT patients receiving methadone or buprenorphine. The presence of opioid withdrawal symptoms was a determinant of non-medical use of opioids and may serve as a clinical indicator of inadequate dosage, medication, or type of follow-up. Sustainability and continuity of care with adequate monitoring of withdrawal symptoms and polydrug use may contribute to reduced harms from ongoing non-medical use of opioids.

    Fifty-five former opioid addicts who have been methadone maintained patients for 10 or more years and whose urine has tested negative for drugs for 2 or more years were compared to 99 former opioid addicts who have been medication-free for 10 or more years. Groups were comparable in age and education, but the medication-free subjects were younger when having started opioids with more severe addiction scores. Methadone maintained patients presented with a higher proportion of psychiatric comorbidity and chronic pain. Their scores of perceived sleep quality and cognitive state were poorer than the medication-free individuals. Possible explanations of the differences are discussed in this article. PMID:26284418

  2. Acquisition and maintenance of medical expertise: a perspective from the expert-performance approach with deliberate practice.

    As a part of a special collection in this issue of Academic Medicine, which is focused on mastery learning in medical education, this Perspective describes how the expert-performance approach with deliberate practice is consistent with many characteristics of mastery learning. Importantly, this Perspective also explains how the expert-performance approach provides a very different perspective on the acquisition of skill. Whereas traditional education with mastery learning focuses on having students attain an adequate level of performance that is based on goals set by the existing curricula, the expert-performance approach takes an empirical approach and first identifies the final goal of training-namely, reproducibly superior objective performance (superior patient outcomes) for individuals in particular medical specialties. Analyzing this superior complex performance reveals three types of mental representations that permit expert performers to plan, execute, and monitor their own performance. By reviewing research on medical performance and education, the author describes evidence for these representations and their development within the expert-performance framework. He uses the research to generate suggestions for improved training of medical students and professionals. Two strategies-designing learning environments with libraries of cases and creating opportunities for individualized teacher-guided training-should enable motivated individuals to acquire a full set of refined mental representations. Providing the right resources to support the expert-performance approach will allow such individuals to become self-regulated learners-that is, members of the medical community who have the tools to improve their own and their team members' performances throughout their entire professional careers.   3. Methadone Maintenance Treatment Program in Prisons from the Perspective of Medical and non-Medical Prison Staff: A Qualitative Study in Iran

    Full Text Available Background As one of the most important components of harm reduction strategy for high-risk groups, following the HIV epidemics, Methadone Maintenance Treatment (MMT has been initiated in prisoners since 2003. In this paper, we aimed to assess the advantages and shortcomings of the MMT program from the perspective of people who were involved with the delivery of prison healthcare in Iran. Methods On the basis of grounded theory and through conducting 14 Focus Group Discussions (FGDs, 7 FGDs among physicians, consultants, experts, and 7 FGDs among directors and managers of prisons (n= 140 have been performed. The respondents were asked about positive and negative elements of the MMT program in Iranian prisons. Results This study included a total of 48 themes, of which 22 themes were related to advantages and the other 26 were about shortcomings of MMT programs in the prisons. According to participants’ views “reduction of illegal drug use and high-risk injection”, “reduction of potentially high-risk behaviors” and “making positive attitudes” were the main advantages of MMT in prisons, while issues such as “inaccurate implementation”, “lack of skilled manpower” and “poor care after release from prison” were among the main shortcomings of MMT program. Conclusions MMT program in Iran’s prisons has achieved remarkable success in the field of harm reduction, but to obtain much more significant results, its shortcomings and weaknesses must be also taken into account by policy-makers.

  4. The lack of integration of clinical audit and the maintenance of medical dominance within British hospital trusts.

    Concerns have been expressed repeatedly about the effectiveness of clinical audit. Some have argued that this is limited by the lack of integration within day-to-day practice and with other NHS policy initiatives. We aimed to explore what mechanisms were being used to develop annual clinical audit programmes within NHS Trusts, and to describe the influence of other initiatives on this; to understand how such influences are exerted; and to understand the role of key players, in order to inform future programme development. Semi-structured face-to-face interviews were performed with Chairs of Clinical Audit Committees, Clinical Audit Managers and Co-ordinators (N = 15) in the former Yorkshire Region of the NHS in England. Concerns about the development, planning and integration of clinical audit focused upon an almost exclusive medical dominance and upon how audit leadership could be delivered within the context of hospital management structures. The lack of an overall plan for the development of clinical audit in most sites was seen as enabling the doctors' agenda to dominate. Purchasing authorities were recognized as being important, but often with limited influence. Other influences on the audit agenda, such as research and development (R&D) and clinical risk management, were rarely well co-ordinated. These findings concur with previous studies in identifying a wide range of constraints on the progress of audit. Several of these constraints operate within the internal environment, for example the doctors' agenda, and concerns about management involvement. Such constraints require resolution in order to facilitate the integration of audit with other initiatives and to achieve the goals of audit effectively. Increase of operating years and upgrade of capacity factor contributed to reduce generation cost and then aging measures for lifecycle extension of NPP was important as well as securing high capacity factor without failures or troubles of NPP with adequate maintenance action. Maintenance program was arranged to decide scope of inspection and repair and measures based on evaluated data. Use of risk information was recommended for reliable maintenance.     This final rule implements sections 9312(c)(2), 9312(f), and 9434(b) of Public Law 99-509, section 7 of Public Law 100-93, section 4014 of Public Law 100-203, sections 224 and 411(k)(12) of Public Law 100-360, and section 6411(d)(3) of Public Law 101-239. These provisions broaden the Secretary's authority to impose intermediate sanctions and civil money penalties on health maintenance organizations (HMOs), competitive medical plans, and other prepaid health plans contracting under Medicare or Medicaid that (1) substantially fail to provide an enrolled individual with required medically necessary items and services; (2) engage in certain marketing, enrollment, reporting, or claims payment abuses; or (3) in the case of Medicare risk-contracting plans, employ or contract with, either directly or indirectly, an individual or entity excluded from participation in Medicare. The provisions also condition Federal financial participation in certain State payments on the State's exclusion of certain prohibited entities from participation in HMO contracts and waiver programs. This final rule is intended to significantly enhance the protections for Medicare beneficiaries and Medicaid recipients enrolled in a HMO, competitive medical plan, or other contracting organization under titles XVIII and XIX of the Social Security Act.     This final rule creates an exception to the usual rule that TRICARE Prime enrollment fees are uniform for all retirees and their dependents and responds to public comments received to the proposed rule published in the Federal Register on June 7, 2013. Survivors of Active Duty Deceased Sponsors and Medically Retired Uniformed Services Members and their Dependents are part of the retiree group under TRICARE rules. In acknowledgment and appreciation of the sacrifices of these two beneficiary categories, the Secretary of Defense has elected to exercise his authority under the United States Code to exempt Active Duty Deceased Sponsors and Medically Retired Uniformed Services Members and their Dependents enrolled in TRICARE Prime from paying future increases to the TRICARE Prime annual enrollment fees. The Prime beneficiaries in these categories have made significant sacrifices for our country and are entitled to special recognition and benefits for their sacrifices. Therefore, the beneficiaries in these two TRICARE beneficiary categories who enrolled in TRICARE Prime prior to 10/1/2013, and those since that date, will have their annual enrollment fee frozen at the appropriate fiscal year rate: FY2011 rate $230 per single or $460 per family, FY2012 rate $260 or $520, FY2013 rate $269.38 or $538.56, or the FY2014 rate $273.84 or $547.68. The future beneficiaries added to these categories will have their fee frozen at the rate in effect at the time they are classified in either category and enroll in TRICARE Prime or, if not enrolling, at the rate in effect at the time of enrollment. The fee remains frozen as long as at least one family member remains enrolled in TRICARE Prime and there is not a break in enrollment. The fee charged for the dependent(s) of a Medically Retired Uniformed Services Member would not change if the dependent(s) was later re-classified a Survivor. PMID:25269153

  6. Did FDA Decisionmaking Affect Anti-Psychotic Drug Prescribing in Children?: A Time-Trend Analysis

    Background Following Food and Drug Administration (FDA) approval, many drugs are prescribed for non-FDA-approved (“off-label”) uses. If substantial evidence supports the efficacy and safety of off-label indications, manufacturers can pursue formal FDA approval through supplemental new drug applications (sNDAs). We evaluated the effect of FDA determinations on pediatric sNDAs for antipsychotic drugs on prescribing of these products in children. Methods Retrospective, segmented time-series analysis using new prescription claims during 2003–2012 for three atypical antipsychotics (olanzapine, quetiapine, ziprasidone). FDA approved the sNDAs for pediatric use of olanzapine and quetiapine in December 2009, but did not approve the sNDA for pediatric use of ziprasidone. Results During the months before FDA approval of its pediatric sNDA, new prescriptions of olanzapine decreased for both children and adults. After FDA approval, the increase in prescribing trends was similar for both age groups (P = 0.47 for schizophrenia and bipolar disorder; P = 0.37 for other indications). Comparable decreases in use of quetiapine were observed between pediatrics and adults following FDA approval of its pediatric sNDA (P = 0.88; P = 0.63). Prescribing of ziprasidone decreased similarly for pediatric and adult patients after FDA non-approval of its pediatric sNDA (P = 0.61; P = 0.79). Conclusions The FDA’s sNDA determinations relating to use of antipsychotics in children did not result in changes in use that favored the approved sNDAs and disfavored the unapproved sNDA. Improved communication may help translate the agency’s expert judgments to clinical practice. PMID:27032095

  7. Screening for metabolic syndrome in long-term psychiatric illness: Audit of patients receiving depot antipsychotic medication at a psychiatry clinic

    Full Text Available Background and Objectives: Metabolic syndrome (visceral obesity, dyslipidaemia, hyperglycaemia, hypertension is a substantial public health problem, especially amongst individuals receiving antipsychotic medication. Methods: We studied routine screening practices for metabolic syndrome amongst psychiatry outpatients receiving injected depot anti-psychotic medication at a clinic in Dublin, Ireland. Results: Our initial audit (n = 64 demonstrated variable levels of documentation of criteria for metabolic syndrome in outpatient files; e.g. weight was recorded in 1.6% of files, serum high density lipoprotein in 12.5%. As our intervention, we introduced a screening check-list comprising risk factors and criteria for metabolic syndrome, based on the definition of the International Diabetes Federation. Re-audit (n = 54 demonstrated significantly improved levels of documentation; e.g. weight was recorded in 61.1% of files. Notwithstanding these improvements, only 11 (20.4% of 54 patient files examined in the re-audit, contained sufficient information to determine whether or not the patient fulfilled criteria for metabolic syndrome; of these, 3 patients (27.3% fulfilled criteria for metabolic syndrome. There was, however, significant additional morbidity in relation to individual criteria (waist circumference, serum triglyceride level, systolic blood pressure and serum fasting glucose. Conclusions: We recommend enhanced attention be paid to metabolic morbidity in this patient group.

    Variant forms of RCM (Reliability Centered Maintenance) have been the maintenance optimizing tools of choice in industry for the last 20 years. Several such optimization techniques have been implemented at the Bruce Nuclear Station. Further cost refinement of the Station preventive maintenance strategy whereby decisions are based on statistical analysis of historical failure data are now being evaluated. The evaluation includes a requirement to demonstrate that earlier optimization projects have long term positive impacts. This proved to be a significant challenge. Based on operational experience, USA has customized the traditional RCM methodology into a streamlined lean logic path and has implemented the use of statistical tools to drive the process. Proactive maintenance should start at the design stage with establishing the functions, functional failures and failure modes. The understanding the failure behavior, especially the relationship between potential failures, functional failures and diagnostic parameters is the most important task. In the next step the designer have to explore the maintenance activities as they have to apply to the design criteria. The models and frameworks have been presented in the paper also a comprehensive review of literature and some results of laboratory investigations.

  3. Optimization potential in maintenance

    Also in the future, the supreme goal of nuclear power plant operation is going to be plant safety. At the same time, the economic component must be taken into account, which has become more important as a consequence of the deregulation in the electricity market. Until now, the electricity generating costs in nuclear power plants have been optimized by increases in electricity generation, and by high plant availabilities in terms of time and capacity. However, these capacity-based influences on the generating costs practically have been exploited to the full extent possible because of the high plant reliability reached. This makes the cost term the focal point in all optimization efforts, i.e. in existing nuclear power plants especially the maintenance costs. At the present high level of safety, which is going to develop even further, growing operating experience will allow a change to be made from preventive maintenance to cost-optimized state-oriented maintenance strategies. Other measures of advanced maintenance strategies are seen in the fields of work organization, personnel management, and personnel and plant use planning for more than one site. Important components in this effort are IT-based plant management systems allowing the complex problems to be planned, executed, controlled, and analyzed. The confidence that NPP structures, systems and components will function as designed is ultimately based on programmes which monitor both their reliability and availability to perform their intended safety function. Unfortunately, lack of concern towards building maintenance, especially preventive maintenance, happens in domestic housing industry in Malaysia. Majority of the maintenance operations of condominiums suffer from lack of planning, lack of proactive maintenance plan, and lack of proper implementation. Thus, this paper reviews the implementation of preventive maintenance strategy, routine maintenance in specific. An extensive review of literature published in 1987 to 2014 is performed for the purpose of this research. The publications are sourced from journal articles, conference proceedings and books. The literature analysis confirms that the routine maintenance of facilities and building services is vital and it can be influential towards the maintenance performance. Subsequently, a theoretical framework is developed, which shows the relationship between routine maintenance of building facilities & services and maintenance performance. The building facilities & services are divided into two categories. They are essential facilities & services that ensure the safety, health, habitability, and operability of buildings; while value-added facilities & services deal with property value, return on investment, and quality living of buildings. Based on the findings, a future research is proposed, which aims to identify the appropriate routine of maintenance for the facilities and services in high-rise residential buildings to improve the maintenance performance.

    Two methodologies for maintenance task selection, Reliability Centre Maintenance (RCM) and Degradation Mode Analysis (DMA), are compared with regard to application in the nuclear industry and potential for application at CANDU nuclear power plants. RCM is the favoured one of the two methodologies. It is more thorough than DMA, is well supported within the US nuclear industry, and - with experience in application - is gaining cost effectiveness. And in the unlikely event a TAS breakdown does occur, prompt and effective troubleshooting can reduce its impact on terminal productivity. To accommodate around-the-clock loading at racks, increasingly unattended by terminal personnel, TAS maintenance, servicing and troubleshooting has become increasingly demanding. It has also become increasingly important. After 15 years of trial and error at petroleum and petrochemical storage and transfer terminals, a number of successful troubleshooting programs have been developed.     The Medical Simulations Working Group conducted a series of medical simulations to evaluate the proposed Health Maintenance Facility (HMF) Preliminary Design Review (PDR) configuration. The goal of these simulations was to test the system effectiveness of the HMF PDR configurations. The objectives of the medical simulations are to (1) ensure fulfillment of requirements with this HMF design, (2) demonstrate the conformance of the system to human engineering design criteria, and (3) determine whether undesirable design or procedural features were introduced into the design. The simulations consisted of performing 6 different medical scenarios with the HMF mockup in the KRUG laboratory. The scenarios included representative medical procedures and used a broad spectrum of HMF equipment and supplies. Scripts were written and simulations performed by medical simulations working group members under observation from others. Data were collected by means of questionnaires, debriefings, and videotapes. Results were extracted and listed in the individual reports. Specific issues and recommendations from each simulation were compiled into the individual reports. General issues regarding the PDR design of the HMF are outlined in the summary report.

  4. Reducing maintenance costs

    The increasingly more expensive maintenance measures, cuts in the cost budget, and the loss of know-how on the part of vendors all require a change of policy with respect to maintenance concepts of the part of operators. This also applies to the existing valve concepts, the drives included. Under these aspects, the current drive, which is self-actuated and actuated by outside media, for a parallel-plate valve of a nomial width of 700 was reconsidered. The effort served to reduce maintenance costs and, at the same time, simplify the drive concept as well as cut back on the number of in-service inspections. Moreover, the number of active components were to be minimized and installation conditions in the plant were to be improved. When the boundary conditions to be observed with respect to process technology had been laid down, the competent technical department developed a concept of modification of the drive. A major constituent part was the demonstration of the functioning capability of the new drive under accident conditions. It was achieved mainly by an analytical approach. In the resultant drive concept, the same control valves are employed to actuate a driving cylinder by means of self-actuation or by an outside medium as a function of pressure. (orig.)

Method Divide 121 critical patients who meet the inclusion criteria into control group (n=61) and observation group (n=60) according to sequence of hospitalization.     Maintenance of Certification (MOC) recognizes that in addition to medical knowledge, several essential elements involved in delivering quality care must be developed and maintained throughout one's career. The MOC process is designed to facilitate and document professional development of American Board of Radiology (ABR) diplomates in the essential elements of quality care in Radiation Oncology and Radiologic Physics. ABR MOC has been developed in accord with guidelines of the American Board of Medical Specialties. All Radiation Oncology certificates issued since 1995 are 10-year, time-limited certificates; diplomates with time-limited certificates who wish to maintain specialty certification must complete specific requirements of the American Board of Radiology MOC program. Diplomates with lifelong certificates are not required to participate but are strongly encouraged to do so.     Traceability relations support stakeholders in understanding the dependencies between artifacts created during the development of a software system and thus enable many development-related tasks. To ensure that the anticipated benefits of these tasks can be realized, it is necessary to have an up-to-date set of traceability relations between the established artifacts. This goal requires the creation of traceability relations during the initial development process. Furthermore, the goal also requires the maintenance of traceability relations over time as the software system evolves in order to prevent their decay. In this paper, an approach is discussed that supports the (semi-) automated update of traceability relations between requirements, analysis and design models of software systems expressed in the UML. This is made possible by analyzing change events that have been captured while working within a third-party UML modeling tool. Within the captured flow of events, development activities comprised of several events are recognized. These are matched with predefined rules that direct the update of impacted traceability relations.     This document is intended to describe the work conducted by the ITER Assembly and Maintenance (A and M) Design Unit and the supporting home teams during the ITER Conceptual Design Activities, carried out from 1988 through 1990. Its content consists of two main sections, i.e., Chapter III, which describes the identified tasks to be performed by the A and M system and a general description of the required equipment; and Chapter IV, which provides a more detailed description of the equipment proposed to perform the assigned tasks. A two-stage R and D program is now planned, i.e., (1) a prototype equipment functional tests using full scale mock-ups and (2) a full scale integration demonstration test facility with real components (vacuum vessel with ports, blanket modules, divertor modules, armor tiles, etc.). Crucial in-vessel and ex-vessel operations and the associated remote handling equipment, including handling of divertor plates and blanket modules will be demonstrated in the first phase, whereby the database needed to proceed with the engineering phase will be acquired.     A remote maintenance monitoring system retrofits to a given hardware device with a sensor implant which gathers and captures failure data from the hardware device, without interfering with its operation. Failure data is continuously obtained from predetermined critical points within the hardware device, and is analyzed with a diagnostic expert system, which isolates failure origin to a particular component within the hardware device. For example, monitoring of a computer-based device may include monitoring of parity error data therefrom, as well as monitoring power supply fluctuations therein, so that parity error and power supply anomaly data may be used to trace the failure origin to a particular plane or power supply within the computer-based device. A plurality of sensor implants may be rerofit to corresponding plural devices comprising a distributed large-scale system. Transparent interface of the sensors to the devices precludes operative interference with the distributed network. Retrofit capability of the sensors permits monitoring of even older devices having no built-in testing technology. Continuous real time monitoring of a distributed network of such devices, coupled with diagnostic expert system analysis thereof, permits capture and analysis of even intermittent failures, thereby facilitating maintenance of the monitored large-scale system.

    CANDU units have long been recognized for their exceptional safety and reliability. Continuing development in the maintenance area has played a key role in achieving this performance level. For over two decades, safety system availability has been monitored closely and system maintenance programs adjusted accordingly to maintain high levels of performance. But as the plants approach mid life in a more competitive environment and component aging becomes a concern, new methods and techniques are necessary. As a result, recent developments are moving the maintenance program largely from a corrective and preventive approach to predictive and condition based maintenance. The application of these techniques is also being extended to safety related systems. These recent developments include use of reliability centred methods to define system maintenance requirements and strategies. This approach has been implemented on a number of systems at Canadian CANDU plants with positive results. The pilot projects demonstrated that the overall maintenance effort remained relatively constant while the system performance improved. It was also possible to schedule some of the redundant component maintenance during plant operation without adverse impact on system availability. The probabilistic safety assessment was found to be useful in determining the safety implications of component outages. These new maintenance strategies are now making use of predictive and condition based maintenance techniques to anticipate equipment breakdown and schedule preventive maintenance as the need arises rather than time based. Some of these techniques include valve diagnostics, vibration monitoring, oil analysis, thermography. Of course, these tools and techniques must form part of an overall maintenance management system to ensure that maintenance becomes a living program. The aim of this paper is to assess the current practice of maintenance process and management in public hospitals buildings in the Gaza Strip. A questionnaire survey that distributed to 13 public hospitals, which are administered by the Ministry of Health (MoH in the Gaza Strip, was used to collect the primary data for this study. The results of this study present an overview of the current situation of the maintenance process in public hospitals buildings in the Gaza Strip. The findings indicated that while the corrective maintenance is implemented in all the 13 public hospitals, preventive maintenance is employed along with corrective maintenance, only in three hospitals. In addition, the findings indicated variances in responding to maintenance requests, while 50% of the maintenance departments took few hours to respond to maintenance requests, the rest took a few days to respond, this can be explained due to lack of spare parts and qualified staff. The study showed also that there is a shortage in training the hospital facility’s users on how to report maintenance problems. The results of this study indicated that most hospitals in the Gaza Strip have no maintenance plan for medical equipment; they do not have quality control system for repair and preventive maintenance. It is recommended to employ experience maintenance staff in order to prepare adequate maintenance plan and detailed check list, which is required for preventive maintenance. In other words, Software maintenance can be defined as; it is the process of providing services to the customers after the delivery of the software. Despite the fact that maintaining software is very challenging, it is the most important routine that must be carried out in the development cycle. If the software is not maintained efficiently it may lead to the death of the software. Maintenance of software may be carried out in two ways. The first one is called „In-house maintenance‟ and the second one is called „Transition maintenance‟. The latter faces the drastic challenges when compared to former, as one team may not provide complete source code to the other, leading to unstructured code, lack of appropriate technique and knowledge about the functioning of the current software. There are a few aspects of software maintenance that set it apart from the other phases. Software maintenance cost comprises more than half of the total software development cost. Also, without software maintenance, it is impossible to change the problems within the product after its release, and many disasters can happen because of immature software. Recognising the importance of software maintenance, this paper proposes a model called “ENTRETIAN MODEL” (Entretian, a French word meaning Maintenance which consists of six basic steps to follow while maintaining software system. This model overcomes certain misconceptions about maintenance phase and it is highly beneficial to the Maintenance Support Team (MST to handle their maintenance activities systematically and efficiently. By employing the proposed model, the MST is able to overcome the technical and managerial issues that are faced earlier in the maintenance phase. An early intervention may be uselessly costly, but a late one may lead to a partial/complete failure of the system, which has to be avoided. To achieve this aim, we model the system by a stochastic hybrid process and propose a numerical method to optimize the maintenance time for this kind of processes.     Living human cells require attachment to a suitable surface and special culture conditions in order to grow. These requirements are modified and amplified when cells are taken into a weightless environment. Special handling and maintenance systems are required for routine laboratory procedures conducted in the Orbiter and in the Spacelab. Methods were developed to maintain cells in special incubators designed for the Orbiter middeck, however, electrophoresis and other experiments require cells to be harvested off of the culture substrate before they can be processed or used. The cell transport assembly (CTA) was flown on STS-8, and results show that improvements are required to maintain adequate numbers of cells in this device longer than 48 hours. The life sciences middeck centrifuge probably can be used, but modifications will be required to transfer cells from the CTA and keep the cells sterile. Automated systems such as the Skylab SO-15 flight hardware and crew operated systems are being evaluated for use on the Space Shuttle, Spacelab, and Space Station research modules.

All models presented so far have been in use in Air Forces, so that the advantages and disadvantages of different models are known. First it shows the current model of aircraft maintenance as well as its basic characteristics. Then the paper discusses two organizational models of aircraft maintenance with their advantages and disadvantages. The advantages and disadvantages of different models are analyzed based on the criteria of operational capabilities of military units. In addition to operational capabilities, the paper presents some other criteria which should be taken into account in the evaluation and selection of an optimal model of aircraft maintenance. Performing a qualitative analysis of some models may not be sufficient for evaluating the optimum choice for models of maintenance referring to the selected set of criteria from the scope of operational capabilities. In order to choose the optimum model, it is necessary to conduct a detailed economic and technical analysis of individual tactical model maintenance. A high-quality aircraft maintenance organization requires the highest state and army authorities to be involved. It is necessary to set clear objectives for all the elements of modern air force technical support programs based on the given evaluation criteria.

    Some trials have demonstrated the benefits of maintenance chemotherapy for advanced colorectal cancer. In chemotherapeutic strategies for advanced colorectal cancer, chemotherapy-related toxicity prevention and quality of life(QOL)maintenance are more important than the introduction of a strong regimen, especially when additional surgery is not possible. In Japan, the combination of a folinic acid/5-fluorouracil/oxaliplatin(FOLFOX)regimen and bevacizumab is a popular first-line chemotherapy regimen. However, despite its effectiveness, neuropathy or hand-foot syndrome after 5 or 6 cycles tends to lead to chemotherapy withdrawal. CAIRO3 trial reported the effectiveness of capecitabine and bevacizumab as a maintenance chemotherapy regimen. Additionally, the ML18147 trial demonstrated that bevacizumab beyond progression(BBP)prolonged overall survival(OS)and progression free survival(PFS)in patients with advanced colorectal cancer. Although those trials demonstrated the effectiveness of continuous or maintenance bevacizumab administration, no trials have compared the effectiveness of cytotoxic drugs with bevacizumab as maintenance therapies.     Maintenance action of nuclear power plant (NPP) was described. Maintenance of NPP aimed at assurance of required function of NPP's equipment so as to prevent release of radioactive materials into the environment as well as attainment of stable operation of NPP. Philosophy of NPP safety was based on defense-in-depth or multiple barriers requiring specified function for the equipment. Preventive maintenance was essential to NPP's equipment and the scope of maintenance was decided on priority with adequate method and frequency of inspection. Most inspection was conducted during periodic inspection at outage. Repair or improvement works were performed if needed. Periodic inspection period was very long and then capacity factor of NPP was low in Japan compared with foreign data although frequency of unscheduled shutdown was very low.     The objective of this research was to develop a decision support system (DSS) to study the impact of introducing new equipment into a medical apparel plant from a maintenance organizational structure perspective. This system will enable the company to determine if their capacity is sufficient to meet current maintenance challenges. The DSS contains two database sets that describe equipment and maintenance resource profiles. The equipment profile specifies data such as mean time to failures, mean time to repairs, and minimum mechanic skill level required to fix each machine group. Similarly, maintenance-resource profile reports information about the mechanic staff, such as number and type of certifications received, education level, and experience. The DSS will then use this information to minimize machine downtime by assigning the highest skilled mechanics to machines with higher complexity and product value. A modified version of the simplex method, the transportation problem, was used to perform the optimization. The DSS was built using the Visual Basic for Applications (VBA) language contained in the Microsoft Excel environment. A case study was developed from current existing data. The analysis consisted of forty-two machine groups and six mechanic categories with ten skill levels. Results showed that only 56% of the mechanic workforce was utilized. Thus, the company had available resources for meeting future maintenance requirements.

Thus, Safety and casks conformity to the according certificate of approval are guaranteed. Nonetheless, based on experience it appeared that some maintenance operations did not seem relevant or were redundant. Then, it was decided to rethink completely our maintenance program to reach the following objectives: - Set up the 'a minima' required inspection operations required to guarantee Safety and conformity to the certificate of approval, - Optimize criteria and periodicities of inspections taking into account: operational feedback, routine inspections carried out for each transport, regulations, environmental impact (ALARA, waste reduction,...), cost-effectiveness (reduction of cask's immobilization period,...). - Set up a maintenance program in Safety Analysis Reports that: stands alone (no need to check the specification or the certificate of approval to have the complete list of inspections mandatory to guarantee Safety), gives objectives instead of means of controls. This approach needs then to be re-evaluated by the competent Authority. Study's scope has been limited to the TNTM 12 cask family which is intensely used. COGEMA LOGISTICS has a high operational feedback on these casks.     The systematic analysis and documentation of operating experiences should be included in a living NPP life management program. Failure mode and effects and maintenance effects analyses are suitable methods for analysis of the failure and corrective maintenance experiences of equipment. Combined use of the information on occurred functional failures and the decision tree logic of the reliability centered maintenance identifies applicable and effective preventive maintenance tasks of equipment in an old plant. In this study the electrical motor drives of closing and isolation valves (MOV) of TVO and Loviisa nuclear power plants were selected to serve as pilot study objects. The study was limited to valve drives having actuators manufactured by AUMA in Germany. The fault and maintenance history of MOVs from 1981 up to and including October 1991 in different safety and process systems at TVO 1 and 2 nuclear power units was at first analyzed in a systematic way. The scope of the components studied was 81 MOVs in safety-related systems and 127 other MOVs per each TVO unit. In the case of the Loviisa plant, the observation period was limited to three years, i.e. from February 1989 up to February 1992.   11. TPX remote maintenance and shielding

    The Tokamak Physics Experiment machine design incorporates comprehensive planning for efficient and safe component maintenance. Three programmatic decisions have been made to insure the successful implementation of this objective. First, the tokamak incorporates radiation shielding to reduce activation of components and limit the dose rate to personnel working on the outside of the machine. This allows most of the ex-vessel equipment to be maintained through conventional ''hands-on'' procedures. Second, to the maximum extent possible, low activation materials will be used inside the shielding volume. This resulted in the selection of Titanium (Ti-6Al-4V) for the vacuum vessel and PFC structures. The third decision stipulated that the primary in-vessel components will be replaced or repaired via remote maintenance tools specifically provided for the task. The component designers have been given the responsibility of incorporating maintenance design and for proving the maintainability of the design concepts in full-scale mockup tests prior to the initiation of final fabrication. Remote maintenance of the TPX machine is facilitated by general purpose tools provided by a special purpose design team.   12. TPX remote maintenance and shielding

    The Tokamak Physics Experiment (TPX) machine design incorporates comprehensive planning for efficient and safe component maintenance. Three programmatic decisions have been made to insure the successful implementation of this objective. First, the tokamak incorporates radiation shielding to reduce activation of components and limit the dose rate to personnel working on the outside of the machine. This allows most of the ex-vessel equipment to be maintained through conventional open-quotes hands-onclose quotes procedures. Second, to the maximum extent possible, low activation materials will be used inside the shielding volume. This resulted in the selection of Titanium (Ti-6Al-4V) for the vacuum vessel and Plasma Facing Components (PFC) structures. The third decision stipulated that the primary in-vessel components will be replaced or repaired via remote maintenance tools specifically provided for the task. The component designers have been given the responsibility of incorporating maintenance design and for proving the maintainability of the design concepts in full-scale mockup tests prior to the initiation of final fabrication.     A series of studies have been conducted to analyze failures that have been experienced by most power distribution companies in the Czech Republic and in one of the Slovak Republics. The purpose was to find ways to optimize the maintenance of distribution network devices. Data was compiled to enable a comparison of results and to create a statistically more important database. Since the number of failures in the area of electrical power engineering have been rather small, the results on element reliability will only be available in several more years to come. The main challenge with reliability analysis is to find reliable and updated input data. As such, the primary task is to change the existing structure of databases of power distribution companies. These databases must be adjusted to get the input data for the calculation functions of reliability centred maintenance (RCM). This paper described the programs designed for analyses of reliability indices and the optimization of maintenance of equipment of the distribution system that will provide basic data for responsible and logical decisions regarding maintenance and basic data for the preparation of an effective maintenance schedule and the creation of a feedback system. 7 refs., 4 figs.

    Health maintenance (HM) practices are essential to prevent illness, promote well-being, and maximize health. Patients with multiple myeloma (MM) are at increased risk for cardiovascular disease and cancers, yet, research on HM practices and preventative care of MM survivors has limited report. The study comprised a descriptive, correlational, and cross-sectional online survey design. Survey of patients with MM was carried out through the International Myeloma Foundation (IMF) and the Association of Cancer Online Resources (ACOR) e-mail list services. The members of the IMF and ACOR e-mail list services were surveyed, of which 237 patients responded. The modified Medical Expenditure Preventive Survey-Preventive Care questionnaire was used; it included items that ask patients regarding their healthcare practices that relate to dental care, cancer prevention, addiction, lifestyles, sensory screening, immunizations, cardiovascular, endocrine, psychosocial, and bone health. Descriptive statistics, Pearson's chi-square, and Spearman's rho correlation coefficient were obtained. In this study, men had statistically significant inferior global health maintenance scores than women (P = 0.002). Being employed (P = 0.054) and married or partnered (P = 0.017) were significantly correlated with better health maintenance patterns among male respondents. In contrast, no statistically significant correlations between sociodemographic factors and health maintenance patterns were found in women. Patients with MM, particularly men, require continued education and close monitoring of health maintenance practices. These findings are consistent with publications looking at gender disparities in healthcare utilization in the United States. Studies show that men, in general, are less likely to seek preventative healthcare screenings. Healthcare providers must incorporate health maintenance promotion during clinic visits. PMID:27119422

    Full Text Available Factors associated with physical well being were examined in adults with end-stage renal disease (ESRD in two large hemodialysis units of the Royal Medical Services in Jordan. Utilizing the Karnofski scale we measured the functional status of 200 Patients who had been on maintenance hemodialysis for at least 12 months. A Marnofski scale of less than 70 incidents frank disability (Inability to perform routine living activities without assistance, in addition current vocational status was assessed as well as any existing comorbid conditions. The mean age of the study group was 45.2 years (range 16 to 70 and included 108 (54% males and 92 (46% females, there were 39 (19.5% diabetic patients and 27 (13.5% patients were receiving erythropoietin (EPO. The mean hematocrit of the entire group was 27.8%. As measured by Karnofski scale, 64 (32% of the patients were unable to perform routine living activity without assistance; dependence on wheelchair was reported by 9 (4.5% patients. The mean comorbidity index of patients who scored less than 70 on the Karnofski scale was 1.5 compared to 0.7 for those who scored at least 70 on the same scale (p< 0.001. Analysis of factors showed that age and diabetes mellitus affected functional status. Of the laboratory variables measured, only serum albumin concentration correlated significant with Karnofski scale. Fourteen (21.8% of the patients who scored below 70, had serum albumin concentration above 40g/L compared to 66 (48.5% of the patients who scored at least 70 on the Karnofski scale (p< 0.001. We conclude that a significant proportion of patients on maintenance hemodialysis is functionally disabled. The elderly, diabetics, patients with high co-morbidity index and those with low serum albumin are most likely to have poor functional status.

    Maintenance optimization process in nuclear power plants (NPPs) has, in recent decades, been driving the transition of maintenance strategies from time-based preventive maintenance to systematic strategies depending on both equipment condition and reliability analysis. Starting with a comparison of various types of maintenance strategies in current NPPs, this paper highlights the evident advantages of condition-based maintenance (CBM) in both technical feasibility and cost-effectiveness. This paper also expounds the theoretical foundation and application methodology of CBM, as well as the complementary relationship between CBM and RCM. Taking Daya Bay nuclear power base as a case example, we introduce in detail the experience of using Reliability-centered Maintenance (RCM) and CBM to drive maintenance improvement in NPPs, which serves as a cornerstone for the application of RCM analysis-based CBM maintenance in other plants.     Safety is the most important issue considered by ETRR-2 management during the reactor operation and utilization. Among others, effective maintenance is one of the most important specific activities to achieve safety. The ETRR-2 Maintenance program and inspection plan is adopted to ensure that the system, subsystem and components continue to operate as desired with the capability to meet the design objectives. Moreover, it is also to ensure that the reactor safety is not affected by subsequent operations. The ETRR-2 Manager has the overall responsibility for all aspects on maintenance while the maintenance head has the direct responsibility. ETRR-2 maintenance group is divided into four units: mechanical maintenance unit, electrical maintenance unit, electronics maintenance unit, and chemical maintenance unit. The intended safety function of every reactor component and system is checked on daily, weekly, monthly, semiannual and annual bases by means of preventive and predictive maintenance program and inspection test plan. Safety and safety related components are given high priority. Deficiencies thus detected are corrected in time. Maintenance activities are carried out in accordance with written and approved procedures. The maintenance procedures are developed by ETRR-2 maintenance personal in cooperation with system designers and based upon the manufacturer recommendations and consideration of the experience gained from similar facilities. Maintenance personnel are kept well trained on all maintenance activities. Documents that define the maintenance activities and associated historical data are subject to managerial control within the QA program for ETRR-2 operation.     The medical physicist is a professional who specializes in the application of the concepts and methods of physics to the diagnosis and treatment of human disease. Medical physicists identify their primary discipline to be radiation therapy (78%), medical imaging (16%), nuclear medicine (3%), or radiation safety (2%). They state their primary responsibility to be clinical (78%), academic (9%), research (4%), etc. Correspondingly, medical physicists reveal their primarily employment to be a private hospital (42%), university hospital (32%), physicist's service group (9%), physician's service group (9%), industry (5%), and government (3%). The most frequent job of medical physicists is clinical radiation therapy physicist, whose clinical duties include: equipment acquisition, facility design, commissioning, machine maintenance, calibration and quality assurance, patient treatment planning, patient dose calculation, management of patient procedures, development of new technology, radiation safety, and regulatory compliance. The number of medical physicists in the United States can be estimated by the number of members of the American Association of Physicists in Medicine (AAPM), which has increased 5.5% annually since 1969, currently being 5,000. New positions plus retirements create a current need >300 new medical physicists per year, which exceeds supply. This is supported by the steady growth in average salaries, being 100,000 for PhDs entering the field and reaching 180,000. Graduate programs alone cannot meet demand, and physicists entering the field through postdoctoral training in medical physics remain important. Details of postdoctoral research programs and medical physics residency programs will provide direction to physics PhD graduates interested in medical physics.   6. Maintenance of nuclear medicine instruments

    Maintenance of instruments is generally of two kinds: (a) corrective maintenance, on a non-scheduled basis, to restore equipment to a functional status by repairs; (b) preventive maintenance, to keep equipment in a specified functional condition by providing systematic inspection, quality control, detection and correction of early malfunctions. Most of the instruments used in nuclear medicine are rather complex systems built from mechanical, electrical and electronic parts. Any one of these components is liable to fail at some time or other. Repair could be done only by a specialist who is able to evaluate the condition of the various parts ranging from cables to connectors, from scintillators to photomultipliers, from microprocessors to microswitches. The knowledge of the intricacies of the various electronic components required for their repairs is quite wide and varied. The electronics industry turns out more and more multi-purpose chips which can carry out the functions of many parts used in the instruments of the earlier generation. This provides protection against unauthorized copying of the circuits but it serves another purpose as well of inhibiting repairs by non-factory personnel. These trends of the instrument design should be taken into consideration when a policy has to be developed for the repairs of the hospital based equipment

  7. Demonstration of reliability centered maintenance

    Reliability centered maintenance (RCM) is an approach to preventive maintenance planning and evaluation that has been used successfully by other industries, most notably the airlines and military. Now EPRI is demonstrating RCM in the commercial nuclear power industry. Just completed are large-scale, two-year demonstrations at Rochester Gas ampersand Electric (Ginna Nuclear Power Station) and Southern California Edison (San Onofre Nuclear Generating Station). Both demonstrations were begun in the spring of 1988. At each plant, RCM was performed on 12 to 21 major systems. Both demonstrations determined that RCM is an appropriate means to optimize a PM program and improve nuclear plant preventive maintenance on a large scale. Such favorable results had been suggested by three earlier EPRI pilot studies at Florida Power ampersand Light, Duke Power, and Southern California Edison. EPRI selected the Ginna and San Onofre sites because, together, they represent a broad range of utility and plant size, plant organization, plant age, and histories of availability and reliability. Significant steps in each demonstration included: selecting and prioritizing plant systems for RCM evaluation; performing the RCM evaluation steps on selected systems; evaluating the RCM recommendations by a multi-disciplinary task force; implementing the RCM recommendations; establishing a system to track and verify the RCM benefits; and establishing procedures to update the RCM bases and recommendations with time (a living program). 7 refs., 1 tab

  8. Demonstration of reliability centered maintenance

    Southern California Edison Company (SCE) has completed a two year demonstration of Reliability Centered Maintenance (RCM) at San Onofre Nuclear Generating Station (SONGS) Unit 2. The demonstration involved the application of detailed RCM analysis to 12 plant systems and the implementation of the preventive maintenance (PM) and testing recommendations from these analyses. Other tasks in support of the successful application and implementation included (1) the selection and prioritization of systems for RCM analysis, (2) employing RCM as the technical basis for technical specification change, and (3) development of an RCM living program. The overall objective of the project is to demonstrate that RCM can be cost-effectively performed in a nuclear plant environment. This report describes in detail the methodologies which evolved at SONGS to perform the above tasks. This report summarizes the recommendations from the analyses which include task deletions, task additions, task modifications, and design changes. A significant portion of these recommendations have been evaluated at SONGS, and the status of task implementation is described herein. Significant maintenance man-hour savings are documented on systems for which the recommendations have been evaluated and implemented. Insights of value for future RCM applications are enumerated.
