Solid-pseudopapillary tumor of the pancreas in a 13-year-old girl - case report

The authors present a case in which primary Ewing's sarcoma of the right petrous pyramid in a 9-year-old girl showed no uptake on a /sup 99m/Tc-pertechnetate nuclide angiogram. Intense uptake was present on a /sup 99m/Tc-polyphosphate bone scan, but a static brain scan was only minimally abnormal. The diagnosis and treatment of Ewing's sarcoma are reviewed.

Ewing's sarcoma of the humerus mimicking fibrous dysplasia on imaging and biological behavior

The solid-pseudopapillary tumor (SPT) of the pancreas is a rare type of exocrine pancreatic neoplasm. SPT predominantly affects young women and female children, and is usually discovered incidentally. This tumor is generally benign with a low incidence of malignancy. A 13-year-old girl was admitted to the hospital with a few weeks' history of mild abdominal pain and jaundice. On physical examination, there was no palpable mass. The laboratory tests showed increased SR, CRP, high bilirubin, amylase and lipase serum levels. Ultrasound imaging revealed a solid lesion in the region of the pancreatic head. On MRI, precise tumor localization in the head of the pancreas with pancreatic duct dilatation and compression of the common bile duct were visualized. Pancreaticoduodenectomy and cholecystectomy was performed with good clinical outcome. Case report 437: Solitary (unicameral, simple) bone cyst of the scapula

We present the case of a 12-year-old girl who presented with a pathological fracture through a benign-appearing osteolytic lesion that was presumed to represent fibrous dysplasia. The fracture healed, and over the next 2.5 years there was further refracturing and healing with progressive osteolysis. A biopsy was performed and revealed Ewing's sarcoma. The disease was locally controlled by neoadjuvant chemotherapy and radiation therapy. The patient is disease free with excellent function 6 years following the discovery of the lesion. We illustrate and discuss the sequence of events. (orig.)

A case of Ewing's sarcoma treated successfully with chemotherapy and radiotherapy

A case has been presented of an expanding, aggressive bone cyst in the scapula of an 11-year-old girl. The lesion had the appearance of a highly aggressive, cyst-like disorder that appeared to be benign. Biopsy and histological preparation confirmed the presence of a solitary bone cyst. The clinical, radiological, and pathological criteria associated with a solitary bone cyst, particularly in the scapula, were discussed and the literature was reviewed. The differential diagnosis was stressed and a number of examples of scapular lesions was presented radiologically, particularly lesions that might be confused with solitary bone cyst. (orig./SHA).

Unusual extraction treatment of Class I bialveolar protrusion using microimplant anchorage.

We encountered a case of Ewing's sarcoma. The patient was a 19 year-old girl, who visited a local physician because of pain in the left scapular region, and was referred to us. Roentgenograms of her left scapula revealed an irregular moth-eaten shadow which was suspected to be a malignant bone tumor. The histological diagnosis of Ewing's sarcoma was comfirmed by needle biopsy. The initial stage of treatment consisted of radiation therapy (6000 rads) and chemotherapy (VAC and ADR). At the end of the initial stage, the symptoms were improved, the size of the local lesion was reduced on the roentgenogram and there was no evidence of distant metastases. (author).

Mesenchymal stem cell transplantation for diffuse alveolar hemorrhage in SLE

This case report describes the treatment of a 16-year-old girl who had severe bialveolar protrusion. Patients with bialveolar protrusion are commonly treated with four first premolar extractions and retraction of the anterior teeth. Unfortunately in this patient, the mandibular left second molar had to be extracted because of extensive caries. To create sufficient space for retraction of the anterior teeth, the mandibular left posterior teeth were retracted with the mandibular posterior microimplant (1.2 mm in diameter, 6 mm long) placed into the retromolar area followed by en masse retraction of the mandibular anterior teeth. Microimplants can provide anchorage for obtaining a good facial profile even without premolar extraction in the case of bialveolar protrusion with the absence of the second molar. PMID:17319776

Pyogenic sacroiliitis is a rare entity in children. Diagnosis is often delayed because of its variable clinical presentation, low suspicion by the examining physician, and rare findings on radiographs. Delayed diagnosis, however, results in complications such as iliopsoas abscess. We report the case of a 12-year-old girl hospitalized with a 21-day history of fever, pain in the left iliac fossa, and flexion contracture of the hip. On examination, she had fever (38.9 ^oC), psoitis, localized tenderness at the left sacroiliac joint, and pain elicited by lateral compression of the pelvis. The abdominal examination was normal. Individual differences in the anti-fat attitudes of preschool-children: the importance of perceived body size.

Case report: Denys- Drash syndrome.

Preschool-age children hold negative attitudes toward overweight peers (i.e., anti-fat attitudes), but little is known about individual differences in these attitudes. The current study investigated actual weight status and perceived body size in relation to preschool-age children's anti-fat attitudes. Sixty-nine 4-6 year-olds (61% girls) were individually interviewed about their body size perceptions using a figure rating scale and anti-fat attitudes using multiple methods (including an adjective rating scale and ratings of acceptability for different body shapes). Results suggested that children's perceived body size, not actual body size, was related to their attitudes about an overweight figure and the number of figures rated as acceptable. Children who perceived themselves as heavier held fewer anti-fat attitudes. Perceived and actual body size ratings were not related to ratings of a thin figure. This paper investigates whether China has reached postnatal nutrient intake equality between boys and girls, despite an exceptionally high ratio of boys to girls at birth, after dramatic technological advances in prenatal sex determination, rapid increases in income, and improved educational opportunities for females. Dominance methods applied to data from the Chinese Health and Nutrition Surveys (selected years 1991-2004) reveal no bias in calorie consumption between girls and boys. We find some weak evidence of protein bias toward boys in 1991, but it disappeared by 2004. (JEL I32, O15, N35)

In many industrialised societies, women remain underrepresented in the sciences, which can be predicted by the gender gap in math achievement at school. Using PISA 2006 data, we explore the role of family background and single-sex schooling in girls' disadvantage in maths in South Korea and Hong Kong. This disadvantage is found to be associated with single-sex schooling, but not with family background. Attending a girls' school confers a benefit only in South Korea, whereas the gendered curriculum counteracts the selectivity advantage of girls' schools in Hong Kong. We find that a gendered social structure prevalent in both societies.

BackgroundMetastases to the stomach from an extra-gastric neoplasm are an unusual event, identified in less than 2% of cancer patients at autopsy. The stomach may be involved by hematogenous spread from a distant primary (most commonly breast, melanoma or lung), or by contiguous spread from an adjacent malignancy, such as the pancreas, esophagus and gallbladder. These latter sites may also involve the stomach via lymphatic or haematogenous spread. We present three cases of secondary gastric malignancy.Methods/ResultsThe first is a 19-year-old male who received a diagnosis of testicular choriocarcinoma in September 2004. Comparison of measured and simulated temperatures revealed thermoregulation in children to be similar to that of adults. The authors used studies of local cerebral metabolic rate for glucose (LCMRGlc) to examine development of cerebral organization in 5 days to 1 year old children. A group (n=8) of infants with diverse pediatric disorders allowed investigation of developmental changes in LCMRGlc, while also providing relevant clinical management information. Patients consisted of questionable and definite neonatal seizures, cerebral embolism from cardiac sources, and otherwise normal infants with facial nevi with consideration of Sturge-Weber. Gradual increase in cortical LCMRGlc coincides with suppression of intrinsic subcortical reflexes present in all newborns. Two retarded children (2 years old) showed LCMRGlc developmental patterns of a few days old, which corresponded to their functional and mental status. An estimated 110 million children between the ages of 6 and 11--60 percent of them girls--will not see the inside of a classroom this year, and another 150 million are likely to drop out before completing primary school. Yet this is a disease with a known cure. We know what tools are needed and what models are proven to work. We also know that the cost of that cure--perhaps $7.5 billion to $10 billion per year--is minuscule compared with the enormous benefits it will bring for health, economics, women's empowerment, and basic human dignity.

A population of male civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease. Field activities started in December, 1982 and were concluded in July, 1984. Diagnosis was made using real-time ultrasonography. Participation in the study was 71.5%. Prevalence of gallstone disease was 8.2% and increased with age from 2.3% in the 20- to 25-year-old age group to 14.4% in the 60- to 69-year-old age group, based on both presence of gallstones and history of cholecystectomy. About one-third of the subjects with gallstone disease had previously been submitted to cholecystectomy. Only 7.7% of the subjects with presence of gallstones complained of at least one episode of biliary pain in the preceding 5 years. Frequency of "minor" dyspeptic symptoms was not different between men with and those without gallstones. PMID:3391519

A population of female civil servants in Rome, Italy, was investigated to determine the prevalence of symptomatic and asymptomatic gallstone disease and to define the associated factors. Case 1 was an 18-year-old female who had had an operation and radiation therapy for Ewing's sarcoma of the left 7th rib. She was referred to our hospital after a recurrent tumor was found. Radiation therapy (tumor dose 46.2 Gy) and chemotherapy were given. The tumor disappeared and there has been no relapse for 1 year and 3 months after the treatment. Case 2 was a 2-year-old-infant. Radiation therapy (tumor dose 74 Gy) was given for primary Ewing's sarcoma of the left 6th rib. The tumor became small and was successfully removed at operation. There has been no relapse or distant metastasis for 8 months following the operation. We emphasize the importance of multidisciplinary treatment in case 1 and the usefulness of preoperative radiotherapy in case 2. (author).

Temporary endovascular control of a bleeding aortoenteric fistula by transcatheter coil embolization.

''Hair-on-end'' skull changes resembling thalassemia were rarely described in the 1950s and 1960s in children with cyanotic congenital heart diseases; these changes were described almost entirely in patients with tetralogy of Fallot or D-transposition of the great arteries. As these lesions have become correctable, the osseous changes, never common, seem now only to exist in a small number of patients with uncorrectable complex cyanotic congenital heart disease who survive in a chronic hypoxic state. We present two cases: a case of marked marrow expansion in the skull of a 5-year-old boy with uncorrectable cyanotic heart disease studied by CT, and a second case of an 8-year-old with tetralogy of Fallot and pulmonary atresia studied by plain skull radiographs. Successful coil embolization of an unusual aorto-superior mesenteric vein fistula.

A 67-year-old man presented with gastrointestinal bleeding resulting from a fistula between the aortic stump and the fourth part of the duodenum 14 months after an axillobifemoral bypass, graft excision, and aortic stump closure for aortic graft infection. The patient initially refused any attempt at definitive surgical treatment so coil embolization of the aortic stump was performed via a brachial approach. Coil embolization may be a useful adjunct in the management of such difficult problems. This approach may buy time and allow careful planning of a definitive treatment or may serve as a palliative procedure in inoperable patients. PMID:15947052

Stress fracture of the clavicle associated with sternocostoclavicular hyperostosis

The aortovenous fistulas are rare, most of them are aortocaval fistula. The non-caval communication of the fistula is sparse. Herein we report a 47-year-old female diagnosed as traumatic aorto-superior mesenteric vein (Ao-SMV) fistula. The abdominal pain, fullness, and frank bruit on abdominal auscultation suggested the diagnosis of an intra-abdominal arteriovenous fistula. The diagnosis of Ao-SMV was further confirmed by the computed tomography (CT) and aortography. The fistula was successfully treated with transcatheter coil embolization. Scintigraphic detection of a radiologically equivocal fracture

We report a case of stress fracture of the clavicle associated with sternocostoclavicular hyperostosis. A 60-year-old man sustained a stress fracture of the right clavicle with no history of trauma. On radiography, hyperostosis of the anterior chest wall and ankylosis of the sternoclavicular joint were evident in addition to the fracture. Fracture healing was uneventful after 2.5 months. Ankylosis of the sternoclavicular joint may have caused increased stress at the midshaft of the clavicle by daily activity or minor trauma. Scintigraphic detection of a radiologically equivocal fracture

This case report makes evident that in a 75 years old patient with a (cured) femoral neck fracture 15 years ago a refracture at the same site following a new trauma could not be detected by conventional X-ray. The follow-up of a radionuclide bone study, however, enabled the dignosis of a refracture. From these findings, it is further evident that the decreased bone metabolism in older patients has to be considered. Thus, although the initial bone scan following the trauma is negative, a follow-up scan should be performed within a 4 day-period.

Primary B-cell lymphoma of thymus is uncommon. We describe the case of a 23-year-old woman with a history of shortness of breath and distended neck veins of 1-month duration. A chest roentgenogram and computed tomographic chest scan of the showed a large anterior mediastinal mass. Tru-cut biopsy revealed primary B-cell lymphoma of thymus. She underwent chemotherapy with no symptomatic improvement. Post-chemotherapy computed tomographic scan of the chest showed no appreciable reduction in tumor size. The 'elementary' particle physics began in 1935, when Hideki Yukawa published his pioneering pi-meson theory, and the problem of strong interaction was finally solved 40 years later by the establishment of the Standard Model. The composite models of hadrons by the Sakata school and Sin-itiro Tomonaga's renormalization theory for quantum electrodynamics played essential roles for finding this beautiful solution. It is really surprising that it took only 40 years to solve such desperately difficult problem. This paper reported on a study in which partial discharge (PD) measurements were performed on 6 underground transmission cable circuits to determine the effectiveness of PD measurements on fluid fill cables. The cables were between 37 and 56 years old. PD activity was detected from the terminations of H11L and H3L Cable 2. However, for cables from H3L Basin TS to Mill Street junction, PD activity was detected originating from within the main cable insulation.

Nasal cavity volume measured in normal persons by MRI

Nasal cavity volume was studied in 69 healthy volunteers (from 8 to 23 years old) by MRI. The right and left nasal cavity areas were measured on horizontal images displayed directly on the MRI console. The nasal cavity volume was obtained by integrated volume calculation. The nasal cavity volume was positively correlated with age, body height and weight up to age 16. The body height and weight reached a plateau at 16 years, but the nasal cavity volume continued to increase. There was no significant difference in the volumes of the right and left cavities. Isolated laevocardia and corrected transposition of the great vessels with total situs inversus

Pulmonary embolism complicated by transcatheter arterial chemoembolization (TACE) in known to be due to the use of large amount of lipiodol as an embolic agent. To our knowledge, massive tumoral pulmonary embolism following TACE and confirmed by surgery has not been described in the literature. In this report, we detail the cas of a 49-year-old man in whom cyanosis and hypotension developed abruptly on the day of TACE.. Chest CT revealed diffuse low-attenuated lesions in both pulmonary arteries. Histopathological specimens after emergent pulmonary arterial embolectomy confirmed the presence of massive tumor emboli of hepatocellular carcinoma.

Incidental discovery of asplenia syndrome, with situs inversus and a normal heart by radionuclide biliary imaging. A case report

Isolated laevocardia combined with corrected transposition of the great arteries and complete situs inversus is rare. Due to discordant interrelationships between atria and ventricles and between ventricles and great arteries, both anomalies result in a normal circulation. In the presented case no associated cardiac malformations were present and the 55-year-old woman remained symptom-free for a long time until she developed an acquired valve disease. A systematic approach to chamber localisation in complex morphologic lesions of the heart is reviewed, and the value of CT, which can be applied successfully in older children and adults is demonstrated. Improvement of muscle strenght independently of analgesic effect following spinal cord stimulation. A case report.

/sup 99m/Tc biliary imaging was performed on a 34-year-old woman who was being investigated for suspected cholelithiasis. A left-sided liver was detected. On subsequent radionuclide imaging, partial visceral situs inversus and asplenia were demonstrated. Extensive roentgenographic investigation, ECG, echocardiogram, and laparotomy confirmed the radionuclide findings. Asplenia syndrome may exist without cardiovascular abnormalities and thus be undiscovered. Radionuclide imaging is important in the diagnosis of this syndrome.

Idiopathic renal hematuria in a military working dog.

Spinal cord stimulation (SCS) is frequently used for relief of chronic benign pain resistant to conservative therapies. Clinical practice suggests, at least in patients with failed back surgery syndrome (FBSS), the possibility that SCS significantly improves motor performances. We present here the case of a 41-years-old female patient with FBSS, who showed a clear improvement in muscle strength after SCS, persisting at 6-months follow-up. We speculate that the electrical stimulation of posterior columns could potentiate the caudal, segmental spinal reflexes resulting in a facilitation of motoneurons activation. PMID:16175150

Diagnostic ultrasound and contrast radiography demonstrated large blood clots in the urinary bladder and a filling defect in the right renal pelvis. At surgery, clotted blood was present in the right ureter and bladder. Following right nephrectomy, the dog returned to training. One month later, elevations in urea nitrogen and creatinine were noted. Hematuria recurred at 3 months and the dog was found dead in its kennel. A 60-year-old male was bitten by a venomous snake (Vipera ammodytes) and gradually developed signs of an allergic reaction including generalized itching, generalized rash, and chest discomfort. This was followed by severe retrosternal pain with electrocardiographic evidence of an inferior myocardial ischemia progressing to acute myocardial infarction. We report a 20-year-old woman who suffered headaches before presenting with a state of fluctuating vigilance. MRI showed diffuse high signal in the basal ganglia bilaterally on diffusion- and T2-weighted images, which had areas of both low and high apparent diffusion coefficient, presumed to correspond to cytotoxic and vasogenic oedema. Bilateral lipoma arborescens of the bicipitoradial bursa

Gallium imaging is increasingly being used for the early detection of complications in patients with AIDS. A 26-year-old homosexual man who was HIV antibody positive underwent gallium imaging for investigation of possible Pneumocystis carinii pneumonia. Widespread cutaneous focal uptake was seen, which was subsequently shown to be due to mycobacterium avium-intracellulare (MAI) septicemia. This case demonstrates the importance of whole body imaging rather than imaging target areas only, the utility of gallium imaging in aiding the early detection of clinically unsuspected disease, and shows a new pattern of gallium uptake in disseminated MAI infection.

Awarable complexity: a study on CRT picture design based on plant images by NPP operators

A 37-year-old military mechanic presented to our institution with a chronic history of a slowly enlarging left elbow antecubital fossa mass. There was no history of other chronic illness or trauma. Magnetic resonance imaging (MRI) and excisional biopsy revealed fatty villi and synovial inflammation within the left bicipitoradial bursa, consistent with lipoma arborescens. Four years later the patient presented with a 6-month history of swelling at the antecubital fossa of the opposite elbow. Diagnostic computed tomography and MRI examinations were performed. Acute lymphoblastic leukemia with multiple cytogenetic abnormalities secondary to treatment of Ewing's sarcoma

Original pictures installed in the 1st and 2nd generation type central control panels (CCP) and new 'Awarable and Complex' pictures were made on personal computers and evaluated. A total 18 of actual plant operators (M=32.3, SD=10.5 years old) participated in the evaluation. The operators rated the new CRT pictures highly. The response times using the new CRT pictures were shorter than those by the original pictures. Acute diaphragmatic paralysis caused by chest-tube trauma to phrenic nerve

We report the case of a 22-year-old man with Ewing's sarcoma who attained a complete remission (CR) after combination radiotherapy and chemotherapy. Secondary acute lymphoblastic leukemia with multiple cytogenetic abnormalities involving chromosome 5 and 7 developed 16 years later. The patient underwent induction chemotherapy and entered a CR. Peripheral blood stem cell transplantation from a matched sibling was performed successfully and he is in complete remission of both ALL and Ewing's sarcoma. Indoor radon concentration measurements in Tarqumia Girl Schools at Western Hebron Region, Palestine

A 3"1/_2-year-old child developed unilateral diaphragmatic paralysis after chest drain insertion. Plain chest X-ray demonstrated paravertebral positioning of the chest-tube tip, and magnetic resonance imaging revealed hematomas in the region of the chest-tube tip and the phrenic nerve fibers. The trauma to the phrenic nerve was apparently secondary to malposition of the chest tube. This is a rare complication and has been reported mainly in neonates. Quantitative bone scintigraphy: follow-up of a femoral osteogenic sarcoma in an adolescent girl treated by chemotherapy and by massive allograft

In this study, radon-222 in indoor air was surveyed in 62 rooms located in four governmental schools, for girls, in Tarqumia town that lies in the north western part of Hebron city in Palestine. The annual effective dose equivalents resulting from the inhalation of radon and its daughters by 2318 pupils and 102 staff members occupying the surveyed rooms were also measured. TASTRAK, a solid state nuclear track detector, has been used to measure the indoor radon concentrations at those schools thus, 124 radon detectors were distributed in the four school buildings. The radon detectors stayed for 70 days between February 2006 and April 2006. The results showed that the radon concentration and the annual effective dose equivalent in these schools were varied from 12 to 232.5 Bq/m"3 with an average of 34.1 Bq/m"3 and 0.62 to 12.0 mSv/y with an average of 1.76 mSv/y, respectively. [Neurodevelopmental follow-up of premature children in Lausanne and Geneva].

Fifteen quantitative bone scintigraphies were performed in an adolescent girl during the follow-up of a femoral osteogenic sarcoma treated by chemotherapy and massive allograft. Three hours after injection of the radiopharmaceutical (7.4 MBq/kg of 99mTc-MDP) bone activity was measured in the inferior limbs at several regions of interest centered on the hips, femurs (proximal, middle, distal) and proximal tibias. The variations of relative bone activities A/S (ratio of corresponding counting rates between two homologous regions in the affected A and in the healthy S limb) and of absolute bone activities (expressed in counts/pixel-second) are interpreted as a function of times during treatment. The quantitative results are discussed with regard to main phenomena influencing bone activity in this particular clinical case: bone growth, chemotherapy and neo-osteogenesis in allograft.

Vitiligo during Treatment of Crohn's Disease with Adalimumab: Adverse Effect or Co-Occurrence?

Preterm children born before 32 weeks of gestation represent 1% of the annual births in Switzerland, and are the most at risk of neurodevelopmental disabilities. A neurological surveillance is thus implemented in the neonatal units, and multidisciplinary neurodevelopmental follow-up is offered to all our preterm patients. The follow-up clinics of the University hospitals in Lausanne and Geneva follow the Swiss guidelines for follow-up. An extended history and neurological examination is taken at each appointment, and a standardized test of development is performed. These examinations, which take place between the ages of 3 months and 9 years old, allow the early identification and treatment of developmental disorders frequent in this population, such as motor, cognitive or behavioral disorders, as well as the monitoring of the quality of neonatal care. Using the Child Behavior Checklist with African American and Caucasian American Adopted Youth

Treatment of asymptomatic internal resorption of a maxillary premolar tooth in a military working dog.

The purpose of this study is to examine the cross-ethnic measurement equivalence of the "Child Behavior Checklist" (CBCL) for 6- to 18-year-olds (2001, parent version) using a sample of African American special needs adopted children, along with a comparison group of Caucasian American special needs adopted children. Reliability and multiple sources of postdictive criterion-related validity were analyzed, and a confirmatory factor analysis of the CBCL within and between participating groups was performed. The results indicate that for practical purposes, the CBCL items appear reliable but not valid in terms of assessing the mental health profiles of both African American and Caucasian youth. These findings are significant because they have important implications for the practical utility of the CBCL in the field for adopted youth. (Contains 6 tables.)

Transient azotaemia is associated with a high risk of death in hospitalized patients

An asymptomatic pink discoloration of a maxillary right fourth premolar tooth was discovered during a routine oral examination on a 9 year-old Belgian Malinois dog. A radiolucent lesion was seen in the pulpal chamber on radiographic examination. The lesion had perforated the mesiobuccal root of the tooth. The primary differential diagnosis was idiopathic internal resorption. The tooth was treated by partial resection (removal of the mesiobuccal root and associated crown). A vital pulpotomy and amalgam restoration was performed on the remaining tooth structure. A follow-up 1 year later demonstrated a successful treatment outcome. Symptomatic hepatic cyst in a child: treatment with single-shot injection of tetracycline hydrochloride

Surgical management of osteoradionecrosis of the temporal bone

The prevalence of hepatic cysts is 0.1% to 0.5% based on autopsy studies, and 2.5% based on US examinations. Percutaneous therapies are a new alternative to surgery. They include simple percutaneous aspiration, catheter drainage alone, and catheter drainage with sclerotherapy. We present an 11-year-old boy admitted to hospital because of abdominal pain. A diagnosis of simple hepatic cyst was made, which was treated with aspiration and tetracycline hydrochloride solution (5%) injection into the cystic cavity. Complete regression was seen on US and MRI examination at 3 months, with total collapse and deflation of the cyst. The cyst regressed totally, leaving a hyperechoic linear scar on US examination at 1 year. On the basis of the clinical and imaging results obtained, percutaneous sclerotherapy of hepatic cysts can be recommended as the treatment of choice and as a valid alternative to laparoscopy in children. The surgical management of osteoradionecrosis of the temporal bone has met with limited success because of the difficulty in accurate assessment of the viability of nonnecrotic bone intraoperatively. Failure to resect all nonviable bone results in recurrence of a necrotic focus. With the use of hyperbaric oxygen therapy to stabilize marginal bone and oral tetracycline to label viable bone preoperatively, removal of all nonviable bone can be accomplished. Postoperatively, a second course of hyperbaric therapy enhances wound healing, thus assuring a successful outcome. This article details a successful systematic approach that was developed to resect a necrotic focus in the temporal bone of a 10-year-old boy who had undergone a full course of radiotherapy for treatment of a rhabdomyosarcoma.

Paraquat is a nonselective contact herbicide of great toxicological importance, being associated with high mortality rates, mainly due to respiratory failure. We report the case of a 22-year-old male admitted to the emergency room with a sore throat, dysphagia, hemoptysis, and retrosternal pain after the ingestion of 50 mL of a paraquat solution, four days prior to admission. Chest CT scans revealed pulmonary opacities, pneumomediastinum, pneumothorax, and subcutaneous emphysema. The patient was submitted to two cycles of immunosuppressive therapy with cyclophosphamide, methylprednisolone, and dexamethasone. Premature Failure of a Riata Defibrillator Lead Without Impedance Change or Inappropriate Sensing: A Case Report and Review of the Literature

Using phantoms of children simulating a child of 6 months of age and another one aged 6 years, the authors measured the amount of radioexposure of the lens of the eye, the thyroid and of the gonads. The two computer tomographs Somatom SF and Tomoscan 300 under examination, produced radiodoses of comparable magnitudes. Both devices allow additional examination techniques of particularly high economy. It was found that in cranial CT the exposure of the lens to radiation is generally below 1 R if the orbitae are not situated in the plane of section. If a distance of 2-3 cm is maintained from the orbitae, the dose remains below 0.5 R. Protecting the gonads is meaningful in a 6-year-old boy, whereas covering-up of the thyroid reduced the dose only slightly.

Premature Failure of a Riata Defibrillator Lead. A 63-year-old woman with a St. Jude Medical Riata 1570 right ventricular lead complained of intermittent hiccups 2 months after implant. Interrogation revealed elevated pacing threshold and diaphragmatic stimulation. Pacing and shock lead impedances remained stable. No inappropriate sensing was noted. Fluoroscopic examination of the lead revealed a thin radio-opaque wire seen between the 2 defibrillator coils away from the main body of the lead. After extraction, a tear in the insulation of the lead was noted allowing the inner wire to protrude. Glomangioma have many synonyms including: glomus tumors, tumors of neuromyoarterial glomi, angioneuromas, angioneuromyomas, neuromyoarterial glomi, painful subcutaneous tubercles, Popoff tumors or subcotaneous glomal tumors. They are common in humans, rare in nonhuman primates and to the best of our knowledge, have only been reported in irradiated rhesus. The neoplasms originate in arterial-venous shunts known as neuromyoarterial glomi which are commonly found beneath fingernails and fingertips, but have been reported in many locations both superficial and deep. The neoplasm can be confused with hemangiopericytomas, hemangiomas, paragangliomas, and leiomyomas, and must be definitely diagnosed ultrastructurally. Omental pedicle flap used to treat a bronchopleural fistula after diaphragma--pericardio-pleuropneumonectomy.

Oligodendroglioma occurring after radiation therapy for pituitary adenoma

In a 47-year-old male patient a bronchopleural fistula was apparent 22 days after extended right-sided diaphragma-pericardio-pleuro-pneumonectomy for pleuramesothelioma. The thoracic cavity was infected. Rethoracotomy was performed and the fistula was closed using an omental pedicle flap. The bronchial stump became tight and the cavity fluid became sterile. No abdominal complications were seen. The patient died 8 months later from malignant pericardial infiltration. The ability of greater omentum to revascularize ischemic tissue, to absorb fluid and to resist local infection is proved and used in several subspecialities of surgery. Nevertheless the use of the greater omentum in the management of bronchopleural fistula has been rarely published. Noninvasive prediction of vertebral body compressive strength using nonlinear finite element method and an image based technique

A 38 year old male dentist developed an oligodendroglioma of the left medial temporal lobe and parasellar region 12 years after radiotherapy with 6600 rads for acromegaly. The 30 cases of radiation-induced gliomas reported in the English literature are reviewed and analysed. The criteria for defining radiation-induced tumours of the central nervous system are proposed as follows: the tumour has a long quiescent ''latency period'', a location in the previously irradiated field, a verified histological difference from a primary condition, and does not arise from a primary condition associated with a genetic syndrome such as neurofibromatosis or tuberous sclerosis. The reported case fulfilled these criteria but appears to be the only reported radiation-induced oligodendroglioma.

Long-term survival following radical surgery after chemotherapy for esophagogastric adenocarcinoma with extensive lymph node metastases: Report of a case

We report the delayed sequelae arising in a case of electrical injury, reviewing the literature on the subject and focusing on the MRI findings of the brain. A 23-year-old male suffered burns to the left parietal scalp, both feet, and the anterior chest wall. Neurological symptoms and MRI abnormalities appeared 14 days after the insult and continued for about three months. T1-weight MR images demonstrated homogeneous hypointensity, while T2-weighted images depicted hyperintense finger-like projections. Contrast-enhanced T1-weighted images demonstrated strong band-like enhancement, indicating meningeal hyperemia. Follow-up MR imaging showed that the lesion had disappeared, indicating that the cerebral edema and meaningeal hyperemia were reversible.

Impacts of gas pipeline rights-of-way (ROWs) through wetlands depend on types of habitat, construction techniques, final elevations, ROW maintenance practices, soil composition, and local climate. In some instances factors unrelated to the presence of the pipeline may have greater impacts on wetland modification than does the pipeline itself. At one site, the required seeding program inhibited natural reestablishment of wetland plants; at another, downstream construction resulted in a major disruption to the adjacent wetland habitat. Our case study is the development of Number Blocks and it combines physical interaction, learning, and immediate feedback. The development was done in collaboration with a class of 7-8 year old children and their mathematics teacher. The article argues that elements from different research methods allowed a structured approach to projects that combines educational research and innovation of new learning technologies. Abstract In this brief report, we have described the fine-needle aspiration cytology (FNAC) of a hepatosplenic T-cell lymphoma (HSTCL) in a 60-year-old female patient. The patient complained of fever and heaviness of left upper abdomen. Ultrasound-guided FNAC of the spleen showed immature lymphoid cells, and a diagnosis of primary non-Hodgkin lymphoma of spleen was offered. Subsequently, the patient underwent splenectomy and wedge biopsy of liver that confirmed the cytological diagnosis. The lymphoma cells were positive for CD3 immunostain. Parents contacted?an average of four agencies (?1.7; range?=?1?14) in the previous year. Approximately one-half of parents were looking for either multiple types of treatment, or help for different problems, across agencies. The complex pattern of help-seeking evidenced in our study likely increases the burden on the mental health care system and on families, and may reduce the likelihood that families will connect with the most appropriate treatment.

Effectiveness of different music-playing devices for reducing preoperative anxiety: A clinical control study

Purpose. We report a case of Ewing's sarcoma in the right distal femur in a 6-year-old male to demonstrate how dynamic contrast-enhanced magnetic resonance imaging (DEMRI) findings predicted histopathology. Materials and methods. DEMRI was performed at presentation and during and after completion of chemotherapy and radiation therapy. Histopathologic studies were done at presentation, at 77 weeks (20 weeks after a pathological fracture), and from the en bloc resection at 104 weeks. Results. Decreased semen quality in a male infected with malaria.

Curative resection of a huge malignant pancreatic endocrine tumor by pancreatoduodenectomy with portal and superior mesenteric vein resection and reconstruction using the right ovarian vein: Report of a case

A 33-year-old male of proven fertility suffered six attacks of malaria while resident in an African country. For this he received anti-malarial drugs. Semen analysis performed after the fourth attack, and repeated during the following 2 years after his return to Israel, revealed severe oligozoospermia, necrozoospermia and occasionally even azoospermia. Immunological examination of the patient revealed an inverse ratio of T-cell subsets and mast cell degranulation in response to palludrin. Twenty-five months after the last attack of malaria a significant improvement was found in semen quality and there was an increase in the ratio of T-helper to T-suppressor cytotoxic cells. Nonfunctioning pancreatic endocrine tumors (PETs) are rare and generally asymptomatic. A 68-year-old woman who had refused treatment for a pancreatic mass, revealed by ultrasonography to be 55 mm in diameter, was referred to us again 29 months later with jaundice. Investigations showed an 82-mm tumor in the head of pancreas, exposed from the papilla of Vater to the duodenal lumen. Case report 403: Extra-mammary Paget disease of the skin with disseminated skeletal metastases

Cardiopulmonary fitness and endurance in children with developmental coordination disorder

The case is presented of a 64-year-old man who complained on admission of discomfort in the right shoulder at the time of excision of a large section of the axillary skin which contained extra-mammary Paget disease. Shortly thereafter, radiological studies of the humerus and scapula showed obvious malignant disease which proved to be metastatic upon biopsy of the humerus and pathological studies of the excised area of extra-mammary Paget disease in the axillary skin. The patient died twelve months after the diagnosis was established with widespread metastatic disease. The literature on the subject was reviewed and it was demonstrated that metastases to the skeleton from lesions of the skin are relatively rare. This case is exceptional because of the rapid fatal course and the extent of metastatic disease originating in extra-mammary Paget disease of the axillary skin.

Aluminium phosphide is a lethal protoplasmic toxin and is also the most common cause of suicidal poisoning in northern India. Machinery management data for willow harvest with a bio-baler

Two children with cutaneous lymphoma and leukemia who were treated with total skin electron beam therapy (TSEB) are described here. Patient 1. A 7-year-old boy was admitted because of a mass over the bilateral parotis and anemia. The white blood cell count on admission was 5,000/ul. Bone marrow examination revealed 70 per cent monoblasts (M5a type by FAB classification). Complete remission was obtained following the regimen of daunomycin, cytosine arabinoside, 6-mercaptopurine and prednisolone. He was again admitted because of a skin nodule on the left thigh 19 months after initial diagnosis. A biopsy of the skin nodule demonstrated monoblastic infiltration. Investigation on the state of health (1993-1997) in ambient residents of Daya Bay Nuclear Power Plant in the early days

Willow harvested in 2-to-3-year rotations has been touted as a reliable source of biomass. A newly designed harvester based on a round baler was developed in 2006. The third generation biobaler was evaluated in 2009 at 2 willow plantations in Quebec. The first site at Godmanchester was a three-year old regrowth with an estimated 52,000 stems/ha of large diameter willow. The biobaler with a flail cutter harvested three plots totalling 4,136 m{sup 2}. Total harvested biomass was 10.36 t DM in 42 bales. The harvest rate averaged 29 bale/h with an average bale size of 1.22 m wide by 1.30 m in diameter. Diesel fuel consumption averaged 0.81 L/bale. Measured losses averaged 11 per cent of the yield. Use of sodium chromate Cr/sub 51/ in diagnosing childhood idiopathic pulmonary hemosiderosis

A 72-year-old man was exposed to the sarin gas attack in a Tokyo subway on March 20 th, 1995. After exposure, he noticed eye discomfort, chest tightness, headache and weakness of the lower limbs and oropharyngeal muscles. Despite these symptoms, he visited a hot spring on the same day with his family. On March 25 th, his muscle weakness progressed, and a low grade fever appeared. His muscle weakness disappeared 8 days after exposure to sarin, but respiratory failure rapidly developed, necessitating artificial ventilation within four day after hospitalization on March 28th. Chemotherapy with erythromycin, imipenem/cilastatin, and steroid pulse therapy was begu. Thin-Section CT findings of lung small peripheral adenocarcinoma detected by CT physical examination

The diagnosis of idiopathic pulmonary hemosiderosis (IPH) may be elusive. A 6-year-old boy had microcytic hypochromic anemia and a hemolytic component. Hemosiderin-laden macrophages were not found in the gastric aspirate. He had no pulmonary signs or symptoms. Extensive hematologic and roentgenologic investigations failed to reveal the cause of the anemia. Quantitative serial scintigraphic scanning showed significant (35%) pulmonary sequestration of autologous erythrocytes labeled with sodium chromate Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy spece Cr51. The half-life of the RBCs was moderately decreased (half-life, 15 days; normal, 25 to 35 days). An open-lung biopsy specimen confirmed the diagnosis of IPH. The Scientific Publications of Richard H. Dalitz, FRS (1925-2006)

Subjects were 18 cases with adenocarcinoma of 20 mm or less in diameter, which were detected and resected surgically from September, 1993 to August, 1998. On these cases, thin-Section CT (TSCT) images were compared with pathological findings. Patients were 15 men and 3 women, with mean age of 63.4-year-old. An average tumor size was 11.5 mm (5-17 mm). According to pathological classification of Noguchi, type A as in 6 cases, type B in 3, type C in 7 and type F in 2. TSCT was carried by vas voltage =150 kVp, vas current =200 mA, beam width =2 mm, WW =2000 HU, and WL =-700 HU. The ratio of CT attenuation value in cancer by TSCT correlated with the ratio of aeration rate in the pathologic specimen. Sprengel deformity in a nine-year-old boy: diagnosis and therapy; Sprengel-Deformitaet bei einem neunjaehrigen Jungen: Diagnostik und Therapie

A 52-year-old woman had a history of left hearing loss for 5 years. An acoustic neurinoma with 3.2 cm in diameter was diagnosed and treated with gamma-knife radiosurgery (19 Gy of marginal dose) 1 year and 4 months ago. She developed headache, nausea, and visual disturbance 1 month prior to admission. Slight left facial palsy appeared after radiosurgery. Magnetic resonance imaging demonstrated the tumor with central necrosis in the left cerebellopontine angle cistern, increasing in size to 3.5 cm in diameter, and hydrocephalus. Tumor removal was performed incompletely, because of the fibrous appearance of the tumor and severe adherence with the surrounding cerebellar tissue. Facial palsy did not worsen after surgery. Since the hydrocephalus was not resolved, a right ventriculo-peritoneal shunt was inserted. Should Male Gender Assignment be Considered in the Markedly Virilized Patient With 46,XX and Congenital Adrenal Hyperplasia?

Up to this time the sigmoid colostomy has been widely accepted and conventional treatment for radiation-injured rectum, but patients without residual malignancy strongly desire to live without colostomy. We have tried to remove the involved rectal segments by sphincter-saving procedures. Four patients underwent these procedures, pull-through procedure in three and low anterior resection in one. Among sphincter-saving procedures, pull-through procedure was most adequate. Provided the following five conditions are fulfilled, pull-through procedure should be considered for severe radiation-injured rectum. (1) No recurrence of initial malignancy in the pelvis. (2) More than 2 cm intact rectal segment above dentate line may be preserved. (3) No radiation-injured segment in upper sigmoid. (4) No severe radiation damage in small intestine. Concentration ratios relative to soil, for plutonium in litter, invertebrate cryptozoans, herbaceous ground vegetation, orthoptera and small mammals were approximately 10"-"1, 10"-"2, 10"-"3, and 10"-"4, respectively. Concentration ratios (CR) for plutonium in biota from the floodplain forest are less than CR values from other contaminated ecosystems in the USA. Presumably, this is due to humid conditions and greater rainfall which minimize resuspension as a physical transport mechanism to biota. Management of Ewing's sarcoma

We simulate mountain-scale coupled heat transfer and gas flow at Yucca Mountain. A coupled rock-gas flow and heat transfer model, TGIF2, is used to simulate mountain-scale two-dimensional transient heat transfer and gas flow. The model is first verified against an analytical solution for the problem of an infinite horizontal layer of fluid heated from below. Our numerical results match very well with the analytical solution. Then, we obtain transient temperature and gas flow distributions inside the mountain. These distributions are used by a transient semianalytical particle tracker to obtain carbon-14 travel times for particles starting at different locations within the repository. Large intracranial vessel occlusion arising after radiotherapy for craniopharingioma. A case report and review of the literature

Forty cases of Ewing's sarcoma of various sites had attended the Department of Radiotherapy at the Wanless Hospital and the Miraj Medical Centre, attached to the Government Medical College, Miraj, Maharashtra, from January, 1970 through December, 1979. There were 28 male patients and 12 female patients in the series with a male to female ratio of 2.3:1. The youngest patient in the series was a female of 6 years of age with the tumour in the scapula and the oldest patient was a 32-year-old male with a lesion in the metacarpal bone. Sixty per cent of the cases were seen in the second decade. The average age of the patients was 13.5 years. Large intracranial vessel occlusion arising after radiotherapy for craniopharingioma. A case report and review of the literature

We report a patient who developed occlusion of the intracranial major arteries induced by radiation therapy for craniopharyngioma. The patient, a 9-year-old boy, presented with deterioration of vision, nausea and vomiting. He underwent a right ventriculo-peritoneal shunt operation, and the tumor was partially removed. Postoperatively, he received focal radiation therapy with total of 60 Gy. Preoperative cerebral angiography, cerebral vessels had no sigh of occlusive lesion or stenosis. One year after irradiation, he started to have repeated cerebral ischemic attacks. But no regrowth of the tumor was detected on CT scans. Two years and 8 months after irradiation, he suddenly had right hemiparesis. Two important greenhouse gases, CO{sub 2} and CH{sub 4}, are closely connected to the carbon cycling of wetlands. Although virgin wetlands are mostly carbon accumulating ecosystems, major proportion of the CO{sub 2} bound annually in photosynthesis is released back to the atmosphere. Main portion of the carbon cycling in wetlands is quite fast while a small proportion of carbon diffusing from soil is released from organic matter, which may be ten thousand years old. Methane is formed in the anaerobic layers of wetlands, from where it is released gradually to the atmosphere. The decomposition in anaerobic conditions is very slow, which means that usually only a few percent of the annual carbon cycling takes place as methane. For the purpose of risk estimates from radiation carcinogenesis including leukemogenesis and radiation genetic effects, the biological significant dose is not the tissue kerma in air but the absorbed dose in organ or tissue with respect to carcinogenic and leukemogenic effects or genetic effects. In order to estimate organ or tissue dose from the tissue kerma in air, a ratio of the organ or tissue dose to the tissue kerma in air for survivors in Hiroshima and Nagasaki was calculated with the aid of the Snyder's mathematical phantoms constructed so as to simulate the body shape of survivors aged 5, 10 years old and adult at the time of atomic bomb detonations. The classical phenomenon of position-effect variegation (PEV) is the mosaic expression that occurs when a chromosomal rearrangements moves a euchromatic gene near heterochromatin. A striking feature of this phenomenon is that genes far away from the junction with heterochromatin can be affected, as if the heterochromatic state {open_quotes}spreads.{close_quotes} We have investigated classical PEV of a Drosophila brown transgene affected by a heterochromatic junction {approximately} 60 kb away. PEV was enhanced when the transgene was locally duplicated using P transposase. Comparative examinations of serum pepsinogen I, II and gastric area using computed radiography in the atrophic gastritis

The relationship between serum PG I, PG II levels and extent of atrophic gastritis was examined. The subjects were 64 patients (male: 32, female: 32, 51.9 years old on average) with established diagnosis of either atrophic gastritis or normal. In the X-ray gastric examination, Fuji Computed Radiography (FCR) was used to obtain clear-cut images of the gastric area. Concerning the serum PG I level, patients in the group with atrophic gastritis showed lower levels than those of the people in the group with no atrophic change, but the variation was wide, and no definite tendency was seen in the relationship between the atrophic change and the serum PG I levels. Cleanup of the 65-year old Department of Energy Laboratory is being conducted under a RCRA Consent Order with the State of New Mexico. This agreement is one of the most recent cleanup agreements signed in the DOE complex and was based on lessons learned at other DOE sites. We report biological changes at several UK Acid Waters Monitoring Network lakes and streams that are spatially consistent with the recovery of water chemistry induced by reductions in acid deposition. These include trends toward more acid-sensitive epilithic diatom and macroinvertebrate assemblages, an increasing proportional abundance of macroinvertebrate predators, an increasing occurrence of acid-sensitive aquatic macrophyte species, and the recent appearance of juvenile (<1 year old) brown trout in some of the more acidic flowing waters. Americans' political participation in the 1993-94 national health care reform debate.

Over 300 cases of femoral neck fractures following radiotherapy for intrapelvic malignant tumor have been reported in various countries since Baensch reported this disease in 1927. In Japan, 40 cases or so have been reported, and cases of bilateral femoral neck fractures have not reached to ten cases. The authors experienced a case of 75 year-old female who received radiotherapy for cancer of the uterus, and suffered from right femoral neck fracture 3 months after and left femoral neck fracture one year and half after. As clinical symptoms, she had not previous history of trauma in bilateral femurs, but she complained of a pain in a hip joint and of gait disturbance. The pain in left femoral neck continued for about one month before fracture was recognized with roentgenogram. The health politics and policy communities are still struggling with the question of "what went wrong" in the 1993-94 health care reform effort. Here I identify which Americans were politically active and inactive during the health care reform debate to explore the role political participation may have had in determining the outcome of the debate. Using data from a national and California random-sample telephone surveys, and controlling for other demographic attributes, I found that those who engaged in political activity specifically related to health care reform were disproportionately more likely to be self-identified conservatives, less likely to favor an employer mandate plan, more likely to be fifty to sixty-four years old, more likely to be men, and more likely to have greater interest in and knowledge of the health care issue. Acute myelogenous leukemia following chemotherapy and radiation for rectal cancer

Ewing's sarcoma classically presents as a hot spot on bone scan as a result of increased vascularity of the tumor and new bone formation. Purpose We report and analyze an uncommon pattern of a 'cold' lesion in Ewing's sarcoma on bone scan and its pathophysiologic significance. Case report A 15-year-old boy complaining of thigh pain. CT scan evoked Ewing's sarcoma or osteitis. MRI evoked chronic osteitis. Scintigraphy showed a fairly intense and heterogeneous uptake on the femoral lesion and no abnormal uptake elsewhere. Biopsy showed none pathologic pattern. Three months later, a second biopsy concluded to Ewing's sarcoma. A lateral cephalometric study of the hard- and soft tissue profile around the face in the mixed dentition

In August 1982, a 44-year-old man was diagnosed as having rectal cancer, histologically diagnosed as well differentiated adenocarcinoma, and abdominoperineal resection and colostomy were performed. Postoperatively, he received chemotherapy with mitomycin C up to a total dose of 100 mg. In September 1986, lung metastasis occurred and he was treated with a combination chemotherapy consisting of cisplatin, pirarubicin and 5-fluorouracil. In the following year, radiation treatment (total: 6900 rad) was given for a recurrent pelvic lesion. Peripheral blood on April 30, 1988, showed anemia, thrombocytopenia and appearance of myeloblasts, and a diagnosis of acute myelogenous leukemia (FAB: M1) was made. It is shown that a three-body bound state turns into a virtual one by increasing the ?? binding energy. This result is consistent with previous results for three equal mass particles. The present approach considers the n-n-18C halo nucleus. Child mortality from solid-fuel use in India: a nationally-representative case-control study - article no. 491

It is shown that when a quantum mechanical oscillator is parametrically excited there are special values of the parameters for which the system will pass periodically through a lattice of coherent states associated with the modular group [Gamma]. It is shown that these [Gamma] transits can be used to determine unknown parameters. A method is given for detecting the transits experimentally and is made possible by the existence of three families of states associated with modular forms that are orthogonal to the lattice. Most households in low and middle income countries, including in India, use solid fuels (coal/coke/lignite, firewood, dung, and crop residue) for cooking and heating. Such fuels increase child mortality, chiefly from acute respiratory infection. There are, however, few direct estimates of the impact of solid fuel on child mortality in India. Methods: We compared household solid fuel use in 1998 between 6790 child deaths, from all causes, in the previous year and 609 601 living children living in 1.1 million nationally-representative homes in India. Analyses were stratified by child's gender, age (neonatal, post-neonatal, 1-4 years) and colder versus warmer states. We also examined the association of solid fuel to non-fatal pneumonias. Solid fuel use was very common (87% in households with child deaths and 77% in households with living children). Relativistic strongly magnetized winds outflowing from fast-rotating compact objects like millisecond pulsars with surface magnetic fields of $\\sim 10^{15}-10^{16}$ G are plausible sources of cosmological $\\gamma$-ray bursts. In such winds, there are at least three regions where extremely powerful X-ray and $\\gamma$-ray emission may be generated. The first radiating region is the wind photosphere that is at a distance of $\\sim 10^9$ cm from the compact object. The second radiating region is at a distance of $\\sim 10^{13}-10^{14}$ cm. In this region, the striped component of the wind field is transformed into large-amplitude electromagnetic waves. The third radiating region is at a distance of $\\sim 10^{16}-10^{17}$ cm, where deceleration of the wind due to its interaction with an ambient medium becomes important. Unsolved problems in the diagnosis and surgical treatment of intramedullary tumors

Most common bacterial species causing peritonitis in the course of peritoneal dialysis (PDP) are coagulase-negative staphylococci, Staphylococcus aureus and streptococci. Haemophilus influenzae is rarely associated with PDP. Hereby we present the first known case of APD-associated peritonitis caused by non-type able H. influenzae (NTHi) presenting the beta-lactamase negative, ampicillin-resistant (BLNAR) phenotype. An 18 year old boy who had been treated with the APD for 12 months due to SLE was admitted in good general condition with diagnosis of PDP. Standard diagnostic and therapeutical procedures were initiated. Dialysis fluid was turbid with cytosis of 435 WBC/ml. From dialysis fluid pure culture of Gram-negative coccobacillus was isolated. Translocation of /sup 14/C-sucrose from mesophyll to vein and stem bark in mulberry plant

Problems and solutions in the diagnosis and operative treatment were clarified by retrospective evaluation of cases of intramedullary tumor in which MRI and surgery had been performed. The subjects were 16 patients (21-75 years old, mean age: 45 years) whose tumor was surgically removed between 1991 and 1998. MRI was performed preoperatively, and T1-weighted images, T2-weighted images, and enhanced images were obtained. Angiography was also performed in some cases. The histological diagnosis based on the MRI findings before surgery were compared with the pathological diagnosis after the operation. The production of nitrous oxide (N{sub 2}O) and carbon dioxide (CO{sub 2}), and the consumption of methane (CH{sub 4}) in a temperate forest soil were studied by a rapid and sensitive gas chromatographic (GC) method for analysing all three greenhouse gases at or below ambient levels. Results of the remote sensing feasibility study for the uranium hexafluoride storage cylinder yard program

The impact of increasing atmospheric C0{sub 2} has not been fully evaluated on western coniferous forest species. Two year old seedlings of Pinusponderosa were grown in environmentally controlled chambers under increased C0{sub 2} conditions for 6 months. These trees exhibit morphological, physiological, and biochemical alterations when compared to our controls. Analysis of whole plant biomass distribution has shown no significant effect to the root to shoot ratios, however needles subjected to elevated C0{sub 2} exhibited an increased overall specific needle mass and a decreased total needle area. Renal artery stenosis after radiotherapy for Ewing's sarcoma

The US DOE manages the safe storage of approximately 650,000 tons of depleted uranium hexafluoride remaining from the Cold War. This slightly radioactive, but chemically active, material is contained in more than 46,000 steel storage cylinders that are located at Oak Ridge, Tennessee; Paducah, Kentucky; and Portsmouth, Ohio. Some of the cylinders are more than 40 years old, and approximately 17,500 are considered problem cylinders because their physical integrity is questionable. These cylinders require an annual visual inspection. The remainder of the 46,000-plus cylinders must be visually inspected every four years. Currently, the cylinder inspection program is extremely labor intensive. Because these inspections are accomplished visually, they may not be effective in the early detection of leaking cylinders. Background: the fact that therapeutic irradiation can induce significant stenosis in the arteries of the head, neck, and chest, as welt as in the aorta and the iliac arteries, is familiar in daily practice and well documented in the literature. During facial surgery for a 68-year-old man, a fire broke out, resulting in first- and second-degree burns after a nasal cannula ignited in an oxygen-rich environment because of improper draping and tenting. DISCUSSION: Operating room (OR) fires can be prevented if any component of the "fire triangle"-fuels, ignition sources, and oxidizers-is reduced or eliminated. The use of supplemental oxygen in the OR via nasal cannulae, nebulizers, and oxygen cylinders must always considered a potential source of fire. Full text: A 51-year-old woman with end stage renal failure secondary to Haemolytic Uraemic syndrome underwent a cadaveric renal transplant. A routine post transplant DTPA scan was performed which demonstrated satisfactory renal transplant perfusion and function. Incidental note was made of tracer uptake in the pelvis in the mid-line, which was suspected to be a uterine fibroid. This was confirmed on ultrasonography and at surgery. One week post transplantation the patient became acutely unwell and at laparotomy a perforated diverticular abscess was drained. Intraoperatively the transplant kidney was examined and the surgeon thought there was a area of infarction. This was confirmed on biopsy. A 36-year-old woman underwent amniocentesis for advanced maternal age. The fetal karyotype had an extra dark staining G band on the long arm of chromosome 11 with no other identifiable abnormalities. FISH studies using a chromosome 11 paint probe confirmed the origin of the extra band. The abnormality was identified as a partial duplication of 11q: 46,XX dir dup (11)(q13.5q21) or (q21q23.1). The germination and growth of Pinus radials Don. plantlets in solid residues deriving from a Kraft pulp industry was evaluated. Plant conditions were monitored by histological studies of roots and shoot-tips, as well as by plant analyses of several essential and non essential elements. The solids employed consisted of ashes, fly-ashes, dregs, grits, primary sludge, brown stock screening rejects and various mixtures of them. This study compared rCBF pattern in normal adult and normal children using statistical parametric mapping (SPM). The purpose of this study was to determine distribution pattern not seen visual analysis in both groups. The three main objectives of this study were: (1) to determine if depletion of proteoglycans from the cartilage matrix that occurs during osteoarthritis causes a measurable increase of cartilage proteoglycan components in the synovial fluid and sera, (2) to observe what effect intracellular cAMP has on the expression of matrix components by chondrocytes, and (3) to determine if freshly isolated chondrocytes contain detectable levels of mRNA for fibronectin. Hystreosalpingogram of 375 patients with infertility were reviewed with reference to radiological feature of each pathology and a comparative study of hysterosalpingography and laparoscopy in 36 patients was presented. Pre-operative MRI of anorectal anomalies in the newborn period

Gonadal function was studied in two groups of children previously treated for medulloblastoma with surgery followed by postoperative craniospinal irradiation. In group 1 but not in group 2, the children also received adjuvant chemotherapy for one to two years. All children in group 1 received a nitrosourea (BCNU or CCNU), plus vincristine in four and procarbazine in three patients. The nine children in group 1 showed clinical and biochemical evidence of gonadal damage with elevated serum FSH concentrations and, in the boys, small testes for their stage of pubertal development. In group 2 (n . 8), each child had completed pubertal development normally, the boys had adult sized testes and the girls regular menses. Gonadotropin values were normal in all eight children. Herlyn-Werner-Wunderlich syndrome: uterus didelphys, blind hemivagina and ipsilateral renal agenesis. Sonographic and MR findings in 11 cases

Nine infants (six boys, three girls) with anorectal anomalies were examined in the immediate newborn period, prior to corrective surgery, with MRI. Three high, one intermediate and five low anomalies were found at MRI - one patient with a `low` lesion was subsequently found at surgery 2 months later to have a high anorectal anomaly. This infant had passed meconium per urethram soon after the MRI study, prompting the need for a protective colostomy and stressing the importance of a thorough clinical examination of babies with anorectal malformations. The MRI results and findings at surgery were in agreement in all other patients (n=8). Hydronephrosis was evident in two and renal agenesis in one patient. Fast quantitative diffusion-tensor imaging of cerebral white matter from the neonatal period to adolescence

Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis is a rare entity, sometimes referred to as Herlyn-Werner-Wunderlich syndrome (HWW). It usually presents after menarche with progressive pelvic pain, sometimes with regular menses, and a palpable mass due to hemihaematocolpos. The diagnosis is generally made only if the suspicion of this genitourinary syndrome is raised. To highlight the imaging diagnostic clues in this rare condition. Suzaku and Optical Spectroscopic Observations of SS 433 in the 2006 April Multiwavelength Campaign

We investigated the isotropic diffusion coefficient (D') and fractional anisotropy (FA) in white matter (WM) during brain development, using an optimised diffusion-tensor imaging (DTI) method with whole brain coverage in a clinically acceptable time. We images 52 children with no evident neurological abnormality (30 boys, 22 girls aged 1 day-16 years) using high-angle DTI with optimised temporal gradient performance. D' and FA were calculated in 10 regions of interest in white matter. We saw that the age-related reduction in D' and increase in FA follow a mono- or biexponential model in white matter, probably depending on the compactness and myelination rate of the fibre tracts. In contrast to other areas, in which adult values were reached during the third year, there is a trend to continuous increase in FA in all deep white-matter areas, suggesting continuing maturation and organisation of deep tracts not detected on conventional MRI. Solar system constraints on a Rindler-type extra-acceleration from modified gravity at large distances

We report results of the 2006 April multi-wavelengths campaign of SS 433, focusing on X-ray data observed with Suzaku at two orbital phases (in- and out-of- eclipse) and simultaneous optical spectroscopic observations. By analyzing the Fe25 K_alpha lines originating from the jets, we detect rapid variability of the Doppler shifts, dz/dt ~ 0.019/0.33 day^-1, which is larger than those expected from the precession and/or nodding motion. This phenomenon probably corresponding to "jitter" motions observed for the first time in X-rays, for which significant variability both in the jet angle and intrinsic speed is required. From the time lag of optical Doppler curves from those of X-rays, we estimate the distance of the optical jets from the base to be ~(3-4) \\times 10^14 cm. New constraints on the primordial black hole number density from Galactic gamma-ray astronomy

We analytically work out the orbital effects caused by a Rindlertype extra-acceleration ARin which naturally arises in some recent models of modified gravity at large distances. In particular, we focus on the perturbations induced by it on the two-body range {\\rho} and range-rate {\\rho}\\cdot which are commonly used in satellite and planetary investigations as primary observable quantities. Benz(a)pyrene levels in medicinal plants and the possibility of contamination of drugs of plant origin

Using electron spin resonance, lattice position and dynamic properties of Mn2+ ions were studied in 0.5 and 2 % manganese doped SrTiO3 ceramics prepared by conventional mixed oxide method. The measurements showed that Mn2+ ions substitute preferably up to 97 % for Sr if the ceramics is prepared with a deficit of Sr ions. Motional narrowing of the Mn2+ ESR spectrum was observed when temperature increases from 120 K to 240-250 K that was explained as a manifestation of off-center position of this ion at the Sr site. From the analysis of the ESR spectra the activation energy Ea = 86 mV and frequency factor 1/?0 ? (2-10)x10^(-14) 1/s for jumping of the impurity between symmetrical off-center positions were determined. Both values are in agreement with those derived previously from dielectric relaxation. This proves the origin of dielectric anomalies in SrTiO3:Mn as those produced by the reorientation dynamics of Mn2+ dipoles.

Absorption Features in Spectra of Magnetized Neutron Stars

Benz(a)pyrene (BP) levels in extracts and solvent cake obtained by alcoholic and aqueous extraction of specimens of 20 different medicinal plants were measured to explore into the possibility of its passage from these plants to drugs. Seventy percent alcoholic extracts were found to contain 40-60% of BP passed from raw material, while aqueous extracts--2-3% (in some cases 10-14%). Maximal concentrations of BP in alcoholic extracts were 0.6-0.7 micrograms/1 and 0.03-0.04 micrograms/1--in aqueous ones. A significant correlation between BP level in extracts and its content in plants was established. BP pathways in the course of solasodine manufacturing from nightshade (Solanum lacinatum) were studied. As little as 1% of BP passed to extract after primary extraction in 2%--sulfuric acid. Solasodine contained about 3 micrograms/kg of BP.

The X-ray spectra of some magnetized isolated neutron stars (NSs) show absorption features with equivalent widths (EWs) of 50 - 200 eV, whose nature is not yet well known. To explain the prominent absorption features in the soft X-ray spectra of the highly magnetized (B ~ 10^{14} G) X-ray dim isolated NSs (XDINSs), we theoretically investigate different NS local surface models, including naked condensed iron surfaces and partially ionized hydrogen model atmospheres, with semi-infinite and thin atmospheres above the condensed surface. We also developed a code for computing light curves and integral emergent spectra of magnetized neutron stars with various temperature and magnetic field distributions over the NS surface. Patients with heterotaxy syndrome often have complex cardiac and extracardiac anomalies requiring further detailed diagnostic evaluation. They often present severe cardiac failure early in life. Newer radiological modalities in the form of spiral computed tomography (CT) and three-dimensional reconstruction of spiral CT allow clear definition of the anatomy of these anomalies. Forum on Flexible Education. Reaching Nomadic Populations in Africa. Summary Report

Purpose: To report on the nephron-sparing, percutaneous ablation of a large renal cell carcinoma by combined superselective embolization and percutaneous radiofrequency ablation. Materials and Methods: A 5 cm renal cell carcinoma of a 43-year-old drug abusing male with serologically proven HIV, hepatitis B and C infection, who refused surgery, was superselectively embolized using microspheres (size: 500 - 700 {mu}m) and a platinum coil under local anesthesia. Percutaneous radiofrequency ablation using a 7F LeVeen probe (size of expanded probe tip: 40 mm) and a 200 Watt generator was performed one day after transcatheter embolization under general anesthesia. Results: The combined treatment resulted in complete destruction of the tumor without relevant damage of the surrounding healthy renal tissue. The patient was discharged 24 hours after RF ablation. Five year prospective study of plasma renin activity and blood pressure in patients with longstanding reflux nephropathy.

This report describes the Forum on Flexible Education: Reaching Nomadic Populations in Africa, which was held in Garissa, Kenya, from 20-23 June 2006. The objectives of the Forum were to share experiences and best practices, create linkages and encourage collaboration in order to make education more accessible to nomadic communities. Representatives from the Kenyan MOE, COL, ComSec and UNICEF provided remarks at the Opening Session of the Forum. They stressed the need to find ways to include nomadic children in the education process and the importance of developing alternative channels for the delivery of education and training. Effect of lead exposure in children

Eight of 100 normotensive children who had pyelonephritic scarring secondary to urinary infection and vesicoureteric reflux were found in an earlier study to have increased plasma renin activity (PRA). Because the risk of these patients becoming hypertensive is between 10% and 20% and because renin activity may play a part in the pathogenesis of the hypertension, PRA and blood pressure were studied in 98 of the original group after five years of follow up. Two patients could not be traced, and other factors that might influence blood pressure or PRA led us to exclude 13 others, 10 of whom were girls taking oral hormonal contraceptives. Increased PRA was found in 11 of the remaining 85 patients but not in five of the eight patients with increased PRA in the first study. Increasing prosperity and population-growth in many developing countries are resulting in accelerated growth in population of vehicles and vehicle-kilometers traveled. In Pakistan also, the number of vehicles has jumped from 0.8 million to about 4.0 million within 20 years, showing an overall increase of more than 400%. Accordingly, the consumption of petrol (motor spirit) has increased from 828,670 metric tons to 1,189,042 metric tons. The high content of lead in petrol is a serious issue, as the end- product is the release of lead into the environment. In Pakistan, prior to July 2001, lead-content in petrol was reported to be as high as 0.35-0.84 gram per liter. Spectroscopic and electrochemical characterisation of thin cathodic plasma polymer films on iron

Regional cerebral blood flow (rCBF) was assessed with single photon emission computed tomography (SPECT), "1"2"3I-N-isopropyl-iodoamphetamine ("1"2"3I-IMP), and the Table-Look-Up method in 51 children (27 boys, 24 girls) considered neurologically normal and aged 1 month to 15 years (mean age, 4 years 1 month; standard deviation (SD) 3 years 11 months) divided into seven age groups. The rCBF was measured in cortical regions, the cerebellum, thalamus, and the head of the caudate nucleus. Curves for reference values and standard deviations were defined for each region. The rCBF rapidly increased until 2 years of age, reaching maximum values during the third to fifth periods (2 to 10 years of age) in each region. Altered glycosaminoglycan metabolism in injured arterial wall

Complimentary spectroscopic, microscopic and electrochemical studies were performed to characterise the barrier properties as well as the interface structure of model iron substrates covered with thin plasma polymer films. Cathodic plasma polymers were deposited which show high barrier properties. The metal surface was pre-treated by a reducing or oxidising plasma. This allowed the adjustment of the oxidation state of the interface layer. The interface structure was characterised by means of X-ray photoelectron sputter profiles, infrared spectroscopy and the application of a Kelvin probe. The investigations show that the measured Voltapotential on the plasma polymer surface can be correlated with the oxidation state of the interface. Reducing plasmas lead to an almost oxide free surface. A study of bacterial isolates from corneal specimens and their antibiotic resistance profile

Glycosaminoglycans (GAG) are believed to be important in the pathogenesis of atherosclerosis. We have previously demonstrated that areas of injured aorta that have been re-endothelialized accumulate increased amounts of lipid and GAG when compared to areas remaining de-endothelialized. We have now examined the net incorporation of labeled precursors into the individual GAG present in both re-endothelialized and de-endothelialized areas of rabbit aorta. Aortic tissue was examined at 2-3 and 10-14 weeks after a denuding injury by incubating tissue minces with (/sup 3/H)glucosamine and sodium (/sup 35/S)sulfate for 24 hr. Following incubation, the aortic GAG were isolated and assayed for uronic acid concentration and radioactivity. Education for All by 2015: Will We Make It?

We aim to examine the spectrum of bacteria causing corneal infections and their antibiotic susceptibility patterns. This will serve as a guideline for empiric therapy of corneal infections. We conducted the study over a period of 18 months from March 2001 through December 2002 in King Abdul-Aziz University Hospital, Riyadh, Kingdom of Saudi Arabia. Corneal specimens taken from 200 patients were inoculated directly onto different types of media. The isolates were identified and then tested against the appropriate topical or systemic antibiotics. Sixty-seven (33.5%) of the total specimens were culture positive and 133 (66.5%) were culture negative. Fourteen (7%) of these showed organisms in the Gram stained smears and correlated well with the culture reports.
