To determine the efficacy and safety of Total Dose Infusion (TDI) of low molecular weight iron dextran for the treatment of iron deficiency anemia compared to oral iron replacement during pregnancy through improvement in hemoglobin (Hb) after intervention. Non-randomized control trial. A group of 100 pregnant women with gestational age greater than 12 weeks with confirmed diagnosis of iron deficiency anemia attending the antenatal clinics were enrolled in this study. Total dose iron infusion of low molecular iron dextran was given to these patients after calculating iron deficit, in a monitored in-patient setting. Three adolescents (ages 14-17) with emotional and behavioral disorders displayed chronic disruptive behavior in their self-contained classrooms at a self-contained alternative school. A descriptive functional behavioral assessment was conducted for each student. Data from file review, structured interviews, and direct observations were used to identify the functions of their disruptive behaviors. Then, function-based interventions were systematically constructed for each student and implemented for an extended period (nearly 6 weeks) within the most problematic situation in their classrooms. These fruits have considerably lower edible quality than tree-ripened fruit. Acute vertebrobasilar occlusion (VBO) is associated with a high risk of stroke and death. Although local thrombolysis may achieve recanalization and improve outcome, mortality is still between 35% and 75%. However, without recanalization the chance of a good outcome is extremely poor, with mortality rates of 80-90%. Early treatment is a fundamental factor, but detailed studies of the exact time management of the diagnostic and interventional workflow are still lacking. Data on 18 patients were retrospectively evaluated. We compared the efficacy of timolol maleate ophthalmic gel-forming solution 0.5% QD with that of levobunolol hydrochloride 0.5% BID, as measured by change in intraocular pressure (IOP), effect on heart rate, and ocular tolerability. The study had a positive-controlled, double-masked, randomized, multicenter, 12-week, two-period (6 weeks each), crossover design. One hundred fifty-two patients with open-angle glaucoma or ocular hypertension were randomized to receive either timolol maleate gel-forming solution QD or levobunolol BID for 6 weeks, followed by a crossover to the alternate treatment. Objective: To investigate the X-ray radiation dose patients dealt with the hepatic intervention. Methods: Thermoluminescence and tissue-equivalent anthropomorphic phantom were used to measure the X-ray radiation dose in patients dealt with the hepatic intervention in 85 patients. Results: The peak skin dose in the patients who accepted hepatic intervention was from 179.95 to 1759.00 mGy, effective dose approached 29.22 mSv (male), 29.27 mSv (female). Conclusion: Hepatic intervention is now regarded as an important therapeutic method for liver cancer. However, the patients were exposed to too much X-ray radiation during interventional procedures. Some more effective protection should be taken to decrease the X-ray radiation dose accepted by patients. (authors)

Pregnant Swiss albino mice of 18 days post conception were exposed to 0.80 Gy, 0.40 Gy and 0.20 Gy of gamma rays from a Cobalt-60 source at different dose rates (.0584 Gy/min and .00091 Gy/min). Post irradiation variations in the cell population of crypts and villus of jejunum were studied in the F_1-generation at 1 day, 3 day and 1,2,4,6 and 12 weeks of post-partum age. In all the exposure groups at 1 day post-partum age, crypts show a decrease in total cells, mitotic figures and goblet cells on one hand and an increase in PNNC on the other hand in comparison to coeval controls. At this interval a decrease in the number of total cells as well as goblet cells/villus column was also noticeable. Dead cells which were prominently seen in crypts were totally absent in villi. Since the publication by the International Commission on Radiological Protection of revised recommendations which set out some general principles for intervention, some ideas have emerged which address interventions in the case of chronic exposure from contaminated lands. In the past there have been interventions proposed or carried out to rehabilitate land contaminated during the era of atmospheric nuclear weapon testing. Some of these interventions are reviewed in the context of these ideas and it is concluded that, by and large, reasonable outcomes have been achieved. The concept of a 'generic action level' (an effective dose of the order of 10 mSv#centre dot#a"-"1 to representative individuals in the critical group) may be useful in future to aid decision makers in determining whether intervention is appropriate. This prospective study describes and evaluates a surgical approach for 3D reconstruction of the posterior maxilla with autogenous mandibular bone in 16 patients (mean age 51 years). Bone blocks were harvested from the mandible and used as lateral or vertical block grafts (onlay); they were also partially milled and used for sinus elevation (inlay). In 4 cases, anorganic bovine bone was added at the periphery of the blocks. 4 months after grafting, implants were placed in a second operation and loaded after 12 weeks. This article describes the design and implementation of an online diabetes self-management intervention for a sample of inner-city African Americans with diabetes. Study participants were randomly assigned to the treatment (26) and control (21) conditions. The results indicate that treatment group participants were more likely to achieve positive outcomes in terms of lowered hemoglobin A1c and body mass index measurements than were control group members. These findings support the development of telehealth interventions to promote effective chronic disease management in medically underserved communities.

The purpose of this study was to determine the incidence of central vein stenosis and occlusion following upper extremity placement of peripherally inserted central venous catheters(PICCs) and venous ports. One hundred fifty-four patients who underwent venography of the ipsilateral central veins prior to initial and subsequent venous access device insertion were retrospectively identified. All follow-up venograms were interpreted at the time of catheter placement by one interventional radiologist over a 5-year period and compared to the findings on initial venography. Positron-emission tomography (PET) and PET/CT (computed tomography) are becoming increasingly important for diagnosis and treatment of cancer. Clinically relevant changes can sometimes be seen on PET that are not seen on other imaging modalities. However, PET is not suitable for guiding biopsy as the images are not obtained in real-time. Therefore, our research group has begun developing a concept for PET/CT-guided interventional procedures. This paper presents the rationale for this concept, outlines our research plan, and includes an initial study to evaluate the relative sensitivity of CT and PET/CT in detecting suspicious lesions. Back pain from osteoporosis is commonly related to compression fractures. The patient with vertebral compression fractures additionally suffers from an ongoing risk of recurrent fractures and postural abnormalities that can result in impaired respiratory function, leading to increased risk of morbidity. Weakened back muscles, especially the back extensor group, are felt to contribute significantly to this risk. The combination of pharmacologic and nonpharmacologic interventions, through physical activity, exercise, and modalities, are potential interventions that could be used to help reduce the pain of osteoporotic compression fractures as well as potentially reducing recurrent fracture rates.

In January 1994, a 6.7 magnitude earthquake in southern California ruptured a pipeline releasing approximately 4,200 barrels of blended San Joaquin Valley crude oil. A smaller volume entered the Santa Clara River and flowed 25 km downstream to an emergency containment dam. Ruptured water mains and chlorinated discharges from a damaged sewage treatment plant also affected water quality in the river. Quantitative injury assessment studies were initiated within days of the spill and included water/sediment chemistry, benthic macroinvertebrate community analyses and aquatic toxicity tests. Water quality values for TPH, BTEX, and chlorine ranged from nondetectable to 78 mg/l (TPH), nondetectable to 5.4 microg/l (total BTEX constituents) and nondetectable to 600 microg/l (residual chlorine) within 72 hours of the spill. Abstract An increase in chronic disease prevalence is contributing to health care cost growth and decreased quality of life in industrialized nations worldwide. Inadequate management of chronic diseases is a leading cause of hospitalizations and, thus, avoidable expenditures. In this study, we evaluated the impact of nurse-delivered care calls, the primary intervention of a proactive chronic care management (CCM) program, in a population aged 65 and older in Germany. Abstract: The clustered logrank test is a nonparametric method of significance testing for correlated survival data. Examples of its application include cluster randomized trials where groups of patients rather than individuals are randomized to either a treatment or a control intervention. We describe a SAS macro that implements the 2-sample clustered logrank test for data where the entire cluster is randomized to the same treatment group. We discuss the theory and applications behind this test as well as details of the SAS code.

The periodic mode is analyzed together with two conventional boundary handling modes for particle swarm. By providing an infinite space that comprises periodic copies of original search space, it avoids possible disorganizing of particle swarm that is induced by the undesired mutations at the boundary. The results on benchmark functions show that particle swarm with periodic mode is capable of improving the search performance significantly, by compared with that of conventional modes and other algorithms.

Purpose: To determine if involved field radiation (IF) is equivalent to standard whole bone radiation (SF) in local tumor control; to establish patterns of failure following treatment; and to determine response, event-free survival (EFS), and overall survival rates from multidisciplinary therapy in Ewing's sarcoma. Methods and Materials: Between 1983 and 1988, 184 children with Ewing's sarcoma were enrolled onto Pediatric Oncology Group 8346 (POG 8346). A total of 178 (97%) met eligibility criteria; 6 had pathology other than Ewing's sarcoma. Induction chemotherapy of cyclophosphamide/doxorubicin (adriamycin )(C/A) x 12 weeks was followed by local treatment either surgery or radiation therapy and C/A, dactinomycin, and vincristine for 50 weeks. Resection was advised for patients with small primary tumors if accomplished without functional loss. In this paper, we investigate the dynamic behaviors of a two-prey two-predator system with impulsive effect concerning biological and chemical control strategy-periodic releasing natural enemies and spraying pesticide at different fixed time. By applying the Floquet theory of linear periodic impulsive equation and small amplitude perturbation method, we prove that there exists a globally asymptotically stable two-prey-eradication periodic solution when the impulsive period is less than some critical value. The conditions for the permanence of the system are given, and meanwhile the conditions for the extinction of one of the two prey species and permanence of the remaining three species are given. Objective: To explore the interventional treatment for biliary re-stenosis after metallic stents placement in patients due to cholangiocarcinoma and evaluate its therapeutic effect. Methods: Percutaneous metallic stents placement or combined with continuously infusion arterial chemotherapeutic and chemotherapeutic embolization were performed in 12 patients with biliary re-stenosis using 12 metallic stents. Results: Once stent placement was 100% successful in all 12 cases, TBIL, ALT, GTP and AKP values 7 days postoperatively were significantly lower than that in preoperation. Jaundice was reduced satisfactorily in 12 patients. 3 patients were undergone continuously arterial chemotherapeutics infusion and chemotherapeutic embolization 4 weeks after stents placement. Acute pneumothorax is a frequent complication after percutaneous pulmonary radiofrequency (RF) ablation. In this study we present three cases showing delayed development of pneumothorax after pulmonary RF ablation in 34 patients. Our purpose is to draw attention to this delayed complication and to propose a possible approach to avoid this major complication. These three cases occurred subsequent to 44 CT-guided pulmonary RF ablation procedures (6.8%) using either internally cooled or multitined expandable RF electrodes. In two patients, the pneumothorax, being initially absent at the end of the intervention, developed without symptoms. One of these patients required chest drain placement 32 h after RF ablation, and in the second patient therapy remained conservative. This paper is to report a case of multi-ruptured complex pseudoaneurysm confirmed by three-dimensional DSA. Under three-dimensional DSA guidance, the thrombin was injected into lumens in the attempt to cure the pseudoaneurysm. As the result was unsatisfactory, fibrin sealant was used to inject into the lumens again, and good curative effect was obtained. In clinical interventional practice, the incidence of pseudoaneurysm has been steadily increasing and the lesions have become more and more complex. A method for cleaning timing optimization in a parallel steam generator system is described. The method is based on the minimization of a suitable objective function, and takes into account the load allocation on steam generators. In order to establish appropriate fouling growth models the mechanism of the particle deposition and removal on heat transfer surfaces is analyzed. The objective function is related to the short time management costs which are based on depreciation of steam generators, fuel costs and the costs of cleaning interventions. There is very little quantitative documentation of actual improvements resulting from the installation of new general ventilation systems in industrial settings. Therefore the performance of the common mixing design principle was compared to the displacement design principle by means of an intervention study in a workshop (V = 12,000 m[sup 3]), where thermoplastics were moulded. An experimental signal-response tracer gas technique was used. In terms of supplying fresh air to the zone of occupancy the displacement system was better than the mixing system by a factor of 2. In terms of the exposure level to a simulated contaminant (tracer gas) the displacement system was better by a factor 1.5-18. (author)

Lung transplantation has become an accepted option for many patients with end-stage pulmonary diseases. Anesthesia and surgery following lung transplantation may be required for various diseases that may affect both systemic organs and the transplanted graft. When a patient with a lung transplant undergoes surgery, there is the potential for interference with lung function, depending on the type of intervention and its anatomical site. Accurate preoperative evaluation, an understanding of the physiology of the transplanted lung, proper airway instrumentation, individualized management of intraoperative ventilation, and fluid balance are essential for a positive perioperative outcome.

This study examined whether cognitive behavioural therapy (CBT) could prevent the development or worsening of panic-spectrum psychopathology and anxiety symptoms in chronic obstructive pulmonary disease (COPD). 41 patients with COPD, who had undergone pulmonary rehabilitation, were randomised to either a four-session CBT intervention condition (n = 21) or a routine care condition (n = 20). Assessments were at baseline, post-intervention, and at 6-, 12- and 18-month follow-ups. Primary outcomes were the rates of panic attacks, panic disorder and anxiety symptoms. Secondary outcomes were depressive symptoms, catastrophic cognitions about breathing difficulties, disease-specific quality of life and hospital admission rates. This paper suggests how nonlinear periodic optimal control of a pumped storage plant can be realized. The control problem consists in maximization of the plant benefits over an operational period. The dynamic behavior of electrooptic bistable devices with delayed feedback is investigated theoretically and experimentally. The operation principle of the system is analyzed by the method of iterated maps. Stable, bistable, periodic, higher periodic, and chaotic solutions are discussed and realized experimentally by using an integrated Mach-Zehnder interferometer on LiNbO3 as a basic nonlinear element. Taking into account the periodic modulator characteristic, the application of this device as a simple and fast bistable and monostable multivibrator is demonstrated. In addition, the synchronization properties of the astable multivibrator are investigated.

For systems that are electronically charged, or contain dipole (or higher) moments, this artifice introduces spurious potentials due to the interactions between the system and multipole moments of its periodic images in aperiodic directions. I describe a method to properly handle the multipole moments of the electron density in electronic structure calculations using periodic boundary conditions. In this paper, we consider the prey-dependent consumption predator-prey (natural enemy-pest) models with age structure for the predator, immature and mature natural enemies are released and pesticide is applied impulsively. We prove that, when the impulsive period is no longer than some threshold, the pest-eradication solution is globally asymptotically stable, or say, the pest population can be eradicated totally. Serious complications due to coronary artery fistulae have been described. Most authors recommend early intervention at the time of diagnosis. We present a case of a fistula originating from a dilated left coronary artery and draining into the right atrium, which was diagnosed prenatally by color Doppler echocardiography. During pregnancy, the echocardiographic findings remained unchanged, and there were no signs of heart failure. After birth, the fistula was confirmed by angiography. Additionally, a persistent left superior vena cava draining into the coronary sinus and a very small ventricular septal defect were detected. The fistula was closed successfully by transcatheter coil embolization. At 17 months old the child was in good clinical condition. Prenatal diagnosis of coronary artery fistulae may be possible and may improve perinatal management and outcome. PMID:12047543

Objective: To evaluate the treatment of malignant biliary stenoses and occlusions using a new stent. Methods: In a prospective study, 25 patients with malignant obstructive jaundice were treated with SMART stents. The handling and the quality of stent expansion were documented. Stent function was assessed 2-4 days after intervention by cholangiography and laboratory tests. A follow-up was performed three months, after stent placement. Results: All lesions were treated successfully, with a total of 35 stents implanted. In 14 patients a further balloon dilatation was performed after stent placement (8-10 mm diameter/40-80 mm length). The mean serum bilirubin level decreased significantly from 11.6 mg/dl to 4.6 mg/dl after intervention (p<0.05). The follow-up showed a mean serum bilirubin level at 4.0 mg/dl. Between 1983 and 1986, 35 of 90 patients with acquired immunodeficiency syndrome (AIDS) had double-contrast esophagograms to rule out opportunistic esophagitis. The radiographs were reviewed without knowledge of the clinical or endoscopic findings. Candida esophagitis was diagnosed radiographically in 17 patients who had varying degrees of plaque formation and viral esophagitis in three who had discrete ulcers without plaques. All three patients with viral esophagitis (herpes in 2 and cytomegalo virus in one) and 15 of 17 with Candida esophagitis had endoscopic and/or clinical corroboration of the radiographic diagnosis. Thus, the authors' experience suggests that fungal and viral esophagitis can often be differentiated on double-contrast esophagography, so that appropriate antifungal or antiviral therapy can be instituted without need for endoscopic intervention.

Imaging sciences, with their inherently technical advances, are specifically influenced by the effects of paradigm shifts in medical education. The teaching of diagnostic radiology should be based on the definition of three core competencies: in vivo visualization of normal and abnormal morphology and function, diagnostic reasoning, and interventional treatment. On the basis of these goals, adequate teaching methods and e-learning tools should be implemented by focusing on case-based teaching. Active contrast material extravasation can be recognized with conventional CT scanners, though it has been considered a rare finding. Helical CT seems to increase the detection rate and especially to boost the radiologist's confidence in this diagnosis. Though active bleeding is defined in severely-injured subjects requiring urgent intervention and may be associated with findings of hypovolaemic shock, it should not be considered itself as a negative prognostic factor. Contrast extravasation is due to ongoing hemorrhage and its detection is critical for urged treatment. Accurate anatomical location permits to choose surgical management or transcatheter embolization and thus decreases time consumption for precis e bleeding site identification.

The development and installation, by Hydro-Quebec, of a computerized pressure monitoring and control system on its fluid-filled high voltage underground cable network in Montreal, was described. The average age of these underground fluid-filled cables is more than 30 years and the cost to replace the circuits is estimated to be 230 million dollars. It is, therefore, crucial to maintain at all times and under all operating conditions a positive oil pressure in order to maintain the dielectric strength of paper insulation, and to prevent air and moisture from entering the cables and accessories. The monitoring and control system is reliable, flexible, user friendly and easy to upgrade. Adolescents with emotional and behavioral disorders (EBD) often fail to master literacy skills, in part because disruptive behaviors interfere with task engagement and persistence. The theory of behavioral momentum explains the persistence of behavior in the face of changing environmental conditions. The current exploratory study examined variables related to behavioral momentum in the context of a continuous reading task. Participants were three adolescents identified with EBD who were instructional on fifth-grade material. Results indicated that when participants read a third-grade paragraph immediately before a fifth-grade paragraph, they decreased the latency to initiate reading of the fifth-grade paragraph and increased words read correctly per minute on the first 10 words of the fifth-grade paragraph. Results are discussed in terms of the theory of behavioral momentum and the nature of interventions that may be developed to increase reading persistence.

In case of a severe nuclear accident at a PWR plant, countermeasures will be initiated in the short term by authorities to reduce the consequences of the atmospheric radioactive releases on the neighbouring population. Various factors influence the level of protection afforded by countermeasures. For instance, a too late intervention would lead to a Jack of efficiency in terms of dose reduction if the actual evolution of the accident is not considered. Thus, implementation of countermeasures should be optimized. In general, the projected doses (those without countermeasure) are compared with those expected when a particular countermeasure or strategy is implemented. In this study we investigated cultural, familial, and individual beliefs that influenced international students? perceptions of therapy and help-seeking behaviors. Included were changes in perception or behavior before and after the students came to the US, and factors that influenced help-seeking behaviors. Six international students from East Asian countries where English is a foreign language, including Taiwan, Mainland China, Japan, and South Korea, were interviewed. We used semi-structured interviews and participant observations for collecting data. The findings indicated changes after students came to the US. Further discussions focus on encouraging and discouraging factors relative to seeing therapists, and culturally sensitive interventions for family therapy clinicians.

An adaptive power system stabilizer (APSS) employing a new self-optimizing pole shifting control strategy and its application to a power system are described in this paper. Based on an identified model of the system, the control is computed by an algorithm which shifts the closed-loop poles of the system to some optimal locations inside the unit circle in the z-domain to minimize a given performance criterion. With the self-optimization property, outside intervention in the controller design procedure is minimized, thus simplifying the tuning procedure during commissioning. Also, a new method of calculating the variable forgetting factor in real-time parameter identification is discussed. Studies show that the proposed APSS can provide good damping of the power system over a wide operating range and significantly improve the dynamic performance of the system.

We describe a 2 parameter family of polygon exchange transformations parameterized by points in a square. Whenever the two parameters are irrational, the polygon exchange has periodic orbits of arbitrarily large period. We show that for almost all parameters, the polygon exchange map has the property that almost every point is periodic. However, there is a dense set of irrational parameters for which this fails. By choosing parameters carefully, the measure of non-periodic points can be made arbitrarily close to full measure. These results are powered by a notion of renormalization which holds in a more general setting. Namely, we consider a renormalization of tilings arising from the Corner Percolation Model.

The primary objective was to assess the effects and clinical safety of a single high-dose of the third generation GnRH antagonist, acyline, on testicular characteristics in male dogs. Seven dogs were followed up weekly for six, 2-week periods (-2, -1, 1, 2, 3 and 4). At the end of the second period, they were given acyline (330 microg/kg sc). Responses to treatment varied among individual animals. Testicular consistency and scrotal diameter were slightly diminished (P>0.05) in Periods 1, 2, and 3. The RDMS is developed for the periodic efficient reliability analysis of the safety systems of Wolsong Unit 1. In addition, this system provides the function of analyzing the effects on safety system unavailability if the test period of a test procedure changes as well as the function of optimizing the test periods of safety-related test procedures. The RDMS can be utilized in handling the requests of the regulatory institute actively with regard to the reliability validation of safety systems.

According to biological and chemical control strategy for pest control, we investigate the dynamics of a predator-prey food chain with impulsive effect, periodic releasing natural enemies and spraying pesticide at different fixed times, by using impulsive differential equation. Choose pest birth rate r{sub 2} as control parameter, we show that there exists a stable pest-eradication periodic solution when r{sub 2} is less than some critical value r{sub 2}* and the system is permanence when r{sub 2} is larger than the critical value r{sub 2}*. By use of standard techniques of bifurcation theory, we prove that above this threshold there are periodic oscillations in prey, middle-predator and top predator. Furthermore, bifurcation diagrams have shown that there exists complexity for the pulsed system including periodic doubling cascade.

Background: Anxiety disorders are known to be common in the general population. Previous studies with adults with intellectual disabilities (IDs) report a prevalence of general anxiety disorder ranging from less than 2% to 17.4%. Little is known about associated factors in this population. This study investigates point prevalence of anxiety disorders and determines the factors independently associated with them. Methods: Information was collected with 1023 adults with IDs who participated in a large-scale, population-based study. All had a comprehensive physical and mental health assessment. The point prevalence of anxiety disorders according to different diagnostic criteria was determined, as were independently associated factors by using logistic regression analysis. Results: Three point eight per cent (95% CI = 2.7-5.2%) of the cohort had an anxiety disorder at the time of assessment. The frequency of peripheral pulmonary artery stenosis in patients after surgical repair of tetralogy of Fallot (TOF) ranges from 20 to 40%. This can be either primary or secondary to the surgical intervention. The influence of resulting lung perfusion alterations on the life quality of patients is difficult to predict. The aim of this study was to compare the utility of the diagnostic procedures in this group of patients, with particular focus on lung perfusion scintigraphy. This study comprised 104 patients who underwent repair of TOF at ages from 5 months to 25 years. The patients have been followed up for from 4.2 to 25 years. On the basis of chest X-ray peripheral pulmonary artery stenosis was suspected in 11 patients, in 12 on the basis of echocardiography examination. It is reported on examination-technical possibilities with craniocerebral Computer Tomography in the peri- and postnatal period. Some typical tomographic images from a 17 1/2 months period in our own patient material of 327 children are demonstrated. We report the of the existence of RV Tauri stars in the Large Magellanic Cloud (LMC). This class of variable star has hitherto been unidentified in the Magellanic Clouds. In light and color curve behavior the RV Tauri stars appear to be an extension of the Type II Cepheids to longer periods. A single period-luminosity-color relationship is seen to describe both the Type II Cepheids and the RV Tauri stars in the LMC.

All Streptococcus pneumoniae strains isolated from paediatric clinical samples at Heraklion University General Hospital in the 10-year period 2000-2009 were tested for serotype and susceptibility to antimicrobials. Among a total of 258 strains, 159 were isolated in the 5-year period 2000-2004, before the introduction of the heptavalent pneumococcal conjugate vaccine (PCV7), and 99 in the post-PCV7 5-year period 2005-2009. The prevalence of PCV7-included serotypes decreased in the post-PCV7 period (p?=?0.0002), but an increase was observed for serotypes 7F (p?=?0.002) and 19A (p?=?0.004). Haemangiomas usually develop within the first few weeks of life, most regressing spontaneously before the age of 7 years. Some may ulcerate or compromise a vital function, in which case systemic corticosteroids, surgery or radiotherapy may be helpful. All of these treatment modalities are associated with significant morbidity. Treatment with the 585 nm flashlamp pulsed dye laser is safe and effective in the management of superficial vascular malformations. We report seven patients, under 12 months of age, who presented with proliferative haemangiomas, causing functional impairment. Ulcerated lesions were present in four patients. The patients were treated with the 585 nm pulsed dye laser (fluences 7.0-9.25 J/cm2), at intervals of 4-8 weeks. All of the lesions showed a significant reduction in size, together with improvement in the colour and integrity of the overlying skin. The synovial and bone uptake of tracer in the knees of patients with rheumatoid arthritis (RA) was quantified using "9"9Tc"m-hexamethyl propylene amine oxime-labelled leucocytes and "9"9Tc"m-methylene diphosphonate (MDP), respectively. Significant neutrophil migration and MDP uptake occurred in the knees of patients with RA irrespective of the disease duration. In all but one patient neutrophil migration was reduced after intra-articular steroid injection. The change in MDP uptake after steroid injection was variable. There was a significant correlation between the percentage reduction in neutrophil migration and pain score, while the latter correlated poorly with the change in MDP uptake. Intracranial atherosclerosis is a major cause of ischemic stroke, and depending on the studied population, it accounts for 8%-15% of all strokes that are due to cerebral atherosclerosis. The prognosis of patients with symptomatic intracranial stenoses seems to depend on the location and extent of intracranial atherosclerosis. Currently, the primary treatment in intracranial atherosclerosis is the control of vascular risk factors such as hypertension, diabetes, hypercholesterolemia, and smoking. Secondary prevention with antiplatelet therapy has been shown to reduce the risk of subsequent vascular events in patients who have suffered a recent ischemic stroke or transient ischemic attack (TIA). This article provides an historical look at how programs and practices for students with emotional or behavior disorders (E/BD) have been evaluated since 1964, leading to a codified, although not universally recognized, set of recommendations for evaluating best practices for students with E/BD set out by The Peacock Hill Working Group (1991). The authors contend that, in addition to the programmatic features of best practice, the addition of the quality indicators and standards for research in special education add a critical dimension for examining the quality of the scientific research of identified best practices. The mortality in undrained abdominal abscesses is high with a mortality rate ranging between 45 and 100%. The outcome in abdominal abscesses, however, has improved due to advances in image guided percutaneous interventional techniques. The main indications for the catheter drainage include treatment or palliation of sepsis associated with an infected fluid collection, and alleviation of the symptoms that may be caused by fluid collections by virtue of their size, like pancreatic pseudocele or lymphocele. The single liver abscesses may be drained with ultrasound guidance only, whereas the multiple abscesses usually require computed tomography (CT) guidance and placement of multiple catheters. The pancreatic abscesses are generally drained routinely and urgently. Conventional film-screen radiography and digital radiography are very different ways of imaging. For digital radiography specific post-processing is the link between imaging conditions and film documentation. Optimisation of the images includes new possibilities of post-processing and a broad range for variation of the dose. Various pollution abatement measures available to combat oil spills were reviewed with emphasis on biodegradation. Microorganisms naturally found in soils and groundwater can efficiently degrade a variety of organic contaminants. For certain sites, local temperature allows the microorganisms to reduce the contaminant concentrations without human intervention. In April 1994, a diesel fuel leak was found at a telecommunication station located in a park. The leak had already contaminated the surrounding soil. The first step of the clean-up procedure was to excavate the contaminated soil, followed by an evaluation to determine the extent of the remaining contamination. It was concluded that by using natural attenuation the deforestation of the remaining affected area could be prevented. In this instance, the natural attenuation process is expected to take five years for completion. 4 refs., 1 tab., 1 fig.

135 patients with histologically proven Ewing's sarcoma, treated at the Institute Gustave Roussy between 1960-1974 were reviewed in order to establish the effectiveness and the complications associated with radiotherapy. In 133 patients, a dose of at least 60 Gy for long bones, 55 Gy for flat bones and 45 Gy for vertebrae was given. 26 major complications (of which more than one occured in certain patients) were observed: 12 cases of growth retardation, loss of joint function in 7 cases, 3 fractures, 2 osteonecroses, 2 cases of delayed healing, one massive soft tissue fibrosis, 1 abdominocutaneous fistula, 2 cases of symptomatic pulmonary fibrosis with associated infection, 2 cases of radiation enteropathy, 2 of mammary hypoplasia and 1 osteosarcoma developing in an irradiated zone. Thirty-five patients with temporal bone fractures were examined; the fractures were sometimes associated with dislocation of the assicular chain caused by road fatalities. Computed Tomography (CT) was performed either because of the presence of clinical symptoms associated with trauma of the temporal bone, because of a hemotympanum discovered during a CT scan of the brain. Thirty-three fractures were detected: 19 longitudinal, 6 transverse, and 8 complex. An incudostapedial dislocation was also detected, together with a displacement of a stapedial prosthesis from the lenticular process of incus, and 3 incus-malleus dislocation associated with fractures. Childhood anxiety is impairing and associated with later emotional disorders. Studying risk factors for child anxiety may allow earlier identification of at-risk children for prevention efforts. This study applied an ecological risk model to address how early childhood anxiety symptoms, child temperament, maternal anxiety and depression symptoms, violence exposure, and sociodemographic risk factors predict school-aged anxiety symptoms. This longitudinal, prospective study was conducted in a representative birth cohort (n = 1109). Structural equation modeling was used to examine hypothesized associations between risk factors measured in toddlerhood/preschool (age = 3.0 years) and anxiety symptoms measured in kindergarten (age = 6.0 years) and second grade (age = 8.0 years). Early child risk factors (anxiety symptoms and temperament) emerged as the most robust predictor for both parent-and child-reported anxiety outcomes and mediated the effects of maternal and family risk factors. ...

The social work literature is replete with studies evaluating social workers' direct practice interventions, but strikingly few have assessed how well social workers are faring in the political arena. This study tests a major theoretical model, the civic voluntarism model, developed to explain why some citizens become involved in politics, whereas others do not. The study sample consisted of 396 randomly selected social workers licensed in 11 states, all of whom completed a 25-minute telephone survey. Social workers were surveyed to determine the role of the following variables in explaining social workers' political activity levels--resources needed to participate, psychological engagement, and attachment to recruitment networks. The results indicate that the civic voluntarism model was significant and accounted for 42 percent of the variance. We investigate the variation of the out-of-phase periodic rhythm produced by two chaotic neurons {bold (}Hindmarsh-Rose neurons [J. L. Hindmarsh and R. M. Rose, Proc. R. Soc. London B {bold 221}, 87 (1984)]{bold )} coupled by electrical and reciprocally synaptic connections. The exploration of a two-parametric bifurcation diagram, as a function of the strength of the electrical and inhibitory coupling, reveals that the periodic rhythms associated to the limit cycles bounded by saddle-node bifurcations, undergo a strong variation as a function of small changes of electrical coupling. We found that there is a scaling law for the bifurcations of the limit cycles as a function of the strength of both couplings. From the functional point of view of this mixed typed of coupling, the small variation of electrical coupling provides a high sensitivity for period regulation inside the regime of out-of-phase synchronization. ...

Radon concentration measurements at 50 locations in the monitoring network at Canonsburg, Pa., are reported for the quarterly period of April 29 to July 28, 1980. In addition, a summary of all data from the date of initiation of the network (covering the period January 29 to July 28, 1980) is presented for comparison purposes. Average background in the Canonsburg area for the quarterly period was 0.21 pCi/liter with a range of 0.16 to 0.26 pCi/liter. This compares with an average for the first quarter of 0.18 pCi/liter and a range of 0.12 to 0.23 pCi/liter. Concentrations ranging from 0.27 to 0.75 pCi/liter at 17 locations exceeded the background range. These locations are predominantly downwind of the site in an easterly direction. A similar pattern is evident from the summary of data for the first two quarters.

The disrupted magnetic braking theory for the period gap of cataclysmic variable systems is used to study the binary evolution of low-mass main-sequence-like stars with white dwarf companions. The model is able to reproduce the observed location and width of the gap provided that the average mass transfer rates above the upper edge of the gap are greater than about 1.9 x 10 to the -9th solar masses/yr. For the case of angular momentum loss by magnetic braking, the slope of the mass transfer rate with respect to orbital period is shown to range from 3.4 to 3.7. For the evolutionary sequences considered, the He-3 abundance at the surface of the secondary exceeds 0.0015 after the complete mixing phase, resulting in modifications in the nuclear burning development of nova explosions. 31 refs.

This study estimates the electricity demand function for the residential sector of South Korea with the aim of examining the effects of improved energy efficiency, structural factors and household lifestyles on electricity consumption. In the study, time series data for the period from 1973 to 2007 is used in a structural time series model to estimate the long-term price and income elasticities and annual growth of underlying energy demand trend (UEDT) at the end of the estimation period. The result shows a long-term income elasticity of 1.33 and a long-term price elasticity of -0.27% with -0.93% as the percentage growth of UEDT at the end of the estimation period. This result suggests that, in order to encourage energy efficiency in the residential sector, the government should complement the market based pricing policies with non-market policies such as minimum energy efficiency standards and public enlightenment. ...

Six different groups of non-smoking young male subjects were studied separately for 18 consecutive days each in a closed controlled-environmental human exposure chamber. Each group was subjected to a 5-day control period in fresh air followed successively by an 8-day period of continuous exposure to 50 ppm, 15 ppm or 0 ppm (control) by volume of carbon monoxide (CO) in air, and a 5-day recovery period in fresh air. The subjects performed a 1-h auditory vigilance task every day at the same time of day in a fixed qualitative, quantitative, and temporal relationship with food intake, consumption of stimulating beverages, physical activity, and sleep. It was concluded that such CO exposure, involving the continuous carriage of carboxyhaemoglobin loads up to 7%, was without significant effect on auditory vigilance.

The purpose of this paper is to report the personal experience with helical CT evaluation of hepatocellular carcinoma treated with various percutaneous interventional procedures. From December 1996 to September 1998 it were examined with helical CT 41 patients (73 nodules in all) with hepatocellular carcinoma treated with percutaneous ablation therapies: conventional ethanol injection in 18 subjects (31 nodules), one-shot ethanol injection 3 (8 nodules), radiofrequency thermal ablation in 16 (25 nodules), and combined chemo embolization and ethanol injection in 4 (9 nodules). CT performed was 4-27 days after the last session, acquiring biphasic volumetric images in 14 patients and triphasic volumetric images in 27. A second treatment with subsequent CT study was performed for 28 lesions; 15 underwent 3 serial studies and 6 underwent 4 studies.
