    In the Scandinavian countries few regulations govern hospital infection control. In Sweden a common procedure manual is used nationwide, consisting of guidelines covering a wide range of nursing and medical procedures performed by the nursing staff. It is revised every fifth year. A recent enquiry to over 150 wards in some 100 hospitals demonstrated that the manual is widely accepted and used.     Substantial advances have been made in our understanding of the movement of species, including processes such as dispersal and migration. This knowledge has the potential to improve decisions about biodiversity policy and management, but it can be difficult for decision makers to readily access and integrate the growing body of movement science. This is, in part, due to a lack of synthesis of information that is sufficiently contextualized for a policy audience. Here, we identify key species movement concepts, including mechanisms, types, and moderators of movement, and review their relevance to (1) national biodiversity policies and strategies, (2) reserve planning and management, (3) threatened species protection and recovery, (4) impact and risk assessments, and (5) the prioritization of restoration actions. Based on the review, and considering recent developments in movement ecology, we provide a new framework that draws links between aspects of movement knowledge that are likely the most relevant to each biodiversity policy category. Our framework also shows that there is substantial opportunity for collaboration between researchers and government decision makers in the use of movement science to promote positive biodiversity outcomes.

In an attempt to increase our understanding of the great dissonance between the policy intention and the experiences of the participants in concrete PMTCT programmes, we will in these concluding remarks draw upon writings in institutional ethnography. Through the concept of 'global texts' we reflect upon the scientific and ideological underpinnings of the WHO policy guidelines on HIV and infant feeding, and the influence that this policy has had across multiple local settings. The particular impact of the global postnatal PMTCT policy guidelines on the position of breastfeeding lies at the core of the discussion.

    This paper analyses a problem, which exist not only in Lithuania but also in most Central and East European countries. Energy intensity is at such high level that it causes financial difficulties nearly in all branches of energy. Resilient interaction between energy sector and economics define the fact that disproportions between energy sector and economics hinder the development of economics. Analysis focusing on proportions between various energy sectors and economies was carried out. These proportions expressed in a set of well-known energy-economic indexes in comparison with similar proportions of other countries revealed the primary reasons of the financial difficulties in energy sector, as well its macroeconomic impact. Concept of state energy conservation policy is presented which can evaluate links between energy and economics according to its proportions. Conditions for successful mastering of renewable energy resources are analysed and state operation mechanism is proposed.

    The United Kingdom's CD4 surveillance scheme monitors CD4 cell counts among HIV patients and is a national resource for HIV surveillance. It has driven public health policy and allowed auditing of national HIV testing, treatment and care guidelines. WE DEMONSTRATE ITS UTILITY THROUGH FOUR EXAMPLE OUTPUTS: median CD4 count at HIV diagnosis; late HIV diagnosis and short-term mortality; the timing of first CD4 count to indicate entry into HIV care; and the proportion of patients with CD4 counts <350 cells/mm3 receiving anti-retroviral therapy (ARV). In 2009, 95% (61,502/64,420) of adults living with diagnosed HIV infection had CD4 counts available. The median CD4 count at diagnosis increased from 276 to 335 cells/mm3 between 2000 and 2009, indicating modest improvements in HIV testing. In 2009, 52% of patients were diagnosed at a late stage of HIV infection (CD4 <350 cells/mm(3)); these individuals had a ten-fold risk of dying within a year of their diagnosis compared to those diagnosed promptly. In 2008, the national target of performing a CD4 count within 14 days of diagnosis was met for 61% of patients. National treatment guidelines have largely been met with 83% patients with CD4 <350 cells/mm(3) receiving ARV. The monitoring of CD4 counts is critical to HIV surveillance in the United Kingdom enabling the close monitoring of efforts to reduce morbidity and mortality associated with late diagnosis and underpins the auditing of policies and guidelines. These routine surveillance outputs can be generated at national and local levels to drive and monitor public health policy and prevention efforts. PMID:23049663

  7. Advanced light source vacuum policy and vacuum guidelines for beamlines and experiment endstations

    The purpose of this document is to: (1) Explain the ALS vacuum policy and specifications for beamlines and experiment endstations. (2) Provide guidelines related to ALS vacuum policy to assist in designing beamlines which are in accordance with ALS vacuum policy. This document supersedes LSBL-116. The Advanced Light Source is a third generation synchrotron radiation source whose beam lifetime depends on the quality of the vacuum in the storage ring and the connecting beamlines. The storage ring and most of the beamlines share a common vacuum and are operated under ultra-high-vacuum (UHV) conditions. All endstations and beamline equipment must be operated so as to avoid contamination of beamline components, and must include proper safeguards to protect the storage ring vacuum from an accidental break in the beamline or endstation vacuum systems. The primary gas load during operation is due to thermal desorption and electron/photon induced desorption of contaminants from the interior of the vacuum vessel and its components. The desorption rates are considerably higher for hydrocarbon contamination, thus considerable emphasis is placed on eliminating these sources of contaminants. All vacuum components in a beamline and endstation must meet the ALS vacuum specifications. The vacuum design of both beamlines and endstations must be approved by the ALS Beamline Review Committee (BRC) before vacuum connections to the storage ring are made. The vacuum design is first checked during the Beamline Design Review (BDR) held before construction of the beamline equipment begins. Any deviation from the ALS vacuum specifications must be approved by the BRC prior to installation of the equipment on the ALS floor. Any modification that is incorporated into a vacuum assembly without the written approval of the BRC is done at the user's risk and may lead to rejection of the whole assembly

These challenges range from the availability and allocation of resources to support the complicated processes involved in developing and updating these policies to the lack of a clearly defined process that includes identification of tasks, assignments of responsibilities, and so forth. For this reason, the APA Board of Directors asked the Policy and Planning Board (P&P) to work with the boards of the four directorates (i.e., the Education Directorate, the Practice Directorate, the Public Interest Directorate, and the Science Directorate) to create a proposal that would (a) make recommendations with regard to how the review and revision of guidelines documents might best be accomplished in accordance with the APA Strategic Plan and existing policies and procedures and (b) make suggestions regarding the association resources that might be required.     Full Text Available Most studies have reported barriers to guideline usage mainly from doctors' perspective; few have reported the perspective of other stakeholders. This study aimed to determine the views and barriers to adherence of a national clinical practice guideline (CPG on management of hypertension from the perspectives of policymakers, doctors and allied healthcare professionals.This study used a qualitative approach with purposive sampling. Seven in depth interviews and six focus group discussions were conducted with 35 healthcare professionals (policy makers, doctors, pharmacists and nurses at a teaching hospital in Kuala Lumpur, Malaysia, between February and June 2013. All interviews were audio-recorded, transcribed verbatim and checked. Thematic approach was used to analyse the data.Two main themes and three sub-themes emerged from this study. The main themes were (1 variation in the use of CPG and (2 barriers to adherence to CPG. The three sub-themes for barriers were issues inherent to the CPG, systems and policy that is not supportive of CPG use, and attitudes and behaviour of stakeholders. The main users of the CPG were the primary care doctors. Pharmacists only partially use the guidelines, while nurses and policy makers were not using the CPG at all. Participants had suggested few strategies to improve usage and adherence to CPG. First, update the CPG regularly and keep its content simple with specific sections for allied health workers. Second, use technology to facilitate CPG accessibility and provide protected time for implementation of CPG recommendations. Third, incorporate local CPG in professional training, link CPG adherence to key performance indicators and provide incentives for its use.Barriers to the use of CPG hypertension management span across all stakeholders.     Data publication is often proposed as a method for ensuring the proper management and curation of research data, while at the same time rewarding the data creator for their efforts in doing so. It takes advantage of the processes already in place to publish research in academic journals, but needs to build on these to take into account the fact that data are vastly more varied in structure, size and complexity than scholarly articles. Peer REview for Publication & Accreditation of Research Data in the Earth sciences (PREPARDE) was a JISC- and NERC-funded project which investigated the policies and procedures required for the formal publication of research data, using as a particular case study connections with, and papers in, Geoscience Data Journal. The focus of the project was in producing outputs that would be of direct use to the editors of Geoscience Data Journal, and which would be generalisable to other scientific domains beyond the geosciences. Several of these outputs have since been taken on and developed further by the Publishing Data Interest Group of the Research Data Alliance, along with other journals such as F1000Research and Scientific Data. In this presentation we will describe key results and conclusions from the PREPARDE project, including guidelines for recommending data repositories as partners in data publication, guidance for reviewers of Geoscience Data Journal who are reviewing datasets as well as data papers, and recommendations for standardisation of cross-linking methods to ensure that the link between the dataset and its citing paper does not become broken. The sustainability of data journals in the stable of other academic journals will also be addressed.   11. The evolution of health policy guidelines for assisted reproduction in the Republic of Ireland, 2004-2009

    Abstract This analysis reports on Irish regulatory policies for in vitro fertilisation (IVF) from 2004-2009, in the context of membership changes within the Medical Council of Ireland. To achieve this, the current (2009) edition of the Guide to Professional Conduct & Ethics was compared with the immediately preceding version (2004). The statutory composition of the Medical Council from 2004-2009 was also studied. Content analysis of the two editions identified the following differences: 1) The 2004 guide states that IVF "should only be used after thorough investigation has failed to reveal a treatable cause of the infertility", while the 2009 guide indicates IVF "should only be used after thorough investigation has shown that no other treatment is likely to be effective"; 2) The 2004 stipulation stating that fertilized ovum (embryo) "must be used for normal implantation and must not be deliberately destroyed" is absent from the 2009 guidelines; 3) The option to donate "unused fertilised ova" (embryos) is omitted from the 2009 guidelines; 4) The 2009 guidelines state that ART should be offered only by "suitably qualified professionals, in appropriate facilities, and according to the international best practice"; 5) The 2009 guidelines introduce criteria that donations as part of a donor programme should be "altruistic and non-commercial". These last two points represent original regulatory efforts not appearing in the 2004 edition. The Medical Practitioners Act 2007 reduced the number of physicians on the Medical Council to 6 (of 25) members. The ethical guidelines from 2004 preceded this change, while the reconstituted Medical Council published the 2009 version. Between 2004 and 2009, substantial modifications in reproductive health policy were incorporated into the Medical Council\\'s ethical guidelines. For control of childhood TB in Pakistan, NTP developed and piloted its guidelines in 2006-2007. The objective of this study was to compare the documented case management practices of pediatricians and its impact on the outcome before and after introducing NTP policy guidelines. Findings An audit of case management practices of a historical cohort study was done in children below 15 years who were put on anti-tuberculosis treatment at all nine public hospitals in three districts in province of Punjab. The study period was two years pre-intervention (2004-05 and two years post-intervention (2006-07 after implementation of new NTP policy guidelines for childhood TB. There were 920 childhood TB cases registered during four years, 189 in pre-intervention period and 731 in post-intervention period. The practices changed significantly in post-intervention period for use of tuberculin skin test (63% of pulmonary cases, 19% of extrapulmonary cases and 67% for site unknown, and for the use of chest x-ray (69% of pulmonary cases, 16% of extrapulmonary cases and 74% for site unknown. Diagnostic scores were recorded for only a minority of cases (18%. The proportion of correct drugs pre- and post-intervention remained same. There were unknown treatment outcomes in 38 out of 141 cases (27% in pre-intervention and in 483 out of 551 cases (87% post-intervention, all among the 692 cases without documented treatment supporter. Conclusions The study has shown that pediatricians have started following parts of the national policy guidelines for management of childhood TB.     Full Text Available Abstract Background Prostate specific antigen (PSA testing is widely used, but guidelines on follow-up are unclear. Methods We performed a systematic review of the literature to determine follow-up policy after PSA testing by general practitioners (GPs and non-urologic hospitalists, the use of a cut-off value for this policy, the reasons for repeating a PSA test after an initial normal result, the existence of a general cut-off value below which a PSA result is considered normal, and the time frame for repeating a test. Data sources. MEDLINE, Embase, PsychInfo and the Cochrane library from January 1950 until May 2011. Study eligibility criteria. Studies describing follow-up policy by GPs or non-urologic hospitalists after a primary PSA test, excluding urologists and patients with prostate cancer. Studies written in Dutch, English, French, German, Italian or Spanish were included. Excluded were studies describing follow-up policy by urologists and follow-up of patients with prostate cancer. The quality of each study was structurally assessed. Results Fifteen articles met the inclusion criteria. Three studies were of high quality. Follow-up differed greatly both after a normal and an abnormal PSA test result. Only one study described the reasons for not performing follow-up after an abnormal PSA result. Conclusions Based on the available literature, we cannot adequately assess physicians’ follow-up policy after a primary PSA test. Follow-up after a normal or raised PSA test by GPs and non-urologic hospitalists seems to a large extent not in accordance with the guidelines.

To counteract this threat in Southeast Asia, sustainable forest management (SFM practices are becoming increasingly important. We assess the tools and guidelines that have been developed to promote SFM and the progress that has been made in Southeast Asia toward better logging practices. We specifically focus on practices relevant to biodiversity issues. Various regional or national mechanisms now inform governments and the timber industry about methods to reduce the impact of production forestry on wildlife and the forest environment. However, so many guidelines have been produced that it has become difficult to judge which ones are most relevant. In addition, most guidelines are phrased in general terms and lack specific recommendations targeted to local conditions. These might be reasons for the generally slow adoption of SFM practices in the region, with only a few countries having incorporated the guidelines into national legislation. Malaysia, Indonesia, and Laos are among the frontrunners in this process. Overall there is progress, especially in the application of certification programs, the planning and management of high conservation value forests, the regulation and control of hunting, and silvicultural management. To reduce further forest loss, there is a need to accelerate the implementation of good forest management practices. We recommend specific roles for governments, the forestry industry, and nongovernmental organizations in further promoting the implementation of SFM practices for biodiversity conservation.

    Full Text Available The period of post-socialist transition in Serbia brings more complex actors environment compared to socialistic period, while institutional arrangements are not enough developed to actively involve different groups of actors in spatial policy formulation process. In order to gather certain knowledge as guidelines for redefining institutional practices in Serbia, institutional framework of Serbia was compared in this paper with institutional framework of three developed European countries, especially in relation to the roles of public, private and civil sector in spatial policy formulation process. The European countries selected for the analysis are United Kingdom, Netherlands and Germany because of diverse national administrative traditions, so different institutional arrangements could be researched. By comparing institutional framework in Serbia with the ones in developed European countries following questions are researched: which actors are missing in Serbia, what are the ways institutional arrangements for different groups inclusion into spatial policy formulation process are formed, what are the differences between the roles of certain groups of actors in decision-making process. Current roles of actors in spatial policy formulation process in Serbia are reviewed and possible directions for public, private and civil sector role redefinition in Serbia are discussed in accordance with experiences of developed European countries.

    Full Text Available Abstract Background In order for clinical practice guidelines (CPGs to meet their broad objective of enhancing the quality of care and supporting improved patient outcomes, they must address the needs of diverse patient populations. We set out to explore the patient attributes that are likely to demand a unique approach to the management of chronic disease, and which are crucial if evidence or services planning is to reflect clinic populations. These were incorporated into a new conceptual framework; using diabetes mellitus as an exemplar. Methods The patient attributes that informed the framework were identified from CPGs, the diabetes literature, an expert academic panel, and two cross-disciplinary panels; and agreed upon using a modified nominal group technique. Results Full consensus was reached on twenty-four attributes. These factors fell into one of three themes: (1 type/stage of disease, (2 morbid events, and (3 factors impacting on capacity to self-care. These three themes were incorporated in a convenient way in the workforce evidence-based (WEB model. Conclusions While biomedical factors are frequently recognised in published clinical practice guidelines, little attention is given to attributes influencing a person's capacity to self-care. Paying explicit attention to predictable threats to effective self-care in clinical practice guidelines, by drawing on the WEB model, may assist in refinements that would address observed disparities in health outcomes across socio-economic groups. The WEB model also provides a framework to inform clinical training, and health services and workforce planning and research; including the assessment of healthcare needs, and the allocation of healthcare resources.

  13. Power sector reform in Africa: Policy guidelines for the sustainability of the sector. Chapter 3

    Since more than a decade and half, a number of African countries have embarked on implementing power sector reform programmes in a bid to address the deficiencies in the management and operations of their power utilities. The rationale for power sector reforms was to: (i) improve the technical, commercial and financial performance of utilities; (ii) boost sector cash flow and enhance utilities' creditworthiness; (iii) facilitate mobilization of resources for capital investment on a commercial basis, thereby releasing public funds for other investments; and (iv) extend access to electricity to poor and rural communities. However, one of the most critical drivers for power sector reform, which is linked to lack of capital to expand and rehabilitate existing systems, is probably pressure from the development finance institutions including the World Bank. Most African countries have thus decided to embark on reforming their power sectors following the announcement of the 1993 World Bank's Electric Power Lending Policy calling developing countries to demonstrate a clear indication to implement comprehensive power sector reform programmes as a precondition of the Bank's continued assistance in the sector. Under this Bank's new policy, developing countries were invited to: - Establish transparent regulatory processes; - Commercialize and corporatize the power enterprises; - Allow for importation of power services in some cases; and - Encourage private investment in the power sector.     Despite improvements in dental caries levels since the widespread introduction of fluoride toothpastes, it is still a disease which is considered to be a priority in many countries around the world. Individuals at higher risk of caries can be targeted with products with a high fluoride concentration to help reduce the amount and severity of the disease. This paper compares guidance from around the world on the use of products with a high fluoride concentration and gives examples of how guidance has been translated into activity in primary care dental practice. A rapid review of electronic databases was conducted to identify the volume and variation of guidance from national or professional bodies on the use of products with a high fluoride concentration. Fifteen guidelines published within the past 10 years and in English were identified and compared. The majority of these guidelines included recommendations for fluoride varnish use as well as for fluoride gels, while a smaller number offered guidance on high fluoride strength toothpaste and other vehicles. Whilst there was good consistency in recommendations for fluoride varnish in particular, there was sometimes a lack of detail in other areas of recommendation for other vehicles with a high fluoride concentration. There are good examples within the UK, such as the Childsmile project and Delivering Better Oral Health, which highlight that the provision of evidence-based guidance can be influential in directing scarce resources towards oral health improvements. Policy can be influenced by evidence-based national recommendations and used to help encourage dental professionals and commissioners and third-party payers to adopt higher levels of practices aimed at oral health improvement.     Because of concerns with the growing threat of global climate change from increasing emissions of greenhouse gases, Congress authorized a voluntary program for the public to report achievements in reducing those gases. This document offers guidance on recording historic and current greenhouse gas emissions, emissions reductions, and carbon sequestration. Under the Energy Policy Act (EPAct) reporters will have the opportunity to highlight specific achievements. If you have taken actions to lessen the greenhouse gas effect, either by decreasing greenhouse gas emissions or by sequestering carbon, the Department of Energy (DOE) encourages you to report your achievements under this program. The program has two related, but distinct parts. First, the program offers you an opportunity to report your annual emissions of greenhouse gases.   16. Sustainable utilisation of forest biomass for energy-Possibilities and problems: Policy, legislation, certification, and recommendations and guidelines in the Nordic, Baltic, and other European countries

    The substitution of biomass for fossil fuels in energy consumption is a measure to mitigate global warming, as well as having other advantages. Political action plans for increased use exist at both European and national levels. This paper briefly reviews the contents of recommendations, guidelines, and other synthesis publications on sustainable use of forest biomass for energy. Topics are listed and an overview of advantages, disadvantages, and trade-offs between them is given, from the viewpoint of society in general and the forestry and energy sectors in particular. For the Nordic and Baltic countries, the paper also identifies the extent to which wood for energy is included in forest legislation and forest certification standards under the 'Programme for the Endorsement of Forest Certification' (PEFC) and the 'Forest Stewardship Council' (FSC) schemes. Energy and forest policies at EU and national levels, and European PEFC forest standards are analysed. With respect to energy policies, the utilisation of wood for energy is generally supported in forest policies, but forest legislation is seldom used as a direct tool to encourage the utilisation of wood for energy. Regulations sometimes restrict use for environmental reasons. Forest certification standards include indicators directly related to the utilisation of wood for energy under several criteria, with most occurrences found under environmental criteria. Objective The objectives of this research were to create a mobile app development guideline for a BYOD hospital environment, apply the guideline to the development of an in-house mobile app called TaskList, pilot the TaskList app within Boston Children’s Hospital (BCH), and refine the guideline based on the app pilot. TaskList is an Apple operating system (iOS)-based app designed for medical residents to monitor, create, capture, and share daily collaborative tasks associated with patients. Methods To create the BYOD guidelines, we developed TaskList that required the use of mobile devices among medical resident. The TaskList app was designed in four phases: (1) mobile app guideline development, (2) requirements gathering and developing of TaskList fitting the guideline, (3) deployment of TaskList using BYOD with end-users, and (4) refinement of the guideline based on the TaskList pilot.     On 3 February 2001 the European Commission adopted the new Community guidelines on state aid for environmental protection. These guidelines outline the methods and criteria used by the Commission when controlling environmental state aid according to Art. 87 and 88 of the EC-Treaty. The study analyses the development of the new guidelines and their potential effects on environmental protection and environmental policy in the Member States of the European Union.     The population health effect and cost-effectiveness of implementing intensive blood pressure goals in high-cardiovascular disease (CVD) risk adults have not been described. Using the CVD Policy Model, CVD events, treatment costs, quality-adjusted life years, and drug and monitoring costs were simulated over 2016 to 2026 for hypertensive patients aged 35 to 74 years. We projected the effectiveness and costs of hypertension treatment according to the 2003 Joint National Committee (JNC)-7 or 2014 JNC8 guidelines, and then for adults aged ≥50 years, we assessed the cost-effectiveness of adding an intensive goal of systolic blood pressure willingness-to-pay threshold of $50 000 per quality-adjusted life years gained, JNC8+intensive had the highest probability of cost-effectiveness in women (82%) and JNC7+intensive the highest probability of cost-effectiveness in men (100%). Assuming higher drug and monitoring costs, adding intensive goals for high-risk patients remained consistently cost-effective in men, but not always in women. Among patients aged 35 to 74 years, adding intensive blood pressure goals for high-risk groups to current national hypertension treatment guidelines prevents additional CVD deaths while saving costs provided that medication costs are controlled.     Full Text Available In this paper we set out to study teacher training in the framework of the First Republic of Portugal (1910-1926. Republicanism embodied a project of the refounding of the nation, which found in education the means of achieving such aspiration. Believing in human plasticity, it elected education, aligned with scientific development, through the path of schooling, as the tool for shaping the new Republican citizen. Teachers had a crucial part to play in this project. Addressing legislation, file sources (exercises, summary books, assignments, inter allia, printed sources (books, workbooks, etc. and specialised media on education and teaching, we discuss training programmes for primary and secondary education teachers. We look into the virtues of its implementation and submit our reading thereof, comparing regular primary education to regular teacher training.     The IAEA Assessment of Safety Significant Events Team (ASSET) Service provides advice and assistance to Member States to enhance the overall level of plant safety while dealing with the policy of prevention of incidents at nuclear power plants. The ASSET programme, initiated in 1986, is not restricted to any particular group of Member States, whether developing or industrialized, but is available to all countries with nuclear power plants in operation or approaching commercial operation.     Northeast Utilities (NU) has developed and implemented a comprehensive probabilistic safety assessment (PSA) program in support of nuclear power plant engineering and operations. Included in this program is a formal policy statement on nuclear safety goals; formalized procedures for review of all plant design changes; a formal procedure for prioritizing new projects; and at least level 1 probabilistic safety studies on all four NU-operated nuclear units. A procedure on risk communication to senior management has also been implemented. This procedure was recently amended to provide quantitative guidelines for determining the acceptability of issues affecting risk. The guidelines described are believed to be unique to the utility industry. The guidelines provide consistency and delineation of authority in making risk management decisions. They are also intended to complement, rather than substitute for, other means of determining the acceptability of plant changes and conditions.     European military transfusion services follow operational guidelines established by their respective national health systems and conform with European Union directives and NATO standards as applicable to member countries. Certain features are common to all of these standards, especially the pre-selection of volunteer, almost exclusively unpaid donors. NATO requirements are very close to European guidelines, with the exception that NATO permits the use of blood products collected in emergency conditions in theater when circumstances allow no better option. Blood product traceability exists for every country but is not always centralized or computerized. Serious adverse event reporting relies on national haemovigilance networks. Military considerations become important mainly in overseas operations, where the overall policy is to implement the relevant national, European or NATO guidelines with adjustments made for unique wartime circumstances and the risk/benefit ratio for the individual patient needing a transfusion. Este artigo tem como objetivo apresentar e discutir os guias alimentares para a população brasileira, buscando identificar aspectos que possam subsidiar as estratégias propostas na Política Nacional de Saúde Bucal. Foram revisados os dois guias oficiais do Ministério da Saúde disponíveis até o momento: o Guia Alimentar para Crianças Menores de 2 Anos e o Guia Alimentar para a População Brasileira. Os guias apresentam recomendações sobre a redução do consumo de açúcar, fundamental na prevenção da cárie, bem como outras diretrizes que podem contribuir para a prevenção da erosão dentária e do câncer bucal. Constituem, assim, um componente importante da estratégia de promoção da saúde geral e bucal, e as recomendações devem ser integradas à Política Nacional de Saúde Bucal.Dietary guidelines are intended to orient dietary habits and policies for health promotion and disease prevention. This article discusses Brazil's national dietary guidelines, aiming to identify issues that can support the strategies proposed by the National Policy on Oral Health. The two currently available official guidelines produced by the Ministry of Health were reviewed: the Dietary Guidelines for Children under Two and the Population Dietary Guidelines. The guidelines recommend reduction in sugar intake, essential for caries prevention, and other measures to prevent tooth decay and oral cancer. These guidelines are thus a key part of an oral and overall health promotion strategy and should be integrated into the National Policy on Oral Health.

  6. Guidelines for an EU waste management policy

    The Commission believes that a legally binding Community instrument is the only option, which will give sufficient assurances to European citizens on a high level of nuclear safety. The first objective of the legislative proposals is, therefore, to guarantee a high level of nuclear safety. The proposals will also enable the European nuclear industry to evolve within a stable legal framework. In addition, in a ruling on 10 December 2002, the Court of Justice of the EU stated that there were clear Community competencies for a) the establishment of a legislative and regulatory framework to govern the safety of nuclear installations, b) the implementation of measures relating to the assessment and verification of safety, c) the emergency preparedness and d) the siting of a nuclear installation and the design, construction and operation of nuclear installations. The European Commission, like the NEA and the IAEA, considers that nuclear safety should be looked at as a whole. All the civil nuclear facilities should be addressed, from their design to their end of lifetime, including the radioactive waste. Three fundamental topics for nuclear safety are consequently part of our proposals: the safety of nuclear facilities, the decommissioning of them at the end of their lifetime and the safe management of spent fuel and of radioactive waste. The proposed Waste Directive encourages progress on the safe management of all forms of radioactive waste, but clearly puts the emphasis on the need for greater progress on the safe long-term management of high-level waste and spent fuel. In particular, it urges Member States to develop repositories, in deep geological structures. Programmes will require the selection of at least one site, the launching of the licensing procedure, the building of the repository and its entry into operation. Member States will have to report on this programme at regular intervals. The reports will be subject to peer reviews by national experts. The Directive would also facilitate co-operation and collaboration on waste management between Member States. It would allow for the development of 'common facilities' - but would in no way require any Member State to accept waste from any other country. Shipments of waste will be subject to agreements between all parties concerned under high safety requirements. What is now needed is for the EU to take the political responsibility -across an enlarged Europe - and make the progress required in order to meet the waste management issue. The adoption of the Waste Management Directive and its implementation in an enlarged EU would be a significant step into such a direction

    In A Theory of Justice, John Rawls demands from citizens who decide upon principles of justice and the rules derived from such principles that they abstract from all particularities that constitute their identity as unique individuals. This demand is unrealistic in policy settings where actual policy-makers convene to provide guidance, establish rules regarding public good, and enact legislation. In practice, I argue, policy-makers, legislators, and others involved in developing social rules that pertain to distributive justice formulate such rules as reasonably partial spectators. To illustrate, I show how clinical practice guidelines are established and mediated by a reasonably partial expert panel whose partial action is publicly justifiable, yet whose claims to impartiality are not.     Decisions regarding thromboprophylaxis in urologic surgery involve a trade-off between decreased risk of venous thromboembolism (VTE) and increased risk of bleeding. Both patient- and procedure-specific factors are critical in making an informed decision on the use of thromboprophylaxis. Our systematic review of the literature revealed that existing guidelines in urology are limited. Recommendations from national and international guidelines often conflict and are largely based on indirect as opposed to procedure-specific evidence. These issues have likely contributed to large variation in the use of VTE prophylaxis within and between countries. The majority of existing guidelines typically suggest prolonged thromboprophylaxis for high-risk abdominal or pelvic surgery, without clear clarification of what these procedures are, for up to 4 weeks post-discharge. Existing guidance may result in the under-treatment of procedures with low risk of bleeding and the over-treatment of oncological procedures with low risk of VTE. Guidance for patients who are already anticoagulated are not specific to urological procedures but generally involve evaluating patient and surgical risks when deciding on bridging therapy. The European Association of Urology Guidelines Office has commissioned an ad hoc guideline panel that will present a formal thromboprophylaxis guideline for specific urological procedures and patient risk factors. The updated AORN "Guideline for cleaning and care of surgical instruments" provides guidance on cleaning, decontaminating, transporting, inspecting, and storing instruments. This article focuses on key points of the guideline to help perioperative personnel implement appropriate instrument care protocols in their practice settings. The key points address timely cleaning and decontamination of instruments after use; appropriate heating, ventilation, and air conditioning parameters for the decontamination area; processing of ophthalmic instruments and laryngoscopes; and precautions to take with instruments used in cases of suspected prion disease.     The US Department of Energy (DOE) has recently issued guidelines for residual radioactivity at Formerly Utilized Sites Remedial Action Program (FUSRAP) and remote Surplus Facilities Management Program (SFMP) sites. Perioperative RNs play a key role in preventing retained surgical items (RSIs). The updated AORN "Guideline for prevention of retained surgical items" provides guidance for implementing a consistent, multidisciplinary approach to RSI prevention; accounting for surgical items; preventing retention of device fragments; reconciling count discrepancies; and using adjunct technologies to supplement manual count procedures. This article focuses on key points of the guideline to help perioperative personnel provide optimal care during a procedure. Key points addressed include taking responsibility for RSI prevention as a team; minimizing distractions, noise, and interruptions during counts; using consistent counting methods; reconciling discrepancies; and participating in performance-improvement activities.     Defining what renewable means has been a challenge, but the authors of a new set of guidelines for the Ecologo certification of renewable low-impact electrcity expect after some delay to have the new rules on the street by March 2000.. There was contention in developing the guideline, according to Terra Choice Environmental Services, the company that created and applied criteria for Ecologo certification under the federal government's Environmetal Choice Program. Interim guidleines were developed and have been in place for renewable power since 1996, and have since been used to certify wood-waste biomass, landfill gas, small hydro and wind power plants, in addition to three green power marketing programs. The first draft of the revised guidelines was issued in March. It was one of the harder guidelines to have been worked on because Canadian power producers did not readily agree on what should qualify as renewable, particularly around the whole area of hydro. While small run-of-river hydro plants will continue to fit within the Ecologo guidelines, larger projects will not. Wind power producers are relatively happy with the latest version of TerraChoice's draft guidelines, released at the end of November. A significant revision of the original rules is a limit on the amount of older generation green power marketers who want to promote the green power Ecologo electricity can sell.     Conflicts of interest (COIs) have been defined by the American Thoracic Society as "a divergence between an individual's private interests and his or her professional obligations such that an independent observer might reasonably question whether the individual's professional actions or decisions are motivated by personal gain, such as direct financial, academic advancement, clinical revenue streams, or community standing." In the context of guideline development, the concerns are not simply about identifying and disclosing direct financial or indirect COIs. Despite this recognition, the management of COIs in guidelines is often unsatisfactory. In response to requests from its international membership and informed by existing syntheses of the evidence and policies of international organizations, the Guidelines International Network Board of Trustees developed guidance on the disclosure of interests and management of COIs. Current approaches are relatively similar throughout the guideline development community, with an increasing recognition of the importance of disclosing and managing indirect COIs. Although there are differences in detail among the approaches, the similarities allow for the formulation of 9 core principles for managing COIs. In formulating these principles, the Guidelines International Network Board of Trustees recognizes that COIs cannot be totally avoided when panel members are being chosen for certain guidelines or in certain settings; thus, the important issue is the management of COIs in a fair, judicious, transparent manner. Several organizations around the world have updated in recent years the guidelines and recommendations for exercise in pregnancy. The December 2015, updated guidelines of the American College of Obstetricians and Gynecologists emphasize that physical activity in pregnancy has minimal risk. Although recommending exercise in pregnancy, the anatomic/physiological changes, absolute and relative contraindications should be considered. Women who exercised regularly before pregnancy, in the absence of contraindications, can continue and engage in moderate to strenuous activities, although information on strenuous activities in pregnancy is still limited. Their construction is complex and labor intensive, and has significant limitations given the necessary process as well as the data available. There has been less focus on their appropriate clinical application as dictated by these inherent limitations. It is worthwhile taking a step back and considering how we know what we think we know based on statistical analysis of biomedical data sets and the real implications of those population data for making predictions about the individual patient we encounter in the clinic. These data as used to establish guidelines for care should be the foundation and starting point for our thoughtful recommendations and decision making, not the final word. The importance of nuance in clinical judgment remains even in this "evidence-based" world. PMID:26992602

    These guidelines were prepared to aid offshore oil and gas operators in the management of waste materials related to petroleum drilling and production operations in offshore areas regulated by the Canada-Newfoundland and Labrador Offshore Petroleum Board (CNLOPB) and the Canada-Nova Scotia Offshore Petroleum Board (CNSOPB). A description of the relevant sections of the regulatory regime applicable to Canada's offshore oil and gas operations was included. Offshore operators are expected to take all reasonable measures to minimize the volumes of waste materials generated by their operations. The guidelines included recommendations for identifying, monitoring, and reporting discharges; performance expectations for specific discharges; requirements for greenhouse gas (GHG) and other air emissions; methods of characterizing and monitoring produced water, drilling muds, and desalination brine.     Full Text Available Abstract Background The World Health Organization (WHO, like many other organisations around the world, has recognised the need to use more rigorous processes to ensure that health care recommendations are informed by the best available research evidence. This is the first of a series of 16 reviews that have been prepared as background for advice from the WHO Advisory Committee on Health Research to WHO on how to achieve this. Objectives We reviewed the literature on guidelines for the development of guidelines. Methods We searched PubMed and three databases of methodological studies for existing systematic reviews and relevant methodological research. We did not conduct systematic reviews ourselves. Our conclusions are based on the available evidence, consideration of what WHO and other organisations are doing and logical arguments. Key questions and answers We found no experimental research that compared different formats of guidelines for guidelines or studies that compared different components of guidelines for guidelines. However, there are many examples, surveys and other observational studies that compared the impact of different guideline development documents on guideline quality.     BACKGROUND: Clinical guidelines are increasingly used in patient management but few clinicians are familiar with their origin or appropriate application. METHODS: A Medline search using the terms \\'clinical guidelines\\' and \\'practice guidelines\\' was conducted. Additional references were sourced by manual searching from the bibliographies of articles located. RESULTS AND CONCLUSION: Clinical guidelines originated in the USA in the early 1980s, initially as a cost containment exercise. Significant improvements in the process and outcomes of care have been demonstrated following their introduction, although the extent of improvement varies considerably. The principles for the development of guidelines are well established but many published guidelines fall short of these basic quality criteria. Guidelines are only one aspect of improving quality and should be used within a wider framework of promoting clinical effectiveness. Understanding their limitations as well as their potential benefits should enable clinicians to have a clearer view of their place in everyday practice.

    The first requirement of the Nuclear Waste Policy Act was for the Secretary of Energy to issue general guidelines for siting repositories. The guidelines were to specify detailed geologic considerations that would be the primary criteria for the selection of sites in various host rocks, as well as factors that would qualify or disqualify any site from development as a repository. These guidelines were clearly intended to provide not only the framework for the siting program but also the stimulus for establishing effective communication and consultation among the parties involved in the program.     Living organisms obtained from modern biotechnology are subject to varying guidelines and regulations at national and international levels. There are different interpretations of the Biosafety Protocol adopted in January 2000. The provisions of the Protocol could be misused to create trade barriers. Worldwide, opinions differ on the agricultural use of genetically modified organisms including crops, livestock and arthropods. Heated debates on the issue continue in both the developed and developing countries with the consequence, that negative attitudes have been created amongst policy makers, scientists and consumers. Concerns have been raised about human, animal and environmental safety with the release and utilization of genetically modifies organisms. In view of the concerns regarding biosafety of GMOs, Zimbabwe has developed 'Biosafety Regulations and Guidelines' aimed at regulating the preparation and implementation of programmes in relation to research, production, importation and release of genetically modified organisms including arthropods. The paper highlights the application of the Biosafety Regulations, Guidelines and the role of policy makers and stakeholder institutions in the country. It also suggests areas where genetically modified arthropods are likely to make an impact in agriculture. Genetically modified arthropods are a product of a new technology and in Zimbabwe's context are likely to stimulate new approach to research and development (R and D) in the protection of stored products against insect pests.     The guidelines for risk assessment of plant protection products (PPPs) and other non-pharmaceuticals were developed over three decades ago and have generally not been updated to incorporate advancements in toxicology and exposure sciences. These guidelines recommend using maximum-tolerated-dose (MTD) even when human relevance of such high doses is mostly limited due to orders of magnitude margin-of-exposure. Conducting animal studies at such high doses often requires further mode-of-action (MoA) studies elucidating human relevance. In order to improve data, ILSI/HESI-ACSA technical committee proposed a tiered approach with emphasis on determining systemic dose of parent and/or metabolite(s) in test animals as biological effects are reflective of systemic rather than administered dose. Any deviation from linearity in systemic dose (saturation of absorption or elimination) in animal studies may have profound toxic effect(s) not expected to occur in likely human exposure scenarios and should be avoided. Toxicity studies should ideally be conducted at kinetically linear doses or slightly above the point of departure from linearity or kinetically-derived maximum dose (KMD) as the systemic dose nonlinearity is a more sensitive parameter occurring much earlier than the MTD endpoints. Therefore, determining systemic dose, especially KMD, in study animals is an improvement to hazard assessment of PPPs and other non-pharmaceuticals allowing toxicologists to better understand findings in animals at systemically linear as well as nonlinear doses to likely human exposures which can easily be accomplished using core study animals as outlined below. Determining systemic dose in studies will also increase the understanding of initial potential MoA of a PPPs and other non-pharmaceuticals and reduce the use of animals by avoiding unnecessary additional MoA studies.     The first phase of the Depleted Uranium Hexafluoride Management Program (Program)--management strategy selection--consists of several program elements: Technology Assessment, Engineering Analysis, Cost Analysis, and preparation of an Environmental Impact Statement (EIS). Cost Analysis will estimate the life-cycle costs associated with each of the long-term management strategy alternatives for depleted uranium hexafluoride (UF6). The scope of Cost Analysis will include all major expenditures, from the planning and design stages through decontamination and decommissioning. The costs will be estimated at a scoping or preconceptual design level and are intended to assist decision makers in comparing alternatives for further consideration. They will not be absolute costs or bid-document costs. The purpose of the Cost Analysis Guidelines is to establish a consistent approach to analyzing of cost alternatives for managing Department of Energy's (DOE's) stocks of depleted uranium hexafluoride (DUF6). The component modules that make up the DUF6 management program differ substantially in operational maintenance, process-options, requirements for R and D, equipment, facilities, regulatory compliance, (O and M), and operations risk. To facilitate a consistent and equitable comparison of costs, the guidelines offer common definitions, assumptions or basis, and limitations integrated with a standard approach to the analysis.     The National Disaster Management Authority (NDMA), ever since it's formation as an apex policy making body for the country in the field of disaster management, has formulated a set of guidelines to assist the various ministries, states and stakeholders in preparing their plans to handle different types of disasters. The guidelines on management of nuclear and radiological emergencies assume great importance in the present context, as our country has very ambitious programme to exploit nuclear energy for peaceful uses. Though, we have an enviable and impeccable record of safety and virtually fail-safe operations in all our nuclear establishments, the possibility, however, remote it may be, of human error, systems failure, sabotage, earthquake, floods, terrorist attacks etc leading to the release of radioactive material in the public domain, cannot be entirely ruled out. With this view, it was decided to prepare the national guidelines by NDMA to manage any nuclear/radiological emergency in public domain. Through these guidelines, we aim to further strengthen our existing nuclear/radiological emergency management framework and generate public awareness, which will go a long way in allaying misapprehensions, if any, amongst the public about the country's nuclear programme. Like in all our guidelines for handling of different types of the disasters, in these Guidelines also, maximum emphasis has been laid on the prevention of nuclear and radiological emergencies, along with a detailed consideration of all other elements of the disaster management continuum. The national guidelines have been prepared and a consensus was arrived on various issues, after widespread consultations and elaborates discussions amongst experts as well as stakeholders. It is assumed that once these guidelines are implemented by the stakeholders and converted into action plans followed by SOPs that will further reduce the chances of accidents in the nuclear arena.     The EMPLOY project aimed to help achieve the IEA-RETD’s objective to 'empower policy makers and energy market actors through the provision of information, tools and resources' by underlining the economic and industrial impacts of renewable energy technology deployment and providing reliable methodological approaches for employment – similar to those available for the incumbent energy technologies. The EMPLOY project resulted in a comprehensive set of methodological guidelines for estimating the employment impacts of renewable energy deployment in a coherent, uniform and systematic way. Guidelines were prepared for four different methodological approaches. In the introduction section of the guidelines policy makers are guided in their choice for the most suited approach, depending on the policy questions to be answered, the data availability and budget. The guidelines were tested for the IEA-RETD member state countries and Tunisia. The results of these calculations are included in the annex to the guidelines.

    Nuclear power currently represents almost one third of the electricity produced and consumed in the European Union. It offers strengths in terms of energy dependence and climate protection. The nuclear option should remain open for the future to those Member States who would like it, but provided nuclear safety is guaranteed. In the public debate following the publication, in 2000,4 of the European Commission', Green Paper on security of energy supply, it became very clear that nuclear safety in an enlarged Europe remained a major concern. The future of the nuclear industry in Europe, therefore, depends on finding and communicating clear answers to safety concerns. In 2007, half a billion consumers will be able to choose their electricity supplier freely in Europe. Public opinion confidence is essential for the security and diversification of electricity supply consumers need. The acceptance of the nuclear option is conditioned by the public' perception of the safety when producing nuclear power and, in particular, in relation to the waste management issue. Recent European public opinion surveys have shown that the public is very concerned about radioactive waste management and also feel poorly informed about it. Th The Commission, therefore, believes that a legally binding Community instrument is the only option, which will give sufficient assurances to European citizens on a high level of nuclear safety. This is the justification of our legislative proposals in the areas of nuclear safety and waste or spent fuel management. The first objective of the proposals is, therefore, to guarantee a high level of nuclear safety. The proposals will also enable the European nuclear industry to evolve within a stable legal framework. But also a similar framework for all power plant operators securing fair competition in a changing electricity market. METHOD A qualitative research outlined by the action research method, with a sample consisting of 16 nurses. Data collection was carried out in three stages and data were analyzed according to the thematic analysis technique. RESULTS It was possible to discuss the demands and difficulties in developing nursing management competencies in hospital organizations and to collectively design a guideline. CONCLUSION The action research contributed to the production of knowledge, confirming the need and the importance of changing the educational processes and evaluations, based on methodologies and instruments for professional development in accordance with human resource policies and contemporary organizational policies.

  20. Dam safety operating guidelines

    As part of Ontario Hydro's dam structure assessment program, the hydraulic design review of several river systems has revealed that many existing dam sites, under current operating procedures, would not have sufficient discharge capacity to pass the Inflow Design Flood (IDF) without compromising the integrity of the associated structures. Typical mitigative measures usually considered in dealing with these dam sites include structural alterations, emergency action plans and/or special operating procedures designed for extreme floods. A pilot study was carried out for the Madawaska River system in eastern Ontario, which has seven Ontario Hydro dam sites in series, to develop and evaluate the effectiveness of the Dam Safety Operating Guidelines (DSOG). The DSOG consist of two components: the flood routing schedules and the minimum discharge schedules, the former of which would apply in the case of severe spring flood conditions when the maximum observed snowpack water content and the forecast rainfall depth exceed threshold values. The flood routing schedules would identify to the operator the optimal timing and/or extent of utilizing the discharge facilities at each dam site to minimize the potential for dam failures cased by overtopping anywhere in the system. It was found that the DSOG reduced the number of structures overtopped during probable maximum flood from thirteen to four, while the number of structures that could fail would be reduced from seven to two. 8 refs., 4 figs., 3 tabs

    The objective of the Department of Energy Natural Phenomena Hazards Project is to provide guidance and criteria for design of new facilities and for evaluation of existing ones subjected to extreme winds, earthquakes, and floods. This paper describes the treatment of wind and tornado hazards. Four facility-use categories are defined which represent increasing levels of risk to personnel or the environment in the event of a high wind event. Facilities are assigned to a particular category, depending on their mission, value, or toxic material content. The assigned facility-use category determines the design and evaluation criteria. The criteria are based on probabilistic hazard assessment. Performance goals are also specified for each facility-use category. A uniform approach to design wind loads, based on the ANSI A58.1-1982 standard, allows treatment of high winds and hurricane and tornado winds in a similar manner. Based on the wind hazard models, some sites must account for the possibility of tornadoes while others do not. Atmospheric pressure changes and missiles must be taken into account when considering tornadoes.     Full Text Available Tropical fevers were defined as infections that are prevalent in, or are unique to tropical and subtropical regions. Some of these occur throughout the year and some especially in rainy and post-rainy season. Concerned about high prevalence and morbidity and mortality caused by these infections, and overlapping clinical presentations, difficulties in arriving at specific diagnoses and need for early empiric treatment, Indian Society of Critical Care Medicine (ISCCM constituted an expert committee to develop a consensus statement and guidelines for management of these diseases in the emergency and critical care. The committee decided to focus on most common infections on the basis of available epidemiologic data from India and overall experience of the group. These included dengue hemorrhagic fever, rickettsial infections/scrub typhus, malaria (usually falciparum, typhoid, and leptospira bacterial sepsis and common viral infections like influenza. The committee recommends a ′syndromic approach′ to diagnosis and treatment of critical tropical infections and has identified five major clinical syndromes: undifferentiated fever, fever with rash / thrombocytopenia, fever with acute respiratory distress syndrome (ARDS, fever with encephalopathy and fever with multi organ dysfunction syndrome. Evidence based algorithms are presented to guide critical care specialists to choose reliable rapid diagnostic modalities and early empiric therapy based on clinical syndromes.

As in the earlier tasks, we will use the IRIS plant as a specific example of an integrated primary system reactor (IPSR) design. The present state of the IRIS plant design – specifically, the lack of a detailed secondary system design – precludes establishing detailed emergency procedures at this time. However, we can create a structure for their eventual development. This report summarizes our progress to date. Section 1.2 describes the scope of this effort. Section 2 compares IPSR ERG development to the recent AP1000 effort, and identifies three key plant differences that affect the ERGs and control room designs. The next three sections investigate these differences in more detail. Section 3 reviews the IRIS Safety-by-Design™ philosophy and its impact on the ERGs.     For several applications of radioactive materials calculations must be executed to determine the radiation risk for the population. A guideline for the risk calculation method of two main sources: nuclear power plants, and other intended and unintended activities with radioactive materials, is given. The standards, recommendations and regulations in this report concern mainly the analysis of the radiological (external) consequences of nuclear power plant accidents, classified as level-3 PSA (Probabilistic Safety Analysis). Level-3 PSA falls within the scales 5-7 of the International Nuclear Event Scale (INES). The standards, etc., focus on the risks for groups of people and the so-called maximum individual risk. In chapter two the standards and regulations are formulated for each part of level-3 PSA: the source term spectrum, atmospheric distribution and deposition, exposure to radiation doses and calculation of radiation doses, dose-response relationships, measures to reduce the effect of radiation doses, design basis accidents, and finally uncertainty analysis.     The United Kingdom (UK) uveal melanoma guideline development group used an evidence based systematic approach (Scottish Intercollegiate Guidelines Network (SIGN)) to make recommendations in key areas of uncertainty in the field including: the use and effectiveness of new technologies for prognostication, the appropriate pathway for the surveillance of patients following treatment for primary uveal melanoma, the use and effectiveness of new technologies in the treatment of hepatic recurrence and the use of systemic treatments.     The aim was to develop clinical guidelines for multi-parametric MRI of the prostate by a group of prostate MRI experts from the European Society of Urogenital Radiology (ESUR), based on literature evidence and consensus expert opinion. Cohort studies from the region show excellent clinical outcomes; however, ART is still being initiated late (in advanced disease in some patients, resulting in relatively high early mortality rates. New data on antiretroviral drugs have become available. Although currently few, there are patients in the region who are failing protease-inhibitor-based second-line regimens. Surveys of international guideline developers found that many do not implement their guidelines. The purpose of this research was to interview guideline developers about implementation approaches and resources. Methods Semi-structured telephone interviews were conducted with representatives of guideline development agencies identified in the National Guideline Clearinghouse and sampled by country, type of developer, and guideline clinical indication. Participants were asked to comment on the benefits and resource implications of three approaches for guideline implementation that varied by responsibility: developers, intermediaries, or users. Results Thirty individuals from seven countries were interviewed, representing government (n = 12 and professional (n = 18 organizations that produced guidelines for a variety of clinical indications. Organizations with an implementation mandate featured widely inconsistent funding and staffing models, variable approaches for choosing promotional strategies, and an array of dissemination activities. When asked to choose a preferred approach, most participants selected the option of including information within guidelines that would help users to implement them. Given variable mandate and resources for implementation, it was considered the most feasible approach, and therefore most likely to have impact due to potentially broad use. Conclusions While implementation approaches and strategies need not be standardized across organizations, the findings may be used by health care policy makers and managers, and guideline developers to generate strategic and operational plans that optimize implementation capacity. Further research is needed to examine how to optimize implementation capacity by guideline developers, intermediaries and users.

Various national guidelines exist for specific diseases. However, most of the guidelines address management at a tertiary level and often appear nonapplicable for the limited resources in primary care facilities. An integrated symptom-based guideline was developed so as to translate the Botswana national guidelines to those applicable in primary care. The Botswana Primary Care Guideline (BPCG integrates the care of communicable diseases, including HIV/AIDS and noncommunicable diseases, by frontline primary health care workers.Methods: The Department of Family Medicine, Faculty of Medicine, University of Botswana, together with guideline developers from the Knowledge Translation Unit (University of Cape Town collaborated with the Ministry of Health to develop the guideline. Stakeholder groups were set up to review specific content of the guideline to ensure compliance with Botswana government policy and the essential drug list.Results: Participants included clinicians, academics, patient advocacy groups, and policymakers from different disciplines, both private and public. Drug-related issues were identified as necessary for implementing recommendations of the guideline. There was consensus by working groups for updating the essential drug list for primary care and expansion of prescribing rights of trained nurse prescribers in primary care within their scope of practice. An integrated guideline incorporating common symptoms of diseases seen in the Botswana primary care setting was developed.Conclusion: The development of the BPCG took a broad consultative approach with buy in from relevant stakeholders.   7. National policy on radioactive waste management

    Every country should have some form of policy and strategy for managing its spent fuel and radioactive waste. Such policies and strategies are important; they set out the nationally agreed position and plans for managing spent fuel and radioactive waste and are visible evidence of the concern and intent of the government and the relevant national organisations to ensure that spent fuel and radioactive waste are properly taken care of in the country. There is a large diversity in the types and amounts of radioactive waste in the countries of the world and, as a result of this diversity, the strategies for implementing the policies may be different, although the main elements of policy are likely to be similar from country to country. In some countries, the national policy and strategy is well established and documented, while in others there is no explicit policy and strategy statement and, instead, it has to be inferred from the contents of the laws, regulations and guidelines. The present paper describes the work undertaken by the International Atomic Energy Agency (IAEA) related to identifying the main elements of national policies for spent fuel and radioactive waste management, recognising that policies and strategies vary considerably depending on, among other things, the nature and scale of applications of radioactive material in a country. An indication is provided of what might be contained in national policies recognizing that national policy and strategy has to be decided at the national level taking into account national priorities and circumstances. The paper is concerned with the contents of policies and strategies and does not address the development of national laws, regulations and guidelines - although these are clearly related to the contents of the national policy and strategy.   8. National planning guidelines for environment ally sustainable development in Scotland and lessons learnt for Pakistan

    This piece of research work reflects the researcher's academic as well as the practical experiences of Scottish planning about the concept, issues and policy formulations. It is in the context of National Planning Policy Guidelines, originated in Scotland with particular reference to it's Fife Region. The first part reflects the general overview of Scotland followed by brief description of the region. Another part deals with the existing strategic issues in the region which are related to land and the environment notably rural planning priorities, agricultural scarce land, conservation of recreational and tourist areas, forest land potentials, landscape resources, national scenic areas, petro-chemical, industrial zones, river pollution and future land use for housing. This study has suggested National Planning Policy Guidelines to these issues. Last section deals with the lessons learnt from Scotland and appropriate application of these guidelines in case of Pakistan. The establishment of the relevant National Planning Guidelines according to our local environmental and socio-economic conditions can also play a significant role to safe guard our rural and urban landscapes and their respective environments. These broad guidelines must therefore be recommended in the broader spectrum in spatial linkage. Moreover these guidelines must therefore synthesize and articulated specifically at structure plans, master plans at district, sub district or local plans and planning processes which can lead Pakistan towards environmentally sustainable development.
