The authors used studies of local cerebral metabolic rate for glucose (LCMRGlc) to examine development of cerebral organization in 5 days to 1 year old children. A group (n=8) of infants with diverse pediatric disorders allowed investigation of developmental changes in LCMRGlc, while also providing relevant clinical management information. Patients consisted of questionable and definite neonatal seizures, cerebral embolism from cardiac sources, and otherwise normal infants with facial nevi with consideration of Sturge-Weber. Gradual increase in cortical LCMRGlc coincides with suppression of intrinsic subcortical reflexes present in all newborns. The temperature elevations in anatomically based human phantoms of an adult and a 3-year-old child were calculated for radio-frequency whole-body exposure. Thermoregulation in children, however, has not yet been clarified. In the present study, we developed a computational thermal model of a child that is reasonable for simulating body-core temperature elevation. Comparison of measured and simulated temperatures revealed thermoregulation in children to be similar to that of adults. Based on this finding, we calculated the body-core temperature elevation in a 3-year-old child and an adult for plane-wave exposure at the basic restriction in the international guidelines. The body-core temperature elevation in the 3-year-old child phantom was 0.03 deg. C at a whole-body-averaged specific absorption rate of 0.08 W kg{sup -1}, which was 35% smaller than in the adult female. This difference is attributed to the child's ...

We present the case of a 12-year-old girl who presented with a pathological fracture through a benign-appearing osteolytic lesion that was presumed to represent fibrous dysplasia. The fracture healed, and over the next 2.5 years there was further refracturing and healing with progressive osteolysis. A biopsy was performed and revealed Ewing's sarcoma. The disease was locally controlled by neoadjuvant chemotherapy and radiation therapy. The patient is disease free with excellent function 6 years following the discovery of the lesion. A 56-year-old man admitted to our hospital for cardiac tamponade due to dilated cardiomyopathy did not respond to treatment by usual medical means or surgery. Pericardio-peritoneal drainage was conducted using a subcostal approach. Seven months later, the patient remains well and free of signs of pericardial tamponade. Brucellosis is a common worldwide zoonotic disease. Cutaneous manifestations are not specific and affect 1-14% of patients with brucellosis. Here, we describe 49-year-old female with fever and a diffuse maculopapular rash due to Brucella melitensis infection. Histopathology of skin biopsy revealed leukocytoclastic vasculitis; positive blood cultures for B. melitensis established the diagnosis of brucellosis. We provide a review of the relevant literature. The authors present a case in which primary Ewing's sarcoma of the right petrous pyramid in a 9-year-old girl showed no uptake on a /sup 99m/Tc-pertechnetate nuclide angiogram. Intense uptake was present on a /sup 99m/Tc-polyphosphate bone scan, but a static brain scan was only minimally abnormal. The diagnosis and treatment of Ewing's sarcoma are reviewed.

Reducing conditions are periodically present in hydric soils and are essential for chemical processes that support wetland functions and values. Indicators of these conditions, i.e., redoximorphic features, can be useful in determining the presence of a hydric soil. However, young wetlands, i.e., those recently formed, may not possess reducing conditions and/or may not exhibit redoximorphic features. Few studies have addressed the time needed for hydric soil development. In this study, we present data on redoximorphic features, including chroma and oxidized rhizospheres, gathered from two sets of wetlands in southwestern Virginia, including (1) constructed wetlands that are 3 years old and (2) accidental wetlands that are 10 to 30 years old. This case report makes evident that in a 75 years old patient with a (cured) femoral neck fracture 15 years ago a refracture at the same site following a new trauma could not be detected by conventional X-ray. The follow-up of a radionuclide bone study, however, enabled the dignosis of a refracture. From these findings, it is further evident that the decreased bone metabolism in older patients has to be considered. Thus, although the initial bone scan following the trauma is negative, a follow-up scan should be performed within a 4 day-period. (orig.).

Nasal cavity volume was studied in 69 healthy volunteers (from 8 to 23 years old) by MRI. The right and left nasal cavity areas were measured on horizontal images displayed directly on the MRI console. The nasal cavity volume was obtained by integrated volume calculation. The nasal cavity volume was positively correlated with age, body height and weight up to age 16. The body height and weight reached a plateau at 16 years, but the nasal cavity volume continued to increase. There was no significant difference in the volumes of the right and left cavities. We report the case of a 22-year-old man with Ewing's sarcoma who attained a complete remission (CR) after combination radiotherapy and chemotherapy. Secondary acute lymphoblastic leukemia with multiple cytogenetic abnormalities involving chromosome 5 and 7 developed 16 years later. The patient underwent induction chemotherapy and entered a CR. Peripheral blood stem cell transplantation from a matched sibling was performed successfully and he is in complete remission of both ALL and Ewing's sarcoma. A population of male civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease. Field activities started in December, 1982 and were concluded in July, 1984. Diagnosis was made using real-time ultrasonography. Participation in the study was 71.5%. Prevalence of gallstone disease was 8.2% and increased with age from 2.3% in the 20- to 25-year-old age group to 14.4% in the 60- to 69-year-old age group, based on both presence of gallstones and history of cholecystectomy. About one-third of the subjects with gallstone disease had previously been submitted to cholecystectomy. Only 7.7% of the subjects with presence of gallstones complained of at least one episode of biliary pain in the preceding 5 years. Frequency of "minor" dyspeptic symptoms was not different between men with and those without gallstones. PMID:3391519

Two cases of primary Ewing's sarcoma of the rib are reported, in which radiation therapy was quite effective. Case 1 was an 18-year-old female who had had an operation and radiation therapy for Ewing's sarcoma of the left 7th rib. She was referred to our hospital after a recurrent tumor was found. Radiation therapy (tumor dose 46.2 Gy) and chemotherapy were given. The tumor disappeared and there has been no relapse for 1 year and 3 months after the treatment. Case 2 was a 2-year-old-infant. Radiation therapy (tumor dose 74 Gy) was given for primary Ewing's sarcoma of the left 6th rib. The tumor became small and was successfully removed at operation. There has been no relapse or distant metastasis for 8 months following the operation. We emphasize the importance of multidisciplinary treatment in case 1 and the usefulness of preoperative radiotherapy in case 2. (author).

''Hair-on-end'' skull changes resembling thalassemia were rarely described in the 1950s and 1960s in children with cyanotic congenital heart diseases; these changes were described almost entirely in patients with tetralogy of Fallot or D-transposition of the great arteries. As these lesions have become correctable, the osseous changes, never common, seem now only to exist in a small number of patients with uncorrectable complex cyanotic congenital heart disease who survive in a chronic hypoxic state. We present two cases: a case of marked marrow expansion in the skull of a 5-year-old boy with uncorrectable cyanotic heart disease studied by CT, and a second case of an 8-year-old with tetralogy of Fallot and pulmonary atresia studied by plain skull radiographs. In the last two years, the VDMA work group 'Functional security' compiled a qualitative and quantitative procedure for the determination and evaluation of turbo-machine specific risks. With the calibrated risk graph for turbo-machines the requirements (Safety Integrity Level SIL) to the protective functions for turbo-machines (steam turbines, gas turbines, generators and compressors) were determined. With consideration of the legislation, the work group compiled a recommendation for the renewal and/or reconstruction of protective functions with old facilities.

A 67-year-old man presented with gastrointestinal bleeding resulting from a fistula between the aortic stump and the fourth part of the duodenum 14 months after an axillobifemoral bypass, graft excision, and aortic stump closure for aortic graft infection. The patient initially refused any attempt at definitive surgical treatment so coil embolization of the aortic stump was performed via a brachial approach. Coil embolization may be a useful adjunct in the management of such difficult problems. This approach may buy time and allow careful planning of a definitive treatment or may serve as a palliative procedure in inoperable patients. PMID:15947052

We report a case of stress fracture of the clavicle associated with sternocostoclavicular hyperostosis. A 60-year-old man sustained a stress fracture of the right clavicle with no history of trauma. On radiography, hyperostosis of the anterior chest wall and ankylosis of the sternoclavicular joint were evident in addition to the fracture. Fracture healing was uneventful after 2.5 months. Ankylosis of the sternoclavicular joint may have caused increased stress at the midshaft of the clavicle by daily activity or minor trauma. Such a fracture is a rare complication of sternocostoclavicular hyperostosis. (orig.)

Primary B-cell lymphoma of thymus is uncommon. We describe the case of a 23-year-old woman with a history of shortness of breath and distended neck veins of 1-month duration. A chest roentgenogram and computed tomographic chest scan of the showed a large anterior mediastinal mass. Tru-cut biopsy revealed primary B-cell lymphoma of thymus. She underwent chemotherapy with no symptomatic improvement. Post-chemotherapy computed tomographic scan of the chest showed no appreciable reduction in tumor size. Surgical excision was carried out with excellent results.

This paper reported on a study in which partial discharge (PD) measurements were performed on 6 underground transmission cable circuits to determine the effectiveness of PD measurements on fluid fill cables. The cables were between 37 and 56 years old. PD activity was detected from the terminations of H11L and H3L Cable 2. However, for cables from H3L Basin TS to Mill Street junction, PD activity was detected originating from within the main cable insulation.

Pulmonary embolism complicated by transcatheter arterial chemoembolization (TACE) in known to be due to the use of large amount of lipiodol as an embolic agent. To our knowledge, massive tumoral pulmonary embolism following TACE and confirmed by surgery has not been described in the literature. In this report, we detail the cas of a 49-year-old man in whom cyanosis and hypotension developed abruptly on the day of TACE.. Chest CT revealed diffuse low-attenuated lesions in both pulmonary arteries. Histopathological specimens after emergent pulmonary arterial embolectomy confirmed the presence of massive tumor emboli of hepatocellular carcinoma.

/sup 99m/Tc biliary imaging was performed on a 34-year-old woman who was being investigated for suspected cholelithiasis. A left-sided liver was detected. On subsequent radionuclide imaging, partial visceral situs inversus and asplenia were demonstrated. Extensive roentgenographic investigation, ECG, echocardiogram, and laparotomy confirmed the radionuclide findings. Asplenia syndrome may exist without cardiovascular abnormalities and thus be undiscovered. Radionuclide imaging is important in the diagnosis of this syndrome.

A 1.5-year-old male Belgian Malinosis Military Working Dog presented with a 1-month history of intermittent hematuria. Diagnostic ultrasound and contrast radiography demonstrated large blood clots in the urinary bladder and a filling defect in the right renal pelvis. At surgery, clotted blood was present in the right ureter and bladder. Following right nephrectomy, the dog returned to training. One month later, elevations in urea nitrogen and creatinine were noted. Hematuria recurred at 3 months and the dog was found dead in its kennel. Necropsy showed a blood-filled left renal pelvis and ureter. PMID:1454185

A 60-year-old male was bitten by a venomous snake (Vipera ammodytes) and gradually developed signs of an allergic reaction including generalized itching, generalized rash, and chest discomfort. This was followed by severe retrosternal pain with electrocardiographic evidence of an inferior myocardial ischemia progressing to acute myocardial infarction. Cardiac enzymes and troponin, serum tryptase, and histamine were elevated. Coronary arteriography showed normal coronary arteries. Gallium imaging is increasingly being used for the early detection of complications in patients with AIDS. A 26-year-old homosexual man who was HIV antibody positive underwent gallium imaging for investigation of possible Pneumocystis carinii pneumonia. Widespread cutaneous focal uptake was seen, which was subsequently shown to be due to mycobacterium avium-intracellulare (MAI) septicemia. This case demonstrates the importance of whole body imaging rather than imaging target areas only, the utility of gallium imaging in aiding the early detection of clinically unsuspected disease, and shows a new pattern of gallium uptake in disseminated MAI infection.

A total 18 of actual plant operators (M=32.3, SD=10.5 years old) participated in the evaluation. The operators rated the new CRT pictures highly. The response times using the new CRT pictures were shorter than those by the original pictures. We encountered a case of Ewing's sarcoma. The patient was a 19 year-old girl, who visited a local physician because of pain in the left scapular region, and was referred to us. Roentgenograms of her left scapula revealed an irregular moth-eaten shadow which was suspected to be a malignant bone tumor. The histological diagnosis of Ewing's sarcoma was comfirmed by needle biopsy. The initial stage of treatment consisted of radiation therapy (6000 rads) and chemotherapy (VAC and ADR). At the end of the initial stage, the symptoms were improved, the size of the local lesion was reduced on the roentgenogram and there was no evidence of distant metastases. (author).

Abstract Adalimumab is a fully human monoclonal anti-tumor necrosis factor-a agent that is approved for the treatment of Crohn's disease. It has a good safety profile, injection site reactions being the most common adverse effect. We report a case of a 54-year-old woman with a 30-year history of Crohn's disease who developed achromic patches on the trunk and upper extremities after initiating treatment with adalimumab. Cutaneous biopsy confirmed diagnosis of vitiligo and laboratory testing ruled out thyroid disease. A 38 year old male dentist developed an oligodendroglioma of the left medial temporal lobe and parasellar region 12 years after radiotherapy with 6600 rads for acromegaly. The 30 cases of radiation-induced gliomas reported in the English literature are reviewed and analysed. The criteria for defining radiation-induced tumours of the central nervous system are proposed as follows: the tumour has a long quiescent ''latency period'', a location in the previously irradiated field, a verified histological difference from a primary condition, and does not arise from a primary condition associated with a genetic syndrome such as neurofibromatosis or tuberous sclerosis. The reported case fulfilled these criteria but appears to be the only reported radiation-induced oligodendroglioma.

A 33-year-old male of proven fertility suffered six attacks of malaria while resident in an African country. For this he received anti-malarial drugs. Semen analysis performed after the fourth attack, and repeated during the following 2 years after his return to Israel, revealed severe oligozoospermia, necrozoospermia and occasionally even azoospermia. Immunological examination of the patient revealed an inverse ratio of T-cell subsets and mast cell degranulation in response to palludrin. Twenty-five months after the last attack of malaria a significant improvement was found in semen quality and there was an increase in the ratio of T-helper to T-suppressor cytotoxic cells. These observations indicate that although malaria and its treatment may affect spermatogenesis, recovery may be expected eventually. PMID:3500921

Forty cases of Ewing's sarcoma of various sites had attended the Department of Radiotherapy at the Wanless Hospital and the Miraj Medical Centre, attached to the Government Medical College, Miraj, Maharashtra, from January, 1970 through December, 1979. There were 28 male patients and 12 female patients in the series with a male to female ratio of 2.3:1. The youngest patient in the series was a female of 6 years of age with the tumour in the scapula and the oldest patient was a 32-year-old male with a lesion in the metacarpal bone. A 52-year-old woman had a history of left hearing loss for 5 years. An acoustic neurinoma with 3.2 cm in diameter was diagnosed and treated with gamma-knife radiosurgery (19 Gy of marginal dose) 1 year and 4 months ago. She developed headache, nausea, and visual disturbance 1 month prior to admission. Slight left facial palsy appeared after radiosurgery. Magnetic resonance imaging demonstrated the tumor with central necrosis in the left cerebellopontine angle cistern, increasing in size to 3.5 cm in diameter, and hydrocephalus. We report a patient who developed occlusion of the intracranial major arteries induced by radiation therapy for craniopharyngioma. The patient, a 9-year-old boy, presented with deterioration of vision, nausea and vomiting. He underwent a right ventriculo-peritoneal shunt operation, and the tumor was partially removed. Postoperatively, he received focal radiation therapy with total of 60 Gy. Preoperative cerebral angiography, cerebral vessels had no sigh of occlusive lesion or stenosis. One year after irradiation, he started to have repeated cerebral ischemic attacks. But no regrowth of the tumor was detected on CT scans. Preterm children born before 32 weeks of gestation represent 1% of the annual births in Switzerland, and are the most at risk of neurodevelopmental disabilities. A neurological surveillance is thus implemented in the neonatal units, and multidisciplinary neurodevelopmental follow-up is offered to all our preterm patients. The follow-up clinics of the University hospitals in Lausanne and Geneva follow the Swiss guidelines for follow-up. An extended history and neurological examination is taken at each appointment, and a standardized test of development is performed. These examinations, which take place between the ages of 3 months and 9 years old, allow the early identification and treatment of developmental disorders frequent in this population, such as motor, cognitive or behavioral disorders, as well as the monitoring of the quality of neonatal care. The purpose of this study is to examine the cross-ethnic measurement equivalence of the "Child Behavior Checklist" (CBCL) for 6- to 18-year-olds (2001, parent version) using a sample of African American special needs adopted children, along with a comparison group of Caucasian American special needs adopted children. Reliability and multiple sources of postdictive criterion-related validity were analyzed, and a confirmatory factor analysis of the CBCL within and between participating groups was performed. The results indicate that for practical purposes, the CBCL items appear reliable but not valid in terms of assessing the mental health profiles of both African American and Caucasian youth. These findings are significant because they have important implications for the practical utility of the CBCL in the field for adopted youth. (Contains 6 tables.)

Paraquat is a nonselective contact herbicide of great toxicological importance, being associated with high mortality rates, mainly due to respiratory failure. We report the case of a 22-year-old male admitted to the emergency room with a sore throat, dysphagia, hemoptysis, and retrosternal pain after the ingestion of 50 mL of a paraquat solution, four days prior to admission. Chest CT scans revealed pulmonary opacities, pneumomediastinum, pneumothorax, and subcutaneous emphysema. The patient was submitted to two cycles of immunosuppressive therapy with cyclophosphamide, methylprednisolone, and dexamethasone. The pulmonary gas exchange parameters gradually improved, and the patient was discharged four weeks later. Glomangioma have many synonyms including: glomus tumors, tumors of neuromyoarterial glomi, angioneuromas, angioneuromyomas, neuromyoarterial glomi, painful subcutaneous tubercles, Popoff tumors or subcotaneous glomal tumors. They are common in humans, rare in nonhuman primates and to the best of our knowledge, have only been reported in irradiated rhesus. The neoplasms originate in arterial-venous shunts known as neuromyoarterial glomi which are commonly found beneath fingernails and fingertips, but have been reported in many locations both superficial and deep. The neoplasm can be confused with hemangiopericytomas, hemangiomas, paragangliomas, and leiomyomas, and must be definitely diagnosed ultrastructurally. A glomangioma at the 6-7 thoracic intervertebral space caused compression of the spinal cord with posterior paralysis in an irradiated 20-year-old female rhesus monkey (Macaca mulatta).

In a 47-year-old male patient a bronchopleural fistula was apparent 22 days after extended right-sided diaphragma-pericardio-pleuro-pneumonectomy for pleuramesothelioma. The thoracic cavity was infected. Rethoracotomy was performed and the fistula was closed using an omental pedicle flap. The bronchial stump became tight and the cavity fluid became sterile. No abdominal complications were seen. The patient died 8 months later from malignant pericardial infiltration. The ability of greater omentum to revascularize ischemic tissue, to absorb fluid and to resist local infection is proved and used in several subspecialities of surgery. Nevertheless the use of the greater omentum in the management of bronchopleural fistula has been rarely published. The reported case shows that the closure of a large bronchopleural fistula is possible by using the attributes of the omental tissue. PMID:2264043

This article explores how action research, design based research and interaction design can be combined and used in the development of educational robotic tools. Our case study is the development of Number Blocks and it combines physical interaction, learning, and immediate feedback. Number Blocks supports the children's understanding of place value in the sense that it allows them to experiment with creating large numbers. The development was done in collaboration with a class of 7-8 year old children and their mathematics teacher. The article argues that elements from different research methods allowed a structured approach to projects that combines educational research and innovation of new learning technologies. Key elements of this approach is acknowledging the users input, developing a theoretical pre-analysis and using an iterative approach.

The case is presented of a 64-year-old man who complained on admission of discomfort in the right shoulder at the time of excision of a large section of the axillary skin which contained extra-mammary Paget disease. Shortly thereafter, radiological studies of the humerus and scapula showed obvious malignant disease which proved to be metastatic upon biopsy of the humerus and pathological studies of the excised area of extra-mammary Paget disease in the axillary skin. The patient died twelve months after the diagnosis was established with widespread metastatic disease. The literature on the subject was reviewed and it was demonstrated that metastases to the skeleton from lesions of the skin are relatively rare. This case is exceptional because of the rapid fatal course and the extent of metastatic disease originating in extra-mammary Paget disease of the axillary skin.

Pyogenic sacroiliitis is a rare entity in children. Diagnosis is often delayed because of its variable clinical presentation, low suspicion by the examining physician, and rare findings on radiographs. Delayed diagnosis, however, results in complications such as iliopsoas abscess. We report the case of a 12-year-old girl hospitalized with a 21-day history of fever, pain in the left iliac fossa, and flexion contracture of the hip. Because Spitzer is an Earth-trailing orbit, losing about 0.1 AU/yr, it is excellently located to perform microlens parallax observations toward the Magellanic Clouds (LMC/SMC) and the Galactic bulge. These yield the so-called ``projected velocity'' of the lens, which can distinguish statistically among different populations. A few such measurements toward the LMC/SMC would reveal the nature of the lenses being detected in this direction (dark halo objects, or ordinary LMC/SMC stars). Cool Spitzer has already made one such measurement of a (rare) bright red-clump source, but warm (presumably less oversubscribed) Spitzer could devote the extra time required to obtain microlens parallaxes for the more common, but fainter, turnoff sources. In Japan, 40 cases or so have been reported, and cases of bilateral femoral neck fractures have not reached to ten cases. The authors experienced a case of 75 year-old female who received radiotherapy for cancer of the uterus, and suffered from right femoral neck fracture 3 months after and left femoral neck fracture one year and half after. As clinical symptoms, she had not previous history of trauma in bilateral femurs, but she complained of a pain in a hip joint and of gait disturbance. In August 1982, a 44-year-old man was diagnosed as having rectal cancer, histologically diagnosed as well differentiated adenocarcinoma, and abdominoperineal resection and colostomy were performed. Postoperatively, he received chemotherapy with mitomycin C up to a total dose of 100 mg. In September 1986, lung metastasis occurred and he was treated with a combination chemotherapy consisting of cisplatin, pirarubicin and 5-fluorouracil. In the following year, radiation treatment (total: 6900 rad) was given for a recurrent pelvic lesion. Peripheral blood on April 30, 1988, showed anemia, thrombocytopenia and appearance of myeloblasts, and a diagnosis of acute myelogenous leukemia (FAB: M1) was made. Problems and solutions in the diagnosis and operative treatment were clarified by retrospective evaluation of cases of intramedullary tumor in which MRI and surgery had been performed. The subjects were 16 patients (21-75 years old, mean age: 45 years) whose tumor was surgically removed between 1991 and 1998. MRI was performed preoperatively, and T1-weighted images, T2-weighted images, and enhanced images were obtained. Angiography was also performed in some cases. The histological diagnosis based on the MRI findings before surgery were compared with the pathological diagnosis after the operation. In 9/16 cases (56%), the preoperative histological diagnosis based on the MR images was the same as the pathological diagnosis after the operation. A 72-year-old man was exposed to the sarin gas attack in a Tokyo subway on March 20 th, 1995. After exposure, he noticed eye discomfort, chest tightness, headache and weakness of the lower limbs and oropharyngeal muscles. Despite these symptoms, he visited a hot spring on the same day with his family. On March 25 th, his muscle weakness progressed, and a low grade fever appeared. His muscle weakness disappeared 8 days after exposure to sarin, but respiratory failure rapidly developed, necessitating artificial ventilation within four day after hospitalization on March 28th. Chemotherapy with erythromycin, imipenem/cilastatin, and steroid pulse therapy was begu. Subjects were 18 cases with adenocarcinoma of 20 mm or less in diameter, which were detected and resected surgically from September, 1993 to August, 1998. On these cases, thin-Section CT (TSCT) images were compared with pathological findings. Patients were 15 men and 3 women, with mean age of 63.4-year-old. An average tumor size was 11.5 mm (5-17 mm). According to pathological classification of Noguchi, type A as in 6 cases, type B in 3, type C in 7 and type F in 2. TSCT was carried by vas voltage =150 kVp, vas current =200 mA, beam width =2 mm, WW =2000 HU, and WL =-700 HU. The ratio of CT attenuation value in cancer by TSCT correlated with the ratio of aeration rate in the pathologic specimen. Preliminary tests have shown that smart-grid systems can significantly reduce home electricity use. A smart grid negates the need for controlled distribution by giving homeowners the ability to monitor rates in real time and modify their use based on energy availability, need and price. Homeowners can also feed the grid alternative power to offset their costs. Studies in the United States have also suggested that the system could reduce carbon emissions by 60 to 211 million tonnes per year. The smart grid would also be less susceptible to major disturbances because it draws power from multiple sources. An added benefit is that the technology could be developed for a fraction of the cost of building traditional energy plants, such as nuclear. Up to this time the sigmoid colostomy has been widely accepted and conventional treatment for radiation-injured rectum, but patients without residual malignancy strongly desire to live without colostomy. We have tried to remove the involved rectal segments by sphincter-saving procedures. Four patients underwent these procedures, pull-through procedure in three and low anterior resection in one. Among sphincter-saving procedures, pull-through procedure was most adequate. Provided the following five conditions are fulfilled, pull-through procedure should be considered for severe radiation-injured rectum. (1) No recurrence of initial malignancy in the pelvis. (2) More than 2 cm intact rectal segment above dentate line may be preserved. (3) No radiation-injured segment in upper sigmoid. (4) No severe radiation damage in small intestine. (5) Patients under 70 year-old, with normal tonus of anal sphincter.

Concentration ratios (CR) for plutonium in biota from the floodplain forest are less than CR values from other contaminated ecosystems in the USA. Presumably, this is due to humid conditions and greater rainfall which minimize resuspension as a physical transport mechanism to biota. Plutonium and radiocesium concentrations are correlated in biota from the forest at Oak Ridge and also from Mortandad Canyon in New Mexico. Preschool-age children hold negative attitudes toward overweight peers (i.e., anti-fat attitudes), but little is known about individual differences in these attitudes. The current study investigated actual weight status and perceived body size in relation to preschool-age children's anti-fat attitudes. Sixty-nine 4-6 year-olds (61% girls) were individually interviewed about their body size perceptions using a figure rating scale and anti-fat attitudes using multiple methods (including an adjective rating scale and ratings of acceptability for different body shapes). Results suggested that children's perceived body size, not actual body size, was related to their attitudes about an overweight figure and the number of figures rated as acceptable. Children who perceived themselves as heavier held fewer anti-fat attitudes. Perceived and actual body size ratings were not related to ratings of a thin figure. In order to estimate organ or tissue dose from the tissue kerma in air, a ratio of the organ or tissue dose to the tissue kerma in air for survivors in Hiroshima and Nagasaki was calculated with the aid of the Snyder's mathematical phantoms constructed so as to simulate the body shape of survivors aged 5, 10 years old and adult at the time of atomic bomb detonations. The relationship between serum PG I, PG II levels and extent of atrophic gastritis was examined. The subjects were 64 patients (male: 32, female: 32, 51.9 years old on average) with established diagnosis of either atrophic gastritis or normal. In the X-ray gastric examination, Fuji Computed Radiography (FCR) was used to obtain clear-cut images of the gastric area. Concerning the serum PG I level, patients in the group with atrophic gastritis showed lower levels than those of the people in the group with no atrophic change, but the variation was wide, and no definite tendency was seen in the relationship between the atrophic change and the serum PG I levels. The health politics and policy communities are still struggling with the question of "what went wrong" in the 1993-94 health care reform effort. Here I identify which Americans were politically active and inactive during the health care reform debate to explore the role political participation may have had in determining the outcome of the debate. Using data from a national and California random-sample telephone surveys, and controlling for other demographic attributes, I found that those who engaged in political activity specifically related to health care reform were disproportionately more likely to be self-identified conservatives, less likely to favor an employer mandate plan, more likely to be fifty to sixty-four years old, more likely to be men, and more likely to have greater interest in and knowledge of the health care issue. The authors have developed laboratory and in situ assays with artificially cultured Utterbackia imbecillis to examine effects of contaminants and existing conditions in embayments and rivers on mortality and reproductive success of unionid mussels. Age sensitivity of U. imbecillis to Cd and Cu was examined using both static acute and 8-day static renewal bioassays. Both aqueous and sediment exposures demonstrated greater sensitivity to Cd than Cu. The development of crud removal technology has started with the finding of the resin aging effect that an old ion exchange resin, aged by long year of use in the condensate demineralizer, had an enhanced crud removal capability. It was confirmed that some physical properties such as specific surface area and water retention capacity were increased due to degradation caused by long year of contact with active oxygens in the condensate water. So, it was speculated that those degradation in the resin matrix enhanced the adsorption of crud particulate onto the resin surface, hence the crud removal capability. Hystreosalpingogram of 375 patients with infertility were reviewed with reference to radiological feature of each pathology and a comparative study of hysterosalpingography and laparoscopy in 36 patients was presented. This results were as follows;1. In 375 cases of infertility, 191 cases (50.9%) were primary infertility and 181 cases (49.1%) were secondary infertility. 2. Simultaneous measurements at Whiteface Mountain, New York, in July 1975 show that the maximum "7Be concentrations are accompanied by increased ozone concentrations. Peaks in "7Be concentrations occurred on July 5-6, 11-12, 16-17, 23, and 27. Ozone peaks were observed on July 7-9, 11, 18, 24, and 27. Isentropic trajectory calculations also showed that the trajectories reaching Whiteface Mountain on July 11-12, 15-16, 23, and 27 had stratospheric origin. One-day delay in ozone peaks on July 7-9, 18, and 24 is attributed to increased tropospheric ozone production. Hereby we present the first known case of APD-associated peritonitis caused by non-type able H. influenzae (NTHi) presenting the beta-lactamase negative, ampicillin-resistant (BLNAR) phenotype. An 18 year old boy who had been treated with the APD for 12 months due to SLE was admitted in good general condition with diagnosis of PDP. Standard diagnostic and therapeutical procedures were initiated. Dialysis fluid was turbid with cytosis of 435 WBC/ml. From dialysis fluid pure culture of Gram-negative coccobacillus was isolated. This presentation provided a geological review of the region as well as estimates of the amounts of oil in place. A simplified geological cross-section of the formation was presented, as well as a chart of the Anticosti Basin of western Newfoundland. Details of exploration wells targeting the Cambro-Ordovician carbonate platform and current west coast land holdings were provided. An outline of the area's petroleum system included details of source rock, reservoirs, traps and seals and migration and timing. A 36-year-old woman underwent amniocentesis for advanced maternal age. The fetal karyotype had an extra dark staining G band on the long arm of chromosome 11 with no other identifiable abnormalities. FISH studies using a chromosome 11 paint probe confirmed the origin of the extra band. The abnormality was identified as a partial duplication of 11q: 46,XX dir dup (11)(q13.5q21) or (q21q23.1). The specific duplicated band could not be identified with certainty. Detailed fetal sonograms were normal. Family studies revealed the identical duplication in the mother but normal karyotypes in both maternal grandparents. The mother had strabismus and a short tongue frenulum which required surgical correction. Menses occurred late in adolescence and complete development of secondary sexual characteristics was delayed until adulthood. Ovarian function was evaluated in 11 women 16 to 43 years of age at treatment who received doxorubicin, cyclophosphamide, and high doses of methotrexate with or without radiotherapy in adjuvant therapy of soft tissue sarcoma. Five women (16-33 yr old) who received chemotherapy alone or combined with radiotherapy only at sites distant from the ovaries (chest wall, thigh, and leg) had minimal menstrual irregularities or temporary cessation of menses during therapy; cyclic menses returned promptly after therapy. This study compared rCBF pattern in normal adult and normal children using statistical parametric mapping (SPM). The purpose of this study was to determine distribution pattern not seen visual analysis in both groups. Tc-99m ECD brain SPECT was performed in 12 normal adults (M:F=11:1, average age 35 year old) and 6 normal control children (M:F=4:2, 10.5{+-}3.1y) who visited psychiatry clinic to evaluate ADHD. Their brain SPECT revealed normal rCBF pattern in visual analysis and they were diagnosed clinically normal. The comprehensive safeguards system of the International Atomic Energy Agency (IAEA) has been one of the main tools in the fight against nuclear proliferation since the entry-into-force of the Nuclear Non-proliferation Treaty three decades ago. In the 1990s some of the inherent weaknesses of this so-called traditional safeguards system were revealed first in Iraq and then in North Korea. The on-site treatment and disposal of replaced heads was thereafter implemented. This is the first time in China for large size radioactive article disposal. To guarantee the safety of old head treatment, issues of radiation protection have to be taken into account. Dose rate distribution of the old head surface and radioactivity were initially calculated according to the operation history and the neutron flux rate of the reactor. Patients with heterotaxy syndrome often have complex cardiac and extracardiac anomalies requiring further detailed diagnostic evaluation. They often present severe cardiac failure early in life. Newer radiological modalities in the form of spiral computed tomography (CT) and three-dimensional reconstruction of spiral CT allow clear definition of the anatomy of these anomalies. A 59-year-old woman was diagnosed with polysplenia and multiple anomalies in an abdominal ultrasonography (US) during a control medical examination due to a trivial dietary mistake. She was then referred to our institution for further examination of these anomalies and an additional thoraco-abdominal computed tomography (CT) examination. We tested the hypothesis that overstorey of eucalypt forest dominated by tall, large diameter trees uses less water than regrowth stands in the high rainfall zone (>1100mmyear^-^1) of the northern jarrah (Eucalyptus marginata) forest in southwestern Australia. We measured leaf area, cover, sapwood area and sapwood density at three paired old and regrowth stands. We also measured sapflow velocity at one paired stand (Dwellingup) from June 2007 to October 2008. Old stands had more basal area but less foliage cover, less leaf area and slightly thinner sapwood. A semiquantitative assay was developed and used to measure the effects of immune responses against 16 independent non-H-2 antigenic loci on erythropoietic stem cells. The assay compares repopulation in genetically anemic WBB6F1-W/Wv recipients that have normal immune responses, and in lethally irradiated WBB6F1 +/+ mice whose immune responses are suppressed by the irradiation. The differences in repopulating ability between these two types of recipients measure how immune responses affect erythropoietic stem cells. Stem cell repopulating abilities for the cells with antigens specified by the Thy-1, H-1, H-24, Ly-1, H-37, and H-17 loci were affected slightly, if at all. Repopulating abilities were moderately reduced by responses against antigens specified by H-15, 16, Ea-2, and Ly-2, 3 loci, and against the differences between the B6 and B10 genotypes, although marrow of these types cured W/Wv recipients. Twenty-four male weanling rats were fed either copper-adequate or -deficient diets until 9 or 11 weeks of age. Deficient rat hearts had increased mitochondria: myofibril compared to adequate rats. Eleven week old deficient rat hearts had decreased mitochondria: myofibril as the hearts increased in weight, but the larger hearts had greater myofibril volume densities. Cardiac mitochondria of deficient rats appeared vacuolated with fragmented cristae and translucent matrix. Valves from copper deficient rats appeared to have less connective tissue and were fragmented in areas. For deficient rats, heart:body weights of 9 wk old rats were negatively correlated with bicuspid valve pathology scores, whereas tricuspid valve scores from 11 wk old rats were negatively correlated with myofibril volume densities. Purpose: To report on the nephron-sparing, percutaneous ablation of a large renal cell carcinoma by combined superselective embolization and percutaneous radiofrequency ablation. Materials and Methods: A 5 cm renal cell carcinoma of a 43-year-old drug abusing male with serologically proven HIV, hepatitis B and C infection, who refused surgery, was superselectively embolized using microspheres (size: 500 - 700 {mu}m) and a platinum coil under local anesthesia. Percutaneous radiofrequency ablation using a 7F LeVeen probe (size of expanded probe tip: 40 mm) and a 200 Watt generator was performed one day after transcatheter embolization under general anesthesia. Results: The combined treatment resulted in complete destruction of the tumor without relevant damage of the surrounding healthy renal tissue. The patient was discharged 24 hours after RF ablation. Abstract The efficacy of iodine and glutaraldehyde as fish egg surface disinfectants were assessed in red porgy (Pagrus pagrus) and white sea bream (Diplodus sargus sargus) eggs, two species of interest for Mediterranean aquaculture. Iodine was effective in reducing the bacterial load of the 1-day-old eggs when applied at 50 mg L-1 for 5 min. The same concentration did not cause any significant change in hatching success or survival of the larvae for the first 5 days. Glutaraldehyde failed to reduce the bacterial load of the fish eggs at concentrations that were safe for the eggs (100 mg L-1 for 5 min), as it had a significant effect in preventing hatching of the developed embryo. Methods and Materials: Twenty-seven of the 288 patients received preoperative neoadjuvant hormonal therapy (leuprolide acetate). Abstract This paper tries to investigate the problem of memory through one of its most intriguing patterns - chiasmus - reflected in old poetry, sacred texts, philosophy and theology, visual arts, as well as biology. It aims to search for some provisory explanation of why man was able once to acquire such excellence in memorizing internally thousands of lines of poetry, whereas now memory is expelled outside the human body and mind in a mere digital file. Contrasting the so-called "wish-dream for immortality" of the contemporary post-human body, this paper takes another path and looks into some old cosmologies and visions in which chiasmus constantly emerged as an enduring cultural paradigm with ontological relevance. Stabilization/solidification of hazardous wastes is used to convert hazardous metal hydroxide waste sludge into a solid mass with better handling properties. This study investigated the pore size development of ordinary portland cement pastes containing metal hydroxide waste sludge and rice husk ash using mercury intrusion porosimetry. The effects of acre and the addition of rice husk ash on pore size development and strength were studied. It was found that the pore structures of mixes changed significantly with curing acre. The pore size shifted from 1,204 to 324 {angstrom} for 3-day old cement paste, and from 956 to 263 {angstrom} for a 7-day old sample. A reduction in pore size distribution for different curing ages was also observed in the other mixtures. From this limited study, no conclusion could be made as to any correlation between strength development and porosity. 10 refs., 6 figs., 3 tabs.

Methods for estimating particle number may be based on at least three different principles. Two of them, isolation of particles and indirect counting, have been predominant until recently. The new methods based on section pairs (disectors) are unbiased and far more efficient. The three principles and their historical developments are described, emphasizing that the newly developed principles have old roots, which were forgotten until recently. It is concluded, that 'learned societies' combined with individual enthusiasm are material for the spread and development of new ideas.

I review findings of various research groups regarding perturbative heterotic string model building in the last 12 months. Attention is given to recent studies of extra U(1)'s and local discrete symmetries (LDS's) in generic string models. Issues covered include the role of U(1)'s and LDS's in limiting proton decay, developments in classification of models containing anomalous U(1), and possible complications resulting from kinetic mixing between observable and hidden sector U(1)'s. Additionally, recent string-derived and string-inspired models are briefly reviewed. Talk Presented at SUSY '97.
