Steam generators for nuclear plants require access to the primary and secondary side for various inspection, cleaning and repair procedures and upon achieving access various inspection and cleaning tasks must be carried out. Steam generators currently at manufacture are therefore provided with; primary side access via the primary manways, steam drum access via the steam drum manway, U-bend region access via the secondary manway and drum-internals access passages, tubesheet secondary side access via tubesheet level inspection ports and tube support plate access via tube support level access ports. A nozzle plug for blocking a nozzle in a nuclear steam generator is improved by the addition of hinges which allow the nozzle plug to be inserted into the steam generator through an access port of substantially smaller diameter than the nozzle. A recess is provided in one of the semi-circular plates allowing the plates to nest, further reducing the necessary size of the access port.

This report describes a technique of inserting an implantable venous access port (portacath) through a thrombosed and occluded vein employing a pre-existing peripherally inserted central catheter (PICC) as the route of access. The PICC was used as a conduit for venous access in a way that has not been described previously in the literature. This procedure was performed in a young patient with cystic fibrosis in an effort to prevent the use of his virgin contralateral veins, which might be used in the future.

Background Laparoscopic inguinal hernia repair has been around since the 1990s. Background This study aimed to evaluate laparoendoscopic single-site (LESS) adrenalectomy via the retroperitoneal approach using the Alexis wound retractor with standard laparoscopic instrumentation. Methods Since October 2009, seven LESS retroperitoneal adrenalectomies have been completed successfully with a homemade single port created using an Alexis wound retractor as an access platform through a 3-cm incision beneath the tip of the 12th rib. METHODS: Surveillance, Epidemiology and End Results Program-Medicare data for seniors diagnosed with CRC were linked to area measures of the sociodemographic characteristics and the capacity of surgeons, medical oncologists, and radiation oncologists. To define the prognostic factors after surgical resection and evaluate the effectiveness of Post Operative Radiotherapy (PORT) in cases with cholangiocarcinoma, 44 cases with proximal cholangiocarcinoma were examined. The mean observation period was 20.6 months, and the survival rates 1, 3 and 5 years after the resection were 79.9%, 59.8% and 39.3%, respectively. Univariate analysis revealed that the presence of lymph node metastasis and absence of PORT were significant poor prognostic factors. Multivariate analysis revealed that the absence of PORT was a significant poor prognostic factor. The survival rates for 1 and 3 years after the resection were 80.0% and 40.0% in the curable A/B and PORT (-) group, and 100% and 53.3% in curable C and PORT (+) group. There were no local reccurences in the pathologically classified hm2 and em2 patients who underwent PORT. ...

The whole computer hardware industry embraced multicores. For these machines, the extreme optimisation of sequential algorithms is no longer sufficient to squeeze the real machine power, which can be only exploited via thread-level parallelism. Decision tree algorithms exhibit natural concurrency that makes them suitable to be parallelised. This paper presents an approach for easy-yet-efficient porting of an implementation of the C4.5 algorithm on multicores. The parallel porting requires minimal changes to the original sequential code, and it is able to exploit up to 7X speedup on an Intel dual-quad core machine.

The global spread of HIV and AIDS has presented a major threat to development, affecting the health of the poor and many aspects of social and economic development. The greatest impact of the epidemic has been felt in sub-Saharan Africa, and Zambia ranks among the worst hit countries. The Free Basic Education Policy in Zambia upholds the right of all children to a universal basic education. This study explored staff and student perceptions of the impact of the epidemic on access to, and the quality of, basic education for AIDS-affected children, orphans and other vulnerable children (OVCs) in the Copperbelt Province of Zambia, where the HIV/AIDS prevalence rate ranges from 34 to 40%, and life expectancy has dropped to 33 years. The purpose of this assessment is to develop a versatile process that will provide a mechanism for evaluating both present and future risks to this environment. Much of the regulatory and environmental interest in the port has centered around a Ballast Water Treatment facility that treats and discharges up to 30 mgd of oily ballast water brought in by crude oil tankers. However, six point discharges and other potential sources of pollution exist in the area. The authors have delineated eleven subareas in the port in order to identify the potential anthropogenic stressors, as well as the receptors that could be exposed to these stressors. An average of 9 million gallons per day of oil-contaminated ballast water off loaded from tankers are treated at the Alyeska Pipeline Service Company Ballast Water Treatment Facility at the terminus of the Trans-Alaska Pipeline in Port Valdez, Alaska. This paper focuses on the results of 2 Prince William Sound Regional Citizens' Advisory Council monitoring programs which detail changes in the chemical composition of treated ballast water as it moves from tankers through the facility and subsequently traces the effluent hydrocarbons into the receiving environment of Port Valdez. The Lake Charles and Port Arthur quadrangles cover approximately 10,950 square miles of land west of the Mississippi River delta area. The area overlies thick sections of the Gulf of Mexico Basin. Surficial exposures are dominated by Recent and Pleistocene sediment. A search of available literature revealed no known uranium deposits. A total of 82 uranium anomalies were detected and are discussed briefly in this report. None were considered significant and all appear to relate to cultural features. Magnetic data appear to be in agreement with existing structural interpretations of the area.

In an existent emergency reactor core cooling device, if a ruptures should occure in a pipeline of a gravitational dropping type reactor core cooling system pool (GDCS) due to some or other causes, a portion of GDCS pool water was flown out of the ruptured port and could not be used for reactor core cooling. Then, a difference pressure detector is disposed to a GDCS pipeline at the inlet of a reactor pressure vessel. When it is judged by the detector, that coolants flow to the outside of the injection pipeline, an injection value disposed to the GDCS pipeline is closed by the difference pressure signal. Even if a rupture should occur on the side of the pressure vessel at downstream to the check value of the GDCS pipeline, since backflow is caused at the pressure container inlet of the GDCS pipeline with the rupture port, the rupture is detected by the difference pressure detector to close the injection valve. The Northeast Gateway Energy Bridge Deepwater Port project was commissioned in 2005 and is based on a proprietary energy bridge shipboard regasification technology. The offshore liquefied natural gas (LNG) facility will be located in federal waters 13 miles southeast of Gloucester and was designed to cope with the extreme weather of the Massachusetts Bay. A dual-buoy system will be capable of ensuring continuous delivery of natural gas. Throughput capacity will range from 400 to over 800 MMcf/d. Vessels used at the port have capacities of 138,000 m{sup 3} and 150,900 m{sup 3} with peak sendout rates of 600 MMcf/d. The vessels have the capacity to rapidly increase and decrease their delivery rates. Outcome from trauma, surgery, and a variety of other medical conditions has been shown to be positively affected by providing treatment at facilities experiencing a high volume of patients with those conditions. An electronic literature search was made to identify English-language articles available through March 2011, addressing the effect of patient treatment volume on outcome for patients with subarachnoid hemorrhage. Limited data were identified, with 16 citations included in the current review. Over 60% of hospitals fall into the lowest case-volume quartile. Outcome is influenced by patient volume, with better outcome occurring in high-volume centers treating >60 cases per year. This presentation gives the results of a research programme aimed at evaluating exhaust emissions from ships moving within confined coastal zones, as in Piraeus harbour. The level of various exhaust emissions were estimated on the basis of a mathematical model applied for each sea traffic element (ship). Hourly and daily records of all ships` movements in different parts of the harbour were compiled, covering extended periods of time and associated with different shipping operational characteristics (peaks, lows, etc). In order to correlate the exhaust emissions from shipping with the quality of the air in the area of interest, land-based measurements for the same pollutants were carried out in selected locations, near the port and in the greater Athens area. This enabled the investigation of time and spatial diffusion of exhaust emissions from shipping within the area of interest, taking into account the influence of prevailing meteorological conditions. Numerous studies have demonstrated the advantages of hydrogen as a fuel for Otto Cycle engines due to high thermal efficiency and low exhaust pollutant levels. Characteristic of hydrogen engine operation using premixed intake charge formation is a problem of pre-ignition resulting in an intake manifold backfire. Additional problems include high NO production when using certain equivalence ratios and power output degradation due to low fuel energy/volume density. Techniques for direct and port fuel injection are discussed as means for overcoming these problems. Emphasis is placed on the need for total engine control, integrating control of fuel injection, ignition timing, intake air throttling, and vehicle subsystems within a central electronic unit. Hung and Quyen's model (analysis of strategic interactions between players in the game theoretic framework) is first recapitulated. Solutions to the embargo game with the seller acting as the Stackelberg leader in both periods. Then the timing coordination issue is discussed and perfect equilibrium outcomes under different structures of leader-follower in the game are compared. Numerical simulations show that the structure corresponding to alternated leadership from one period to the other yields the perfect equilibrium outcome that is Pareto improving with respect to the structure where the buyer is first mover in both periods and Pareto dominant for some specific values of the parameters embedded in the game.

Anesthetic techniques vary widely in the endovascular repair of abdominal aortic aneurysms (EVAR). Previous studies have demonstrated the feasibility of using local anesthesia. However, the ideal anesthetic technique has not been determined. This study examines whether anesthesic technique influences the outcomes of EVAR. Data regarding demographics, risk factors, procedural characteristics, recovery characteristics, treatment complications, acute ( 0.05, ANOVA). From these results we concluded that EVAR with local anesthesia is a safe and efficacious method that may reduce recovery times and postoperative medical morbidity compared to use of genera1 or spinal/epidural anesthesia.

Objectives This paper examines the determinants of the insureds decision to use their health insurance card when seeking outpatient and inpatient health care in Vietnam. Methods Uses Vietnams latest Household Living Standard Survey data and random-intercept logistic regression to assess the influence of the observed individual, household and commune/ward factors on the insureds decision to access health insurance benefits while controlling for the unobserved commune/ward-specific factors. Results Compared to the compulsory enrollees, the voluntary enrollees and the beneficiaries of the Health Care Fund for the Poor are less likely to use their card when seeking inpatient care. Stenting for stenosis of the proximal vertebral artery (VA) is commonly performed via a femoral approach. However, iliofemoral occlusive disease such as arteriosclerosis obliterans sometimes prevents safe transfemoral access. In certain situations where both femoral access and ipsilateral brachial access are difficult because of a concomitant vascular diseases or particular anatomic setting, a contralateral brachial approach using the brachiobrachial pull-through technique may allow efficient and accurate stenting. A case of VA origin symptomatic stenosis successfully treated with stenting using the new pull-through technique from the contralateral brachial artery to the brachial artery on the affected side is described. The great advantage of the helical ionization cooling channel (HCC) is its compact structure that enables the fast cooling of muon beam 6-dimensional phase space. This compact aspect requires a high average RF gradient, with few places that do not have cavities. Also, the muon beam is diffuse and requires an RF system with large transverse and longitudinal acceptance. A traveling wave system can address these requirements. First, the number of RF power coupling ports can be significantly reduced compared with our previous pillbox concept. Secondly, by adding a nose on the cell iris, the presence of thin metal foils traversed by the muons can possibly be avoided. We show simulations of the cooling performance of a traveling wave RF system in a HCC, including cavity geometries with inter-cell RF power couplers needed for power propagation.

By means of an analogous simulator, which can simulate a power supply system, a method to determine the transfer function of a Power System Stabilizer (PSS) is developed. The purpose of the PSS is to damp unwanted electro-mechanical oscillations. The method is very systematic and there is no need for specified values of the parameters used in the system. It is assumed that the power generator system is linear for small variations. The system can be considered to be a black box. Measurements executed by means of this method are completely controlled by the person who enforces the measurement. By applying this method carefully, irregularities are almost out of the question. The equipment necessary are a PC with two AD-ports and an adjustable sinus-generator. A comparison is made with a more traditional method, in which a spectrum analyzer is used. 4 figs., 7 refs.

The ACP Multi-Array Processor System (ACPMAPS) is a highly cost effective, local memory parallel computer designed for floating point intensive grid based problems. The processing nodes of the system are single board array processors based on the FORTRAN and C programmable Weitek XL chip set. The nodes are connected by a network of very high bandwidth 16 port crossbar switches. The architecture is designed to achieve the highest possible cost effectiveness while maintaining a high level of programmability. The primary application of the machine at Fermilab will be lattice gauge theory. The hardware is supported by a transparent site oriented software system called CANOPY which shields theorist users from the underlying node structure. 4 refs., 2 figs.

A geological and geotechnical framework was provided in order to facilitate understanding of the damage to structures resulting from soil conditions during the earthquake of January 17, 1995 at Hyogo-ken Nanbu (Kobe), Japan. Special attention was paid to the large scale failures of the quay walls in the port of Kobe, resulting from liquefaction. The performance of structures in improved and unimproved ground were compared. It was observed that locations with some previous site improvements through densification behaved satisfactorily, despite unanticipated high levels of ground motion. Soil-structure interaction problems, such as pile foundations, bridge piers, lifelines, and an underground subway station were also described. Reference was made to the significance of these failures for the seismic design in the Fraser Delta in British Columbia in view of the area`s proneness to liquefaction during a major earthquake. 31 refs., 5 tabs., 45 figs.

Two patients who developed large intracranial vessel occlusion after standard radiation therapy for brain tumor are described. This form of vascular occlusion is usually seen in patients who have previously been treated by radiotherapy for intracranial tumor who then develop a relatively acute change in neurologic status. Histology of the lesion mimics accelerated focal arteriosclerosis. The clinical and radiographic manifestations of one case were highly atypical. The vasculopathy became evident shortly after termination of radiation therapy for a fourth ventricular ependymoma, and the angiographic picture stimulated a diffuse arteritis. The second patient was more typical, with clinical symptoms developing 12 years after radiation therapy for an oligodendroglioma. Occlusion of a proximal vessel that had been included in the radiation port was demonstrated radiographically and confirmed by pathologic examination.
