Objective: Analyse the difference in school careers and secondary school qualification levels between unilateral hearing aid users and bilateral hearing aid users. Study design: Retrospective questionnaire study. Setting: Postal-based questionnaire. Participants: Names of adults known to have been fitted with unilateral or bilateral hearing aids during childhood were retrieved. This resulted in 292 names. Participants were selected using the following criteria: availability of the medical record, presence of bilateral hearing loss, completed secondary school education, normal IQ and a minimum aided word-recognition score of 70% at 10years of age. The questionnaire was sent to 50 potential participants of whom 40 responded, resulting in two groups comprising 19 unilateral and 21 bilateral ...
Background: Some former retrospective studies have suggested that patients with Ewing's sarcoma might have a very high risk for developing secondary sarcomas if treated with radiotherapy. We have evaluated the risk of second malignancies (SM) in patients treated in the German Cooperative Ewing's Sarcoma Studies CESS 81 and CESS 86. Materials and methods: From January 1981 through June 1991, a total number of 674 patients was registered in the two multicentric Ewing's sarcoma trials CESS 81 (1981 through 1985) and CESS 86 (1986 through June 1991). The systemic treatment consisted in both studies of a four-drug-chemotherapy (VACA= vincristine, actinomycin D, cyclophosphamide and adriamycin; or VAIA= vincristine, actinomycin D, ifosfamide and adriamycin) and a total number of four courses, each lasting nine weeks, was recommended by the protocol. Local therapy was either complete surgery or surgery plus postoperative radiotherapy with 36-46Gy or definitive ...

The outcome was retrospectively examined and prognostic factors were discussed for arterial chemo-infusion therapy (ACT) through an implanted reservoir system and radiation therapy (RT) in patients with unresectable intrahepatic cholangiocarcinoma (CCC). ACT plus RT were conducted to 25 patients (mean age 61.7 y, M/F 12/13, stage III-IVB) during 10years from 1996 to 2005 with various regimens of cisplatin (CDDP), 5- fluorouracil (FU), epirubicin (FARM), mitomycin C (MMC) plus 39-50 Gy of radiation in a fractionated manner partly involving internal RT (2 cases, 15 Gy) and RT for metastatic bone (1, 25 Gy). The survival rates at 1, 2 and 3 years after ACT+RT were respectively 52.5, 43.7 and 28.1%; total efficacy (PR) was 36% (9/25) at 6 months; and significantly good prognostic factors were found to be female and absence of metastasis remote and/or lymphatic around artery. Authors concluded that ACT+RT was a meaningful ...
The radiographic appearance of Ewing's sarcoma was studied retrospectively in 22 patients who survived 5 years or longer after diagnosis and treatment. Expected changes from treatment, including regression of the extraosseous soft tissue mass, periostitis, and reconstitution of the cortex, occurred in all patients. Local recurrence occurred in one patient 10years after complete remission whereas secondary osteosarcoma occurred more than 5 years after complete remission in two other cases. Both recurrent and secondary tumors presented as new lytic foci at the site of the original primary lesion. Lytic changes from radiation (radiation osteitis) may develop more than 2 years after treatment and in this sample; such findings were widely distributed in the radiation port. The authors conclude that bone remodeling and postradiation changes occur slowly over 2 years after treatment, and that any localized lysis at the primary ...
Abstract Objective: This study aims to gain an overview of hot topics related to research focused on Chinese herbal drugs by authors inside China and outside of China in the past 10years. The aim of this study was the evaluation of contralateral hip fractures after a previous hip fracture. For this retrospectiveanalysis patients were selected from the database of the LUMC, a teaching hospital in the south-west of the Netherlands. We analyzed all patients with a second fracture of a hip between 1992 and 2007. The exclusion criteria were high impact trauma and patients with diseases or medication known to have a negative effect on bone metabolism. A total of 1,604 hip fractures were identified. The possible predictive factors for the second fracture and descriptive statistics related to surgery (Hb and HT before and after the operation, total amount of intra- and postoperative blood loss, type of osteosynthesis, complications, time of death after the last fracture, time betw...
Within about 10years, microelectronic devices will be made with more than a billion (10"9) electronic components per chip. To implement such a sophisticated technology it will be essential to have a fundamental understanding of the solid state interaction between the different materials in thin film semiconductor structures. This is the main purpose of this research program. Characterization and analysis is carried out mainly by Rutherford backscattering spectrometry and channelling using accelerated nuclear particles from the Van de Graaff accelerator, while radioactive isotopes provide information about interaction mechanisms. 6 figs., 1 ref.
The operation of pumped-storage plants (PSP) is based on the daily load variations of power demand. It performs power generation at peak load times during the day, and pumps water up into the reservoir during the off-peak times at night. In the Taipower system in Taiwan, there are two pumped storage plants with ten units. These had a total installed capacity of 2,602 MW or a 12 percent capacity at the end of 1995. The plants were able to reduce system generation costs and improve reliability. A Pumped-Storage Plant Benefit Analysis (PSPBA) program was established using the principle of economic dispatch in 1985. This paper explains the rules of the program, the analysis results of the past 10years, and a a cost benefit evaluation. It was found that the advantages of PSP are not only in supplying the power for peak load, but also in decreasing the total fuel cost. 5 figs., 4 refs.

Purpose: To identify results and prognostic factors on long-term survival and local control following treatment of localized Ewing's sarcoma. Patients and Methods: Between 1979 and 2004, a total of 60 children and young adults with Ewing's sarcoma were treated. Patients with distant metastases at presentation (n = 6) and recurrent cases (n = 2) were excluded from this analysis. Patients were exclusively treated within ongoing national and international protocols CESS-81, CESS-86, EICESS-92, EURO-EWING-99. All patients received local irradiation with a total dose of 45-60 Gy; in addition, 41 (79%) of the patients had local surgical procedures, 27 (52%) of them with clear margins. Results: Overall survival rates at 5 and 10years were 56% and 45%, respectively. Patients #<=# 14 years at diagnosis had overall survival rates of 66% and 61% at 5 and 10years compared to older patients ...
The Commissariat a l'Energie Atomique (CEA, French Atomic Energy Commission) has set up over the last 10years an innovative methodology aiming at characterizing radiological contaminations. The application of the latter relies on various tools such as expertise vehicles with impressive detection performances (VEgAS) and recently developed software platform called Kartotrak. A Geographic Information System tailored to radiological needs constitutes the heart of the platform; it is surrounded by several modules aiming at sampling optimization (Stratege), data analysis and geostatistical modeling (Krigeo), real-time monitoring (Kartotrak- RT) and validation of cleaning efficiency (Pescar). This paper presents the different tools which provide exhaustive instruments for the follow-up of decontamination projects, from doubt removal to the verification of the decontamination process. Advances in the diagnosis of Ewing's sarcoma of the rib were considered and retrospectiveanalysis of the treatment of 32 patients (76% of them were children at the age of 10-15 y.o.) in the clinic of children oncology was performed. It was shown that the diagnosis of Ewing's sarcoma of the rib in children ought to be combined taking into account the course, morphology and radiation examination techniques. Most optimum treatment consisted of combined therapy (cyclic polychemotherapy, lesion focus irradiation at the summary focus dose of 57 Gy, surgery with subsequent polychemotherapy for not less than 1.5 years
This is a retrospectiveanalysis of bone scans in 72 patients with Ewing's sarcoma. Sites of primary disease were found to be evenly distributed among the axial skeleton and the proximal and distal extremities. Primary disease sites in the axial skeleton were more frequently associated with metastases. Fifty-three of these patients had their first scans done at presentation; among them, 25 were found to have metastases. Unsuspected metastases were picked up in 28 of all the patients. Metastases at presentation were seen to be associated with a poorer prognosis.

Daya Bay NPP has been operating safely and stably over 10years since 1994, and its' seismic analysis of nuclear island was in accordance with the approaches in RCC-G standard for the model M310, in which the Simplified Impedance Matrix Method (SIMM) was employed for the consideration of SSI. Thanks to the rapid progress being made in upgrading the evaluation technology and the capability of data processing systems, methods and software tools for the SSI analysis have experienced significant development all over the world. Focused on the model of reactor building of the Daya Bay NPP, in his paper the more sophisticated 3D half-space continuum impedance method based on the Green functions is used to analyze the functions of the soil, and then the seismic responses of the coupled SSI system are calculated and compared with the corresponding design values. It demonstrates that the design method provides a ...
Purpose: During recent years, more intensified systemic and local treatment regimens have increased the 5-year survival figures in localized Ewing's sarcoma to more than 60%. There is, however, concern about the risk of second malignancies (SM) in long-term survivors. We have analyzed the second malignancies in patients treated in the German Ewing's Sarcoma Studies CESS 81 and CESS 86. Materials and Methods: From January 1981 through June 1991, 674 patients were registered in the two sequential multicentric Ewing's sarcoma trials CESS 81 (recruitment period 1981-1985) and CESS 86 (1986-1991). Univariate and matched multivariable analyses were performed. Results Overall, study population had in...
The Nuclear Data Interface (NDI) code library and data formats are the standards for multigroup nuclear data at Los Alamos National Laboratory. NDI's analysis, design, implementation, testing, integration, and maintenance required a ten person-year and ongoing effort by the Nuclear Data Team. Their efforts provide a unique, contemporary experience in producing a standard component library. In reflection upon that experience at NDI's decennial, we have identified several factors critical to NDI's success: it addressed real problems with appropriate simplicity, it fully supported all users, it added extra value through the code to the raw nuclear data, and its team went the distance from analysis through maintenance. In this report we review these critical success factors and discuss their implications for future standardization projects.
SummaryBackground The choice of sampling techniques in bronchoscopy with sampling from a visible lesion will depend on the expected diagnostic yields and the costs of the sampling techniques. Aims The aim of this study was to determine the most economical combination of sampling techniques when approaching endobronchial visible lesions. Methods A cost minimization analysis was performed. All bronchoscopies from 2003 and 2004 at Haukeland university hospital, Bergen, Norway, were reviewed retrospectively for diagnostic yields. 162 patients with endobronchial disease were included. Potential sampling techniques used were biopsy, brushing, endobronchial needle aspiration (EBNA) and washings. Costs were estimated based on registration of equipment costs and personnel costs. Sensitivity analyse...
During the past 10years, five infants with Kasabach-Merritt syndrome (K-M) receiving radiation therapy were reported. We investigated whether radiation therapy for K-M was useful and what the unfavorable factors of K-M were. During the past 10years, we have treated five infants with K-M. The syndrome occurred at ages ranging from birth to 4 months. The incidence of female to male ratio was 3:2. Among 5 cases, the site of hemangioma was as follows; shoulder, anterior chest wall, lower abdominal wall, face and neck and inguinal site. All 5 cases received medication to control the coagulopathy including prednisone and blood transfusion at first. Because the platelet count and the bleeding tendency did not improve in any case, these cases received radiation therapy. Total dose ranged from 5 to 10 Gy and fraction-size ranged from 0.5 to 1.75 Gy. Irradiation session was 2 or 3 times per week. In 5 cases, 4 ...
Stable isotopes are valuable tools for research on mineral bioavailability and metabolism. They can be used as tracers with no exposure to radiation and they do not decay over time. Attempts to use stable isotopes of minerals as metabolic tracers were first described only 25 years ago. There were relatively few reports of their use over the next 15 years, but interest in stable isotopes has expanded markedly in the last 10years. The advantages of stable isotope tracers are so great that scientists have been willing to accept the laborious and costly nature of mineral isotope analysis, and substantial progress has been made in the field. New applications for stable isotopes and new analytical methods have been introduced recently. However, limitations to the approach and methodological problems remain to be resolved. This review describes early work in the field and discusses the advantages and disadvantages of stable ...
The natural gas industry has proposed an increase in the DOE gas R&D budget from about $100 million to about $250 million per year for each of the next 10years. The proposal includes four programs: natural gas supplies, fuel cells, natural gas vehicles and stationary combustion systems. This paper is a qualitative assessment of the gas industry proposal and recommends a natural gas R&D strategy for the DOE. The methodology is a conceptual framework based on an analysis of market failures and the energy policy objectives of the DOE`s (1991) National Energy Strategy. This framework would assist the DOE in constructing an R&D portfolio that achieves energy policy objectives. The natural gas supply program is recommended to the extent that it contributes to energy price stability. Stationary combustion programs are supported on grounds of economic efficiency and environmental quality. The fuel cell program is ...
The standard protocol in multislice spiral CT (MSCT) angiography for coronary arteries with fixed tube current-time settings leads to an overexposure and thus to an unnecessary high radiation dose in patients with lower weight when compared to heavier patients. The purpose of this study was to estimate the effect of reducing the radiation dose by adapting the tube current-time settings individually. Fifty patients underwent retrospectively ECG-gated MSCT of the heart. In 25 patients (group A{sub 1}) a standard protocol with constant tube current-time settings was used (4 x 1-mm collimation, 120 kV, 400 mAs{sub eff}). Subsequently, artificial image noise was added to the data of these patients simulating a directive for weight-adapted tube current-time settings (group A{sub 2}). In the other 25 patients (group B) an alternative protocol with individually weight-adapted tube current-time settings was applied. The data of all groups were evaluated by a regression ...

Soluble TWEAK is a member of the TNF-alpha family of cytokines that has been shown to predict mortality in patients with heart failure. Pulmonary artery hypertension is a devastating disease, in which right ventricular function has been shown to be the major determinant of prognosis. In this hypothesis-generating study, we sought to evaluate the potential usefulness of sTWEAK in the prediction of disease severity in patients with pulmonary artery hypertension. We therefore conducted a retrospectiveanalysis of sTWEAK serum levels in 95 stable patients with PAH. For all patients data on hemodynamic parameters, biomarkers and functional exercise tests were available. Compared to controls, patients with PAH showed significantly decreased levels of sTWEAK [median 314?pg/ml (interquartile range...
Acute kidney injury (AKI) is a common complication after allogeneic stem cell transplantation (SCT). Although various risk factors for AKI have been reported, the influence of pretransplant comorbidity on the incidence of AKI has not been well investigated. We performed a retrospectiveanalysis of 207 consecutive patients undergoing myeloablative or nonmyeloablative SCT between 2001 and 2009, using the hematopoietic cell transplantation-specific comorbidity index (HCT-CI) as a representative of pretransplant comorbidities. According to Risk, Injury, Failure, Loss, and End-stage kidney disease (RIFLE) criteria, 158 patients (76.3%) developed AKI, and 92 patients (44.4%) developed severe AKI (RIFLE class I or class F) within 100 days after SCT. The cumulative incidence of severe AKI within 1...
Research on hematological disorders relies on suitable animal models. We retrospectively evaluated the use of the hematological parameters hematocrit (HCT), hemoglobin (HGB), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), mean corpuscular volume (MCV), red blood cell count (RBC), white blood cell count (WBC), and platelet count (PLT) in the phenotype-driven Munich N-ethyl-N-nitrosourea (ENU) mouse mutagenesis project as parameters for the generation of novel animal models for human diseases. The analysis was carried out on more than 16,000 G1 and G3 offspring of chemically mutagenized inbred C3H mice to detect dominant and recessive mutations leading to deviations in the levels of the chosen parameters. BACKGROUND.The National Breast and Cervical Cancer Early Detection Program (NBCCEDP) covers the direct clinical costs of breast and cervical cancer screening and diagnostic follow-up for medically underserved, low-income women. Personal costs are not covered. In this report, the authors estimated personal costs per woman participating in NBCCEDP mammography screening by race/ethnicity and also estimated lifetime personal costs (ages 50-74 years).METHODS.A decision analysis model was constructed and parameterized by using empiric data from a retrospective cohort survey of mammography rescreening among women ages 50 years to 64 years who participated in the NBCCEDP. Data from 1870 women were collected from 1999 to 2000. The model simulated the flow of resources incurred by a woman participat...
Purpose Suprasellar meningiomas usually present with visual deterioration, including decreased visual acuity and/or visual field defects. Suprasellar meningiomas have a close relationship with the optic apparatus, arteries of the anterior circulation, pituitary stalk and hypothalamus, which makes safe surgical resection a challenge especially with dissection around an already compromised optic apparatus. In this report 21 patients operated on for a suprasellar meningioma over a 4-year period are reviewed. Postoperative outcome and visual recovery are evaluated, including analysis of its determinants. Methods Over a 4-year period (2002?2006), patients surgically treated for suprasellar meningiomas were included in this retrospective study. All tumors were located at the tuberculum sellae an...

An important unresolved issue in outcomes analysis for lung complications is the effect of poor or completely lacking heterogeneity corrections in previously archived treatment plans. To estimate this effect, we developed a novel method based on Monte Carlo (MC) dose calculations which can be applied retrospectively to RTOG/AAPM-style archived treatment plans (ATP). We applied this method to 218 archived nonsmall cell lung cancer lung treatment plans that were originally calculated either without heterogeneity corrections or with primitive corrections. To retrospectively specify beam weights and wedges, beams were broken into Monte Carlo-generated beamlets, simulated using the VMC++ code, and mathematical optimization was used to match the archived water-based dose distributions. The derived beam weights (and any wedge effects) were then applied to Monte Carlo beamlets regenerated based on the patient computed tomography ...

To determine the presence, prevalence, and clinical importance of /sup 99m/Tc pertechnetate uterine uptake, this retrospectiveanalysis of 71 Meckel's scans was undertaken. Specifically, each study was evaluated for the presence of a focal accumulation of radiotracer cephalad to the bladder. Patients received an intravenous dose of 150 microCi/kg of /sup 99m/Tc pertechnetate. Each study consisted of 15 one minute anterior serial gamma camera images, and a 15, 30, and 60 minute anterior, right lateral and posterior scintiscan. Menstrual histories were obtained from all patients except two. No males (33/33), nor premenstrual (13/13), menopausal (4/4) or posthysterectomy (2/2) patients revealed a uterine blush. Eleven of 15 patients (73%) with regular menses demonstrated a uterine blush. They were in the menstrual or secretory phases of their cycle. Four demonstrated no uterine uptake, had regular periods, but were in the proliferative ...
The aim of this study was to evaluate typical magnetic resonance imaging (MRI) findings in early rheumatic diseases manifesting at the soft tissues of the hand using a retrospectiveanalysis. A total of 186 MRI examinations of patients with clinical suspicion of a rheumatic disease were evaluated in a consensus reading by two experienced radiologists. All imaging patterns were assessed with respect to their type and localization. Under blinded and non-blinded conditions diagnoses were correlated with final clinical diagnosis. The most frequent diagnoses were rheumatoid arthritis (RA, 45.7%) and psoriatic arthritis (PsA, 15.6%). The mean correlation between clinical and MRI diagnosis (r) was 0.75 in blinded and 0.853 in non-blinded reading (p <0.001). The following extra-articular imaging patterns were found: synovitis (59.1%), tendovaginitis (91.4%), dactylitis (14.5%), and bone marrow edema (18.3%). Only dactylitis was specific for a ...
Two most important causes of portal hypertension are cirrhosis of liver and non-cirrhotic portal fibrosis (NCPF). The purpose of this study was to assess the scintigraphic appearances of "9"9"mTc sulfur colloid liver scan in differentiating liver cirrhosis from NCPF. Materials and Methods: Retrospectiveanalysis records of 146 patients (91 male and 55 female) with diffuse hepatocellular disease was done for liver size, liver uptake, spleen size, spleen uptake, colloid shift to bone marrow and lungs. Results: Cirrhotic livers showed patchy and lower uptake than NCPF (59% vs. 20%, P-value 0.041). Spleen size was significantly increased in NCPF compared to cirrhosis (100% vs. 67%, P-value 0.0137). Significant colloid shift to bone marrow was noted in cirrhosis (84% vs. 7%, P-value<0.0001). Conclusion: "9"9"mTc sulfur colloid liver scan is a non-invasive procedure having a useful adjunctive role in clinical differentiation of cirrhosis from ...
Purpose: The aim of this study was to demonstrate the typical appearance of ganglioneuromas in computer-assisted tomography (CT), and magnetic resonance imaging (MRI). Material and methods: Retrospectiveanalysis of diagnostic imaging (9 CT, 6 MRI) in 9 children aged 3 to 15 years with the histological diagnosis of ganglioneuroma. Results: The tomographies showed large (max. 13.4 cm in diameter) round or oval tumors with sharp delineation. The sites of the tumors were the retroperitoneum (5), the mediastinum (3), and the adrenal gland (1). Intraspinal tumor involvement occurred in 4 cases. On comparing CT with MRI, MRI was more accurate in defining the intraspinal involvement. The ganglioneuromas were of hypodense appearance in the native CT scan and showed moderate enhancement upon administration of contrast media. In five patients tumor calcifications with a disseminated sprinkled pattern were seen in CT. In MRI T_1-weighted scans the tumors ...
Tuberous sclerosis complex (TSC) is an inherited disorder characterized by the presence of cortical tubers, the majority of which are solid and show high signal on FLAIR images. Low-signal tubers are less frequent. To evaluate cystic cortical tubers in patients with TSC on the basis of their appearance on FLAIR images. MR examinations of 73 patients were retrospectively reviewed and 17 children (aged 25 days to 12.3 years) with a typical cyst-like tubers were selected for further analysis. The age of the patients, the number of lesions, and the shape and MR intensity were analyzed. Cyst-like cortical tubers were detected in 82% of children below 7 years of age and in 18% of those older then 7 years (P=0.00086). Most of the cystic lesions were of the shrunken type (72%), the expansile type being less frequently seen (28%). FLAIR images confirm the cystic character of some of the cortical tubers in TSC patients although the pathogenesis of cystic ...
OBJECTIVE:- Tumor necrosis factor (TNF) inhibitors can be used to treat inflammatory bowel disease (IBD) but may lead to anti-nuclear antibody (ANA) positivity and lupus-like reactions. Because of its unique structure, certolizumab has lower rates of these complications. We sought to investigate whether patients who have had lupus-like reactions to infliximab or adalimumab would be able to tolerate certolizumab. METHODS:- We performed a retrospectiveanalysis on the 23 patients at the Roberts Inflammatory Bowel Disease Center who received certolizumab for the treatment of Crohn's disease from March 2008 to June 2009. We identified 6 patients who were switched to this drug because of lupus-like reactions from prior anti-TNF therapy and had documented ANA after the reaction and prior to cert...

Sulfur K-edge X-ray absorption spectroscopy is a powerful in situ probe of sulfur biochemistry in intact cells and tissues. Under favorable circumstances the technique can provide quantitative information on the chemical identify of the sulfur species that are present in a sample. Prange et al. have recently reported an X-ray absorption spectroscopic study of bacterial sulfur storage globules. Unfortunately there are substantial problems with the experimental technique employed that, they contend, lead to completely erroneous conclusions. In the more recent of their two papers Prange et al. employed a curve-fitting method similar to that used by us (for more than 10years). In essence, the method employs simply fitting a linear combination of the spectra of standard compounds to that of the unknown, in this case cultures of bacterial cells. This type of analysis can provide quantitative estimates of the individual sulfur ...
BACKGROUND: Denys-Drash Syndrome (DDS) is an uncommon disorder that appears sporadically and in rare cases may be inherited as an autosomal dominant trait It manifests either at birth or within the first year of life and typically consists of the triad of congenital nephropathy, Wilms tumour and intersex disorder. CASE REPORT: A 10year-old Caucasian girl was referred to the Dental Department, at Glasgow Royal Hospital for Sick Children by her Paediatric Nephrologist Consultant. The patient was being teased by her peers over her markedly discoloured teeth. The dental history revealed that the patient was a regular dental attendee from an early age. She was dentally anxious having only experienced dental treatment under general anaesthesia (GA) when she was 4 years old. Apparently her primary dentition also showed a generalised discolouration. TREATMENT: This consisted of multiple visits for diet analysis and tooth brushing ...

To evaluate the predictive factors for treatment response and prognostic factors affecting survival outcomes after concurrent chemoradiotherapy (CCRT) for patients with anal squamous cell carcinoma. Medical records of forty two patients with histologically confirmed analsquamous cell carcinoma, who had complete CCRT between 1993 and 2008, were reviewed retrospectively. Median age was 61.5 years (39-89 years), and median radiotherapy (RT) dose was 50.4 Gy (30.0-64.0 Gy). A total of 36 patients had equal to or less than T2 stage (85.7%). Fourteen patients (33.3%) showed regional nodal metastasis, 36 patients (85.7%) were treated with 5-fluorouracil (5-FU) plus mitomycin, and the remaining patients were treated by 5-FU plus cisplatinum. The median follow--up time was 62 months (2-202 months). The 5-year overall survival, locoregional relapse-free survival, disease-free survival, and colostomy-free survival rates were 86.0%, 71.7%, 71.7%, 78.2%, respectively. Regarding ...
Purpose: Treatment results and the pattern of relapse were evaluated in the multimodal treatment of Ewing's sarcomas of the chest wall. Methods and Materials: In a retrospectiveanalysis, 114 patients with non-metastatic Ewing's sarcoma of the chest wall were evaluated. They were treated in the CESS 81, CESS 86, or EICESS 92 studies between January 1981 and December 1993. The treatment consisted of polychemotherapy (VACA, VAIA, or EVAIA) and local therapy, either surgery alone (14 patients), radiotherapy alone (28 patients) or a combination of both (71 patients). The median follow-up was 46.6 months (range 5-170). A relapse analysis for all patients with local or combined relapses was performed. Results: Overall survival was 60% after 5 years, event-free survival was 50%. Thirty-seven patients had a systemic relapse (32.4%), 11 patients had a local relapse alone (9.6%), and 3 patients had a combined local and systemic ...
Purpose: To examine the role of brachytherapy in the treatment of cholangiocarcinomas in a relatively large group of patients. Methods and Materials: Using the Surveillance, Epidemiology and End Results database, a total of 193 patients with cholangiocarcinoma treated with brachytherapy were identified for the period 1988-2003. The primary analysis compared patients treated with brachytherapy (with or without external-beam radiation) with those who did not receive radiation. To try to account for confounding variables, propensity score and sensitivity analyses were used. Results: There was a significant difference between patients who received radiation (n = 193) and those who did not (n = 6859) with regard to surgery (p < 0.0001), race (p < 0.0001), stage (p < 0.0001), and year of diagnosis (p <0.0001). Median survival for patients treated with brachytherapy was 11 months (95% confidence interval [CI] 9-13 months), compared with 4 months for patients ...
Purpose: To conduct a retrospectiveanalysis within a large university teaching hospital, comparing outcomes between patients receiving either radical surgery or radiotherapy as curative treatment for bladder cancer. Patients and Methods: Between March 1996 and December 2000, 169 patients were treated radically for muscle-invasive bladder cancer. Data were collected from patient notes. Statistical analyses were performed using Kaplan-Meier methods and Cox proportional hazards regression analysis to compare radiotherapy and surgical outcome data. Results: There was no difference in overall, cause-specific, and distant recurrence-free survival at 5 years between the two groups, despite the radiotherapy group being older (median age, 75.3 years vs. 68.2 years). There were 31 local bladder recurrences in the radiotherapy group (24 solitary), but there was no significant difference in distant recurrence-free survival. In a more ...
Purpose: To evaluate the role of stereotactic radiosurgery (SRS) and whole-brain radiotherapy (WBRT) for the treatment of brain metastases in patients with renal cell cancer (RCC). Patients and methods: 88 patients were treated with either SRS (n = 51) or SRS + WBRT (n = 17) for one to three lesions, or with WBRT (n = 20) for more than three brain metastases. Overall survival (OS), intracerebral control (IC) and local control (LC) were retrospectively analyzed. Six potential prognostic factors were assessed: age, gender, number of brain metastases, extracerebral metastases, recursive partitioning analysis (RPA) class, and interval from tumor diagnosis to irradiation. Results: The median times for OS, IC, and LC from the time of diagnosis were 11, 9, and 10 months. The median OS times for SRS, SRS + WBRT, and WBRT were 12, 16, and 2 months. Addition of WBRT to the SRS improved IC (p = 0.032) but not OS (p = 0.703). On multivariate analyses, ...
Purpose: to compare several morphometric parameters in MRI with the functional status of the articular disc in a large patient group suffering from internal derangement (ID) of the temporomandibular joint (TMJ). Materials and methods: in a retrospective study, 320 analyzable high resolution MRI examinations of the TMJs obtained in a 1.5 T unit were evaluated in 184 patients with clinically suspected ID. The analysis included the anatomical structures and a number of morphometric parameters previously described in the literature. The parameters were compared with the position of the articular disc. Results: the disc position was categorized as ''normal'' (NDP, 21.9%, n = 70), ''anterior displacement with reduction on opening'' (AMR, 51.6%, n = 165) and ''anterior displacement without reduction on opening'' (AOR, 26.6%, n = 85). Materials and methods: A 5-day workshop with 12 theoretical and 9 'hands-on' modules was designed and stepwise improved, according to the trainees' feedback. All trainees were trained in small groups (6 to 10 participants per workshop). The workshops consisted of mini-lectures, repeated micro teaching exercises and video-supported feedback concerning the following key-competencies: Communication of goals; methods to trigger interactivity; design of slides in power point presentations; effective feedback-techniques; and use of media, time-management, skills teaching, assessment methods (e.g. OSCE and others), evaluation and general presentation skills. The evaluation was based on two components: (A) trainees' scores in two objective structured ...
Purpose: Extrahepatic cholangiocarcinoma is a rare malignancy. Despite radical resection, survival remains poor, with high rates of local and distant failure. To clarify the role of radiotherapy with chemotherapy, we performed a retrospectiveanalysis of resected patients who had undergone chemoradiotherapy. Methods and Materials: A total of 45 patients (13 with proximal and 32 with distal disease) underwent resection plus radiotherapy (median dose, 50.4 Gy). All but 1 patient received concurrent fluoropyrimidine-based chemotherapy. The median follow-up was 30 months for all patients and 40 months for survivors. Results: Of the 45 patients, 33 underwent adjuvant radiotherapy, and 12 were treated neoadjuvantly. The 5-year actuarial overall survival, disease-free survival, metastasis-free survival, and locoregional control rates were 33%, 37%, 42%, and 78%, respectively. The median survival was 34 months. No patient died perioperatively. Patient ...

An epidemiological study is under way in the Schneeberg/Schlema region where radon concentration in dwellings were found to vary between 10 Bq/m"3 and 10 kBq/m"3. For several reasons, indoor radon levels are expected to have considerably varied over the past decades. To take these potential fluctuations into account, indoor radon concentration measurements should be combined with retrospective measurements. In this way, possible outliners in the epidemiological study could be sorted out. (A.K.)
Surgical resection is the only curative treatment strategy for intrahepatic cholangiocarcinoma (CC). Therefore, accurate staging is essential for appropriate management of patients with CC. We assessed the usefulness of 2-["1"8F]fluoro-2-deoxy-d-glucose (FDG) positron emission tomography (PET) in the staging of CC. We undertook a retrospective review of FDG PET images in 21 patients (10 female, 11 male; mean age 57 years) diagnosed with CC. Ten patients had hilar CC and 11, peripheral CC. Patients underwent abdominal magnetic resonance imaging (MRI) (n=20) and computed tomography (CT) (n=12) for the evaluation of primary tumours, and chest radiography and whole-body bone scintigraphy for work-up of distant metastases. For semi-quantitative analysis, the maximum voxel standardised uptake value (SUV_m_a_x) was obtained from the primary tumour. All peripheral CCs showed intensely increased FDG uptake, and some demonstrated ring-shaped uptake ...
The aim of this retrospective study was to determine the safety and efficacy of chemoembolization (TACE) as palliative treatment for patients with unresectable intrahepatic cholangiocarcinoma (CCA) and to compare the results with those in the literature. Fifteen patients with histology-proven CCA (5 men, 10 women) had received palliative treatment with TACE over a 6-year period. The treatment protocol comprised repeated TACE at a minimum of 8-week intervals. TACE was performed with a mixture of 10 ml Lipiodol and 10 mg mitomycin C injected into the tumor-supplying vessels. Follow-up investigations after 8-10 weeks comprised contrast-enhanced multislice spiral CT and laboratory control. Statistical evaluation included survival analysis using the Kaplan-Meier method. During the investigation period 58 TACEs (3.9 #+-# 3.8; 1-15) were performed in 15 patients. Mean tumor size was 10.8 #+-# 4.6 cm (range, 2.0-18.0 cm). Unifocal tumor disease was ...
The aim of the study was the attempt to evaluate the influence of two different methods of cardiac perfusion SPECT reconstruction (FBP and ITW) on clinical efficacy in diagnosing the coronary artery disease as well as the cardiac ischemia detection in three areas of heart vascularized by main coronary arteries: LAD, LCX and RCA with the use of artificial neural networks (ANN). The study was performed retrospectively with the use of the diagnostic image records as well as clinical dataset of 43 patients. Myocardial perfusion stress/rest SPECT study and X-ray coronarography data were evaluated for each patient. The results of coronary angiography were considered the reference method. The cardiac SPECT data were reconstructed using the two different methods: filtered backprojection (FBP) and iterative Wallis method (ITW). The local perfusion deficits denominated in stress and rest study in three main vessel cardiac segments were the main input values for the ANN. The ...
Objective: To retrospectively evaluate the results of conservative therapy using escorpal iodine-125 (model 6711, Amershan) plaque for brachytherapy of choroidal melanoma. Materials And Methods: We evaluated 49 patients with choroidal melanoma treated at 'Hospital do Cancer', Sao Paulo, Brazil, from March 2001 to January 2003. The following clinical parameters were analyzed and correlated with local control and survival rate: gender, age, lesion dimension, treatment duration, as well as doses in apex and base of the tumors. Results: The maximum diameter and height of the base of the tumors treated were 17 mm and 12 mm, respectively. Doses at those points ranged from 213 to 463 Gy (median: 347 Gy) and 51 to 250 Gy (median: 91 Gy), respectively. The actuarial rates of ocular preservation, disease free survival and ocular preservation in 2-years were 96%, 93.5% and 96.3%, respectively. A tumor height < 6 mm was the only predictive factor for ...
(1) To determine the accuracy of computed tomography (CT) in the evaluation of ligament tear and avulsion in patients with tibial plateau fracture. (2) To evaluate whether the presence or severity of fracture gap and articular depression can predict meniscal injury. A fellowship-trained musculoskeletal radiologist retrospectively reviewed knee CT and MRI examinations of 41 consecutive patients presenting to a level 1 trauma center with tibial plateau fractures. Fracture gap, articular depression, ligament tear and footprint avulsions were assessed on CT examinations. The MRI studies were examined for osseous and soft tissue injuries, including meniscal tear, meniscal displacement, ligament tear, and ligament avulsion. CT demonstrated torn ligaments with 80% sensitivity and 98% specificity. Only 2% of ligaments deemed intact on careful CT evaluation had partial or complete tears on MRI. Although the degree of fracture gap and articular depression was significantly ...
The aim of this retrospectiveanalysis was to evaluate feasibility and effectiveness of definitive chemoradiotherapy without split-course technique in anal cancer patients. From 1993 to 2003, 81 patients were treated; 13 were excluded due to various chemotherapeutic regimes, thus 68 patients were analysed. In case of acute grade 3 toxicities, treatment was halted until improvement or resolution independent of dose. Short interruption was defined as completing treatment without exceeding eight cumulative treatment days beyond scheduled plan, other patients were considered to have had prolonged interruption. Median follow-up was 46 months. Median overall treatment time was 53 days corresponding to an interruption of eight cumulative treatment days. Thirty-five patients (51%) had treatment interruption of <8 days. No acute grade 4 toxicities were observed; one fatality occurred during treatment due to ileus-like symptoms according to acute ...
We conducted a retrospective review of the North American Renal Transplant Cooperative Study (NAPRTCS) Registry transplant and dialysis arms to assess anemia and growth patterns in children who returned to dialysis after a failed renal transplant from January 1, 1992 to February 3, 2004. Of the 1807 potential study subjects, 1451 had transplant removal data (TxIn vs. TxOut) available for analysis. Four hundred and twenty-one of 1451 patients (29%) had a transplant nephrectomy at the time of entry into the NAPRTCS Registry Dialysis arm. Anemia rates steadily decreased from 72.2% at 30 days after dialysis initiation to 59.5% at 12 months after dialysis initiation. Factors associated with anemia at 30 days after dialysis initiation included hemodialysis, lack of Epo use, and patients who comprised earlier study era cohorts. At one yr after return to dialysis, earlier study cohort era was the only factor associated with anemia status. Patients did ...
We performed both a laboratory and field intercomparison of two novel glass-based retrospective radon detectors previously used in major radon case-control studies performed in Missouri and Iowa. The new detectors estimate retrospective residential radon exposure from the accumulation of a long-lived radon decay product, Pb-210, in glass. The detectors use track registration material in direct contact with glass surfaces to measure the alpha emission of a Pb-210 decay product, Po-210. The detector's track density generation rate (tracks cm{sup -2} hr{sup -1}) is proportional to the surface alpha activity. In the absence of other strong sources of alpha emission in the glass, the implanted surface alpha activity should be proportional to the accumulated Po-210 and hence, the cumulative radon gas exposure. The goals of the intercomparison were to: (1) perform collocated measurements using two different glass-based ...
Purpose: Patients with ataxia-telangiectasia (A-T) show greatly increased radiation sensitivity and cancer predisposition. Family studies imply that the otherwise clinically silent heterozygotes of this autosomal recessive disease run a 3.5 to 3.8 higher risk of developing cancer. In vitro studies suggest moderately increased cellular radiation sensitivity of A-T carriers. They may also show elevated clinical radiosensitivity. We retrospectively examined patients who presented with severe adverse reactions during or after standard radiation treatment for mutations in the gene responsible for A-T, ATM, considering a potential means of future identification of radiosensitive individuals prospectively to adjust dosage schedules. Material and Methods: We selected 20 cancer patients (breast, 11; rectum, 2; ENT, 2; bladder, 1; prostate, 1; anus, 1; astrocytoma, 1; Hodgkins lymphoma, 1) with Grade 3 to 4 (RTOG) acute and/or late tissue radiation side effects by reaction ...
Population residing in the northern part of the Chelyabinsk oblast and the south eastern part of the Sverdlovsk oblast of Russia affected to accidental exposure since 1957. Objective: To investigate the predictive value of ADC value in grading of neuroepithelial tumors. Methods: The clinical data and images of 70 patients with neuroepithelial tumors pathologically proven were collected and analyzed retrospectively. All the patients were classified into low (WHO I or II) and high (WHO III or IV) grade groups which included 40 and 30 cases respectively according to the 2007 WHO classification of tumours of the central nervous system. All the patients underwent plain and contrast-enhanced MR scan and DWI before surgery. The minimum ADC (MinADC) value was measured postoperatively on ADC maps. The Ki-67 labeling index (Ki-67 LI) of tumor tissue was determined by immunohistochemistry. MinADC values for two groups were analyzed using student t test, while the age and Ki-67 LI for the two groups was analyzed using Mann-Whitney test (P<0.05 considered to be significant). Also the MinADC value and Ki-67 LI were analyzed with Pearson ...
The Forsmark area consists of a number of natural wetlands. As a part of the evaluation of wetlands in the safety assessment for the area, possible future wetlands are being studied with respect to hydrology and transport mechanisms. A sensitivity analyses is performed to point out the governing parameters for the wetland hydraulics. The analysis of future wetlands is carried out using the hydrological model system Mike SHE. Mike SHE has been used to describe the near-surface hydrology for a regional model area in Forsmark. Three types of areas have been chosen. Today's lake Bolundfjaerden is because of its shallow depth likely to develop into a mire in the future. As it is situated in the downstream part of the regional model area, the runoff to the lake from upstream surface water system is significant. Lake Eckarfjaerden is situated in the upstream part of the catchment at a higher altitude and with a smaller inflow. Lake Puttan is situated above a ...
Through the collaborative efforts of undergraduates and faculty at Gettysburg and Franklin & Marshall Colleges, we present the compilation of 10years of differential photometry for three K-type stars in the Pleiades. These young stars have rotational light curves with V-band amplitudes of a few percent (10% in the most active) due to BY Draconis-type behavior (modulation due to rotation of a star with non-uniform surface brightness). With 10years of photometry reduced, measured, and compiled we are now in a position to probe the V-band variations in these stars for indications of the extent of brightness asymmetry in the photosphere, and characteristics of the long-term activity of these stars. With the practice of 10years safe operation, Daya Bay Nuclear Power Station has established and continuously improved the management system for radiation protection and optimization (ALARA) which contains 3 basic requirements: all workers are trained, all employees are engaged in totally, and work management is implemented for the whole process. At the same time, strong efforts have been made to build the 'infrastructure' as a platform for its effective operation. This article introduces the contents and characteristics of the system and basic experiences of its effective implementation. In order to implement the management system effectively, it is necessary for NPPs to strengthen the responsibility system for radiation protection and the leading role of the radiation protection personnel, especially the role of technical support and supervision during the work with high radiation risk, emphasize the organic combination and actively mutual ...
The diagnosis of dialysis-related amyloid (AB-amyloid) has been based usually on clinical and radiological criteria. Following the discovery that beta 2-microglobulin was the major protein of this amyloid, we isolated and radiolabelled uremic plasma beta 2-microglobulin. After intravenous injection, gamma-camera images of selected joint areas were obtained from 42 patients who were on regular hemodialysis therapy. Positive scans involving the shoulder, hip, knee and carpal regions were found in 13 of 14 patients treated for more than 10years and 10 of 16 patients treated for 5 to 10years. Patients treated for less time had negative scans. Specificity was indicated by negative scans in non-amyloid inflammatory lesions in control hemodialysis patients. Up to 48-fold tracer enrichment was detected in excised AB-amyloid containing tissue as compared to amyloid-free tissue. These findings suggest that ...

The Idaho National Laboratory (INL) Ten-Year Site Plan (TYSP) describes the strategy for accomplishing the long-term objective of transforming the laboratory to meet Department of Energy (DOE) national nuclear research and development (R&D) goals, as outlined in DOE strategic plans. The plan links R&D mission goals and INL core capabilities with infrastructure requirements (single- and multi-program), establishs the 10-year end-state vision for INL complexes, and identifies and prioritizes infrastructure needs and capability gaps. The TYSP serves as the basis for documenting and justifying infrastructure investments proposed as part of the FY 2013 budget formulation process.
Geothermal heat pump systems are a promising new energy technology that has shown rapid increase in usage over the past 10years in the United States. These systems offer substantial benefits to consumers and utilities in energy (kWh) and demand (kW) savings. The purpose of this study was to determine what existing monitored data were available mainly from electric utilities on heat pump performance, energy savings, and demand reduction for residential, school, and commercial building applications. Information was developed on the status of electric utility marketing programs, barriers to market entry, incentive programs, and benefits.
The multicell proportional chambers at the backing calorimeter of the ZEUS experiment at the storage ring HERA are supplied with a gas mixture of Ar/CO[sub 2] by an open gas system. Flow proportional counters with the build-up [sup 55]Fe sources are used as gas system monitoring detectors. The ZEUS experiment is planned to collect data for about 10years. Therefore, the lifetime of the Ar/CO[sub 2] mixture should be known. In this paper, the results of laboratory tests and preliminary measurements of the aging effects for monitoring counters are presented. ((orig.))
The suitability of calcium silicate bricks (CSBs) for the retrospective measurement of gamma dose using luminescence techniques has been investigated. Bricks of this type are distinguished from fired clay bricks by containing negligible clay and requiring comparatively low temperature treatment during manufacture. They have been used widely in the construction f buildings in the Former Soviet Union since the 1970s but hitherto have not been used for retrospective dosimetry measurements. A procedure based on the se of the 210 deg. C thermoluminescence (TL) peak of quartz was tested with granular quartz extracted from three types of CSB, one of which had been taken from a settlement downwind of Chernobyl. The degree to which the residual geological TL signal within the temperature range of the 210 deg. C L peak had been reduced during manufacture varied with brick type; the levels of residual TL corresponded, in the samples tested, to absorbed ...
Purpose: We present an update analysis of the multiinstitutional Ewing's sarcoma study CESS 86. Methods and Materials: From January 1986 through June 1991, 177 patients with localized Ewing's sarcoma of bone, aged 25 years or less, were recruited. Chemotherapy consisted of four 9-week courses of vincristine, actinomycin D, cyclophosphamide, and adriamycin (VACA) in low-risk tumors (extremity tumors =# 100 cm"3). Local therapy was an individual decision in each patient and was either radical surgery (amputation, wide resection) or resection plus postoperative irradiation with 45 Gy or definitive radiotherapy with 60 Gy (45 Gy plus boost). Irradiated patients were randomized concerning the type of fractionation in either conventional fractionation (once daily 1.8-2.0 Gy, break of chemotherapy) or hyperfractionated split-course irradiation simultaneously with the VACA/VAIA chemotherapy (twice daily 1.6 Gy, break of 12 days after 22.4 Gy and 44.8 Gy, total dose and ...
Background. To determine total upper limb function following selective neck dissection over a mean follow-up of 1.6 years. Methods. A retrospective questionnaire study in a tertiary head and neck surgical unit. One hundred forty-eight patients who underwent selective neck dissection for head and neck cancer from January 2000 to December 2005 were invited to participate. The main outcome measure was ipsilateral upper limb dysfunction as measured by the Disability of Arm, Shoulder and Hand (DASH) questionnaire. Results. Sixty-five patients responded to the invitation to join the study from 148 invited. Despite accessory nerve conserving surgery for all the selective neck dissections studied, 23% reported no upper limb dysfunction, 54% reported mild upper limb dysfunction, 15% reported modera...
Summary The aim of the study was to perform a retrospective review of records to test a possible significant association between sudden, modest weight loss (t-test and Wilcoxon test p values were 0.95 and 0.61, respectively. A Kolmogorov-?Smirnov test p value was 0.2139. A one-half standard deviation difference in time to delivery, approximately 2.5 days, was detectable with 0.95 probability. It was concluded that these data may be the first time this question has been investigated and provide evidence that sudden, modest weight loss at the end of pregnancy is not associated with shorter time to onset of natural labour.
Purpose: To quantify left ventricular function derived from retrospectively ECG-gated multislice spiral CT (MSCT) data sets in comparison to MRI. Materials and Methods: In 16 patients (14 males, 2 females, mean age 56.8 #+-# 11.5 years), retrospectively ECG-gated MSCT angiography of the coronary arteries and breath-hold steady state free precession cine MRI were performed. From MSCT data-sets, 20 axial image series were reconstructed every 5% of the RR interval. Multiplanar images were reformatted in the short axis orientation from axial images. End-systolic and end-diastolic images were selected. From these images end-systolic volume (ESV), end-diastolic volume (EDV) and stroke volume (SV) as well as the ejection fraction (EF) and myocardial mass (MM) were determined using the Simpson's method and compared with MRI. Furthermore, image quality was assessed for both imaging modalities using a four point grading scale. Results: All parameters ...
We reviewed retrospectively the magnetic resonance (MR) images of 14 soft-tissue lesions of fibromatosis (desmoid tumors) encountered in 11 patients. The lesions were typically inhomogeneous in texture and round to oval in configuration. Margins were well-defined in 78% of the lesions at presentation and were infiltrating in all recurrences. (orig./DG).
In this retrospective study, 28 cases of Ewing's sarcoma are reviewed for the onset of metastasis. Bone scans demonstrated bone metastasis in three out of 28 patients at presentation. Of the 22 patients free of metastases at presentation, ten subsequently developed bone metastases. In six of these patients, the bone scan was the earliest demonstrator of metastatic disease. Bone scans are recommended at presentation and periodically during follow-up.

Radiotherapy has been reported to be of benefit in prolonging the survival of patients with cholangiocarcinoma. This study examined whether radiotherapy in addition to endoscopic stenting improved survival. The survival advantage of radiotherapy in those with a type II/III stricture is seen only in the first 10 months after diagnosis. The costs of radiotherapy and significantly increased time spent in hospital, however, raise doubts over its routine use in the management of non-resectable cholangiocarcinoma. (Author).
Paget Schroetter syndrome, or effort thrombosis of the axillosubclavian venous system, is distinct from other forms of upper limb deep vein thrombosis. It occurs in younger patients and often is secondary to competitive sport, music, or strenuous occupation. If untreated, there is a higher incidence of disabling venous hypertension than was previously appreciated. Anticoagulation alone or in combination with thrombolysis leads to a high rate of rethrombosis. We have established a multidisciplinary protocol over 15?years, based on careful patient selection and a combination of lysis, decompressive surgery, and postoperative percutaneous venoplasty. During the past 10?years, a total of 232 decompression procedures have been performed. This article reviews the literature and presents the Exet...
Energy in general is essential for economic and social development, prosperity, health and security of citizens. of the other hand, world population over the last 10years has increased by more than 12%, and now it is exactly about 6.4 billion people and it means more demand for energy. Meanwhile, global primary energy consumption has seen an increase of 20%. Energy supply has some sources and unfortunately most of them have impact on life cycle in biosphere. However, the developed countries, that are only 16% in the population in 2000, consume the energy of 80%. This article deals with the threat of global climate change and its implications throughout the world.
Background:- Discrepancies exist in optometric education, practice and regulation across the Asia-Pacific region and the competence of optometric practitioners in adopting new lens technologies may vary widely. Over the past 10-years, a continuing professional development program, Varilux Academy Asia-Pacific (VAAP), was implemented and conducted in countries across the Asia-Pacific region to improve practitioners' understanding of optometric fitting principles, with special emphasis on progressive addition lenses (PAL). The aim was to demonstrate the effectiveness of VAAP and to compare the competence of practitioners across the Asia-Pacific region in new lens fitting technologies. Methods:- From 2002 to 2008, all VAAP participants from 12 countries across Asia-Pacific were invited to com...
The surgical management of osteoradionecrosis of the temporal bone has met with limited success because of the difficulty in accurate assessment of the viability of nonnecrotic bone intraoperatively. Failure to resect all nonviable bone results in recurrence of a necrotic focus. With the use of hyperbaric oxygen therapy to stabilize marginal bone and oral tetracycline to label viable bone preoperatively, removal of all nonviable bone can be accomplished. Postoperatively, a second course of hyperbaric therapy enhances wound healing, thus assuring a successful outcome. This article details a successful systematic approach that was developed to resect a necrotic focus in the temporal bone of a 10-year-old boy who had undergone a full course of radiotherapy for treatment of a rhabdomyosarcoma.
The Superphenix 750-MW (thermal), once-throughtype steam generators are the result of over 10years of intensive research and development. Detailed design and manufacture of the components lasted from 1977 to 1982. The main difficulties encountered during the construction of these prototype units concerned: demonstration of satisfactory high-temperature properties of Alloy 800 tubes; proven resistance of tube welds to thermal fatigue and of structures and shells to sodium/ water reaction effects; prevention of the tube bundle to flow-induced vibration; necessity of manual welding of shells to prevent hot cracking hazards related to the boron content of the steel; and welding of heat exchange tubes. None of these difficulties, however, invalidated initial major design choices, but provided a wealth of technical and technological experience and knowledge for the steam generators of the future. Plans for the future include revision of the ...
Twenty-four long-term survivors of Ewing's sarcoma were identified as being at risk for a second primary tumor. Among this group of patients followed from 3 to 22 y, 4 new bone tumors were observed, whereas 1.2 x 10"-"3 were expected. All new tumors arose in heavily irradiated areas. The risk associated with radiation after 3 years was 7.2 cases/million person-years per rad. The cumulative cancer risk over 10years for irradiated patients was 35% (SE, 15.1%). Intensive chemotherapy (cyclophosphamide and vincristine administered in five or more courses) seemed to exert an enhancing effect, increasing the rate of development of new tumors.
TIMETAL LCB was introduced more than 10years ago targeting automotive suspension spring applications. The alloy development aim was to use a low-cost formulation by selecting less expensive raw materials than typical beta titanium alloys. Following the first successful application of TIMETAL LCB suspension springs to series production vehicles, the 2000 Volkswagen Lupo FSI, the springs have been used for Ferrari Challenge Stradale since 2003. The most recent development effort was focused on the generation of metallurgical and mechanical service data for the alloy, and the implementation of efficient low cost processing of coils. This paper will introduce various properties of TIMETAL LCB products that can be useful in the design and the consideration of spring applications. Recent progress in the processing of TIMETAL LCB will also be introduced and discussed.
Adding capacity to coal-fired plants in small increments of an overall integrated program may be the solution to the 10-year lead time required for the construction of a new plant. A preliminary study evaluates the technical and economic feasibility of a modular natural gas and coal-fired combined-cycle power plant which can be installed in three distinct phases. The plant sizes are suitable for large industrial and utility applications. The Modular Block Power Plant (MBPP) concept offers the advantages of phase construction, lower capital cost, lower cost electricity, lower air emissions, lower water requirements, and reduced solid waste discharge. Other advantages include part load availability, reduced coal inventory requirements, and easier plant siting. 11 figures, 4 tables.
Introduction Postoperative alopecia is a relatively rare event, and therefore both patients and surgeons are puzzled once it develops even though it is said to improve spontaneously with time in most cases. We report a parieto-occipital pressure-induced alopecia firstly developed in a patient who had undergone repeated surgery for 10?years after a traffic accident. Case report A 29-year-old male underwent segmental osteotomy at the upper and lower frontal edentulous areas for distraction osteogenesis. Throughout the operation, he was in the supine position with the hair covered with a paper cap and the head on a plastic vinyl chloride-covered soft foam horseshoe-shaped urethane sponge placed on the horseshoe-shaped headrest. About 2?weeks after the surgery, two patches of parieto-occipital...
Full text. The electric energy demand in Lebanon is estimated to grow at an average of 3-5% per year for the coming 10years. Such an increase in energy demand is problematic for Lebanon since its economy is almost totally dependent on imported fuel which contributes to 97% of the overall energy requirements. Solar water heaters (SWH) are regarded as the most important element in a long term energy conservation and management strategy for this country, but their promotion is a national issue requiring the participation of many stake holders and decision makers. Additionally, the success of solar energy penetration into the existing energy market is constrained by many factors such as technical and financial limitations, decision criteria and policy instruments. This paper will explore the feasibility of SWH, and will work out, using the Analytic Hierarchy Process technique, a policy to ensure a large scale diffusion of SWH in the energy market
This paper discusses organizational models of planning technology development in coal mining. The following 4 planning systems are analyzed: all-union national system coordinated by the Ministry of Coal Mining of the USSR, all-union associations of coal mine associations (in the Ukrainian SSR the plan is developed by the Ministry of Coal Industry of the Republic), associations of coal mines, coal mines or groups of coal mines. Methods for developing programs for technology development at 4 levels are reviewed. The following planning periods are used: 10years or more (from 1980 to 1990), a 5 year period and a 1 year period. System for technology development in the Soviet coal industry from 1981 to 1985 is evaluated: research and development programs (20 comprehensive state programs, 36 branch programs). (In Russian)
This brochure describes the strategy employed by the administration of Malaspina University--College in Nanaimo, British Columbia, to implement energy-efficient plans and integrating systems and plans that would allow the institution to consume less energy, fewer resources and improve the health and well-being of its students and staff. An assessment study to identify the most problematic areas, and to develop a prioritizing strategy for their replacement was the first step, followed by rigorous adherence to a 10- year cyclical replacement schedule. The program includes replacement of rooftop ventilation units, installation of high-performance glazing units to replace single-glazed windows, implementation of an Energy Management System to monitor heating, ventilating and air conditioning systems, and a Power Smart New Building Design and Building Improvements Program. The combined cost avoidance associated with these projects is estimated to ...
Occurrence of genetic variants during micropropagation is occasionally encountered when the cultures are maintained in vitro for long period. Therefore, the micropropagated multiple shoots of Vanilla planifolia Andrews developed from axillary bud explants established 10years ago were used to determine somaclonal variation using random amplified polymorphic DNA (RAPD) and intersimple sequence repeats markers (ISSR). One thousand micro-plants were established in soil of which 95 plantlets (consisting of four phenotypes) along with the mother plant were subjected to genetic analyses using RAPD and ISSR markers. Out of the 45 RAPD and 20 ISSR primers screened, 30 RAPD and 7 ISSR primers showed 317 clear, distinct and reproducible band classes resulting in a total of 30 115 bands. However, no ...
In 1980 through 1982 a Basewide Energy Systems Plan (BESP) was developed under the direction of the Corps of Engineers at Ft. McClellan to assist the installation in meeting the requirements of the Army Facilities Energy Plan and the ten (10) year Energy Use Reduction Goal. Some of the energy saving projects recommended by the BESP have been implemented. In FY 1977, the total base energy consumption was 1,105,221 MBtu (Million British thermal units). By FY 1987, this had been reduced to 870,900 MBtu, a reduction of twenty-one percent (21%). This resulted in a cost avoidance for FY 1987 over base year FY 1977 of $2,960,986, or a thirty-two percent (32%) reduction.
Abstract Remediation of ground water containing anionic contaminants presents a great challenge. Because of its low cost, surfactant-modified zeolite (SMZ) has been studied for >10years for potential uses as permeable barrier materials to remove anionic contaminants from water. In this study, zeolite aggregates with particle size of 3.4 to 4.8 mm were modified by hexadecyltrimethylammonium (HDTMA) bromide, a cationic surfactant, to a surfactant loading level of 80 mmol/kg and a concurrent counterion bromide loading level of 34 mmol/kg. While no retardation of chromate transport was observed for unmodified columns, a retardation factor of 60 was found for chromate transport through the SMZ columns. Slow but persistent desorption of HDTMA occurred throughout the chromate transport experimen...
The assessment and control of fetal exposure to radiation in the workplace is an issue that is complicated by both biological and political/social ramifications. As a result of the dramatic increase in the number of women employed as radiation workers during the past 10years, many facilities using radioactive materials have instituted fetal protection programs with special requirements for female radiation workers. It is necessary, however, to ensure that any fetal protection program be developed in such a way as to be nondiscriminatory. A study has been initiated whose purpose is to balance the political/social and the biological ramifications associated with occupational protection of the developing embryo/fetus. Several considerations are involved in properly balancing these factors. These considerations include appropriate methods of declaring the pregnancy, training workers, controlling the dose to the embryo/fetus, measuring and ...
BACKGROUND: Calibration by inverse modelling was performed with the MACRO transport and fate model using long-term (>10years) drainflow and isoproturon (IPU) data from western France. Two lack-of-fit (LOF) indices were used to control the inverse modelling: sum of squares (SS) and an alternative statistic called the vertical-horizontal distance integrator (VHDI), which is designed to account for offsets in observed and predicted arrival times of peak IPU concentration. With these data, SS was artificially inflated because it is limited to comparison of predicted and observed IPU concentrations that are concurrent in time. The LOFs were used along with the index of agreement (d) and the correlation coefficient (r) to ascertain the fit of the calibrated models. RESULTS: Predicted arrival ti...
This survey paper categorises, compares, and summarises from almost all published technical and review articles in automated fraud detection within the last 10years. It defines the professional fraudster, formalises the main types and subtypes of known fraud, and presents the nature of data evidence collected within affected industries. Within the business context of mining the data to achieve higher cost savings, this research presents methods and techniques together with their problems. Compared to all related reviews on fraud detection, this survey covers much more technical articles and is the only one, to the best of our knowledge, which proposes alternative data and solutions from related domains.
Purpose: Radiotherapy (RT) has been the predominant local treatment for Ewing's sarcoma of bone at the University of Florida. Twice-daily hyperfractionated RT was initiated in 1982 to improve local control and functional outcome. This retrospective review compares the results of once-daily vs. twice-daily RT in patients with primary Ewing's sarcoma of an extremity, with emphasis on functional outcome. Methods and Materials: Between June 1971 and January 1990, 37 patients were treated at the University of Florida for nonmetastatic Ewing's sarcoma of bone with a primary lesion in an extremity. Three patients underwent amputation. Of 34 patients treated with RT, 31 had RT alone and 3 had a combination of RT and local excision. Before 1982, 14 patients received once-daily RT; since 1982, 17 patients have received twice-daily RT. Doses of once-daily RT varied from 47 to 61 Gy at 1.8-2 Gy per fraction. Doses of twice-daily RT varied, depending on the response of the ...
In MRI T{sub 1}-weighted scans the ...
The detector for retrospective radon exposure measurements is developed. The detector consists of the multilayer package of solid-state nuclear track detectors LR-115 type. Nitrocellulose films works both as {alpha}-particle detector and as absorber decreasing the energy of {alpha}-particles. The uncertainties of implanted {sup 210}Pb measurements by two- and three-layer detectors are assessed in dependence on surface {sup 210}Po activity and gross background activity of the glass. The generalized compartment behavior model of radon decay products in the room atmosphere was developed and verified. It is shown that the most influencing parameters on the value of conversion coefficient from {sup 210}Po surface activity to average radon concentration are aerosol particles concentration, deposition velocity of unattached {sup 218}Po and air exchange rate. It is demonstrated that with the use of additional information on surface to volume room ratio, air exchange rate ...
Radon exposure assessment in case-control studies on radon and lung cancer is generally based on contemporary radon concentration measurements, which can be affected by significant changes in the building structures or in living habits. Another method to estimate the radon exposure of the subjects is the recently developed retrospective dosimetry technique based on the {sup 210}Po surface activity from glass objects. In order to compare the results obtained by the two methods, a study has been carried out in a sample of 26 dwellings in Rome, with radon concentration values ranging from 28 to 623 Bq m{sup -3}. Retrospective detectors based on CR-39 and LR 115 were exposed on 50 glass objects in bedrooms and living rooms. The correlation factor between the two sets of data, after removing six extreme values, is 0.67, which is similar to results obtained in other validation studies of similar sample size. The correlation increases to 0.83 if the ...
This report summarizes preliminary 2-D reactive-transport simulations on the injection, storage and transport of supercritical CO{sub 2} in dolostone formations in the Ordos Basin in China. The purpose of the simulations was to evaluate the role that basin heterogeneity, permeability, CO{sub 2} flux, and geochemical reactions between the carbonate geology and the CO{sub 2} equilibrated brines have on the evolution of porosity and permeability in the storage reservoir. The 2-D simulation of CO{sub 2} injection at 10{sup 3} ton/year corresponds to CO{sub 2} injection at a rate of 3 x 10{sup 5} ton/year in a 3-D, low permeable rock. An average permeability of 10 md was used in the simulation and reflects the upper range of permeability reported for the Ordos Basin Majiagou Group. Transport and distribution of CO{sub 2} between in the gas, aqueous, and solid phases were followed during a 10-year injection phase and a ...
On the basis of soil losses records during 10years, three levels of water erosion were established for two soil (Typical Hapludox and Oxic Dystropept) located on high and medium terraces of alluvial flat plain of piedmont in the Eastern Plains in La Libertad Research Center of CORPOICA. Eighteen 3 x 10 m"2 run-off plots were fitted out on a nonrandom arrangement of nine plots by landscape and three soil use and management treatments: zero grazing Brachiaria decumbens pasture for six years, up land rice, soybean and maize rotations for six years and bare soil for 10years. Soil losses under these treatments allowed to define three degrees of erosion: slight (N_3 moderate (N_2) and severe (N_3) respectively. From each plot soil samples were taken at two depths for physical analyses. infiltration and resistance to cone penetration were measured in the field. Without exception water erosion produced a ...
A report on the long-term follow up of the first cooperative Scandinavian Sarcoma Group study in Ewing's sarcoma of bone is presented. Fifty-two previously untreated patients entered the study between 1984 and 1990. Half of the tumors were located in the extremities and one quarter in the pelvis. The combined modality treatment consisted of 5 cycles of chemotherapy - including vincristine, methotrexate, doxorubicin, cyclophosphamide, bleomycin and dactinomycin. The first two cycles were followed by local resection or amputation in 35 patients and by radiotherapy alone in 17 patients. When surgery was not performed, was incomplete or yielded poor margins radiotherapy was given at a dose of 40-60 Gy. Local tumor relapses developed in 10 patients and in all but one patient were accompanied by metastatic disease. Five patients had metastasis at diagnosis and distant metastases developed after primary treatment in 27 patients after a median of 14 months. The median follow-up time for the 20 ...
Background and purpose: Local treatment for non-metastatic Ewing's sarcoma family tumors (ESFTs) is controversial. Results achieved in a single institution in patients with ESFT of the humerus are presented. Materials and methods: Patients treated between 1983 and 2000 for ESFT of the humerus were included. The impact of local treatment (surgery, radiotherapy or both) on outcome was assessed. Results: 55 patients: 34 males (62%); 21 females (38%); mean age: 17.9 (range: 3-40). Local treatment: surgery in 27 patients (49%), radiotherapy in 17 (31%) and surgery followed by radiotherapy in 11 (20%). After a mean follow-up of 15 years (range: 7-25 years), 27 patients (49%) remained continuously disease free, 27 (49%) relapsed and one died of chemotherapy toxicity. The local recurrence rate was 13% overall: 18% (3/17) after radiotherapy, 7% (2/27) after surgery and 19% (2/11) after surgery followed by adjuvant radiotherapy (p = ns). On the contrary, the ...
Snapping or grating scapula is a condition of the shoulder girdle in which the patient complains of pain and an audible snapping sound which may be associated with pain. We evaluated the diagnostic value of the computed tomography (CT) scans using the clinical diagnosis of a grating scapula as the gold standard. This retrospective study reports the sensitivity, specificity, likelihood ratio and receiver operator characteristic curve for the CT scans and the k value as a measure of the interobserver agreement. None of the parameters validated the examination. We therefore concluded that CT scans are inappropriate for differentiation between a grating scapula and a normally functioning shoulder except in cases where bony abnormalities are present. (orig.)
Intraspinal synovial cysts were diagnosed in six patients during a 5-year period and retrospectively studied. Plain films of the lumbar spine showed degenerative changes in all patients. Lumbar myelography showed a posterior and lateral defect caused by extradural compression. Using CT without contrast enhancement established the diagnosis in five of the six patients. In four cases MRI was performed, three before and after injection of gadolinium diethyline-triamine penta-acetic acid (Gd-DTPA). Including the patient with a false-negative CT, MRI was positive in all patients. (orig.)
Abstract Background- Vitiligo, characterized by destruction of melanocytes, causes a patchy depigmentation of the skin. It has been hypothesized to have an autoimmune pathogenesis. Autoimmune disorders are more common among women and may be associated with adverse pregnancy outcomes, such as recurrent abortions, intrauterine growth restriction (IUGR), and pre-eclampsia. Objective- The purpose of this study was to investigate whether patients with vitiligo have increased rates of gestational complications. Methods- A retrospective comparative study was undertaken comparing pregnancy complications of patients with and without vitiligo. The population was composed of all singleton deliveries that occurred at the Soroka University Medical Center in Israel during the years 1988-2006. Women lack...
The objectives of the research in the field of epidemiology , performed at the Belgian Nuclear Research Centre SCK-CEN are (1) to study cancer mortality and morbidity in nuclear workers in Belgium; (2) to document the feasibility of retrospective cohort studies in Belgium; (3) to participate in the IARC study. For radiobiology, the main objectives are: (1) to elucidate the mechanisms of the effects of ionizing radiation on the mammalian embryo during the early phase of its development, (2) to assess the genetic risks of maternal exposure to ionizing radiation, (3) to elucidate the mechanisms by which damage to the brain and mental retardation are caused in man after prenatal irradiation. The main achievements in these domains for 1997 are presented.
Assessment on the back end fuel cycle, in PEACER (Proliferation-resistant Environmental-friendly Accident-tolerant Continuable and Economical Reactor) that was designated as a new transmutation concept, was performed. Recovery system of uranium and TRU for PEACER is based on pyroprocessing. In the assessment of Long-Lived Fission Products (LLFP) wastes, initially {sup 90}Sr and {sup 137}Cs are dominant contributor nuclides until 30 years and especially {sup 90}Sr and {sup 137}Cs have the highest activity and decay heat than other LLFP. In this study, recovery of {sup 90}Sr and {sup 137}Cs is recommended for reducing of wastes loading. The acceptable decontamination factor is investigated by the toxicity of PEACER spent fuel. The acceptable decontamination factor is about 1.02E+05 for the actinides from PEACER spent fuel after 10years cooling, 4.26E+05 after 100 years cooling, 1.97E+04 after 300 years cooling, 9.52E+03 after 1000 years cooling.

The main objectives of this study was to determine the bone density for the purposes of i) determining the age of peak bone mass in each study group and ii) quantifying the differences in bone density as functions of age and sex. The results of estimating the age dependence of mineral density for the femoral neck, spine and total body of healthy women and men using DEXA are given. The age changes in concentrations of major minerals and electrolytes found for compact and trabecular bones of healthy male femoral neck with regard for the wet and dry tissue calculations are represented. Information about femoral neck water content depending on the age of healthy men is tabulated. The number of performed observations and analyses is evidently insufficient to make any final decision, however some preliminary conclusions can be drawn. In accordance with DEXA results, BMD maximum for the femoral neck, spine and total body of healthy women is found for 31-35 age period while that of healthy men ...
All Streptococcus pneumoniae strains isolated from paediatric clinical samples at Heraklion University General Hospital in the 10-year period 2000-2009 were tested for serotype and susceptibility to antimicrobials. Among a total of 258 strains, 159 were isolated in the 5-year period 2000-2004, before the introduction of the heptavalent pneumococcal conjugate vaccine (PCV7), and 99 in the post-PCV7 5-year period 2005-2009. The prevalence of PCV7-included serotypes decreased in the post-PCV7 period (p?=?0.0002), but an increase was observed for serotypes 7F (p?=?0.002) and 19A (p?=?0.004). Pan-susceptibility rates and susceptibility to cotrimoxazole increased in the post-PCV7 period (p?=?0.01 and p?=?0.008, respectively), but serotype 19A emerged as a contributor to multi-resistance (p?=?0.007). PCV7 was followed by decreased S. pneumoniae resistance and prevalence of vaccine-related serotypes but increased prevalence of serotypes 7F and 19A. ...
This report provides an overview of the scope and status of the US Department of Energy (DOE`s) Salt Repository Project (SRP) at the time when the project was terminated by the Nuclear Waste Policy Amendments Act of 1987. The report reviews the 10-year program of siting a geologic repository for high-level nuclear waste in rock salt formations. Its purpose is to aid persons interested in the information developed during the course of this effort. Each area is briefly described and the major items of information are noted. This report, the three salt Environmental Assessments, and the Site Characterization Plan are the suggested starting points for any search of the literature and information developed by the program participants. Prior to termination, DOE was preparing to characterize three candidate sites for the first mined geologic repository for the permanent disposal of high-level nuclear waste. The sites were in Nevada, a site in volcanic ...
Two local repair procedures, one without (9) and the other with (14) a bulbocavernosus muscle graft were performed on 20 patients with a radiation induced rectovaginal fistula. Four patients had two procedures successively. The initial success rate of both procedures was 7/9 and 14/14 respectively. Though the initial result of the bulbocavernosus graft was obviously better, in many of the local repair procedures, subclinical radiation damage progressed, resulting in recurrence of rectovaginal fistula (5), rectovesical fistula (4), pararectal abscess (2) etc. After a mean follow up of around 10years, the success rate of fistula repair decreased to 5/9 and 13/14 and only 2/9 and 6/14 finally remained without a colostomy. A local repair operation should be restricted to carefully selected cases. The musculus gracilis is proposed as a better vascular graft. If the general condition of the patient does not allow more aggressive reconstructive ...
This work assesses the performance of a modified two-dimensional gamma scan system in spent fuel pin studies. The techniques for a two-dimensional gamma scan studied have been developed at the Hot Cell of Institute of Nuclear Energy Research (INER). Samples are acquired from the spent fuel pin, TPC-SP-C1, which was irradiated in a commercial reactor core (the first of its kind in Taiwan) for 2 years and then deposited in a cooling pool for 10years. The spent fuel pin was then transferred into INER for further examination. The gamma scanning system was driven by a step motor which had an accuracy within 0.1 mm in both X-Y directions. Data obtained from this system are presented in both an isotopic distribution and contour plot. Results in this study closely correspond to those in other investigations, thereby confirming the effectiveness of this modified system. (author)
Reducing conditions are periodically present in hydric soils and are essential for chemical processes that support wetland functions and values. Indicators of these conditions, i.e., redoximorphic features, can be useful in determining the presence of a hydric soil. However, young wetlands, i.e., those recently formed, may not possess reducing conditions and/or may not exhibit redoximorphic features. Few studies have addressed the time needed for hydric soil development. In this study, we present data on redoximorphic features, including chroma and oxidized rhizospheres, gathered from two sets of wetlands in southwestern Virginia, including (1) constructed wetlands that are 3 years old and (2) accidental wetlands that are 10 to 30 years old. Under conditions described for these sites, there is strong evidence that discernable redoximorphic features form in accidental wetlands within 10years, but not within 3 years in constructed wetlands. ...
This report presents the results obtained from the application of the accident consequence assessment code, called OSCAAR, developed in Japan Atomic Energy Research Institute to the Iput dose reconstruction scenario of BIOMASS Theme 2 organized by International Atomic Energy Agency. The Iput Scenario deals with {sup 137}Cs contamination of the catchment basin and agricultural area in the Bryansk Region of Russia, which was heavily contaminated after the Chernobyl accident. This exercise was used to test the chronic exposure pathway models in OSCAAR with actual measurements and to identify the most important sources of uncertainly with respect to each part of the assessment. The OSCAAR chronic exposure pathway models almost successfully reconstructed the whole 10-year time course of {sup 137}Cs activity concentrations in most requested types of agricultural products and natural foodstuffs. Modeling of {sup 137}Cs downward migration in soils is, ...
This report for the Swiss Federal Office of Energy (SFOE) presents the results of a study carried out on the Norwegian electricity market 10years after its liberalisation. The similarity of the Norwegian market to the Swiss electricity market is discussed. Similarly to the proposed situation in Switzerland, the liberalisation in Norway foresaw no privatisation of public utilities and a model for the regulation of grid access was introduced. The report describes and comments on the various phases in which the liberalisation occurred and examines the various instruments used, e.g. to ensure that individual grid operators did not make undue profits from their monopoly. The methods used for the monitoring of grid operators' costs are described and the mechanisms involved in the definition of prices for grid services are examined, including measures taken when profits were too high or too low. The report is concluded with a discussion of ...
NGC 2770 has been the host of three supernovae of Type Ib during the last 10years, SN 1999eh, SN 2007uy and SN 2008D. SN 2008D attracted special attention due to the serendipitous discovery of an associated X-ray transient. In this paper, we study the properties of NGC 2770 and specifically the three SN sites to investigate whether this galaxy is in any way peculiar to cause a high frequency of SNe Ib. We model the global SED of the galaxy from broadband data and derive a star-formation and SN rate comparable to the values of the Milky Way. We further study the galaxy using longslit spectroscopy covering the major axis and the three SN sites. From the spectroscopic study we find subsolar metallicities for the SN sites, a high extinction and a moderate star-formation rate. In a high resolution spectrum, we also detect diffuse interstellar bands in the line-of-sight towards SN 2008. A comparison of NGC 2770 to the global properties of a galaxy ...
Certain innovative coke-oven repair techniques are reviewed that represent an engineered approach to a successful rehabilitation of all types of coke-oven batteries. These techniques have been developed during the last 10years and experience gained on a number of repair projects has shown that these techniques operate as a cohesive and comprehensive method of end flue and through-wall repairs to gain additional years of operating life to coke-oven batteries. Extended operations approaching 10 to 15 additional years of service at lower costs than a pad-up rebuild and, while meeting the environmental emission regulations, are attainable using the techniques of: Proper tie-in joint preparation; Improved bricking up methodology; Preheating refractory during bricking up; Installation of spring-loaded bracing system; and installation of flexible coke-oven doors. Repair methods that do not incorporate the above techniques are subject to premature ...
For the purpose of risk estimates from radiation carcinogenesis including leukemogenesis and radiation genetic effects, the biological significant dose is not the tissue kerma in air but the absorbed dose in organ or tissue with respect to carcinogenic and leukemogenic effects or genetic effects. In order to estimate organ or tissue dose from the tissue kerma in air, a ratio of the organ or tissue dose to the tissue kerma in air for survivors in Hiroshima and Nagasaki was calculated with the aid of the Snyder's mathematical phantoms constructed so as to simulate the body shape of survivors aged 5, 10years old and adult at the time of atomic bomb detonations. The ratios were corrected for the angular distribution of atomic bomb radiations, assuming an anisotropic angular distribution for the survivors exposed to the atomic bombs in open air and the isotropic distribution for the survivors incide a Japanese house or other structures. The ...
Experience at SRS, ORNL, BNL, and Georgia Institute of Technology involving irradiated aluminum clad fuel and target elements, as well as studies of non-irradiated aluminum indicate that some types of aluminum assemblies can be kept in a continually well-deionized water atmosphere for up to 25 years without problems. SRS experience ranges from 2.75 years for the L-1.1 charge kept in deionized D[sub 2]O[sup 1] to greater than 10years for assemblies stored in the Receiving Basin for Off-site Fuel (RBOF)[sup 2]. Experience at Georgia Institute of Technology reactor in Atlanta yielded the longest value of 25 years without problems. The common denominators in all of the reports is that the water is continually deionized to approximately 2 M[Omega] (2 [times] 10[sup 6]ohms) resistivity and the containers for the water are stainless steel or other non-porous material. This resistivity value is equivalent to a value of 0.5 micromhos or microSiemens ...

The BNL Neutral Beam Development Group has been active in the program for the development of high energy, high power neutral beam systems since 1973. These injectors are based on the production, acceleration and neutralization of negative hydrogen or deuterium ions and are supposed to be used for plasma heating and current drive in the next generation of fusion devices. Over the span of 10years the group has studied plasma-surface type of negative hydrogen ion sources, transport and acceleration of negative ion beams and neutralization of negative ions in gases and plasmas. As the required source parameters (current, pulse length, efficiency) were changing over this period of time, the group developed several types of sources, resulting finally in the design of a steady state device operating with an excellent gas efficiency and having the possibility of scaling-up to the size necessary for a high power neutral beam system. The funding of this ...
Abstract Silver-impregnated wound dressings continue to be routinely used for the management of infected wounds, or wounds that are at risk of becoming infected. The ability of antimicrobials that have been incorporated into wound dressings to kill microorganisms within the dressing requires appropriate evaluation using in vitro models. In vitro models that have been exploited for this purpose have included the corrected zone of inhibition and the log reduction assay. However, these and other related culturable-based assays are purported to have poor correlation with the overall microbicidal barrier activity of an antimicrobial wound dressing. This is because culturable-based methods only retrospectively indicate bacterial cell death and do not take into account viable but nonculturable st...
Background. There are no suitably powered epidemiological studies of `transient azotaemia' (TA). The objective of this study was to describe the epidemiology of TA and its independent association with hospital mortality. We hypothesized that TA would be associated with an independent increase in the risk of death. Methods. We retrospectively studied all patients admitted to a university-affiliated hospital in Australia between January 2000 and December 2002. Patients were excluded if they were <15 years old, were on chronic dialysis, had kidney transplant or if their length of hospital stay was <24 hours. We defined TA as rapidly recovering acute kidney injury (AKI) (return to no-AKI risk, injury, failure, loss, end stage (RIFLE) class within 72 hours of onset). We performed descriptive and ...
There is limited information on the incidence of acute kidney injury (AKI) in patients with traumatic brain injury (TBI) although AKI may contribute to morbidity and mortality. We investigated the incidence of AKI in patients with moderate and severe TBI and the association of AKI with risk factors and outcomes in these patients. We studied all TBI patients over 16 years of age admitted to the two designated trauma hospitals in the state of Victoria, Australia from 1 January to 31 December 2008. Patients were included if they had head trauma and presented with a Glasgow coma scale (GCS) <13. Prospectively collected data from the hospital trauma registries, ICUs, and pathology databases were analyzed retrospectively. Risk injury failure loss end (RIFLE) criteria were used to categorize rena...
Background The crucial role of cigarette smoking in the development of pneumothorax is unclear because nonsmokers can also develop primary spontaneous pneumothorax. The purpose of this study was to clarify the pathophysiologic effects of cigarette smoking and its clinical correlations in primary spontaneous pneumothorax. Methods Included were 115 specimens of lung tissue from patients with primary spontaneous pneumothorax who underwent video-assisted thoracoscopic surgery from January 2001 to December 2002. We reviewed the clinical features of 56 smokers and 59 nonsmokers with an average follow-up of 67 months. The pathologic findings of resected lung specimens were analyzed retrospectively. Results There were no statistical differences in sex, age, body height, body weight, body mass inde...
Purpose Degeneration of the gingival margin and periodontal bone loss between segments can occur in various segmental osteotomies. However, treatment and management of these problems have not yet been resolved; improvement of the conventional method is necessary. The purpose of this retrospective study is to evaluate the usefulness and advantage of orthodontic devices in osteotomies. Patients and Methods Forty Japanese adults presented with jaw deformities diagnosed as mandibular prognathism with maxillary protrusion, bimaxillary protrusion, and anterior crowding. Of these 40 patients, 20 (group 1) underwent anterior segmental osteotomy or dento-osseous osteotomy along with our original orthodontic periodontal management. The remaining 20 (group 2) patients underwent conventional procedure...
Women treated of localised stages of cervix cancer have a very good prognosis. But do health care professionals have a tendency to underestimate the potential need for rehabilitation of this group? To be able to answer this question this study aimed to develop knowledge concerning these women's own perception of their health during a long period after surgical cancer treatment. A total of 398 women who underwent radical hysterectomy in the period from the 1 January 1983 to 31 December 2000 participated in the study at Aarhus University Hospital, Skejby in Denmark 2001-2003. All patients initially filled in an internally validated questionnaire. Subsequently a sample of 100 women filled in the questionnaires Short Form -36 and Sense of Coherence to evaluate their self-reported health. Analy...
RIFLE criteria have been used to determine the incidence of acute kidney dysfunction (AKD) after orthotopic liver transplantation (OLT). However, no studies have focused on the incidence of AKD after OLT in patients with normal pre-OLT kidney functions. Using the RIFLE criteria, we determined the incidence and risk factors for AKD after OLT in patients with normal pre-OLT kidney function. We retrospectively analyzed the records of 112 patients who underwent OLT from January 2000 to February 2009 with normal prior kidney function. We investigated three levels of renal dysfunction outlined in the RIFLE criteria: risk (R); injury (I); and failure (F). Preoperative, intraoperative, and postoperative variables were collected. AKD occurred in 64 (57%) OLTs with risk, injury, and failure frequenc...
Study Type - Therapy (case series)Level of Evidence 4 OBJECTIVE To present our experience of retroperitoneoscopic partial adrenalectomy (RPA) for small adrenal tumours, as with modern imaging methods small adrenal lesions are being diagnosed more commonly, and retroperitoneoscopic adrenal surgery for small adrenal tumours (PATIENTS AND METHODS We retrospectively reviewed the records of 389 consecutive retroperitoneoscopic adrenalectomies from September 2005 to December 2008, 88 of which were small adrenal tumours and treated by RPA. Ultrasonography and computed tomography (CT) were used in all patients before RPA, and magnetic resonance imaging or positron emission tomography/CT in some patients. We used RPA for adrenal tumours and total adrenalectomy for adrenal cancer. During the surgery...
AbstractBackground: The aim of this study was to review an experience with retroperitoneal endoscopic adrenalectomy (REA). This is the procedure of choice for adrenal tumours at this institution. Methods: Between 1997 and 2008, 112 REAs were performed in a single university centre. Data were retrieved retrospectively from a prospectively collected database, including information on patient demographics, surgical procedure, complications and hospital stay. Results: One hundred and twelve REAs were carried out successfully in 105 patients, including seven bilateral adrenalectomies. Thirty nine patients with unilateral adrenal disease had a phaeochromocytoma, of whom 16 had multiple endocrine neoplasia syndrome type 2, 21 patients had Cushing's disease and 20 had Conn's disease. Median body m...
Purpose. To examine the impact of residual limb osteomyelitis (RLO) on the rehabilitation of lower limb amputees. Method. Retrospective review of the casenotes of patients with RLO. Information sought included details of amputation, clinical features of investigations for and management of RLO and its effect on rehabilitation. Results. There were seven transfemoral and three transtibial amputees. Indications for amputation were vascular disease in nine cases, trauma in one. In each case, delayed wound healing or residual limb pain prompted radiological, hematological and microbiological investigations. Average time between amputation and diagnosis was 187 days. One patient died before treatment commenced. Two transtibial amputees were treated with intravenous antibiotics while rehabilitati...
HCV infection continues to spread at an alarming rate among IDU populations. The available evidence suggests that HCV is acquired relatively quickly following onset of injection. However, there are few prospective studies of HCV acquisition, particularly among IDU populations in resource-poor settings. A sample of young male heroin injectors with recent onset of injection (<4?years) was recruited in Hanoi, Vietnam for a prospective assessment of the early course of injection (n?=?179). Both behavioral and biological assessments (including detailed retrospective assessment of injection initiation) were conducted at baseline and repeated at 6-month intervals for a period of 16?months. Variables associated with HCV infection (p value?p value?=?0.0005). In multivariate logistic regression anal...
Abstract Recently, a study of patients with rheumatoid arthritis who developed herpes zoster while taking a tumor necrosis factor (TNF)- inhibitor reported a decreased incidence of postherpetic neuralgia. The objective of this study was to investigate whether patients on TNF- inhibitors who developed herpes zoster have a lower incidence of subsequent development of postherpetic neuralgia. A retrospective review of herpes zoster patients on TNF- inhibitors (infliximab, etanercept, or adalimumab) was conducted in 12 dermatology clinics. Medical records of such patients were reviewed thoroughly to confirm herpes zoster and TNF- inhibitors and any subsequent development of postherpetic neuralgia (pain score -3 out of 10 after 90 days of shingles onset) was noted. A total of 206 cases were revi...

Patients and method Between 2002 and 2010, 300 consecutive patients underwent percutaneous nephrolithotomy operation for treatment of staghorn kidney stone disease. Forty-five of the patients were older than 65?years and considered to be in elderly group. Thirty-seven of the patients were between the ages 18 and 36?years and considered to be the control group. Surgical parameters and outcomes were compared between groups. Results There were no significant differences between groups for stone area, operation time, difference in hemoglobin levels...
Epiphyseal plates of growing bones are sensitive to ionizing radiation and this poses problems for the paediatric radiotherapist. In this case report we describe a child in whom electron beam therapy to the groin delivered only low dose radiation to the proximal hip epiphyses - 130 cGy fractionated over 5 weeks - but who suffered late maldevelopment. We explore the success of various radiotherapy techniques in minimizing the dose to the hip epiphyses in four other children. The appropriate positioning of the limb, the choice of treatment modality and, where possible, individual shielding blocks are considered and their effect on the dose to the hip estimated. Absorbed doses of radiation delivered to the midpoint of the proximal femoral epiphyses have been retrospectively determined. (author).
SummaryPurposeWomen with epilepsy have increased frequency of reproductive health problems compared to women without epilepsy. In puberty, reproductive hormonal changes during sexual maturation may affect epilepsy and induce the debut of seizures as indicated in some studies. On the other hand, epileptic activity affects sex hormone function, which may induce alterations in pubertal endocrine maturation and thereby menarche age. We wanted to investigate the relation between epilepsy and menarche age in a larger population of female epilepsy patients.MethodsA retrospective, questionnaire study of a cohort of 265 female outpatients from three Norwegian hospitals and 142 controls, aged 18-45 years was conducted. Parameters regarding epilepsy and reproductive health issues were registered. Per...
Over three years ago, the Core Integration team of the National Science Digital Library (NSDL) implemented a digital library based on metadata aggregation using Dublin Core and OAI-PMH. The initial expectation was that such low-barrier technologies would be relatively easy to automate and administer. While this architectural choice permitted rapid deployment of a production NSDL, our three years of experience have contradicted our original expectations of easy automation and low people cost. We have learned that alleged "low-barrier" standards are often harder to deploy than expected. In this paper we report on this experience and comment on the general cost, the functionality, and the ultimate effectiveness of this architecture.
Abstract Aim- According to the revised Bethesda Guidelines, colorectal cancer (CRC) occurring under age 50-years should be screened to exclude Lynch syndrome. However, in current practice in East Anglia, tumour screening is initiated only after genetics referral, reserved for those with a strong pedigree. This study aimed to determine how many patients with young-onset CRC undergo tumour screening in hospitals in East Anglia. Method- A retrospective case notes review over 5-years in four hospitals was undertaken to determine what proportion of those with young-onset CRC underwent referral for tumour screening and to assess local practices in terms of patient counselling and management. Results- One hundred and twenty-two patients were included. There was an average yearly caseload of 6-9 p...
In a retrospective single-centre study 170 consecutive patients were included who received a Kotz modular prosthesis after resection of bone tumours of the proximal femur to evaluate the management of prosthetic infection. Infection occurred in 12 of 166 patients available for follow-up (six males; six females; mean age, 47?years; range, ten to 75?years) after a mean of 39?months (range, one to 166?months; infection rate, 7.2%). Mean follow-up was 54?months (range, four to 200?months). One patient died of septic shock. Two patients were treated by wound revision only. Treatment of infection in the remaining patients was one-stage revision in eight and hip disarticulation in one. Infection control by one-stage revision was achieved in five of eight patients; re-infection occurred in three p...
The consequences of exit, voice, loyalty, and neglect behavior were examined using retrospective questionnaires (Study 1) and 2 social interaction diary studies (Study 2). Exit and neglect were generally associated with more negative partner responses and reduced feelings of value and closeness, and were reported to be more harmful to the relationship. Voice was associated with more positive partner responses and greater value and intimacy, and was perceived to be most beneficial to the relationship. In contrast, loyalty was perceived to have the same detrimental consequences as destructive behaviors (Study 1) and did not predict more positive outcomes within daily interactions (Study 2). These results indicate that, despite good intentions, loyal intimates are often left feeling ignored a...
The treatment of acetabular bone defects presents a great challenge in revision total hip arthroplasty (THA). The purpose of this study was to evaluate the clinical and radiological outcome of revision THA using jumbo cups for acetabular reconstruction after applying the bone-grafting technique. We studied 17?patients with acetabular defects ranging from Type 2A to Type 3A according to Paprosky`s classification. According to the AAOS-score twelve patients were classified as Type II and five patients as Type III. Uncemented press-fit cups with an outer diameter larger than 64?mm were used in all cases. Fifteen patients received morselized bone allografts. In eight patients an additional screw fixation was necessary. The mean follow-up period was 82?months (range 33?149). The mean Harris Hip...
The United States deployed 118 military working dogs (MWDs) to the Persian Gulf theater during Operations Desert Shield and Desert Storm. This study is a retrospective descriptive study of medical records of these deployed dogs, with the objective to determine whether there were infectious or parasitic diseases with a zoonotic potential in a sentinel population of MWDs that may be of concern to Persian Gulf veterans. Fifty-one percent of visits to veterinary treatment facilities during deployment were for illness or injury. Potential zoonotic conditions accounted for 21% of the total visits, 41% of the "sick-call" visits, and 63% of presentations for illness to veterinary treatment facilities. This study did not determine whether the diseases treated were transmitted between MWDs and the troops. Although the etiologic agents were not determined in these cases, no evidence was found supporting new or reemerging illnesses in this population of dogs. PMID:11272705
Objective:- To review the rate of inadvertent cystotomy during laparoscopic hysterectomy performed by the Sydney West Advanced Pelvic Surgery Unit (SWAPS). Eighty-one percent of these were performed laparoscopically. There were 14 (1.1%) bladder injuries reported during this period. ...
Background In scientific literature, when the consequences of illness are discussed, one finds that the deficit perspective is prevailing: Illness as the cause for loss of abilities and a loss of contentment. On the other hand, particularly with creative people, an increase of creativity can be observed, following a confrontation with and having endured a period of illness. When one considers the occurrence of illness retrospectively in a biographical context, one may find often a biographical standstill. Although illness and disease are considered as a collapse, one may observe also psycho-spiritual maturation processes during the confrontation with illness. In several cases illness can thus result in personality maturation and a higher level of psychological health. Aims Despite of the f...
Modern scientific computing involves organizing, moving, visualizing, and analyzing massive amounts of data from around the world, as well as employing large-scale computation. The distributed systems that solve large-scale problems will always involve aggregating and scheduling many resources. Data must be located and staged, cache and network capacity must be available at the same time as computing capacity, etc. Every aspect of such a system is dynamic: locating and scheduling resources, adapting running application systems to availability and congestion in the middleware and infrastructure, responding to human interaction, etc. The technologies, the middleware services, and the architectures that are used to build useful high-speed, wide area distributed systems, constitute the field of data intensive computing. This paper explores some of the history and future directions of that field.
Context. Cardiovascular medication overdoses can be difficult to treat. Various treatment modalities are currently recommended. Objective. To describe patient outcomes and adverse events of high-dose insulin therapy in consecutive overdose patients in cardiogenic shock after implementation of a high-dose insulin protocol (1-10 U/kg/h, while avoiding or tapering off vasopressors). Methods. This is an observational consecutive case series of patients identified from a registry. Data were collected by retrospective chart review of patients treated by our toxicology service with this protocol from February 2007 until March 2010. Results. Twelve patients were treated with high-dose insulin. The mean age was 36.5 years (SD 11.7). Seven patients had pre-existing vasopressor therapy, and all were ...
The main objectives of research in the field of health effects at the Belgian Nuclear Research Centre SCK-CEN are: (1) to study cancer mortality and morbidity in nuclear workers in Belgium; (2) to document the feasibility of retrospective cohort studies in Belgium; (3) to participate in the IARC study; (4) to elucidate the mechanisms of the effects of ionizing radiation on the mammalian embryo during the early phases of its development; (5) to assess the genetic risks of material exposure to ionizing radiation; (6) to elucidate the cellular mechanisms leading to brain damage after prenatal irradiation; (7) to advise authorities and to provide the general population with adequate information concerning the health risk arising from radiation exposure. Progress and major achievements in these topical areas for 1997 are reported.
339 CT und 95 MRT examinations in 210 partients were evaluated retrospectively to determine the value of CT and MRT for follow-up of head and neck tumors and for diagnosing recurrences. Semi-quantitative evaluation of tumor extent during and after radiotherapy showed advantages for MRT since changes induced by treatment, particularly oedema, produced less contrast loss in the images. CT had a sensitivity of 81% for the recognition of recurrences; this was 92% for MRT. Specificity for CT was 76% and for MRT 86%. Concerning the reliability of individual diagnostic criteria, space occupying lesions were the most valuable in CT diagnosis. For MRT, space occupying lesions and infiltration into neighbouring structures were of equal value. Because of differences in the nature of the signals, MRT proved better than CT in characterising recurrent masses and this improved the differentiation between scarring and local tumour recurrence. A retrospective study of Ewing's Sarcoma at The Armed Forces Malignant Diseases Treatment Centre, Command Hospital, Pune, is presented. A total of 38 cases over a period of thirteen years have been studied. Males predominate slightly, and the common age group on presentation has been in the second and third decade. Humerus, pelvis and femur are the bones commonly affected. The initial line of management has been primarily radiotherapy, which has been over the years supplemented by combination chemotherapy. All the cases achieved remission, though only 32 cases (84 per cent) had complete remission, and the remaining only partial remission. The overall survival in those offered aggressive therapy has been over 50 per cent in two years. The present policy in the management of these cases is discussed. (author). 4 tabs.
AbstractBackground: So far, infliximab (IFX) therapy for the treatment of Crohn's disease (CD) has generally been guided by clinical symptoms. Data on treatment response as ascertained by endoscopy in IFX therapy are scarce. The aims of this study were to measure the endoscopic response rate during IFX induction and maintenance therapy in luminal CD, and also evaluate the role of endoscopy in monitoring IFX therapy. Methods: Data obtained from 71 patients with active luminal CD and treated with IFX were analyzed retrospectively. The endoscopy findings were scored according to mucosal activity as: 0 (remission), 1-2 (mild), 3-4 (moderate), and 5-6 (severe). A positive endoscopic response was determined by a decrease in score of at least two points and mucosal healing was assigned a score of...
Background/Purpose: Congenital esophageal stenosis (CES) is a rare malformation. Endoscopic dilations represent a therapeutic option. This study retrospectively evaluated the efficacy and safety of a conservative treatment of CES. Patients and Methods: Patients diagnosed with CES since 1980 by a barium study or endoscopy were reviewed. Endoscopic ultrasonography (Olympus UM-3R-20-MHz radial miniprobe, Olympus Corporation, Tokyo, Japan), available from 2001, allowed for the differential diagnosis of tracheobronchial remnants (TBR) and fibromuscular hypertrophy (FMH) CES. All children underwent conservative treatment by endoscopic dilations (hydrostatic and Savary). Results: Forty-seven patients (20 men) had CES. Fifteen were associated with esophageal atresia; and 8, with Down syndrome. Mea...
Background There is increasing recognition that lower nurse staffing levels are associated with higher morbidity and mortality among medical and surgical patients. The degree to which this applies to elderly patients with hip fractures is unclear. Questions/purposes We conducted a pilot study using administrative data as an initial step in investigating the relationship between nurse staffing levels and in-hospital mortality among elderly patients with hip fractures. Patients and Methods We retrospectively reviewed administrative data for 13,343 patients 65?years or older with a primary diagnosis of hip fracture admitted to 39 Michigan hospitals between 2003 and 2006. We used logistic regression to calculate the change in predicted probability of in-hospital death conferred by differences ...
Abstract Background: Follicular lymphoma (FL) is one of the most common non-Hodgkin's lymphomas of B cells, being closely associated with a t(14;18) translocation. Detection of t(14;18), which is present in 70-95% of FL, might aid in FL diagnosis. Objective: To compare the efficacy of routine polymerase chain reaction (PCR) and fluorescence in situ hybridization (FISH) techniques in detecting t(14;18) in paraffin-embedded tissue samples of FL patients at different stages. Combined with other immunophenotypic biological determinants, detection of t(14;18) might help to determine patients at increased risk according to the FL International Prognostic Index (FLIPI) and therefore facilitate appropriate treatment. Design and Methods: This study was mainly based on a retrospective examination of...
Twenty one examinations of chest FCR were performed in 15 malignancies with pulmonary metastases. The rate for demonstrating, less than 5, from 5 to 15, and more than 15 mm pulmonary nodules using with FCR was retrospectively evaluated on the basis of the lesions detected with CT. The demonstrating rate for pulmonary nodules less than 5 mm in diameter was only 8 % ; for 5 - 15 mm nodules, about 87 %. However, several 5 - 15 mm nodules were not demonstrated due to overlap of the pulmonary vessels and bony structures. All nodules more than 15 mm were demonstrable. Chest FCR may demonstrate more pulmonary nodules than conventional radiography. However, the further studies should be required for the improvement of detectability of them with FCR. (author).
In a retrospective study of 21 patients, in whom the acetabular fractures were suspected on initial radiographs, we compared and analysed the computed tomographic findings and plain radiographic findings. The results were as follow: 1. In patients with multiple trauma, no further change in position was required during CT examinations. 2. CT showed intraarticular loose bodies, which were invisible on plain radiographs. 3. CT was useful in detecting the fractures of acetabular rims, medial wall of acetabulum, and femoral head. 4. CT permitted better evaluation of shape, extent, and degree of separation of fracture fragments. 5. CT was helpful in detecting the associated fractures and soft tissue injuries. 6. CT also demonstrated the adequacy of reduction, the position of metallic fixation devices, and the presence or absence of remaining intraarticular osseous fragments after surgery.

Background For the past 14 years, we have been performing laparoscopic adrenalectomy via the lateral transabdominal as well as the posterior retroperitoneal approach. The aim of this study is to describe patient selection criteria for each approach with comparison of perioperative outcomes. Methods In patients with smaller tumors, low body mass index (BMI), history of previous abdominal operations, appropriate body habitus, and bilateral pathology, we have performed preferentially the posterior approach. Data regarding clinical pathology, tumor size, BMI, estimated blood loss (EBL), operating time (OT), morbidity, mortality, and duration of stay were analyzed retrospectively. Data are expressed as mean +- standard error of the mean (SEM). Results One hundred seventy-two laparoscopic adrena...
Atrial fibrillation (AF) is common after coronary artery bypass grafting (CABG) and increases the morbidity and cost. Amiodarone reduces AF after CABG. Ranolazine, an antianginal agent, also prolongs atrial refractoriness and inhibits after depolarizations and triggered activity; effects that could decrease AF after CABG. The present study compared amiodarone versus ranolazine for the prevention of AF after CABG. A retrospective cohort study of patients undergoing CABG at Aspirus Hospital from June 2008 to April 2010. The patients received either amiodarone (400 mg preoperatively followed by 200 mg twice daily for 10 to 14 days) or ranolazine (1,500 mg preoperatively followed by 1,000 mg twice daily for 10 to 14 days). The primary end point was any identified AF after CABG. A total of 393 ...

Aim of the work was to describe computerised tomography (CT) findings of calcified renal metastases focusing on differential diagnosis problems. Abdominal CT scans of 8000 patients with tumor, performed from 1984 to 1998, have been retrospectively reviewed.
CT findings were compared retrospectively between 9 cases with malignant lymphoma of the anterior mediastinum and 8 cases with invasive thymoma. CT findings of malignant lymphoma were as follows: (1) The majority of the tumors were bilateral and extended beyond the anterior mediastinum to the other mediastinum compartments. (2) Their margin was either smooth or lobulated. (3) Their density was either homogeneous or heterogeneous. In the case with heterogeneous density, ring, triangle and/or band-like low density areas, just like interspace of fused lymph nodes, were demonstrated. Calcification or cysts could not be shown. (4) Patent inherent vessels without deviation were occasionally demonstrated running through the tumors. (5) The tumors faced and contacted widely to anterior chest wall and often pressed the lung laterally at the anterior parietomediastinal pleural reflection. (6) Pleural implants were not demonstrated. (author).
The objective of this study was to evaluate the association between the number of blood cultures collected and the appropriateness of care for suspected bacteremic community-acquired urinary tract infection (UTI) in the elderly. We retrospectively evaluated the medical records of 129 patients with UTI >65?years old admitted to a large community-based training hospital in Japan from 1 January 2006 to 31 December 2009. We assessed the association between the number of blood cultures collected and the appropriateness of care received, as well as other factors. Two-thirds of the patients were women, and patients >85?years old accounted for 45.0% of the cases. Most of the organisms isolated from the urine and blood were Escherichia coli (65.4?67.0%). More than two blood cultures were collected ...
BackgroundAlthough benign ampullary tumors are removed endoscopically, due to their potential to progress to malignant disease, the favored treatment for adenocarcinoma is pancreaticoduodenectomy. We reviewed our institutions experience in order to identify which patients were at highest risk of disease progression following surgical resection, as well as evaluate whether localized T1 tumors are best treated by pancreaticoduodenectomy. MethodsWe retrospectively reviewed 157 patients who presented with an ampullary mass, from 2001 to 2010, and identified 51 with benign adenoma and 106 with adenocarcinoma. ResultsPatients with malignant tumors most often presented with larger tumors and jaundice, which alone was predictive of survival (OR = 67). Forty-five percent of patients with pathologic...
The progression of hip dysplasia was investigated in 116 military working dogs. Medical records were reviewed for any clinical history of hind-limb lameness. Pelvic radiographs were studied for evidence and degree of hip dysplasia, degenerative joint disease, or both. The number of months each dog worked was determined. Each dog's age at termination from service and cause of death (or euthanasia) were recorded. The mean months of work for normal and dysplastic dogs were evaluated using the Student's t-test. No significant difference was found in the total number of months worked between normal and dysplastic dogs (p greater than 0.05). PMID:8875358
The progression of hip dysplasia was investigated in 116 military working dogs. Serial pelvic radiographs were graded for degree of dysplasia and degenerative joint disease (DJD). Norberg angles, angles of inclination, and joint space widths were measured. There was a significant correlation between the Norberg angle and the degree of dysplasia (p less than 0.0001). Angles of inclination and joint space width measurements did not demonstrate a correlation to canine hip dysplasia. Dysplastic dogs had a significant estimated risk for development of DJD compared to normal dogs (p less than 0.0001; odds ratio of 70.2). Dogs with normal hip conformation at 24 months of age or older did not develop moderate nor severe DJD. PMID:8875357
This study was a retrospective evaluation of the efficacy of stereotactic radiosurgery (SRS) in patients with >4 metastases to the brain. Materials and Methods: Between January 2004 and December 2006, 68 patients with ?4 multiple brain metastases were included and reviewed retrospectively. Twenty-nine patients received SRS and 39 patients received whole brain radiotherapy (WBRT). Patients with small cell lung cancers and melanomas were excluded. The primary lesions were non-small cell lung cancer (69.0%) and breast cancer (13.8%) in the SRS group and non-small cell lung cancer (64.1%), breast cancer (15.4%), colorectal cancer (12.8%), esophageal cancer (5.1%) in the WBRT group. SRS involved gamma-knife radiosurgery and delivered 10?20 Gy (median, 16 Gy) in a single fraction with a 50% marginal dose. WBRT was delivered daily in 3 Gy fractions, for a total of 30 Gy. After completion of treatment, a follow-up brain MRI or a contrast-enhanced ...
Ischemic bowel disease is a rare disorder whose incidence is increasing as the mean age of the population increases. Diagnosis by clinical, laboratory and radiologic means is often difficult, and delay in definitive therapy results in substantial morbidity and mortality. A series of 26 consecutive patients, with proved acute superior mesenteric ischemia, was retrospectively reviewed: the authors reports the diagnostic methods performed preoperatively, the site and the cause of infarction and the time passed between the first radiograph and surgery. Plain abdominal radiographs were performed in 25 of 26 patients, screening abdominal US in 23 cases and CT in 19 cases. All radiological examinations were retrospectively reviewed by the three authors, independently, to recognize the different signs of infarction. On plain abdominal films, the findings warranting a presumptive diagnosis of bowel infarction were air-fluid levels (84% of cases), ...
Xeroradiography (XR) was retrospectively compared with a newly developed computed radiography (FCR) for diagnostic availability for breast cancer. FCR provides with the image processed by a computer. Subjects were 65 breast cancer patients for XR and 51 for FCR. Both methods were performed preoperatively. No significant difference was observed in distribution of age between the two groups but the average tumor size in XR group was larger than that in FCR group. Tumor shadow appeared in 44 of 65 xeromammograms (67.7%) and in 34 of 51 computed radiograms (66.7%). Microcalcification was detected in 27 of 65 xeromammograms (41.5%) and in 21 of 51 computed radiograms (41.2%). Consequently, diagnosis of breast cancer could be achieved in 47 of 65 patients (72.3%, established: 39, suspective: 8) through XR and in 41 of 51 (80.4%, established: 33, suspected: 8) through FCR. Thus FCR is thought to be more useful for breast cancer because it not only offers good diagnostic ...

Abstract. Purpose: To determine whether vitrectomy with 25-gauge instruments contributes to better postoperative visual recovery after macular hole (MH) surgery. Methods: The medical records for 46 consecutive eyes operated for MH by a single surgeon were retrospectively examined. Vitrectomy had been performed with a 25-gauge instrument in 23 eyes (25-G group) and with a 20-gauge instrument in 23 eyes (20-G group). Postoperative visual acuity (VA) in logMAR (logarithm of the minimum angle of resolution) units after 1 week and 1, 3, 6, 9 and 12 months, operating time, and volume of intraocular irrigating fluid were compared between the two groups. Results: Mean preoperative logMAR VA was 0.72 in the 25-G group and 0.68 in the 20-G group (p = 0.282, unpaired t-test). One week after surgery, ...
Morphine sulfate (MS Contin), a proven analgesic in the treatment of cancer pain and chronic benign pain, seems to be a good analgesic for the treatment of burn pain. MS Contin is morphine sulfate incorporated in a wax cellulose matrix delivery system. This wax cellulose delivery system gives MS Contin its duration of action. Ten patients were enrolled in an open-labeled, nonrandomized study. The study was designed to examine the analgesic efficacy of MS Contin in the burn population. Each patient remained in the study for 6 days. The efficacy of the analgesic regimen was subjectively measured by the visual pain scale. The MS Contin group was retrospectively compared with a group of patients who were given continuous intravenous infusions of morphine. The two groups were matched according to age, burn size, surgical procedures, and hospital stay. The analgesic qualities of MS Contin were comparable to those of continuous intravenous morphine sulfate infusions. MS ...
Objective was to present our 8 year experience in the prevention of the obturator nerve reflex during transurethral resection of bladder tumors. This study was performed in Ataturk University Hospital between 1999 and 2007. We retrospectively reviewed the records of 89 patients with inferolateral bladder tumors, who underwent transurethral resection under epidural or general anesthesia and requested obturator nerve reflex inhibition. Epidural anesthesia was administered to 57 patients, while the remaining 32 patients underwent general anesthesia via mask; and succinylcholine was administered prior to resection. Of the 57 patients received epidural anesthesia, 18 were diagnosed as inferolateral bladder tumors during endoscopy and had to undergo general anesthesia. Obturator nerve block was attempted preoperatively in 39 patients. However, a nerve identification failure, hematoma and 4 obturator nerve reflex events, despite the block, were observed and these patients ...
The objective of this study was to determine the prevalence of concha bullosa and nasal septal deviation and their potential relationships to maxillary sinusitis. 883 CT scans taken at Creighton University School of Dentistry from 2005 to 2008 were retrospectively reviewed for the presence of concha bullosa, nasal septal deviation, and maxillary sinusitis. 67.5% of patients exhibited pneumatization of at least one concha, 19.4% of patients had a deviated septum, and 50.0% had mucosal thickening consistent with maxillary sinusitis. 49.3% of patients who had concha bullosa also had evidence of maxillary sinusitis. Only 19.5% of patients with concha bullosa also had nasal septal deviation, whereas 19.7% of patients with sinusitis also presented with nasal septal deviation. Although concha bullosa is a common occurrence in the nasal cavity, there did not appear to be a statistically significant relationship between the presence of concha bullosa or nasal septal ...
Purpose: To assess the prevalence, significance and practical value of two recently described CT signs of instable aneurysm: the crescent sign and the periluminal halo (a low-attenuating internal layer of the thrombus around the patent lumen). Material and methods: Among the CT examination performed in the last 5 years, they retrospectively selected the nonruptured aneurysms with a diameter exceeding 4 cm (no. = 93 average diameter 5.1 cm, unenhanced images in 28 cases and enhanced in 84) and the ruptured aneurysms (no. = 16, average diameter 6.7 cm, unenhanced images in 9 cases and enhanced in 9). They studied the prevalence of the crescent and halo sing, their relationship with the aneurysm diameter, and the effect of contrast enhancement. Results: The crescent sign was identified with a statistically significant prevalence in ruptured ( 37.5 % or cases) over asymptomatic aneurysms (5.5 %); the halo had the same frequency in the 2 groups (12.5 % and 9.5 %). Both ...
The improvement of the ecological conditions at waste storing reservoirs is an important task of the restoration activity at Production Association (PA) ``Mayak`` (South Urals). The radionuclides mostly {sup 90}Sr, {sup 137}Cs, and chemical pollutants deposited in the reservoir water and in the bottom sediment are very dangerous sources for the contamination of Techa River below the reservoirs and the contamination of groundwater in the surrounding formations. The spreading of radioactive contaminants has both hydrogeological and the chemical features. The thermodynamic approach used to account for physical-chemical interactions between water and the bed rocks based on Gibbs free energy minimization of multicomponent system (H-O-Ca-Mg-K-Na-S-Cl-C-Sr) permitted the authors to calculate the corresponding ionic and complex species existing in the solutions, and to characterize the processes of precipitation and dissolution. The model takes into account the input and output surface and ...
Six patients with eosinophilic granuloma were studied retrospectively in order to correlate the MRI appearances with the pathology. Ages ranged from 2 years 6 months to 11 years. The bones involved were the humerus, ulna, radius, femur, clavicle and ilium. Plain films, MRI and pathology specimens were obtained. A lytic lesion with indistinct margins, endosteal erosions and periosteal reaction was seen in all cases on plain radiographs. Bone marrow signal was decreased on T1-weighted images and increased on T2-weighted images throughout the bony lesion in all cases. T2-weighted images showed extensive soft-tissue abnormalities suggesting inflammatory changes in four cases. In two cases abnormalities were limited. Extensive changes correlated histologically with an early phase lesion. Localized minor changes were associated with a mid-phase lesion. Inflammatory soft-tissue changes could be associated with eosinophilic granuloma. The size of the soft tissue mass ...
PurposeTo evaluate the efficacy of photorefractive keratectomy (PRK) for residual refractive error after penetrating keratoplasty (PKP) for keratoconus. SettingPrivate ophthalmic clinic. DesignCase series. MethodConsecutive patients who had PRK augmented with topical mitomycin-C (MMC) after PKP for keratoconus were retrospectively reviewed. Patients were divided into a a low cylinder group (refractive cylinder 6.00 D). Visual acuity, refraction, and keratometry were analyzed preoperatively and 1, 3, 6, and 12 months postoperatively. ResultsThe study comprised 47 eyes (41 patients). The spherical equivalent (SE) decreased from -4.24 D +- 3.23 (SD) preoperatively to -0.71 +- 1.03 D 12 months postoperatively in the low cylinder group and from -4.19 +- 3.54 D to -2.45 +- 3.42 D, respectively, ...
Objective. The aim of this study was to assess the capability of high-resolution images obtained with a commercially available pelvic phased-array surface coil to demonstrate normal hip anatomy. Design. We retrospectively analyzed the oblique coronal magnetic resonance (MR) images of hips of 36 consecutive patients acquired on a 1.5-T clinical imager using a pelvic phased-array coil as a receiver, a 16-20 cm field of view, and 5 mm slice thickness. Patients. Thirty-six patients were studied, age 15-81 years. There were 20 males and 16 females. Results and conclusions. The articular cartilage, cortex, superior labrum, and iliofemoral ligament were well visualized on proton density weighted fat saturation (PDF) images. The femoral and obturator vessels, obturator nerve, and various muscles were easily seen on T1-weighted images. High-resolution imaging of the hip is achievable in a reasonable amount of time using newer phased-array surface coils and may play an ...
The MRI and CT appearances in 48 patients with histologically confirmed benign and malignant pleural abnormalities were compared retrospectively. Abnormal pleural changes were shown in 47 out of the 48 patients by high signal intensity of the pleura in T_2-weighted sequences and in contrast enhanced T_1-weighted sequences on MRI. CT showed abnormalities in 45 out of 48 patients. Delineation of pleural and pulmonary changes by CT was possible in 13 out of 23 cases, and pleural disease from effusions in 15 out of 28 cases. T_2-weighted MRI was successful in 14 out of 23 and 4 out of 28 cases, respectively. T_1-weighted images after contrast were successful in 20 out of 23 and 22 out of 28 cases, respectively. Indications of malignant pleural disease were the presence of mediastinal or circumferential involvement or involvement of the entire pleura, thickness of more than 10 mm and nodular changes. The most reliable sign of malignancy was infiltration of the thoracic ...
The primary aim was to evaluate delayed contrast-enhanced MRI in depicting perineural spread of hilar cholangiocarcinoma (CCC) and consequently to determine the capability of MRI/MRCP for staging CCC. Fifteen patients that underwent MRI/MRCP and surgical treatment were retrospectively included. Two radiologists evaluated MR images to assess delayed periductal enhancement, extent of bile duct stenosis, liver parenchymal and vascular involvement and presence of liver atrophy. An agreement between delayed enhancement of the bile duct walls and perineural neoplastic spread showed a very good correlation factor (0.93). The overall accuracy in detecting biliary neoplastic invasion was higher for delayed T1-weighted images (93.3%) than for the MRCP images (80%), and T1-delayed image increased the MR accuracy in assessing the neoplastic resectability (p < 0.05). MRI correctly predicted vascular involvement in 73% and liver involvement in 80% of the cases. The number of ...
Recent technological developments have facilitated intensified searches for genetic markers under selection in nonmodel species. Here, we present an approach for the identification of candidate gene variation in nonmodel organisms. We report on the characterization of 82 single nucleotide polymorphisms (SNPs) and on the development of a specific genotyping assay for 30 SNPs in 18 candidate genes for growth and reproduction in Atlantic cod (Gadus morhua). These markers can be used for scanning natural populations for signatures of selection in both contemporary and archived historical samples, for example in retrospective studies assessing the effects of environmental changes, such as increasing temperatures, and selection imposed by high fishing pressure. Furthermore, these gene markers may be of interest to aquaculture, serving as a starting point for linking phenotypic traits important for productivity with genotypes and potentially be of use for marker-assisted ...
Bone matrix gelatin, prepared by sequential chemical treatment including decalcification with 0.6 N hydrochloric acid [9], was used as an alloimplant for the treatment of benign bone tumours, tumorous conditions of bone, acetabular dysplasia, delayed union, traumatic bone defects and other disorders. The bone matrix gelatin implanted into bone defects was incorporated successfully in 98% of implantations, excluding cases of infection, tumour recurrence and recurrence of tumorous conditions. The material was also implanted into ten bone sites as an onlay but in five it was resorbed without new bone formation. The incorporation of the bone matrix gelatin into the recipient bed was completed from 6 to 33 months (average 14.9 months) after implantation. Wound infection complicated 5 of 165 implantations (3%) in previously uninfected sites. Low grade fever persisting after the tenth post-operative day (a probable sign of immunological reaction) occurred in 4 of 160 implantations (2.5%), ...
A retrospective study was made of all radionuclide (RN) bone images performed at our institution over a two-year period to evaluate femoral head viability after nonpathologic fracture of the femoral neck. Twelve patients had avascular femoral heads during the perioperative period, of which nine had adequate follow-up. Seven of these nine patients had follow-up bone images. Revascularization occurred in four patients, while three had persistent absence of femoral head uptake. With clinical follow-up ranging from four to 29 months (median: 14 months), only two of these nine patients developed clinical or radiographic evidence of osteonecrosis. RN bone imaging performed in the perioperative period does not reliably predict the development of post-traumatic osteonecrosis of the femoral head and, at present, should not be used to determine prospectively method of treatment of femoral neck fracture.

To estimate of the dispersion trend of 3H and 14C discharged from the Wolsung Nuclear Power Plants, the present level of 3H and 14C in environmental samples in the vicinity of the Wolsong site was studied. Tree-ring cellulose analyses were carried out for retrospective evaluation of 3H and 14C in the environment around the Wolsong Nuclear Power Plants. 3H released from the Wolsong Nuclear Power Plants has affected an area up to a 25-km radius from the site, while almost all 14C was deposited within a 2-km radius, reaching to a natural level over 2 km. Organically bound tritium concentrations in tree rings were strongly correlated with the gaseous tritium discharge rate, while delta14C (excess) in tree rings ranged from 204 per thousand to 460 per thousand, which did not significantly vary with year. To assess the potential of CT in evaluating the histological severity of acute appendicitis in comparison with surgical and pathological findings. The CT images of 75 patients with surgically proven appendicitis, including 10 cases of catarrhal, 34 of phlegmonous, and 31 of gangrenous appendicitis, were retrospectively analyzed for the following five CT findings: hazy periappendiceal densities, enlarged appendix, increased enhancement of the appendiceal wall, increased enhancement of the periappendiceal intestinal wall, and deficiency of the appendiceal wall. By comparing all the CT findings and the pathological severity of appendicitis (catarrhal, phlegmonous, and gangrenous), the prevalence of the five CT findings was calculated for each pathological category. Abnormal CT findings were noted in only one case of catarrhal appendicitis. Increased enhancement of the appendiceal wall was observed in all 29 cases of phlegmonous appendicitis (100%), but in only 66.7% ...
This work is a retrospective study of 86 patients with squamous cell (51) and basal cell (35) carcinoma of the face. The median age was 67 years. The staging is as following: 31 cases T{sub 1} and T{sub 2}, 55 patients T{sub 3} and T{sub 4}. All patients were considered NO. Electron beam was used in all cases and energy ranged from 5 to 15 MeV depending on the thickness of the tumor. The total tumor dose was 60-70 Gy delivered in 6 weeks. In 7 cases of very old patients the authors have used concentrated irradiation: 3 patients received 21 Gy in one fraction and 4 patients 48 Gy in 8 fractions. 96.8% patients T{sub 1} i T{sub 2} are NED contrary to 56.4% T{sub 3} i T{sub 4}. Cosmetic results were considered as good in 52, fairly good in 7 and bad in 2 patients.
In this report we assessed retrospectively the usefulness of Thallium-201 chloride scintigraphy for diagnosing malignant tumors of the maxillofacial region. Thallium-201 chloride scintigraphy was performed on 74 patients with malignant tumors and 8 patients with benign tumors, and a retention index was calculated from the early and delayed scintigrams (delayed accumulation ratio/ early accumulation ratio). This retention index depended on the blood flow of tumors and the affinity of tumors with Thallium-201 chloride. The retention index was large for malignant tumors (avg.: 1.05) and small for benign tumors (avg.: 0.78). Concerning the degree of malignancy (the histological type and the degree of tumor involvement), the retention index was higher in highly malignant tumors (1.20 for poorly-differentiated tumors and 1.19 for aggressive-involvement-type tumors) and lower for tumors of low malignancy (1.02 for well-differentiated tumors and 0.98 for low ...
To determine clinical predictors and accuracy of {sup 123}I-FP-CIT SPECT imaging in the differentiation of drug-induced parkinsonism (DIP) and Parkinson's disease (PD). Several clinical features and {sup 123}I-FP-CIT SPECT images in 32 patients with DIP, 25 patients with PD unmasked by antidopaminergic drugs (PDu) and 22 patients with PD without a previous history of antidopaminergic treatment (PDc) were retrospectively evaluated. DIP and PD shared all clinical features except symmetry of parkinsonian signs which was more frequently observed in patients with DIP (46.9%) than in patients with PDu (16.0%, p<0.05) or PDc (4.5%, p<0.01). Qualitatively {sup 123}I-FP-CIT SPECT images were normal in 29 patients with DIP (90.6%) and abnormal in all patients with PD, and this imaging technique showed high levels of accuracy. DIP and PD are difficult to differentiate based on clinical signs. The precision of clinical diagnosis could be reliably ...
Background. A change in practice at our institution resulted in increased use of anesthesia for CT scan of the chest in children who required sedation. Objective. To determine whether there is a difference in the frequency or severity of pulmonary atelectasis on CT scan in children sedated by anesthesiologists compared with children sedated by radiologists using intravenous pentobarbital. Materials and methods. Retrospective blinded review of 60 CT scans of the chest performed in 41 children. Forty-one studies in children sedated by radiologists (median age 29 months) were compared with 19 studies in children sedated by anesthesiologists (median age 25 months). Results. Atelectasis sufficient to obscure pulmonary metastases was shown in 5 of 41 (12 %) radiology sedations and 13 of 19 (68 %) anesthesiology sedations (P < 0.01). Higher grades of atelectasis were recorded in children under anesthesia (P < 0.01). Conclusion. Atelectasis is more frequent and more ...
In cases of obstructive colorectal cancer (CRC), preoperative diagnosis of the proximal lesion is often difficult when the primary lesion impedes the passage of the endoscope. The aim of this study was to evaluate the usefulness of fluorine-18-fluorodeoxyglucose positron emission tomography (FDG-PET) in such cases. A total of 52 obstructive CRC patients who underwent preoperative FDG-PET and subsequent surgical resection were retrospectively reviewed. The correlation between characteristics of the proximal lesion and FDG-PET findings was analyzed statistically. There was a significant correlation between the proximal lesion size and the maximum standardized uptake value (P=0.00016). Abnormal FDG accumulation in the proximal colon indicated the existence of proximal cancer or adenoma with a sensitivity of 50% and a specificity of 100%. There was a significant difference in the distribution of tumor size between the cases with proximal abnormal accumulation and those ...
To quantify the number of cases and clinical courses of Acanthamoeba keratitis treated at the King Khaled Eye Specialist Hospital (KKESH) between December 1982 and May 2005, and to review their clinical course and response to medical and surgical therapy. Retrospective review of medical records of all patients with Acanthamoeba keratitis diagnosed by the KKESH Microbiology or Pathology or Pathology Laboratory since the opening of the hospital. During a period of over 22 years, only six cases of confirmed. Acanthamoeba keratitis were diagnosedand treated at KKESH. In two cases the diagnosis was made by corneal biopsy, while four cases required examination of a histopathology specimen obtained at the time of therapeutic keratoplasty to establish the diagnosis. One eye was cured by medical therapy, while five eyes required one (4eyes) or two (1 eye) therapeutic penetrating keratoplasty (PKP) to achieve a clinical cure. Three eyes achieved good visual acuity (20/40, ...
Ensyn is a profitable commercial company which derives its revenues from the conversion of wood residues into liquid biofuel and chemicals. The technology, Rapid Thermal Processing (RTP{sup (TM)})is based on extremely fast ``cracking`` of biomass which results in light liquid yields exceeding 70% by weight, from wood. Whether producing chemicals or liquid biofuel, the RTP plant is configured identically and operated essentially in the same mode. Chemicals production simply allows economical production to occur at a lower plant capacity, as low as 2 tonnes/day, than is feasible for a dedicated fuel plant (typically greater than 100 tonnes/day). Ensyn has developed the commercialisation of RTP{sup TM} from bench to industrial scale in 10years. A variety of crative funding initiatives in the early years allowed for capital to be raised for R and D without the loss of intellectual property (IP). The transition years of technology demonstration, ...
Petro-Canada's Terra Nova offshore oil platform was designed with the expectation that its Floating, Production, Storage and Offloading (FPSO) vessel would be taken off location for maintenance and modifications work within the first 10years of operation. Petro-Canada's 2006 extended Turnaround project was initiated to prepare for the maintenance and transport of the FPSO. The project aimed to execute an extended turnaround in approximately 90 days. The cost of the project was estimated at $190.5 million. Shutdown occurred early on May 7, 2006 due to the failure of a generator gearbox. Staff were increased from 80 to 120 to complete critical preparation work. On June 17, the FPSO was disconnected and departed for Conception Bay. A turret cover plate was installed, and the FPSO began its voyage to a Rotterdam shipyard in the Netherlands. In order to complete the journey, a comprehensive marine transit manual and emergency ...
Calgary-based TAG Oil is an exploration company that manages 4.1 million acres of major producing oil and gas fields in New Zealand. The enormous Maui field, with 4 tcf of natural gas in place, has dominated the gas market in New Zealand by meeting nearly 90 per cent of the country's energy demand at costs much lower than world prices. However, the maturing field is in decline and will cease production by 2008. New gas field discoveries will only meet 60 per cent of the country's energy requirements for 5 additional years. Unless new large reserves of gas are discovered, the supply and demand situation will get worse. Lead time to place new production on-stream requires 5 to 10years, which creates a large supply gap over the next decade. Public resistance to coal-fired power plants, new hydroelectric dams and nuclear power has left the country with no viable alternative to natural gas. TAG Oil has taken this unique opportunity to create value ...
Dual energy X-ray absorptiometry (Hologic QDR-1000) was used to quantitatively analyze the diagnosis and prevention of osteoporosis. The peak bone mineral density (BMD) of the spine appeared in normal men in their twenties and in normal women in their thirties. There was acceleration of bone loss in the 50 to 60 year age bracket (premenopause and postmenopause) in normal women. On the contrary, the peak BMD of the femoral neck in normal men and women appeared in their twenties after which it decreased slightly with age. Comparison showed that the femoral neck BMD of normal women was lower than that of men throughout all ages. The fracture threshold, 0.756 g/cm[sup 2] for the spine, was obtained by scanning 73 females with spinal fractures, the mean BMDs for L2-L4 at the 90th percentile level were used as the fracture threshold. The fracture threshold of femoral neck fracture was the femoral proximal BMD of the 9th decile. Classification by the Public Welfare Silver Science ...
Fusion energy is one of only a few truly long-term energy options. Since its inception in the 1950s, the vision of the fusion energy research program has been to develop a viable means of harnessing the virtually unlimited energy stored in the nuclei of light atoms--the primary fuel deuterium is present as one part in 6,500 of all hydrogen. This vision grew out of the recognition that the immense power radiated by the sun is fueled by nuclear fusion in its hot core. Such high temperatures are a prerequisite for driving significant fusion reactions. The fascinating fourth state of matter at high temperatures is known as plasma. It is only in this fourth state of matter that the nuclei of two light atoms can fuse, releasing the excess energy that was needed to separately bind each of the original two nuclei. Because the nuclei of atoms carry a net positive electric charge, they repel each other. Hydrogenic nuclei, such as deuterium and tritium, must be heated to approximately 100 million ...
There is extensive evidence that exposure to asbestos causes pulmonary parenchyma fibrosis, pleural disease, and malignant neoplasm in asbestos-exposed workers. However, few data concerning brake-lining workers are available in the literature. In this study, we aimed to assess the long-term effects of chrysotile asbestos exposure on lung function and the risk of asbestos-related diseases in brake-lining workers. Seventy-four asbestos-exposed workers who processed brake-lining products and 12 unexposed office workers were offered pulmonary function tests (spirometry and transfer actor) in 1992 and 1999. In 1999, the mean duration of asbestos exposure was 0.00#+-#4.07 and 11.02#+-#4.81 years (7-31 years) in non smoking and smoking asbestos workers, respectively. Transfer factor (T_L, CO) and transfer coefficient (K_C_O) decline were significant in the 7-year follow-up in both smoking and non smoking asbestos workers. However, lung function indices of he control group, whom were all ...

This paper reports, to complement current knowledge on brain development, late regional brain maturation assessed with quantitative MR imaging. Axial and coronal head spin-echo (SE) images were obtained in 60 healthy individuals aged 5--56 years, with a double-echo, flow compensated imaging sequence obtained with a 1.5-T Magnetom spectroscopy and imaging system. T2-weighted images were calculated from the intensity differences in SE images at echo times (TEs) of 15 and 90 msec (TR = 2.5 second). The mean T2 values were determined at 16 sites in each cerebral hemisphere. T2 values of the six frontal subcortical white matter (FSCWM) sites and of the internal capsule (IC) were evaluated. Mean T2 values in the IC decreased until age 10years, whereas this decrease continued in the FSCWM past age 15 years before reaching a plateau. Differential age-dependent patterns of mean T2 values emerged between the six FSCWM sites. The spread of T2 values ...
As human activity in and around Puget Sound increased, so did the contaminant levels in the sediment. Sediment cores collected in 1 982 revealed inputs of chemicals to the Sound, including lead (Pb), mercury (Hg), silver (Ag), copper (Cu) and petroleum hydrocarbons, began to increase above background in the late 1800s and peaked between 1945 and 1965. Synthetic organic compounds, such as polychlorinated biphenyls (PCBs) and DDT, first appeared in sediments deposited in the 1930s and reached a maximum in the 1960s. The presence of the subsurface maximum concentrations suggests that pollution-control strategies have improved the sediment quality of central Puget Sound. Additional sediment coring was performed in 1991 and samples were collected at six locations in the main basin of Puget Sound. Sediment ages were determined using Pb"2"1"0 radio isotope dating. Sedimentation rates were approximately 1 to 2 cm/yr and deposition rates ranged from 480 to 1000 mg/cm2/yr. The contaminant level ...
Recent investigations have identified elevated concentrations of polychlorinated dibenzo-p-dioxins (dioxins) in marine sediments and wildlife of Queensland, Australia. While it has been demonstrated that the contamination is widespread and predominantly land-based, limited information exists on the pathways and fate of these compounds within the near-shore marine system. This environment supports unique and threatened species including green sea turtles (Chelonia mydas). Adult green turtles are predominantly herbivorous, feeding on seagrass and algae. Apart from initial migration to feeding grounds (at {proportional_to}10years of age) and intermittent migrations to breeding grounds (at {proportional_to}30-50 years and thereafter), green turtles remain and feed within relatively small home ranges. Long life-span (50 years or more), near-shore feeding grounds and highly specialized food requirements render green turtles potentially vulnerable to ...
Regional cerebral blood flow (rCBF) was assessed with single photon emission computed tomography (SPECT), "1"2"3I-N-isopropyl-iodoamphetamine ("1"2"3I-IMP), and the Table-Look-Up method in 51 children (27 boys, 24 girls) considered neurologically normal and aged 1 month to 15 years (mean age, 4 years 1 month; standard deviation (SD) 3 years 11 months) divided into seven age groups. The rCBF was measured in cortical regions, the cerebellum, thalamus, and the head of the caudate nucleus. Curves for reference values and standard deviations were defined for each region. The rCBF rapidly increased until 2 years of age, reaching maximum values during the third to fifth periods (2 to 10years of age) in each region. The rCBFs then decreased, reaching adult levels at 10 to 15 years of age. The rCBF reached maximum values later in the frontal region than in other regions. The rCBF during the early period increased most prominently in the occipital ...
Introduction: Previous studies have published the correlation between myocardial perfusion SPECT (MP) during cold pressor test (CPT) and intracoronary acetylcholine and its usefulness as independent marker of endothelial dysfunction (ED). Objective: To analyze the incidence of positivization of MP exercise studies in the follow up of asymptomatic patients with moderate cardiovascular risk (CV) and ED detected by PF. Material and Methods: Of 301 patients of the PARADIGMA Registry (normal exercise MP SPECT and clinical probability < 20% of events at 10years [moderate risk by Framingham index]) 55 had positive PF (+) (18.3%). Prospectively and consecutively, 15 asymptomatic patients with PF (+), and a control group (CG) of 15 patients with negative PF, with paired sex, age and coronary risk factors (CRF), that accomplished a 12 #+-# 2 months follow up, and that underwent a new exercise and resting MP SPECT were analyzed. An MP extension score ...
During outbreaks, Microdiprion pallipes (Fall.) is the most destructive of the pine sawflies in Sweden. Its distribution includes most provinces, but damaging outbreaks have until recently occurred only in two inland areas in northern Sweden. These areas are characterised by high elevation, a harsh climate, and slow tree growth. The four recorded outbreak periods showed a 10year periodicity. Outside these areas, a lesser outbreak occurred in 1988 to 1990, on the east coast (province of Uppland). Outbreak patterns, life history variation, and mortality factors were studied. Factors that may explain the distribution of outbreaks and the population patterns were identified.Experimental and observational evidence on the potential of various factors to influence fecundity, dispersal, and survival was evaluated. In the outbreak areas, there were few major population factors. Parasitism by Rhorus substitutor (Thunb.) was the largest cause of larval ...
Environmental monitoring has been conducted at the U.S. Department of Energy`s (DOE`s) Hanford Site in southeastern Washington State since 1945. Fish and wildlife have been monitored since 1945, however, a major emphasis on mammals did not occur until the 1970s. This report focuses on the 10-year period from 1983 through 1992. The objectives of this report are to evaluate {sup 90}Sr and {sup 137}Cs concentrations in Site wildlife populations and, when possible, evaluate trends in concentrations over this period of time. No distinct trends in radionuclide concentrations were apparent in most species sampled. Many measurements were at or below the analytical limit of detection. This evaluation found that concentrations of {sup 90}Sr in rabbit and deer bone were elevated in animals collected from areas adjacent to industrialized areas. Similarly, radionuclide concentrations in duck muscle from waterfowl collected at B Pond were elevated with {sup ...

The authors have developed a complex of activation methods of analysis using a nuclear reactor (nuclear activation analysis) and a cyclotron (charged-particle activation analysis). The methods have been used to determine the concentrations of more than 20 elements in five medicinal plants native to Uzbekistan: Syrian rue (Peganum harmala L.), plantain (Plantago lanceolata), peppermint (Mentha piperata L.), sage (Salvia officinalis L.), and ziziphora (Ziziphora bungeana Yur.). The results of radio-activation analysis were compared with the results of standard spectral analysis performed in another laboratory and the accuracy of the procedures developed was evaluated on the basis of the results.
To get a better understanding of the condensation oscillation and chugging process during blowdown of a BWR after a LOCA a large number of large scale pressure suppression experiments have been performed in several countries. ECN has performed a detailed analysis of the results of one of these experiments, e.g. the DAS M10 experiment of GKSS. This analysis includes the following aspects: Power Spectral Density (PSD) analysis of the data, analysis of the acoustic frequencies of the downcomers and an analysis of the eigenfrequencies of the downcomers. The results of these analyses and an evaluation are given in this paper. It is shown that all dominant frequencies in the experimental results can be correlated with the vent-acoustic or eigenfrequencies. (orig.).

Thermal-fluid flow analysis and demonstration test were performed for a spent fuel storage system. The commercial computational fluid dynamics (CFD) code, FLUENT was used for the numerical analysis. Effective thermal conductivities of a spent fuel assembly and a fuel basket were derived to optimize a thermal analysis model. Also, a porous model, which can simplify a complex configuration of a fuel assembly, was used in the thermal analysis. Demonstration test were performed to verify the thermal analysis method and procedure using a half scaled-down model and an electrically heated dummy fuel. The numerical analysis results were compared with the experimental data. Thermal analyses of the storage system were carried out for normal and off-normal conditions by using the verified analysis method.

The aim of this study was to document vascular complications that occurred following cadaveric and living donor kidney transplants in order to assess the overall incidence of these complications at our center as well as to identify possible risk factors. In a retrospective cohort study, 1500 consecutive renal transplant recipients who received a living or cadaveric donor kidney between December 1988 and July 2006 were evaluated. The study was performed at the Nemazee Hospital, Shiraz, Iran. The assessment of the anatomy and number of renal arteries as well as the incidence of vascular complications was made by color doppler ultrasonography, angiography, and/or surgical exploration. Clinically apparent vascular complications were seen in 8.86% of all study patients (n = 133) with the most frequent being hemorrhage (n = 91; 6.1%) followed by allograft renal artery stenosis (n = 26; 1.7%), renal artery thrombosis (n = 9; 0.6%), and renal vein thrombosis (n = 7; 0.5%). ...
Problems and solutions in the diagnosis and operative treatment were clarified by retrospective evaluation of cases of intramedullary tumor in which MRI and surgery had been performed. The subjects were 16 patients (21-75 years old, mean age: 45 years) whose tumor was surgically removed between 1991 and 1998. MRI was performed preoperatively, and T1-weighted images, T2-weighted images, and enhanced images were obtained. Angiography was also performed in some cases. The histological diagnosis based on the MRI findings before surgery were compared with the pathological diagnosis after the operation. In 9/16 cases (56%), the preoperative histological diagnosis based on the MR images was the same as the pathological diagnosis after the operation. In 8 of the 11 cases (73%) whose images showed clear tumor margins and well shaped the tumor, the tumors were completely removed. The concordance rate between the pathological diagnosis during and after the operation was 67%. ...
Acute vertebrobasilar occlusion (VBO) is associated with a high risk of stroke and death. Although local thrombolysis may achieve recanalization and improve outcome, mortality is still between 35% and 75%. However, without recanalization the chance of a good outcome is extremely poor, with mortality rates of 80-90%. Early treatment is a fundamental factor, but detailed studies of the exact time management of the diagnostic and interventional workflow are still lacking. Data on 18 patients were retrospectively evaluated. Time periods between symptom onset, admission to hospital, time of diagnosis, and beginning of intervention were correlated with postinterventional neurological status. The Glasgow Coma Scale and National Institute of Health Stroke Scale (NIHSS) were used to examine patients before and after local thrombolysis. Additionally, multivariate statistics were applied to reveal similarities between patients with neurological improvement. Primary ...
Objective: To compare the therapeutic effect of unilateral biliary duct drainage with bilateral biliary duct drainage in the treatment of Bismuth-Corlette II-IV type hilar cholangiocarcinoma in order to provide rational and proper clinical options. Methods: Biliary duct drainage was performed in 80 patients with hilar cholangiocarcinoma. The patients were divided into unilateral drainage group (n = 52) and bilateral drainage group (n = 28). The clinical data and the therapeutic results of the two groups were retrospectively analyzed and compared. Results The two groups were quite comparable in the patient's age, sex, clinical type, drainage way, preoperative total and direct bilirubin level, white blood cell, total protein and albumin, etc. After the treatment, the mean reduction value of the total bilirubin and direct bilirubin was 82.73 #mu#mol/L and 31.71#mu#mol/L respectively in unilateral drainage group, and was 80.28 #mu#mol/L and 50.89#mu#mol/L respectively ...
We wanted to evaluate the status of self-expandable nitinol stents implanted in the P2 and P3 segments of the popliteal artery in Korean patients. We retrospectively analyzed 189 consecutive patients who underwent endovascular treatment for steno occlusive lesions in the femoropopliteal artery from July 2003 to March 2009, and 18 patients who underwent stent placement in popliteal arterial P2 and P3 segments were finally enrolled. Lesion patency was evaluated by ultrasound or CT angiography, and stent fracture was assessed by plain X-rays at 1, 3, 6 and 12 months and annually thereafter. At the 1-month follow-up, stent fracture (Type 2) was seen in one limb (up to P3, 1 of 18, 6%) and it was identified in seven limbs at the 3-month follow-up (Type 2, Type 3, Type 4) (n = 1: up to P2: n = 6: P3). At the 6-month follow-up, one more fracture (Type 1) (up to P3) was noted. At the 1-year follow-up, there were no additional stent fractures. Just four limbs (up to P2) at ...
To assess the efficacy and resulting complications of tetracycline sclerotherapy in renal, hepatic, ovarian, and perivesical cysts. We retrospectively reviewed 23 cases of benign cysts (16 renal, 4 hepatic, 2 ovarian, and 1 perivesical) in 22 patients in whom the condition was diagnosed or confirmed by either ultrasound, CT, or cytology, and who underwent percutaneous tetracycline sclerotherapy. Using a 21-gauge Chiba needle, the target cyst was punctured under ultrasound guidance. Prior to the injection of 1500 mg of tetracycline diluted in 5 ml of normal saline, almost all the cystic content was aspirated, and at the end of the procedure the tetracycline was left in the cyst. During a period of between 3 and 22 months, 18 of the 23 cases were followed up. In six of the 18 cases followed up, the cysts either decreased in size by 10%, or collapsed completely. In seven cases a collapse of over 50% was noted, and in the remaining five the cyst recurred. In one of ...
Retrospective analyses were performed to evaluate the therapeutic efficacy of intra-oral cone (IOC) radiation therapy for carcinoma of the oral tongue, especially about its faculty of local control and complications. Thirty-nine patients with squamous cell carcinoma of the oral tongue were treated with IOC radiation therapy alone or in combination with external irradiation. IOC radiation therapy was given not only for T1 and T2, but also for T3 tumors. Hypofractionation was used for IOC radiation therapy. The 2 year local control rates, including surgical salvage, for T1, T2, T3 and T4 were 100%, 73.3%, 40% and 0%, respectively. The 5 year actuarial survival rates for Stage I, II, III and IV were 100%, 65.3%, 35.4% and 0%, respectively. The incidences of radiation complications, i.e. soft tissue ulceration and osteoradionecrosis, were 42.9% and 2.9%, respectively. It is suggested that relatively high rate of soft tissue ulceration was due to hypofractionation. We ...
Purpose: To evaluate the outcome of repeat stereotactic radiosurgery (SRS) for acoustic neuromas, we assessed tumor control, clinical outcomes, and the risk of adverse radiation effects in patients whose tumors progressed after initial management. Methods and Materials: During a 21-year experience at our center, 1,352 patients underwent SRS as management for their acoustic neuromas. We retrospectively identified 6 patients who underwent SRS twice for the same tumor. The median patient age was 47 years (range, 35-71 years). All patients had imaging evidence of tumor progression despite initial SRS. One patient also had incomplete surgical resection after initial SRS. All patients were deaf at the time of the second SRS. The median radiosurgery target volume at the time of the initial SRS was 0.5 cc and was 2.1 cc at the time of the second SRS. The median margin dose at the time of the initial SRS was 13 Gy and was 11 Gy at the time of the second SRS. The median ...
Stereotactic radiosurgery (SRS) precisely delivers high-dose radiation to a small target (usually less than 3-4 cm in diameter), in a single session with steep dose-fall, employing various radiation methods. SRS provides good tumor control for small brain metastases from various primary cancers, with minimal untoward effects on surrounding normal brain. This excellent tumor control prevents neurological death and maintains good activity of daily life. Although surgery with whole-brain radiation therapy (WBRT) remains an important option for patients with a solitary brain metastasis, SRS with or without WBRT should be considered in patients with a limited number of small tumors and a good prognosis. Many reports, as well as both retrospective and prospective reviews, have shown WBRT before or after SRS to improve local control and reduce new distant lesion emergence. However, upfront WBRT does not improve survival. There are two major delivery techniques, Gamma ...
The purpose of this study is to evaluate the radiologic findings of the extraosseous Ewing's sarcoma. Six patients with pathologically confirmed extraosseous Ewing's sarcoma were retrospectively reviewed. Patients included two men and four women with an average age of 21.5 years (age range 9-48 years). Plain radiographs (six patients), magnetic resonance (MR) images (five patients), computed tomographic (CT) scans (three patients) and whole body scintigraphy (two patients) were reviewed and analyzed. Images were evaluated with regard to lesion location, size, margin, muscle or bone involvement and intrinsic imaging characteristics on CT and MRI. The tumors were located in the thigh (three patients), back (two patients) and upper arm (one patient). The tumors ranged in size from 2.3 cm to 7.5 cm (mean, 5.2 cm), were mainly well circumscribed and showed no evidence of calcification prior to treatment. Margins were well defined in four out of the six patients. Four ...
Our aim was to investigate the diagnostic reliability of multidetector-row computed tomography (MDCT) for preoperative assessment of local tumoral spread in hilar cholangiocarcinoma. Thirteen of 30 consecutive patients with hilar cholangiocarcinoma who underwent surgery, excluding 17 patients who underwent biliary drainage or preoperative portal embolization, were retrospectively evaluated. Using MDCT systems of 4 detector rows or 16 detector rows, plain and dynamic contrast-enhanced images of three phases were obtained. Extent of tumor spread and lymph node metastasis were assessed with MDCT and compared with histopathological findings. The Bismuth-Corlette classification of hilar cholangiocarcinoma with MDCT were type I, 1 patient; type IIIa, 3 patients; type IIIb, 4 patients; and type IV, 5 patients; those with histopathological findings were type I, 1 patient; type IIIa, 2 patients; type IIIb, 4 patients; and type IV, 6 patients. One patient diagnosed as type ...
Radioimmunotherapy (RIT) is a new treatment option for patients with non-Hodgkin lymphoma (NHL). Response to RIT currently remains difficult to predict using conventional prognostic factors and could be refined using functional imaging. The goal of this work is to evaluate the value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) in predicting response to Yttrium 90-labeled monoclonal antibodies for patients with NHL. Thirty-five patients with NHL who had undergone 18F-FDG PET prior to RIT with either 90Y-ibritumomab tiuxetan (group A; n=17) or 90Y-epratuzumab tetraxetan (group B; n=18) were included in this retrospective study. Four functional criteria were determined for each tumour lesion in a given patient: maximum and mean standard uptake values (SUVmax and SUVmean), functional lesion volume (LVol) and total lesion glycolysis (TLG, product of the volume and the SUVmean). For each patient, we determined highest SUVmax and SUVmean, cumulative ...
To analyze the enhancement patterns and morphology of peripheral cholangiocar-cinomas (CCs), as seen on three-phase helical CT. Three-phase helical CT scans of 25 cases of 24 patients with pathologically-proven CCs were retrospectively reviewed. After intravenous administration of contrast media, scans of the hepatic arterial phase (HAP) were obtained at 30 seconds, of the portal venous phase (PVP) at 70 seconds, and of the delayed phase (DP) at 3 minutes. Linear, dense, marginal enhancement was seen in 17 CCs (68%), mainly on HAP images (64%) while thick, band-like, peripheral enhancement was seen in 18 (72%), on either PVP (52%) or DP (44%). Internal enhancement was observed in 23 CCs (92%) on PVP (68%) and/or DP (88%), and 19 CCs (76%) among them showed incomplete enhancement of stippled and/or septated patterns. Most (76%) were seen throughout all three phases to be hypoattenuating, as compared to the surrounding parenchyma. On HAP, wedge-shaped parenchymal ...
To evaluate the radiological patterns of vascular invasion in peripheral cholangiocarcinomas. Hepatic arteriography and portography in 20 cases with cholangiocarcinoma including 12 cases with angiographic CT were retrospectively analyzed. The arteriography showed no arterioportal shunt, hypertrophy of tumor vessel, or tumor staining extending to central portion of the mass in all cases. However, doughnut shaped peripheral tumor staining was seen until late hepatogram phase in 12 cases and compensatory hyperperfusion around the mass was seen in six cases (eight cases if include arterial CT). Encasement of tumor vessel was seen in 12 cases, and hypertrophy of feeding vessel in nine cases. On portography, the filling defect on segmental portal branch could be demonstrated only in 11 cases. Shape of the portal defect was tapered narrowing in six cases, abrupt narrowing in two cases but intraluminal nodular filling defect was not seen. Remaining three cases were ...
A retrospective review was made of 2,659 autopsy patients obtained during the past 31 years and a half in Hiroshima Atomic Bomb Hospital. Of the patients, 1,328 (49.9%) were A-bomb survivors. Five hundred and six A-bomb survivors (38.1%) had been exposed at up to 2,000 m from the hypocenter. There was no correlation between the incidence of malignancy and the presence of exposure to A-bombing. The incidence of multiple malignancy was slightly higher in A-bomb survivors exposed at the place nearer the hypocenter. In the group of A-bomb survivors, cancer of the lung was the most common for men; and cancer of the stomach was the most common for women. The incidence of malignancy has been high in the exposed group during the early eras of examination; however, this figure has recently become high in the non-exposed group. Regarding the kinds of malignancy, there was no significant difference between the exposed and non-exposed groups. The incidence of leukemia was high ...
Recently the association of a rare disease named ''nephrogenic systemic fibrosis'' (NSF) with the administration of gadolinium-containing contrast media, especially gadodiamide (Omniscan, GE-Healthcare), was described. NSF is a scleroderma-like disease characterised by widespread tissue fibrosis. Until now, NSF cases were observed only in patients with kidney disease. Almost all patients were suffering from chronic renal insufficiency, 90 % of them required renal replacement therapy. The true incidence of the disease is unknown. First retrospective analyses of selected collectives of patients with end-stage renal disease showed 2 - 5 % cases of NSF after administration of Gadolinium-containing contrast agents with an odds ratio of 20 - 50 in comparison to non-exposed controls. NSF is a serious adverse reaction, which may result in severe disabilities and even death. Therefore all radiologists applying gadolinium-based contrast ...
Background: In the German CESS-studies, Ewing's sarcoma patients with pulmonary metastases at diagnosis were considered as candidates for additional bilateral lung irradiation. We have retrospectively analyzed the impact of radiotherapy on survival. Material and methods: Out of 42 patients presenting with pulmonary metastases of Ewing's sarcoma between 1981 and June 1992, 30 were evaluable. One patient died of progressive disease prior to lung irradiation. The other patients had a complete radiographic remission either after chemotherapy (n=25) or chemotherapy plus resection of lung metastases (n=4). 22 patients received bilateral lung irradiation with doses of 12 to 21 Gy (once daily 1.5 Gy or twice daily 1.25 Gy), six had no further treatment and one had bone marrow transplantation for consolidation. Results: Ten patients are in complete remission, nine of them had received lung irradiation and one had a complete resection of three lung metastases. Two patients ...
Purpose: To assess the relationship between large intracranial vessel occlusive vasculopathy (vasculopathy) and radiation therapy, and to clarify the clinical efficacy of magnetic resonance (MR) imaging in the diagnosis and screening of the vasculopathy. Methods and Materials: We retrospectively evaluated the medical records and serial MR images for 32 pediatric patients, in whom radiation therapy had been given to fields including the circle of Willis and major cerebral arteries. All children had periodically undergone follow-up neurologic assessment and MR imaging examinations at Kanagawa Children's Medical Center for more than one year after radiation therapy (range 1.3-14 years). Patients who had not remained free of tumor progression up to the time of final evaluation were excluded. Results: Vasculopathy developed in 6 of 32 patients 2-13 years after radiation therapy. Three of them presented with transient ischemic attacks (TIA) and the other three showed ...
From 1977 through 1984, 293 previously untreated patients with biopsy proven carcinoma of the uterine cervix were treated by whole pelvis irradiation and high intensity 60Co remote afterloading (RAL) intrauterine tandem techniques in Haiti. The treatment results were analyzed retrospectively to evaluate the therapeutic results and prognostic factors of a strict protocol involving 40 Gy to the whole pelvis (2 Gy/day, 5 days/week). In addition, on the 5th day of the 3rd week, the first outpatient 60Co remote afterloading intracavitary insertion, delivering 7.5 Gy to point A with each insertion, repeated 3 times by a week separation for a total of 4 times. The total TDF for external beam plus RAL was 158 and 175 for early and late effects respectively. One hundred-four patients were evaluable after 1 year or more follow-up, with a median of 26.5 months. No evidence of disease (NED) by Stage at 1 year was: Stage I of 100% (3/3), Stage II of 82% (9/11), Stage III of 80% ...
Objective: Solid pseudopapillary tumor of the pancreas (SPTP tumor) is a rare pancreatic neoplasm with low malignant potential, which usually affects female patients in the second or third decades of life. It is a non-functional, slow-growing neoplasm that very often reaches considerable size before the first symptoms appear. Symptomatology is frequently related to tumor size. Surgical excision is usually curative in most cases. Infrequently the tumor can appear in male patients or in aged women, which can make the diagnosis more difficult. Some patients develop liver metastases in the follow-up that can be resected. Our purpose is to review the radiological and pathological findings of SPTP with emphasis on these infrequent cases. Subjects and methods: The medical records and radiological findings of patients who underwent surgery for SPTP between 2000 and 2005 were retrospectively reviewed. Study eligibility required that patients had undergone surgical resection ...

The first Brazilian historical mortality cohort study on miners was conducted. The cohort consisted of 3224 workers in the underground coal mining industry in southern Brazil. This industry has been operating since 1942 without compliance with any regulatory standards, since there were no relevant national regulations. Over almost 60 years, about 5000 workers were exposed to high levels of radiation. However, later radiation exposure was significantly reduced, particularly that due to radon exposure. Recent radon concentration measurements indicated an average annual exposure to radon progeny of 2.1 WLM, ranging from 0.2 to 7.2 WLM. As radon exposure in the past was unknown, it can be suggested that mine workers have not been working safely as regards the health hazard related to radon and radon progeny exposure. The cohort inclusion criteria are as follows: (a) all male employees who had worked for at least one year at the coal mine; (b) workers with complete workplace information ...
This work describes an experimental verification of the two-photon dose calculation engines available on the Helax-TMS (version 6.1) commercial radiotherapy treatment planning system. The performance of the pencil beam convolution and the collapsed cone superposition algorithms was examined for 4, 6, 15 MV beams, under a range of clinically relevant irradiation geometries. Comparisons against measurements were carried out in terms of absolute dose, thus assessment of the accuracy of monitor unit (MU) calculations was also carried out. Results show that both algorithms agree with measurement to acceptable tolerance levels in most cases in homogeneous water-equivalent media irradiated under full scatter conditions. The collapsed cone algorithm slightly overestimates the penumbra width and this is mainly due to discretization effects of the fluence matrix. The accuracy of this algorithm strongly depends on the resolution of the patient density matrix. It is recommended that the density ...
In not infected knee prostheses bone scintigraphy is a possible method to diagnose mechanical loosening, and therefore, to affect treatment regimes in symptomatic patients. However, hitherto studies showed controversial results for the reliability of bone scintigraphy in diagnosing loosened knee prostheses by using asymptomatic control groups. Therefore, the aim of our study was to optimize the interpretation procedure and to evaluate the accuracy using results from revision surgery as standard. Methods: Retrospectively, we were able to examine the tibial component in 31 cemented prostheses. In this prostheses infection was excluded by histological or bacteriological examination during revision surgery. To quantify bone scintigraphy, we used medial and lateral tibial regions with a reference region from the contralateral femur. Results: to differentiate between loosened and intact prostheses we found a threshold of 5.0 for the maximum tibia to femur ratio of the ...

The number of multislice CT (MSCT) scans performed in polytraumatized children has increased rapidly. There is growing concern regarding the radiation dose in MSCT and its long-term consequences, especially in children. To determine the effective dose to polytraumatized children who undergo whole-body MSCT. A total of 51 traumatized children aged 0-16 years underwent a polytrauma protocol CT scan between November 2004 and August 2006 at our institution. The effective dose was calculated retrospectively by a computer program (CT-Expo 1.5, Hannover, Germany). The mean effective dose was 20.8 mSv (range 8.6-48.9 mSv, SD{+-}7.9 mSv). There was no statistically significant difference in the effective dose between male and female patients. Whole-body MSCT is a superior diagnostic tool in polytraumatized children with 20.8 mSv per patient being a justified mean effective dose. In a potentially life-threatening situation whole-body MSCT provides the clinicians with ...
Faced with increasing indications for the use of iodine-131 labelled lipiodol in the treatment of hepatocellular carcinoma (HCC), it appears essential to develop an individualized dosimetrY so we can predict the potential effectiveness of this treatment and adjust the activity to be injected. We developed a dosimetric protocol based on imagery and dedicated to calculation of the tumoral dose during the treatment of hepatocellular carcinoma by iodine-131 labelled lipiodol. This concept was developed on a SPECT- CT gantry, and integrates corrections for the phenomena of attenuation, diffusion and dead time. The dose is calculated according to the formalism proposed by the Medical Internal Radiation Dose Committee (MIRD). This protocol was applied to a series of 41 patients in the framework of a retrospective study. The results obtained in terms of biodistribution are compatible with previously published data. The total biological response rate is 42% (n= 41) for the ...
The objective was to characterize imaging findings of benign notochordal cell tumors (BNCTs). Clinical and imaging data for 9 benign notochordal cell tumors in 7 patients were reviewed retrospectively. Conventional radiographs (n = 9), bone scintigrams (n = 2), computed tomographic images (n = 7), and magnetic resonance images (n = 8) were reviewed. Eight of the 9 lesions were stained with hematoxylin-eosin and microscopically examined. There were 3 male and 4 female patients with an age range of 22 to 55 years (average age, 44 years). Two patients had two lesions at different sites. The lesions involved the cervical spine in 4 patients, the lumbar spine in 2, the sacrum in 2, and the coccyx in 1. The most common symptom was mild pain. The lesions of 2 patients were found incidentally during imaging studies for unrelated conditions. Five patients underwent surgical procedures. One patient died of surgical complications. All other patients have been well without ...
Lipiodol CT is a important modality for the diagnosis of hepatocellular carcinoma and compared is usually performed at 2-3 week after Lipiodol injection. Therefore, we assessed and the diagnostic value and merits of immediate CT after chemoembolization from there of 2-3 week delayed Lipiodol CT. Thirty three cases of chemoembolization which were performed both immediate CT after chemoembolization and 2-3 week delayed Lipiodol CT were reviewed retrospectively. They were divided into four grades according to pattern of lipiodol uptake by three radiologists. The diagnostic value of immediate Lipiodol CT was compared to delayed Lipiodol CT. Grade 0 was two cases(3.0%) and Grade 1 was seven cases(21.2%). In the cases of Grade 2(23/33, 69.7%), tumor uptake could be dishing wished from parenchymal uptake of Lipiodol by its density and pattern, Tumor uptake showed dense and homogeneous pattern, but parenchymal uptake revealed less dense and hepatogram-like wedge shaped ...
Purpose/Objective. Daily Cone-beam computed tomography (CBCT) in room imaging is used to determine tumour shrinkage during a full radiotherapy (RT) course. In addition, relative interfractional tumour and lymph node motion is determined for each RT fraction. Material and methods. From November 2009 to March 2010, 20 consecutive lung cancer patients (14 NSCLC, 6 SCLC) were followed with daily CBCT during RT. The gross tumour volume for lung tumour (GTV-t) was visible in all daily CBCT scans and was delineated at the beginning, at the tenth and the 20th fraction, and at the end of treatment. Whenever visible, the gross tumour volume for lymph nodes (GTV-n) was also delineated. The GTV-t and GTV-n volumes were determined. All patients were setup according to an online bony anatomy match. Retrospectively, matching based on the internal target volume (ITV), the GTV-t or the GTV-n was performed. Results. In eight patients, we observed a significant GTV-t shrinkage ...
Background. Metabolic disturbances such as anoxia and hypoglycaemia are important in causing maldevelopment of the neonatal brain. While there have been some pathology studies of the effects of neonatal hypoglycaemia on brain development, reports of MRI findings in such infants have been rare. Objectives. To describe the MRI findings in neurologically handicapped children who had suffered from neonatal hypoglycaemia and to evaluate the relationship between the neurological impairment and neonatal hypoglycaemia. Materials and methods. We retrospectively evaluated the MRI findings in eight full-term infants with neonatal symptomatic hypoglycaemia who later exhibited neurological handicap. The age at which the MRI scans were obtained ranged from 9 months to 8 years 10 months (mean 4 years 1 month, median 4 years). Results. The most striking findings were prolonged T1 weighting and T2 weighting in the parieto-occipital periventricular deep white matter in six patients, ...
Aim of this retrospective study was to assess the capability of contrast-enhanced helical computerised tomography (CT) in sigmoid diverticulitis, especially relative to selection of the appropriate treatment. The findings of 41 patients were reviewed from 1998 to 1999 June. Contrast-enhanced helical CT is the imaging modality of choice in assessing mild or severe acute diverticulitis because it provides useful information for appropriate treatment planning in the emergency setting. This technique is most useful in questionable cases and in patients with suspected severe diverticulitis where a choice must be made between different treatment options.
The authors carried out a retrospective study on 4 years' work on a series of patients with proved active tuberculosis examined in the Departments of Bronchopneumology and Infectious Disease of the Arezzo Civil Hospital. The population was heterogenerous from the immunologic viewpoint. This paper compared the effectiveness of Computed Tomography (CT) with that of conventional chest radiography (CR) in the diagnosis and prognosis of tuberculosis. All patients underwent chest CR in double projection and conventional CT studies of the chest; 16 patients were also submitted to high resolution CT (HRCT). Consolidation was the most frequent parenchymal lesion in immunocompetent patients; it was often associated with cavitation, with no clear topographic prevalence in the upper lobes. Pleural effusion, as the only sign of disease and with empyema features, was found in 10 immunocompetent patients. No predominant pattern was found in the other two groups, even though ...
Background and purpose: stereotactic radiosurgery (SRS) and also fractionated stereotactic radiotherapy (SRT) offer high local control (LC) rates (> 90%). This study aimed to evaluate three-dimensional (3-D) tumor volume (TV) shrinkage and to assess quality of life (QoL) after SRS/SRT. Patients and methods: from 1999 to 2005, 35/74 patients were treated with SRS, and 39/74 with SRT. Median age was 60 years. Treatment was delivered by a linear accelerator. Median single dose was 13 Gy (SRS) or 54 Gy (SRT). Patients were followed up ? 12 months after SRS/SRT. LC and toxicity were evaluated by clinical examinations and magnetic resonance imaging. 3-D TV shrinkage was evaluated with the planning system. QoL was assessed using the questionnaire Short Form-36. Results: Median follow-up was 50/36 months (SRS/SRT). Actuarial 5-year freedom from progression/overall survival was 88.1%/100% (SRS), and 87.5%/87.2% (SRT). TV shrinkage was 15.1%/40.7% (SRS/SRT; p = 0.01). Single dose ( 0.05). ...
The aim of this study was to compare multidetector-row computed tomography (MDCT) findings between cases of sclerosing cholangitis with autoimmune pancreatitis (SC-AIP) and infiltrative extrahepatic cholangiocarcinoma (IEC). We retrospectively assessed MDCT findings from 16 IEC cases and 13 SC-AIP cases. MDCT findings were analyzed with regard to location, length, wall thickness, contour, stricture wall enhancement pattern, proximal duct diameter, and the presence of diffuse concentric thickening in the proximal duct and gallbladder wall thickness. Stricture length, stricture wall thickness, and proximal duct diameter were significantly smaller for SC-AIP than for IEC: 19.3#+-#8.7 vs. 31.8#+-#12.0 mm (P=0.004), 2.1#+-#1.3 vs. 4.1#+-#1.3 mm (P<0.001), and 9.2#+-#3.9 vs. 13.3#+-#5.0 mm (P=0.012), respectively. SC-AIP was correlated with stricture location in both the intrapancreatic and hilar hepatic bile ducts, concentric stricture contour (P<0.001), and ...
Thalassaemic patients are in need of frequent assessment of bone age because of growth failure and pubertal disorders. To compare the ''rapid'' Greulich and Pyle (G and P) method with the third edition of the Tanner and Whitehouse (TW3) method for determining skeletal maturity and predicting final height in thalassaemic patients. A total of 191 radiographs from 58 patients (28 male, 30 female) were retrospectively evaluated by two investigators, one for each method. In 47 radiographs from 15 patients having attained their adult height, predicted final height was calculated according to each method. The mean bone ages determined by both the G and P and TW3 methods were lower than mean chronological age, although the differences were not statistically significant (10.04 {+-} 3.69 years and 9.98 {+-} 3.39 years vs. 10.78 {+-} 3.96 years, respectively). Both methods had a tendency to over-estimate final height. Overall, the TW3 ...
Attention is given to recent research on gear fault diagnostic techniques. A mathematic model for vibration signals of defective gears is presented. Novel methods of gear fault diagnosis, such as zoom complex envelope analysis, wideband demodulation techniques, bispectrum analysis, correlation spectrum analysis, and maximum entropy spectrum analysis, are demonstrated. The wide-band demodulation technique is found to exhibit a high SNR. Bispectrum and correlation spectrum are excellent for some mechanical fault forms.
Based on the reliability analysis methods of FMEA and FTA, according to the result of ETA of PRA in Daya by NPP, the top events of the fault trees of reactor protection system and the success criteria were established. By using RISK-SPECTRUM procedure, the unavailability and the minimal cut-sets (MCS) of the fault trees were obtained. The results of analysis was put into the visual risk analysis software of Daya bay NPP as the support of data

The computer vision approach to image analysis is discussed from two aspects. First, this approach is constrasted to the pattern recognition approach. Second, how external knowledge and information and models from other fields of science and engineering can be used for image and scene analysis is discussed. In particular, the connections between computer vision and computer graphics are pointed out.
The report consist of the following appendices: A. Cost annualization model; B. Facility level analysis; C. Market model methodology; D. Summary of Demand and Supply Elasticity Literature; E. Sensitivity analysis; and F. Cost effectiveness analysis.
Radionuclide X-ray fluorescence analysis was used for the determination of Cu, Y and Ba in very thin high-temperature superconducting films. The precision of the method is better than 3% for about 1 #mu#m thick films. The atomic emission ICP spectrometry was used to testify results of XRF analysis. An acceptable agreement of both methods was obtained. (author) 4 refs.; 2 tabs.
A novel CFD/structural analysis was performed to predict functionality of a cross parachute under loadings near the structural limits of the parachute. The determination of parachute functionality was based on the computed structural integrity of the canopy and suspension lines. In addition to the standard aerodynamic pressure loading on the canopy, the structural analysis considered the reduction in fabric strength due to the computed aerodynamic heating. The intent was to illustrate the feasibility of such an analysis with the commercially available software PATRAN.
A methodology of tsunami PSA was developed in this study. A tsunami PSA consists of tsunami hazard analysis, tsunami fragility analysis and system analysis. In the case of tsunami hazard analysis, evaluation of tsunami return period is major task. For the evaluation of tsunami return period, numerical analysis and empirical method can be applied. The application of this method was applied to a nuclear power plant, Ulchin 56 NPP, which is located in the east coast of Korean peninsula. Through this study, whole tsunami PSA working procedure was established and example calculation was performed for one of real nuclear power plant in Korea
The Fuels Material Examination Facility/Fuel Assembly Area is proposed to be used for fabrication of mixed oxide fuel to support the Fast Flux Test Facility (FFTF) tritium/medical isotope mission. The plutonium isotope mix for the new mission is different than that analyzed in the FMEF safety analysis report. A reanalysis was performed of three representative accidents for the revised plutonium mix to determine the impact on the safety analysis. Current versions computer codes and meterology data files were used for the analysis. The revised accidents were a criticality, an explosion in a glovebox, and a tornado. The analysis concluded that risk guidelines were met with the revised plutonium mix.
